I always prefer the lateral suprapatellar approach to inserting of knee joint. Because most of gonartrhrosis patients have inflammatory fluid in knee joints. We should take this fluid out from their knee joint before any terapotic injection. This aspiration technique is more easy and comfortable at suprapatellar area instead of position where your choice. The resistance is from the needle, its small for that liquid and you dont have also feeling if you are in the joint or not. I use always bigger needle.
We also do this injection along with PRP in an outpatient setting. We inject HA first then followed by PRP. A lot of people in the clinic seem to like it and they tend to ask for it even in advanced stage knee degenerative OA due to lack of financial capabilities for replacement and/or fear of operations.
very nice sir...i routinely give hyaluronate injections to my patients irrespective of osteoarthritis stage...but to my surprise all my patients will become painfree.... my doubt is how long does this injection work??? plz tell me sir so that i can give assurance to them.

Got the HA injection today, (6-28-2024) Three cc lidocaine and 6cc synvisc one. I forgot to ask the Dr, how long till I can play golf again? A week? Two weeks?
Sir if patient has fixed flexion deformity with grade 4 knee OA , wats ur take on approach sir . Can we give this try or replacement is the only option?
Patient suitability is based on the medical history, pain impact on ADL, stage of degen changes and mainly patient willingness. Usually CSI (steroid with or without LA) shows quick response but may last up to 12 weeks, again depending on the maintenance and activity levels. HA shows late response with long lasting effect up to 6 months as per the evidence. As far as the adverse effects are concerned, CSI holds the evidence of chondrotoxicity whereas HA is less likely, and there is no sound evidence to support PRP though.
