I like the subtle reference to re-training foreign physicians who come to the US. I believe there is movement to change the process and would enjoy hearing your thoughts.

Congratulations! I love your answers to the questions. As for retraining for immigrants, my Mum was a nurse in the UK and when she came here was told that she would have to go through training again. Instead, she chose to become a unit clerk in ICU/CVICU and happily did that all the time I was growing up and until she retired.

Congratulations on your well-earned accomplishment! I totally get you on starting over again. I myself am undertaking a fellowship in another country at the moment. Telling your younger self on starting over again is no big deal -- you're working towards entering a noble profession. But starting from scratch in a different country is less appealing because you know it's partly just for formalities and bureaucracies. You've proven yourself already with over 10+ years of training and passing certification exams. To start over again only because another country refuses to accept your training puts a disheartening, gargantuan obstacle ahead of you. Unfortunately, that's the reality in many countries and let's face it, we're older now and long night calls hit harder compared to when we were medical students or residents. I, too, just _might_ not start over again. Nevertheless, it is a time to celebrate! Congratulations again and enjoy your Europe trip! I wish you all the best in your practice.

Thanks for sharing Doc Schmidt - I feel exactly the same about 'would I do it again' (14 years into my medical journey - still a couple more years to go until I finish!!) - glad I did it once. But would not re-do it again from scratch. Best wishes for your new attending role.

Just wondering: Does anybody ever fail out of this 10 year process of becoming a doctor? Do they stop you early on if you're just not cut out for it, or are there different points along the way where some people stop or are stopped? I'd hate to go 9+ years and then fail.

Huge numbers, just the failure to match for residency is between 10-25 percent depending on source and different variables. The best success rate is via a US program and the MD instead of DO path. That first US/MD combo was measured in 2023 as 93,7 percent.

I have had experiences I'm glad I had, and some I'm glad I survived, that I don't think I could do again at my current age. But that doesn''t mean I wouldn't tell my younger self, yeah, do that, you'll be glad you did. So there's a huge difference between, "Was it worth it?" and, "Would you do it again?" Do it again at that same age and with the same external demands on my time and attention? Or, do it again at the age I am now and with the current set of external demands? Very different things.

Be careful about spending or borrowing against future income. Given it's a one off and your salary is guaranteed to take a huge hike, this won't be a problem. But for some it can become an issue.

While you are a doctor in training in the US, do you get paid for those hours worked over 40 hours? If so, do you get paid overtime rates, e.g 1.5x or 2x?

I absolutely love hearing what the 'medical' staff has to say*. Don't take it personally. But with my personal medical history and study along with certain family members who know ALL about how our system works.......it's all a $$$. Not to actually HELP fellow man. My grandmother was a nurse from way back when who left due to knowing what really took place behind closed doors and helped others at her own free will. Other members worked in the health department to government. I have been screwed do to our peoples Ego vs's actually love for others and serious medical help. I have been many peoples paychecks for decades do to my Epilepsy yet I myself always struggled on keeping them under control and abuse by our system along with others. I always cared about People vs's that money concept as that is rather similar to a religion in a way*.

Rookie numbers. I think the avg debt is like $250,000 for med school graduates. And trust me, there are people with $500,000 plus is debt after med school. It's crazy.
