It sounds so good but a little scary. What are the infusion sets used with this pump? What about occlusions? How does it compensate for activity…based on CGM which is interstitial fluid trends not venous blood sugar levels? Can you still have a hypoglyemic rxn? Does it partner with Dexcom?

Good point, doesnt matter how good the tech is if your insurance company isnt going to cover it. I hope this someday changes but unfortunately its the sad reality for most people nowadays.

This is interesting. It's using statistics of previous insulin delivery to estimate a bolus and then it will increase/decrease the trickling supply of insulin based on the CGM readings it receives post delivery. If it were that simple, why wouldn't Loop or AndroidAPS be doing something like this? Is the risk of hyperglycemia too large and other programmers did not take this approach? This is just another algorithm, so I'm curious as to why such a simple approach would not have been taken by established algorithms. That being said, the insulin/glucagon system sounds perfect... it can self-correct. Looking forward to that.

I agree, the mechanics are the same, it’s just a change in the algorithm. No idea how they are able to achieve such impressive results with just software changes. We’ll see if these results hold up once it’s released to the general population.

It does sound amazing. I'm on 3 years with T-1, the last year I've been using the OP5 and is so much easier and less stressful. I was wondering if Insulet ever looked into adding a Glucagon vial in the pod to act like this. I think one big thing over the algorithm and other pumps is insulin choice and how sensitive people are to it. I tried a few, ended up needing a rapid-acting to keep from going low way later after using just a normal fast-actin insulin. For this Bionic one I think it would almost require a rapid insulin to work well.

Since I had my pancreas removed in 2013. Islet Cells transplanted into my liver worked for a while. But now I am on insulin and my highs and lows are insane. This is what I need!!

@kimellah589 Hello! I was thinking about the islet transplant as well. I even spoke with someone about the pouch system they were thinking about. Is there any way we could exchange information. I'd love to ask your experience with the transplant before I take on something like this. Glad I saw your comment.

My son was diagnosed this year February 26, 2023 A1C was 9.1 and today is a 5.2. This sounds cool but also scary at the same time. His weight changes and he's only 10. At least it's something to think about options Tslim and Omnipod and this.

You do not need to change the weight unless you lose 15% of the weight you entered when first setting up. It will ask you if you've gained or lost weight I believe every 90 days. I just got my iLet set up and went through the training. As he said the iLet learns your bodies patterns with the help of the dexcom & just daily routine. It will also stop giving insulin automatically when you get to 100 and I think you can set it up to whatever number you'd like (don't quote me on the whatever number part) it also will give you lots of alerts when your starting to go low , very low & extremely low so you have to to treat the low before it gets very low.

I have had diabetes for 50 years in other words, all my life and I’m still am excepting of it. My hba1c is 7.2. I want one of these pumps where do I get it? When does it come out? I don’t currently wear a pump I control with pens. And I am in Australia

Just got FDA approved here in the US very recently. I’m hoping it will also get approval in other countries in the coming months. So hopefully soon as many people could definitely benefit from it.

@@diabetesdailydigest in the words of Veruca out of Charlie and the chocolate factory” I want it now daddy I want to Golden Goose” I’m telling you what it would be as good for me. I am over diabetes. I also have a spinal infection and recently lost my mother so this would be a godsend. I have had this spinal infection for three years. Most people only have discitis for a year to a year & a half. Osteomyelitis discourses isn’t very pleasant. I haven’t been able to walk until recently and yet only can walk with a walker. Damn diabetes, I curse it. As I said before, I am well-controlled and have been praying for a cure. This sounds like the next best thing.

I also keep my A1C in the low 5,s and control of my settings. It works better for me, and my eyes.

Thanks for the video, as a type 1 as well this looks so impressive and I couldn't agree more that if there was no change in results that would still be impressive. Adding carbs corrections ratios all this info and bells and whistles can be nice but a product that acts as a pancreas and takes little effort from the user sounds fantastic. Hopefully when this comes out I can get one because it sounds absolutely incredible.

That’s a good question. Sadly here in the US our health benefits are often tied to our employment. Many of my patients who either don’t have insurance or can’t afford their meds, we process the medications through patient assistance programs. It’s a process but most of the time we can get coverage for our patients, certainly isn’t an ideal situation and I hope it gets better here one day to get everyone the meds they need.

