As we age, our bones become thinner and weaken. For older people this can lead to life-altering fractures. Our bones are growing, living tissue made up of calcium-based minerals, collagen and other proteins. Understanding your bone health - how well the minerals and proteins are working together to resist fracture - can help you and your doctor determine the best course of treatment. Paul Hansma, Ph.D., inventor and professor emeritus of physics at UC Santa Barbara, has been studying the protein 'glue' within our bones and how to measure it. In this program, Hansma discusses a new assessment called Bone Score, which, along with other diagnostic tests like measuring bone density, can give a more complete picture of overall bone health. The Bone Score assessment medical device was recently approved by the FDA. Hansma is a founder and investor in Active Life Scientific, a UC startup company that manufactures the equipment for Bone Score tests and runs the bonescore.com website. I am an 81 year old male who has been on Prolia for 5 years and I am concerned that when oesteoclasts are suppressed by the action of Prolia, it fakes out the DEXA results. This is because, without oesteoclasts doing the maintenance work of removing old bone cells, the old bone cells are left behind and are measured by the next DEXA as an improvement to bone density. The DEXA, as I understand it, is simply measuring X-ray opacity. Old bone cells and healthy bone cells all look alike to the DEXA. If this is the case, DEXA seems like a meaningless test for judging the strength (quality) of the bones and, thereby, the fracture risk. If my understanding of the situation is sound, it means that your bone strength test is extremely valuable and should be immediately adopted by the physicians in this field of medicine.

In my 70s. Just had a bone density test. Result. Statistically increased hip density 9.6, Statically increased spine bone density 6.4 All I do last 2 years, yoga, pilate, home made keffir, DAILY home made chicken broth plus magnesium, boron, manganese. No calcium suplements! By the way 10 years ago dr.topd me I will need a hip replacement. That's forced me to collect informations and implementation of steps preventing future deterioration of my bone density.

@BobYou may be right. Dr. Adolfo Diez Perez told me that he thought it was not necessary to take calcium supplements. He thought that it was best to get calcium from food.

Hi there and thanks for sharing! I am a 40 years old male and diagnosed with osteoporosis at 37! Doctors cant find the reason. All my tests are perfectly fine (vitamin D,calcium etc) but I have osteoporosis!! I dont want to go on drugs. From all th research I have done I came really close to the good and simple adivce you give! Is your diet like low carb or nothing to do with this? I would be really thankfull for any advice you can share with me! All the best!

@@blastfilmmaking3658 Wow, not sure if like many issues tests are better or rates are incr and younger? Maybe both. I did learn at times we won’t and don’t need to know why only the keys to get better. Educate yourself and trust your instincts. We need to help MDs help us. Find ones you trust.

@Bob Interesting if you look at OP rates per country and even those with traditionally higher rates of bread, pasta and oils (Italy, France, UK) have lower rates then US and Japan. Not sure if science knows enough yet to identify factors in causality or treatment with high accuracy. We’ll see what happens in 5-10-20 yrs. Good science is always re/testing theories and outcomes based on new data and understanding. Looking fwd to more clarity.

Sadly I saw my mother go downhill after 3 hip operations. The final one left her with a shorter leg which at her age it was too complicated to correct. It left her disabled and caused her back and knee issues. Her quality of life deteriorated and she became scared of venturing outside with her disability. This in turn caused her to lose a lot of muscle and go to skin and bones. I honestly have no faith in the medical profession and don't believe in Pharma drugs which to my mind are lethal! Chemicals do not sit comfortably in the human body and people develop all sorts from ingesting them! I practice what I believe by using natural remedies of which there is an abundance and proven over the centuries. I also make sure I take bone strengthening exercises with a rebounder which interestingly helps with the heart, the lymphatic drainage, lubricates the joints so not harsh when hitting the mat, and so much more! The Pharma industry is all about profits and not curing anyone! Many people know this and I'm disgusted with the doctors who just peddle drugs because they are on the Pharma payroll! I certainly wouldn't trust any of the recommendations the FDA puts forward as they are controlled by Big Pharma so they have conflicts of interest. Finally, I came across something really concerning over a week ago. Someone mentioned when they went to their dentist he would not do any work on them after they told him they took a particular osteoporosis drug because it was known to crumble the jaw bone!

I noticed those symptoms also, My solution was is Exercise before the muscles start to fall asleep i noticed the results Walkn Squats, Planks, Stretching ect.Sorry to hear about your mother's situation, I remember my Aunt at 82 dealing with it also she fell several time's?

I also witnessed some of the disability and decline in quality of life of, especially, elderly people after hip operations. Operations as a result of hip fracture are particularly problematic. I really hope that the bonescore test becomes widely available to help with these problems.

