The best advice I was given by my lactation consultant was the milk shake. I was dealing with over supply and baby was only getting fore milk (this will turn their poop green btw) and concerns about getting back to birth weight. My LC told me that the fat sticks to the milk ducts and you can shake the breast like you are shaking a drink and mix that fat in for baby. I know it sounds weird but it works!
@marycahillclc

Yes that can definitely help 👍 thanks for sharing! ❤️ It’s important to also get supply under control. Milk isn’t stored in our breasts, it’s produced while we nurse or pump so it would be hard to shake your breast through the whole nursing or pumping session. 🥰

@marycahillclc

@@deahnabarcelona9915 hi mommy! Do you have a forceful letdown/over supply? How old is baby? Your breasts will start to feel much softer and less full but still make lots of milk. When your breast feels full, that’s inflammation of the breast milk isn’t stored in your breast. It’s made while you nurse or pump. If you don’t have a forceful letdown/over supply, it’s best to offer both breasts each feeding in the first few months. To do this, you would nurse on the 1st breast for as long as possible. When baby seems to be done nursing, offer the 2nd side. Baby may not want it, or just nurse a few seconds or nurse for several minutes. We want to nurse this way because (you’re right🥰) we want baby to get the fattier hind milk but when we offer the other side in that feeding, it helps to keep supply up if you’re a mom with an average milk supply. ❤️

Hi Mary, thank u so much for this informative video. My baby is 11weeks old. I guess I suffer from a forcefull letdown,since my baby tend to choke with milk whenever I breastfeed him.I changed the breastfeeding position. I oppted for side lying position. It helpped with the choking,But I believe it made his silenced reflux worse. Yesterday he choked while he was laying on his back and we had to took him to the emergency room. My questions are: 1* can the side lying position make the reflux problem worse? 2* what can I do to reduce my milk oversupply? 3* what can I do to reduce the reflux? Im really suffering with the breastfeeding, It becomes something scary and stressful for me. Thank u so much❤

@@douniaelbaz817 That is very scary. I’m so sorry that you and baby had that happen.😥 Reflux is a symptom of something happening. Sometimes we don’t know exactly what is causing it but if mom has a forceful letdown/over supply, that’s often the cause. Side lying is often one of the best with this condition but it can still be difficult. Keeping baby more upright for 5 or 10 minutes after eating can help. Hi Mary, Thanks for your video series on OS/FL. They are really informative! I have read your responses to comments in your videos too, and they are truly helpful as I can relate to some of the questions that viewers had posted. I hope you could enlighten me on some matters as follow: 1. Does baby suffering from reflux due to OS/FL tend to struggle clearing out secretions (e.g.; saliva) especially upon waking up? My baby tends to grunt, move his head left and right, and cough out or gag when waking up-not always but often since i suspect i am overproducing. 2. Me and baby exhibit the signs of OS/FL that you have mentioned in your videos. Hence, i started 3-hour block feeding. It makes the other breast really engorged and painful before its turn to be fed on, but i am managing it with cold compress and i am refraining from pumping it out despite the pain and pressure. A. In a 3-hour block feeding, supposed that the designate breast to be fed on has not been emptied during the 3-hour period, should i left it be and proceed to the other breast during the nect block period? B. I wonder how long does it usually take for block feeding to work in controlling my milk supply? C. Would the absence of engorgement be the indication that OS/FL has been corrected? D. What is the risk of developing mastitis with block feeding especially that the other breast is left unlatched on for approx 6 hours in a 3-hour block feeding session (with 3 hours being the block feeding session and another 2-3 hours before baby usually demands for another feed)? How to prevent it from developing? 3. From your videos, i gathered that lactose in foremilk makes baby gassy and irritates his GI tract. Is there a way to avoid this while mommy addresses the OS/FL issue or does baby just have to deal with it while the issue is being addressed? How bad could continuous ingestion of high amount of foremilk affect baby? Looking forward to hearing back from you. Would really appreciate if you could spare your time to clarify my concerns. Thank you!