Wow - thanks for sharing, but . . . "Bigger/Smaller meal than yesterday?" - sounds such an easy decision to input 😛 But what about random carbs - like if you're on a crazy day with variation in exercise, snacking to get thru, so not taking 3 regular meals? I'm in UK, using G6 + manual pump. Perfect to manage this kind of abnormal day 👍 I'm about to switch to Ypsomed myLife Loop - which appears to leave me with enough control for these silly days (but only time will tell 🙏). I love the concept of this tool with Insulin & Glycogen (& would jump at the chance to help 'em develop it) - but would not be keen to rely on it (yet) for my crazy days 🤔

I have had the pump for three weeks now and the only problem I had was in the evening I would go low how ever, I’m not the typical type 2 diabetic so what I did -after consulting with my doctor was deceased my weight by 5 pounds and the pump works flawlessly.I highly recommend recommend this pump

I got to set in on this Ilet insulin pump research. I wore it for one year and 8 months. You know this being research they didn't just hook up the pumps and that was it. I was still required to do daily finger sticks to ensure the pump matched what the sensor said and to check to see if the insulin pump calculated the bolus correctly. We were never just turned lose at any time and not go unchecked. The pump did learn to raise or lower the dose. for example, if you went for a walk every day at 2:30 pm it learns that and will dose accordantly. For example, if your BS was 100 and that was your target number it will not give insulin, if it is say 200 it will give insulin knowing what it will take to bring your BS down to 100 I guess it learns to give a slower dose. They didn't go in great detail or the high-tech stuff about it with us. The pump will be a game-changer in the future. It isn't there Yet. I will say this company and this pump will be the ones that will take diabetic treatment to new levels. They need to fine toon the pump and it will be fully automated to the pump that all we will need to do is refill the pump every couple of days and that is it. ILet has almost put us there and I believe will in couple short years.

As others have said, this is both exciting and scary. I imagine after so long manually entering data on my pump, to just hand everything over to this machine would be frightening for a while.

Super exciting and scary all at the same time. Being diabetic for so long and having our routines like we do it’s going to be a huge change not to mention the leap of faith on so many levels of things we are so used to programming into our pumps. We have waited for break throughs time and time again and we always hear those famous words….” A cure/breakthrough product is right around the corner “ and “x” years goes by with no progress, so this is so welcomed. Once it’s out most of the time insurance will fight the first year, which in a sense, is OK because I would like them to work all the bugs and glitches out anyway. So excited and can’t wait to see where this leads to.

I agree, in the beginning it will be very hard to let go and take that leap of faith to let the tech take over. I believe these improvements in technology will be what eventually will be the “cure” for us rather than the beta cell transplants and other treatments which have been promised for decades.

Do you know it there are target ranges? The current pumps are not designed for those of us eating very low carb. My target is 95-130 so I'm correcting and my basal is minimal at best. Thank you!

From the screenshot at 3:02, the 0.5% reduction in HbA1c was from a baseline of 7.6%, so the average A1c on the system was 7.1%

No that I know of. Its possible once the product actually comes out to market they may actually add some features like a target range. I do know they did mention a "usual, lower, or higher" target you can set for your glucose ranges but I believe this is for your post meal glucose levels.

Hi, these are great news. I am just wondering about the cost of a device of this nature - this should blow everything else out of the water in a blink if it works out alright. We should be able to at least see what the pump is doing, so that we can act accordingly and to be aware and at least predict - I'd do that in a 1st phase pump device, and then upgrade it when the data + results support a removal of reading of the details.

"... if it works out alright." That is the question, isn't it. I'm very skeptical of it being able to achieve a good A1c. Lower than 6. It may be OK at getting close to an A1c in the 7s. For those who don't care how well their diabetes is managed, it may be the solution. But for those who care and strive for the best possible diabetes management, no.

Sounds good but when you can't always trust the Dexcom G6 that would be an issue. I am on Tandem control IQ and I calibrate the sensor often. Nothing like it saying you are 230 but finger sicks say 73. This pump would not work for me with Dexcom so off often.

Sounds too good to be true. Also, I don't think there is any company that can promise a 0% malfunction rate, and iLet can't be an exception. A malfunction in this device (eg too much bolus injection) is too much of a risk to take. People that aim a max of 6 A1c, in my opinion, should stick with the old school.