Animals in the wild have the strangest bone density, the same as neanderthal human with the remaining of their teeth and bone reveals. There are three main branches globally; the westerner with extra fat deposited, the tall black African with very healthy teeth and bone, and the lean east Asians. Medicine has no miracle when it comes to western diet and life style. Daily lemon juice will wash away and cleans the breakable bones and recycle the old bone materials. Nothing works like it. Also garlic heals the broken bone faster than any drug.

I was diagnosed with osteoporosis at the age of 47! I was going to be given an IV of “reclast,” (I believe) and declined, now at age, 57, I’m a -4.7( we’ll that was a year ago) I won’t make it to 60!!!!! So scary😞

The bone need both Vitamin D and Vitamin K2 for the bone to absorb the Calcium.. there are three types of collagen... Type 2 collagen supports joints and is used to address joint inflammation/arthritis. Type 1,3 collagen supports hair, skin, and nail .....

I live in SB! Thank you soooo much Dr. Hansma!! Trying to avoid drugs like Prolia, etc! My Dr. Is working with me( Thank God!) I do go to Samsun ( another Thank God!) and feel even more hope and encouragement from your video! @@mocoolboy8717 I’m very proactive in my medical stuff so I’m doing dietary stuff that I’ve researched, and am checking out a new thing called ‘bone strength’ testing… they only do it here in Santa Barbara … I’m researching it… I did quit all inflammatory food and I can tell you that the difference in my knee pain is unbelievable! I have none now, and I was considering surgery as my pain was brutal… so I’m walking and doing weight bearing stuff, and researching all I can on this osteo stuff… I’m not gonna do regular meds like prolia, etc…♥️☮️🙋‍♀️‼️

@@blastfilmmaking3658 hi dear! Both actually… I was shocked at some of the foods that are inflammatory! I’ve TOTALLY stopped sugar… ( the worst one) and just started eating clean… grass fed, low carb( clean carbs) , and only dairy I do is grass fed butter, also do organic kefir and cottage cheese… I also avoid stuff that has oxalates… I eat so clean I squeak!!!♥️🕉🙋‍♀️‼️😂🤣☮️

I heard back from Dr. Adolfo Diez Perez: "When you refer to the study of the effect of different drugs on BMSi you say that the patients were organ transplant recipients and this is not the case. All of them were starting glucocorticoids for different reasons but none had been transplanted. The case is the same but someone listening might go the paper and capture the difference." As he says the case is the same: patients starting glucocorticoids were divided into four groups that received different treatments. Changes in BMSi (now called bonescore) happened in just 7 weeks!

DEXA scan has been in use for the last 30 years. Just ask your doctor and he can set you up for the DEXA scan

Are you talking about Trabecular Bone Score via Echo machine? Not sure where you go to get "bone score" ? I don't see it being discussed in osteo groups.

@11:54 advised and read largest bone loss in first 3-6 mths but obv much earlier. Advice in Cda was/is 1500 mg Ca and 1K iu D. I researched and asked many questions but too late. Finding my way w/my med team. Dr Hansma pls add factor of longterm PPIs to OP risk. Often given w/GC steroids. Very hard to stop and some studies show not needed w/GCS. Thank you.

hi thanks for sharing ,I have bone metastasis because im a breast cancer patient ,i was advice to take ZOLEDRONIC but till now i have'nt start yet because as per my oncology i need to ask clearance from dentist ,but he told me that i there are many things to do ,restoration , tooth extraction,root canal etc which will cost 40k reason till now i have not yet start the zoledronic, is there a possibilty that i my bones will break immediately? tHANKS

Only offered in California? If it so easy to do, why isn't it more widely available? Some are having Trabecular testing with Echo in NC. If we know bone density has alot of variation from place to place and technician to techician and it's not an accurate measure of strength, why the heck are we using it to prescribe medication?

What is known about bone strength/score in different sites in the body? According to your presentation and on the website, the bone score is assessed at patients’ shins. Is there any data on whether this bone score is the same as the one in the patients’ spinal vertebrae? Is there data on differences between cortical and trabecular bone? DXA scores can differ significantly when taken at the forearm and lumbar spine, for example.

bonescore is only clinically measured on the tibia. Fortunately this measurement on the tibia correlates with fracture rates more generally. For example, in the first clinical trial, the measurements on the tibia were lower for patients who had had a hip fracture on the other leg. Low bone score is also correlated with increased rates of vertebral fracture. There is current, ongoing research on cadavers to answer your questions, but clinically, measurements will probably only be done on the tibia because there is a knowledge base there and the site is convenient.

Actually bone loss and bone strength are independent. So increasing bone strength does not necessarily reverse bone loss. However, since some medications that reverse bone loss have been shown to also increase bone strength, it is very possible that actions and medications that increase bone strength may also reverse bone loss.