@@joyanntan1289 Let me start by asking, how old is baby? Is this your first baby? If not, how was BF with other babies? I can’t say for sure that baby might wake and have trouble with secretions but it would make sense to me. If most people consumed a large amount of high lactose before bed, they might wake up phlegmy. When you start to block feed, you will probably need to do a little pumping on the side not nursed on. It’s important to go down very slowly so that you don’t get a clogged duct. There will be things that can put you back to the beginning and you’ll have to wean back down again SLOWLY. If you get a clogged duct, you’ll need to keep moving milk so it doesn’t turn to mastitis. It’s a bummer to have to start again but it’s necessary 🥲 You must pump if you’re uncomfortable. If you are engorged, pump for just a minute or two, then wait 5 to 10 minutes and see how you feel. That breast will probably soften up in the 5 to 10 minutes after pumping. If not, pump a little more. I know that engorgement seems like the breast hasn’t been “emptied” but it’s not really milk in the breast, it’s actually inflammation/swelling of the tissue around the milk ducts. Your body is wanting to make milk at that moment but if you start letting it know that you don’t need it, production will start to go down. Theres no way to know how long it will take to get supply under control. Every mom is very different. Some moms get it under control in days or weeks and some moms may never get it totally controlled. The further out you are postpartum helps. Supply regulates over time for most moms. Yes, less engorgement shows that it’s getting under control. Some moms try to pump a little bit before nursing to remove some fore milk but this is a BAD idea. Removing extra milk will just increase supply and make it worse but if you have some previous pumped milk (or milk that you needed to pump when you’re engorged), you can give a little bit of that at the beginning of a feeding. This works because it reduces supply a little faster. It reduces supply because of less milk removal and because there’s less breast stimulation. Some moms will add in formula or donor milk to reduce baby’s time at the breast to help control supply. I hope that helps. Let me know if I didn’t answer all of your questions or if you have more questions. Moms wish they had an over supply but they don’t understand that it can be harder than low supply. 🥲🥰

@@marycahillclcReally appreciate you taking the time to answer my questions. Baby is 2 weeks and 3 days old; first baby. I have started block feeding just two days ago. Regarding your advice on pumping out engorged breast, when is the best time to pump-is it as soon as i feel it being full or do i have to wait for its turn to be nursed on and pump just before or after the nursing session? How much is enough pumping-is it til engorgement is gone or just until it becomes less uncomfortable (but still engorged)? For me, engorgement seems to be only completely gone once baby nursed on the breast for a good 12-18 minutes. Does clogged milk duct feel painful or not necessarily? My right breast seems to react to block feeding quite well-feeling less engorged than before even when not nursed on. Usually survives the 6 hour period of not being nursed on without being extremely hard and painful. However, the left breast still gets engorged pretty quickly-within 2-3 hours from being nursed on. Also, i was unable to bear the engorgement yesterday so i pumped out for 5 mins and felt just a bit of a relief but it is still engorged. I seem to notice some slightly hard lumps on that breast too, but they aren’t really painful. Could those be clogged ducts?

@@joyanntan1289 Is this your first baby? 2 weeks is really soon to know if you truly have a FL/OS unless you’ve had this condition before. I wouldn’t be block feeding now unless you had this issue before. Most moms have a forceful letdown/over supply in the first month but not the actual condition FL/OS. It is important to pump as soon as you feel uncomfortable. In the beginning, moms with a FL/OS, need to pump longer. It’s important to slowly wean down. A nipple shield can help and sometimes might be necessary but it can also cause problems. Sometimes baby won’t want to latch without it in the future. @marycahillclc
@marycahillclc 5 месяцев назад
I just responded to your earlier question on the other video. I’m glad that you watched this one too❤️ Breastfed babies do eat a smaller amount but much more often. Breastmilk also digest faster than formula. Babies will often seem extra hungry and fussy in the afternoon and evening but this is usually because they are either, tired, overstimulated, have growing pains or painful gas. When Mom has a forceful letdown/over supply, it can cause lots of tummy issues. Baby may want to nurse even more because it’s comforting to suckle, eat and be held by mommy. Managing your condition can usually help a lot. When mom has a FL/OS, baby ends up getting a larger amount of foremilk. Foremilk has more lactose in it. That doesn’t mean the baby has a lactose sensitivity, but anyone that consumes a large amount of lactose will usually have an upset tummy. Foremilk also has slightly less fat in it. This means that baby will take in a larger volume of milk to get the fat content that they need. Do you pump breastmilk at times? If so, how much milk do you usually get? And is that amount in place of the feeding or between feedings?

