Rotator cuff tears can nearly always heal naturally without surgery. In this video, we walk you through at home treatment to fix your rotator cuff injury.
There are 3 phases that span several weeks depending on your needs. You might even be able to skip phase 1 or 2 if your shoulder pain is not too bad and you can tolerate the phase 3 exercises.
We often get asked if therapy bands (rubber bands) are needed and the answer is NO! In fact, using rubber bands for rotator cuff problems is really only appropriate at the very end if needed.
The exercises and advice in this video are based on the latest medical research, expert experience, and past success with real patients that have escaped having a rotator cuff surgery.
Disclaimer! This treatment should not cause more shoulder pain. In fact, you should feel shoulder pain relief and possibly mild muscle soreness. If you get consistent increased pain when doing any part of this treatment program, you should seek individualized medical help. There may be more than a rotator cuff injury in your case and you need a customized plan to heal properly.

Yesss. This is why I feel like I trust him more than anyone I’ve actually met In person over my pain. He acts like he actually listens to his patients and has the knowledge to back up how to help them. Not just training, but like he’s listening, which I think is more important than the training at times!

THIS GUY HEALED ME FREE ON YOU TUBE!! I had a bad rotator cuff tear and the doctor wanted to operate. I wanted to try non evasive therapy first however money is short and I can’t afford insurance. I found this guy and his exercises not only gave me immediate relief to get through my work day as a baggage handler but in a few short months I regained complete range of motion with little to no pain at all!

Did you happen to have shaking when you tried to use your arm? I have a partial tear but I have shaking with my fine motor skills, applying makeup, eating, putting earrings in, etc. I’m really hoping this can heal me with me having the shaking.

I received a work innjury 15 months ago cleaning outdoor windows for 6 hours, it wasn't in my job description but I didn't know that. I'm almost 67 and the physio treatment I wa s receiving hasn't fixed it, I've lost a lot of fitness and gained weight as my job was very physical, I have become a lot less active. My latest MRI conclusion says I have mild supraspinotis tendinosis, moderate subacromial bursitis and subacromial spur. I get pins and needles in my upper posterior shoulder, down my arm into my fingers. My shoulder is doing a lot of clicking and my physio had me doing all the exercises you said are no good, no wonder it hasn't healed! So I am now doing self managed physio from youtube, I have bought all the stuff. I never lost full movement but it causes a lot of pain and makes me feel generally unwell, often the day after a regular busy day at home just doing washing and cleaning the house. I now understand how pain is more than just the part thats hurt, it is a whole body malaise. Anyway I just want to be pain free again and do the recreational things I so enjoyed, like swimming in the sea. Do you think these exercises here are for me, thankyou if you have time to repsond. I really enjoyed your video on describing in depth that a rotator cuff injury is. I do not want to go down the surgery path.

Thank you, Dr. David! Your video on hip flexor was awesome and was a tremendous help for me! Now I have rotary cuff syndrome, a .4 tear I've been told (fraying). And THANK YOU for always explaining each problem so thoroughly!!

Thank-you so much for your terrific videos on diagnosing and treating RCT, and differences between impingement, and tendinitis. You are a very good teacher, and explain it all so well. I am 56, and have had this condition for about 10 weeks. I have been doing stage one exercises for about 3 weeks, but the R.O.M in the affected shoulder is greatly reduced. When lying on my back that arm barely moves past 90 degrees when I am gently rotating it. The R.O.M is half what it should be. How can I improve and move on to cure this? I have not worn a sling, as I have to work, and can manage to compensate during the day, so I don’t aggravate it, while still functioning. Thankyou in advance from 🇦🇺Melbourne, Australia.

I have to be honest, I was very sceptical in the beginning. Partly also because I was down psychologically as I could not understand why did this upper body injury happen to me. But then as I got into it, did the shrugs 🤯, got the pulley etc, I started to feel like I had my arm back again, I have been “staring” at you in the video and I cannot believe you’re sharing your advice for free. Once again, thank you very much kind Sir and I wish you nothing but blessings in your life🙏🏼😊

Thank you so much for this video. These are the only excersizes I have faith in after having physio that hasn’t worked for what I think is a rotator cuff tear (based on your video of how to tell if you have a rotator cuff tear) The physio I was given to do were the ones you said not to do, no wonder they didn’t help. Your initial excersize in phase 1 helped tremendously with the pain. I initially got the injury from repetitive strain in my work for 4 years and kept going for a year in agony then eventually got a steroid injection. My question is even though the injury is older and I’ve just recently had my 2nd steroid injection, will it benefit me to start from phase 1 and use a sling etc or am I too late to heal the tear?

My pain is not directly in the shoulder, but in the deltoid muscle and sometimes hurts all the way down my arm. I have had that same pain in my other arm, in my neck, and shoulder and did have 5 tears in that arm which required surgery. My left arm is the one that now has pain and am wondering just what that arm pain is telling me. No real motion restrictions, just pain mainly while in bed and sometimes during the day. Is this the kind of pain that comes from shoulder tendinitis or tear?

Is working great much thanks sir! Will continue and see how it goes. For exercise, what is okay to do in terms of gym, free weights, swimming, martial arts etc and at what phase would you recommend? As some people suggest still training weights but to be careful and change form to cater for the injury... any feedback welcome!

i don’t understand when to move from phase 1 to 2. do you have to regain full range of motion like you do if you did the phase 1 exercise with your other side/good side shoulder or just if it feels easy?