There have been studies showing that collagen supplements help with bone mineral density. I am not aware of any studies yet about the effect of collagen supplements on bonescore.

I’ve taken it for several years and my osteoporosis is reversed. I walk four miles a day and do weight exercises, as well as taking calcium with vitamin K2. It worked for me.

Research boron 3mg and trace mineral research liquid (low sodium). P.s I just read boron and calcium increase estrogen. I'm going to research about magnesium that should balance it out, hopefully.

There are side effects to Fosamax, but it does help as Libby Collins wrote. A bonescore test can help you and your physician decide if the benefits outweigh the side effects.

I would like to try jumping on one leg to make the bones stronger. What did the people do in the study that showed jumping on one leg increased bone score? How long did they jump on one leg and how often? Thank you!

my Functional Doctor has me doing the "astronaut" double tap exercise. When astronauts come back from space they have lost a lot of bone density and this is one of the exercises they do for 6 months. I do it 3 times a day on concrete 10 repetitions. How to do this exercise: You stand on both feet, get on your toes and drop down on your heels and "thump thump" up. Maybe you can ask your doctor if it is good to do for you and get help doing it correctly. Doesn't hurt to ask, right? I can feel it in my spine and hips:) GOODLUCK

All I hear is ' medication is the solution'....see a functional medicine doctor or dietician, or search on the internet (FREE) for nutrition to improve bone health. Last time I looked, the natural world did not have pills growing on a bush! And chemical intake will frequently create an additional problem down the road. Respected medical studies are quoted by many doctors and hospitals such as Mayo Clinic. In your web search, type in "bone loss nutrition studies". Also, " bone loss prevention protein requirements studies". I just finished reading a NASA study proving correct nutrition and weight bearing exercise as preventing bone loss in space! As well, friends, our balance tends to wane as we age; please steady yourself with a hand on a wall, counter, or tall back chair, etc, in case you begin to fall. (jumping up and down on 2 feet seems inherently ⁶safer....). On the same path at 67, and respectfully submit these suggestions!

You might benefit from watching some videos on the RU-vid Chronic Pain Science Channel. The cause of ongoing chronic pain is often not what people expect. Many have overcome long term chronic pain using techniques specific to overcoming chronic pain.

It seems likely based on the study mentioned about 10 minutes into the video, but no study, to my knowledge, has yet been done on rebounding on a rebounder. Hopefully a study will be done. In the meantime you or other individuals can get a bonescore test, do the rebounding, and get retested to find out for yourself.

I submit that the only relevance for the individual is whether his bones easily break or not (fracture risk). Why is DEXA relevant to anything? Density could be an age-related anatomical change that is not related to bone strength. The only consideration for the individual fracture risk is bone strength possibly measured by bone quality. But you do not explain how your bone quality measurement is made. The only iron-clad bone quality measurement is by extracting a bone sample and measuring its strength - not attractive because it is invasive. The recent development of high-resolution peripheral quantitative computerized tomography (HRpQCT) imaging has overcome many of the above limitations. HRpQCT imaging allows for noninvasive bone biopsy at the distal radius and tibia, providing in vivo assessment of bone geometry, true 3-D (volumetric) BMD and microarchitecture at extremely high resolution (60 to 82 μm) which (supposedly) correlates with individual fracture risk. Another development is termed trabecular bone score (TBS). This method uses standard DXA spine images to measure texture inhomogeneity - that is, how well-structured or poorly structured the trabecular bone appears when assessed as individual voxels. The FRAX score now optionally includes TBS. You do not mention either of these new measurements that address more directly bone strength. Bone strength is the only relevant issue for assessing fracture risk. Fracture risk is the only relevant consideration for predicting an individual's potential for breaking a bone.

Forteo is very risky to the heart. Had to stop Forteo after heart stopped, passed out and ended up in the ER immediately after injection.

I am sorry you had that problem! One of the main reasons for the bonescore test is to be sure that people really need strong drugs like Forteo, with their side effects.

Jumping on one foot made that leg stronger, but not the other one. I would guess that a more general exercise program like recommended in the videos I show at 23:30 in this video would be a better choice.

Lijm Anderson had a great response to this question 6 days ago: my Functional Doctor has me doing the "astronaut" double tap exercise. When astronauts come back from space they have lost a lot of bone density and this is one of the exercises they do for 6 months. I do it 3 times a day on concrete 10 repetitions. How to do this exercise: You stand on both feet, get on your toes and drop down on your heels and "thump thump" up. Maybe you can ask your doctor if it is good to do for you and get help doing it correctly. Doesn't hurt to ask, right? I can feel it in my spine and hips:) GOODLUCK