Hi Mary thanks for this video. I am struggling with nursing my second child. She’s almost 4 weeks old. I think I have forceful letdown, but it’s something new that I haven’t experienced with my first child. I feel the letdown and then I see the baby chocking, becoming fussy, crying, unlatching. Baby doesn’t stay latched for more than 5 mins and she wants to be nursed very often. I tried to keep her more vertical or to lay down, but milk is really too much and it’s spraying even in those positions. I am not sure how to cope with this. Baby is growing well but I hate seeing her not satisfied and always nervous.
@marycahillclc
@marycahillclc 2 месяца назад
@@CarmelaGreco If you have this condition, you’ll have it with every baby. Some babies have very little problems and another can really struggle. This might be very normal behavior for this baby. Some newborns eat almost non stop. If she’s having at least 6 or more wet diapers every 24 hours and gaining well, she’s probably doing fine but if you’re concerned, talk with your doctor and a local lactation counselor. When mom has a FL/OS, baby can get all they need in a short amount of time (some babies of moms with a FL/OS, take a long time to eat because they are clamping down to slow the flow). She will probably get better at handling it as time goes on. Have you taken any of the steps to control it? Are you doing any extra pumping? How is engorgement? Does baby seem firmly latched? Is baby having green or foamy poopy diapers? Has baby been evaluated for a lip or tongue tie? Was your last baby diagnosed with reflux or dairy sensitivity?

Block feeding can be different for everyone. You might just nurse on one breast each feeding or make it an amount of time, like 3 hours or 4 hours.you’ll have to do what controls it best You don’t want the breast that didn’t get nursed on to become engorged. As you start to block feed, you may need to pump the other side just a little. A pump is better than hand expressing when a mom has a forceful letdown. @marycahillclc

No, that’s not going to be damaging because you know that you have this condition. The biggest problem is when mom believes she has the condition but really doesn’t. Even then, it can be increased if needed. How is baby and you doing with the block feeding? Are you avoiding extra pumping when possible?

Baby is almost 6 weeks and I’ve been dealing with all of this. - Block feeding leaves me engorged and having a stronger FL. I’m afraid of clogged ducts or worse, should I pump to get the remaining milk out after feeding? - If I pump before a feeding, I can get an ounce or so out in just 3 mins, should I dump it, since the baby already has too much for milk intake?
@marycahillclc
@marycahillclc 7 месяцев назад
You have to go very slow when block feeding. You will need to pump on the side not nursed on but just a short pumping time. Then apply cold compresses. After you pump for a few minutes, apply the cold compresses. If you don’t feel relief at that point, you can pump a small amount more to get comfortable. Over time, that extra pumping will be less and less. It’s important to not stay overly engorged or you risk getting a clogged duct. Mom‘s with this condition are at higher risk for clogged ducts so go slow on reducing your pumping time. Breast have very little milk actually stored in them. Milk is made while you pump or nurse. The full feeling that you have is engorgement. If you are pumping before baby nurses to slow down the flow or to give baby more hind milk, that will work for a few days at most. Then your supply and fore milk will increase even more. Your supply will lessen a bit in a few weeks. For most moms their supply regulates down at about 6 to 8 weeks but for a mom with a forceful letdown/over supply, it’s usually at 3 to 4 months. You’ll still have the condition at that point, but it does get a little better. When you do need to pump, it is a good idea to store that milk incase you ever have an emergency need for it. if it doesn’t get used, you can always put it in milk baths. Is baby having lots of symptoms?