Hi Dr! Does this treatment still apply and can I follow the same methods if I have suffered an acute rotator cuff injury from falling off my bike? I've watched all your other videos and it identifies as a rotator cuff tear, however I am already able to do all of the exercises reasonably well (I suspect just a small tear). I also do a lot of calisthenics so already have a lot of strength in this area and surrounding muscle groups. Should I still follow this routine?

QUESTION:Will this help if you've had the tear for years, or is it too late for the tendons to heal. My friend is an active 84 yo who tore her rotator cuff abt 3-4 yrs ago. I'd like to recommend this video to her, but not if it doesn't apply. She does yoga & tai chi & your video makes me think some of the movements may be actually be making her shoulder worse...Thanks very much.

Hi Bellylaughter, Our hearts go out to your friend dealing with a rotator cuff tear. While we can't provide a definitive assessment without proper evaluation, if she retains at least 10% range of motion, there's hope for healing with consistent effort. We offer a Rotator Cuff Tear Recovery Program tailored to support her journey. This program mirrors our in-person treatments, minus manual manipulation, and is designed to be progressive, meeting her at her current stage of recovery. We currently have a sale on all of our programs. She can use MOM25 upon checkout to receive 25% off. The Rotator Cuff Tear Recovery Program is a comprehensive program designed to get your shoulder pain under control to allow the rotator cuff tendons to heal. Then it guides you to correct the underlying root muscle imbalances so that you can fix the source of the problem (rather than just treat the symptoms) and get long-term relief from shoulder pain. This program works even if you've had rotator cuff surgery in the past but continue to have rotator cuff problems. To learn more please click this link: www.epmanualphysicaltherapy.com/rotator-cuff-tear-recovery-program/ We also have a rotator cuff help playlist that she can try out. She can access it here: "Rotator Cuff Tear Help" ru-vid.com/group/PLJVIPiFvv1mbQ_-1YFfEePnrcxfwwf9_k We hope this info was helpful. We hope your friend can feel some relief soon. Thank you so much for sharing our information and being part of our community! Please take care. 🙌

Thank you for these videos!!! 😃👍 I fell November 20, 2023. My doctor said it wasn't the rotator and sent me to 2 mos therapy. Didn't help. Did the MRI. My doc now says I need surgery saying it is the torn tendon underneath (tripped over hubbies weights in the gym while videoing my daughter ... fell on my elbow and tore my mis-named rotator). Dr., should I not feel ANY discomfort in my shoulder when I lift the weight over my head (phase 3)? Should I feel NOTHING like the other shoulder, or just discomfort or minimal pain? I feel it (light discomfort/the detached feeling). Should I just do the pully and wall shrugs until I feel ZERO discomfort with weights, or should I do the weights if I can? (Side note: I can do 15lbs pretty comfortably with bicep curls, hammers, and 10lbs for kick backs). THANK YOU FOR YOUR TIME!!💞💃

Is there an alternative technique for that first shoulder rotation exercise? It is not reasonable for me to lay down in my cubicle every hour. Nor could I could fit in there layed out anyway. Is there a way I could get the same effect sitting at my desk?

I think I ruined mine because I put KT tape on it and it made it stop hurting but now I can no longer lift the arm past my shoulder. I cannot reach behind my back. I cannot remove my shirt without assistance and I cannot raise the arm high enough to put my hair in a ponytail. I think the KT tape restricted the arm to remove the pain but it may have semi-healed incorrectly :(

Thank you Dr. David!! You are really great. I have 2 questions I really hope you can answer: 1) is what you taught in this video appropriate for a 62 year old female?, and 2) where does internal rotation and adduction, reaching the hand behind the back fit in with this? My wife's worst pain is reaching bahind the back, and her ROM is worsening over the past few months). Should this motion be worked on, or avoided/limited, and will these exercises improve that motion as well? ps - I am a PT in Maryland, but specialize in geriatrics. I wish you were local 😊

I am so glad I found your videos. Very helpful. I injured my shoulder about six months and just recently "finally" had a MRI that showed I have a complete rotator cuff tear. I am frustrated because my doctor sent me to therapy first that had me do exercise that made things worse and I finally realized I needed to stop that. If the tear is complete, do these exercises work as well and help to prevent surgery? Also, will Rock Tape help the pain and healing process?

Hey brotha, thx for your videos buddy. I fell into a hole the snow and it was three feet deep, I had stepped backwards and fell to the side, but when I posted up to keep from hitting the ground and breaking a leg, my palm hit the ground and I heard a rip like the tearing of jeans and hit the ground holding my shoulder that hurt right away. Well I dig your videos and I get the recovery info and excesizes. Please elaborate for the guys that just hurt themselves. 18:38 in the video.. How long should we wait and let chill? I been down two days and I tied to do the first excersize last night laying on your side and I couldn't get past 90° and up at all. Pain was excruciating and mobility zero. So to the question.. How long to let a fresh injury chill before we begin the part one excersizes? Thanks Dan in Colorado

I will incorporate the exercises from your video into my regimen. My main concern is doing something that will make the tear worse.

i read somewhere not to use a sling cause its more likely to cause frozen shoulder? I bought a shoulder compression sling. It goes over the shoulder and around the chest and keeps compression on it. it said on the package for rotator cuff injury. Should i not use it and use a sling? i seemed to have more pain in the sling due to the position of my arm. ??

Hello Alexander, great question! The sling may provide benefit, but if it does not seem to help, we would recommend hanging from a pull-up bar or something similarly sturdy that can be held on to and allow hanging. This can be done for as long as you can hold it multiple times throughout the day.
