It is description of decision making in treatment planning of grossly decayed or unfavorably fractured teeth. Role of ferrule and methods to increase it are explained here.
Always wanted to know about ferrule effect and how to increase ferrule height . But none of the videos on youtube had a clear explanation. Thank you for explaining it so simply making it easy to understand.
Because if margin of restoration is encroaching biological width, it will damage the tooth attachment to bone and pocket formation will happen. It will ultimately fail the treatment.🙏🙏
@@Endocrisps i really struggled so much to understand that ferrule is length rather than width. can you evaluate my understanding of these special cases. case 1 : if i have an upper central with 1 mm remaining tooth structure. orthodontic extrusion 1 mm is the only way to perserve the tooth right ? what if total remaining becomes 3 mm, do i still have to make post and core ? and it's 1:1 crown root ratio. case 2: if i have an upper central with 3 mm remaining tooth structure no need for post , only a core build up right ?
Look there are two things First is when to save when to extract. This decision depends on ferrule available. Ferrule is considered when dentinal walls of width 1 mm and length atleast 1.5- 2 mm all around present. For this do crown preparation then evaluate width of dentinal walls. Second is post is needed or not? This is a vast topic that is different for single and multirooted teeth. For Incisors, if more than 50% tooth structure is lost, there is need of post👍
this makes it clear a. if an incisor remaining 2mm ferrule after prep --> restorable b. then 2mm is less than 50% of average crown length --> need post and core. if the ferrule was 3 mm --> just core build up Thank you so much , you don't how much this helped me.
2 ways Either put wire in canal with zinc phosphate cement and make a hook or put orthodontic button on related tooth. Then put braces on adjacent teeth. Then place wire or elastics to extrude.🙏🙏
@@Endocrisps thanku sir for ur reply.even i prefer cast post over the fiber post but i hav seen few clinician who prefer fiber post in such cases according to them cast post require more tooth destruction and ferrule is difficult to achive.thats y i was in doubt
Because main aim of post is to retain the core and in cast post, post and core are single unit. So cast post serves the best function of a post. But it has some drawbacks like its very rigid and lead to tooth fracture, another is it took multiple appointments also.🙏🙏