Hi Mary, thanks for the video, it answers a lot of questions. I know that I’ve been having oversupply, I can totally see the letdown a lot, specially when baby is feeding on one side.. I believe the oversupply is because I used to pump a lot as well as nurse because I was under the impression of emptying breasts.. I’ve considerably reduced the amount of pumping.. I have been getting engorged breasts more frequently now, my baby is 7 weeks.. Should I pump when I have engorged breasts or right after I nurse? Also at night time, the feeding gap is more than the daytime so engorgement is more during that time.. in that case do you recommend pumping only for couple minutes or more?
@Ntandon18
@Ntandon18 9 месяцев назад
Also this oversupply has kind of led the baby to nursing strike, and I’ve been trying to get her through this by nursing when she’s just woken up or using nipple shield and removing the shield sometimes in between and let her directly latch once she’s in between the feedings. But mostly she would feed on just one side and that too for a few minutes like 5-8 minutes, so the breast is still not empty.. I’ve read a few articles online that pumping engorged breasts make things worse. Is that true and what are your recommendations?
@marycahillclc
@marycahillclc 9 месяцев назад
@@Ntandon18 You can cause engorgement by over pumping but forceful letdown/over supply that last past 6 or 8 weeks, isn’t caused by you. It’s a condition that you’ll always need to work on reducing. Also, you’ll have it with every baby but some babies handle it better. I would just pump when you’re starting to feel engorged instead of just adding pumping after a feed. Pump as little as possible but keep as comfortable as possible. Apply lots of cold compresses. Those long stretches can be hard. You can apply cold cabbage leaves in your bra for 15 to 20 minutes before bed. Taking Ibuprofen can help too. ❤️ She is probably getting all that she needs in those 5 to 8 minute feedings. Just be sure that she’s having plenty of wet diapers. You’re doing great to nurse when she’s sleepy and removing the shield when she gets feeding. Also, try to not force her to nurse. If she fusses, apply the shield or bottle feed then keep slipping back to the breast. Spend lots of time skin to skin helps. Have skin to skin time be just relaxing with her. ❤️

Hi Mary, thanks for your helpful information. I was confused if I have a forceful letdown since my baby pull off the breast, bite & pull back my nipple and crying sometimes while breastfeeding, she also never say no whenever I offer her to breastfeed which makes me also confused is she hungry or just using it as a pacifier. Since she turned 3 months old I realized that her diapers has no more white yarn & balls as it used to be the first 3 months and now she only has 1-3 dirty diapers and sometimes not at all. She's now 4 months old and she only gained 0.5 kg in the last month, Dr told me to help her with formula but she refused to take the bottle. I also tried pumping both sides after 2 hours of feeding her and I got 80 ml but she also refused to take it. For the entire 4 months I've been feeding her from one side each time and whenever she wants to but now I'm trying to feed her both sides every 3 hours, hopefully this will work.

Did you watch my videos on how to determine if you have a forceful letdown? That will help. I’ll link it here for you. A forceful letdown often looks like a low supply because of how baby acts. At 4 months, baby’s weight gain slows down. A 4 to 6 month old should gain 92 to 126 grams per week. It’s very normal for an exclusively breastfed baby to start to skip days pooping. @@marycahillclc I got almost the same concern as the mum writing above. My little girl is almost 5 months. She is a fast eater from the first day. She gained 1kg each month for the first 3 months. Then she has slowed down, last 7 weeks she gained only 1kg but her length got 6cm more. She has normal wet diapers. Can you tell me how a fussy baby is like? Can my baby eat too little? Does the hot weather make them lose their appetite? Today I decided to use the pump, and I got only 100ml both side after 3 hours! It freaked me out because 3 days ago she skipped her breakfast and I pumped out 180ml from 1 side (very engorged breast)! Also when using the pump, is it normal that I pump only for 5-7mins? Because my feeding section sometimes lasts for 5 mins, normally it’s 8-10mins, it is very rarely longer than 15 mins, she eats only from 1 side, and after she finishes, I can always squeeze out some more milk, even spraying out. Sorry for a lot of questions. I really hope to hear from you. Big thanks 😘

