One important thing I forgot to mention was that low back pain that only occurs with lumbar EXTENSION can be a fairly reliable sign of foraminal stenosis. Lumbar foraminal stenosis is different from cervical foraminal stenosis seeing as it is almost exclusively caused by atropathy (arthritic filling of the nerve canal). Your treatment options limit severely when dealing with lumbar foraminal stenosis because the only thing that makes it better is lumbar flexion. Lumbar flexion tends to make all other low back problems worse, which is why I generally recommend foraminotomy to these patients, as it puts you in a position where you're damned if you do and damned if you don't.