@@gjngin hi mommy! I love all of the questions 💕. Babies slow way down on weight gain as they get older. Birth to 4 months baby should gain 155 - 241 grams a week 4 to 6 months 92 - 126 grams a week 6 to 12 months 50 - 80 grams a week so baby is gaining great!😊 Babies can possibly be under fed but if you are seeing 6 or more wet diapers every day then you know baby is getting enough. Pumping is not at all an indicator of how much milk you have. It’s common for moms to sometimes only see drops when they randomly pump at 5 months postpartum. If baby wiggles when nursing, that’s often because of tummy pain when they have gas. Is that what you were meaning about what fussy is like?

That’s awesome! Thanks for mentioning what worked best for you. I suggest that moms wait a bit for a couple of reasons. It can help with engagement in the first weeks. For some moms (especially with first baby) waiting for a block feeding time can allow for engorgement, leading to clogged duct then possibly mastitis. Another reason is (especially for first time moms), it can seem like you have a forceful letdown/over supply but it’s just hormones causing this extra milk. We want supply established in the beginning. For moms who aren’t on their first baby, block feeding can work well because you already know that you have the condition. ❤️

You could start to head towards block feeding. If you’re getting engorged that often, you’ll want to go slow. Don’t let yourself get overly engorged. You are doing AMAZING! 4 months breastfeeding twins 😃 It can be hard to block feed twins. You would need to feed both on one breast, each feeding (one right after the other). You could just do that part of the day, to see how it goes. Maybe mid day. You would just need to keep it at about the same time every day. I know that it’s scary to cut down your supply but with a FL/OS, you’ll be able to increase it again in just a day. Not being engorged anymore is a good sign that your body is adjusting well. It’s ok that you don’t feel your letdown. Some moms do and others don’t. 👍 For baby A, has the choking gotten better? For baby B, How slow is weight gain? Engorgement is inflammation of the tissue around the milk ducts. Applying cold compress after feedings and between feedings will help. Many moms do well because it can normally be a little harder to keep up production for twins but oversupply moms with twins don’t have to worry about enough supply. Each baby handles oversupply differently. Most of my mom‘s with twins, tend to see a big difference from one baby to the other twin. One will tend to handle it better. If a mom has a large over supply, she sometimes needs to resort to pumping and then bottle feed babies in part or exclusively. When a baby’s mouth is on the breast it gives more stimulation. If they’ve been cluster feeding in the evenings, that would be increasing your supply a bit during that time of day. At around six weeks babies will usually have a big growth spurt and want to cluster feed. Not because they need or want any more to eat but because it hurts to grow so fast. Breast milk has natural painkillers in it plus, mommy‘s breasts is comforting. For the one that’s not wanting to latch, just be sure that you watch that she is always having six or more wet diapers every 24 hours. She may be taking in more milk in a shorter amount of time. Baby chokes n I cry seeing me getting unsuccessful everytime I feed. What I recall when my baby was placed reclined she used to feed well. I think even if it hearts my back. I will come back to the reclining baby. In side lying position as well baby chokes so that position also gets scary for me. God help me as I have been determined to breastfeed my bub atleast 6months. Breast are now soft n supple with multiple let down if at all baby suckles,most of the time she cries and I have to put her off breast. She is slow weight gainer still manages to have between 15-20, percentile weight gain. Now ab days I am making her sleep n dream feeding her. Thats the only time I see her peacefully suckling. But this is too painful to watch as she cries a lot before sleep. And I get exhausted in making her forcefully sleep. I need hope that I can feed her well. No feeding pillow can help in my case. After consulting my lactation consultant I bought my second feeding pillow as well and it was a fail too..

@@garimasat can you lay back on a stack of pillows to support your back? Or a have you tried a sitting up Koala hold? Are you pumping once a day for a bottle or to store? That’s excellent that your breasts are feeling soft now.
