@@epmanualphysicaltherapy it’s been 11 months since my diagnosis and steroid shot. My dr said the shot should last anywhere from three months to nine months at the most. I’m the one thankful for you!

I don't have these issues, but sometimes it feels like some issues are starting to build in my knees and shoulders, as I approach the 50s. Plus, I did have a fall few years ago but I recovered. Watching your videos really rebuilds the confidence that sometimes starts to feel shaky with the approaching old age. Stay blessed.

I bought your knee arthrtis program and am in phase 2, at walking. I feel that it much ressembles the way I have taught myself to walk with a hard cover book on top of my head, half an hour at a time. First it was really difficult, the book kept dropping, but now it's essy. What's important, I don't get tired and my feet and legs don't get sore. All that your course has tauht me so far, is useful, some of it really eye-opening (and glute tightening).

Cartilage can heal and adapt I totally agree, in fact weightlifting has helped my joint pain, and there is research showing weightlifters have thicker cartilage. Maybe this is because of less damage due to having stronger and more supple muscles and joint ligaments, less inflammation, growth factors but either way it has helped get rid of my right knee and hip pain almost entirely. I do 5x5 weightlifting. Plant based diet. I also find glucosamine chondroitin msm has helped but takes a whole to get into the system so to speak.

Hi Chris! as you have conquered your knee pain I'm addressing you for my question. I was diagnosed with chondromalacia patella grade 3 two months ago, so I rested and took Collagen, Glucosamine MSM and Diacerein supplement. Now I developed strong grinding in my knee which wasn't there earlier. Is it because of the degenerative deterioration of my cartilage or is it possible because of a scar tissue! I mean it could be the result of healing process. can you suggest any thing!? I'm 32 years. Thanks in advance!

Have you worked with people that have had microfracture surgery? It doesn’t seem like an easy one to bounce back from but eventually it has worked really well for a friend of mine.

Hi, really enjoy and learn from your videos. However could I just clarify if I understand correctly. You need to take the pressure of the knee to get scar tissue to form. You also need to do some exercises to support the knee function and keep the fluid flowing thru the knee joints. Not quite clear how this is to be done concurrently.

I'm confused about the scar tissue being 70%-80% as strong as cartilage. I watched that part again, too. I had an open surgery for ligament reconstruction. It's that knee that's so stiff in the morning and hurts when lowering to sit and raising to stand. Is there an easier way to explain how the scar tissue can be good?

My right knee has torn meniscus and torn cartilage, I cant go up and down the stairs i cant put pressure on my knee and when i do put pressure on my knee it feels like my bones are scraping each other and it hurts a lot. Also when i bend and stretch my knee it cracks. Also my anterior cruciate ligament is torn, what would you suggest me to do?

A couple of months ago (not sure what caused it) I noticed my knee was hurting and it got worse. I was unable to bend my knee at all. Then me on my own doing stretches it got better but the pain was still there. One day I went to get into my Son's car and I heard a pop and the pain got worse, so to the Ortho I went. X-Ray showed nothing but an MRI did show a tear ligament. The Ortho did not give me the option for physical therapy. He wants to operate. Friends are telling me to go for a second opinion. My knee now does not hurt as much unless I over do it. These were his findings: Radial tear posterior horn of medial meniscus near the tibial attachment with intrasubstance degeneration in the remainder of the posterior horn. Full-thickness chondromalacia over the patellar apex and Mild chondromalacia in the medial and lateral compartment. Do I need surgery or can this fix with PT. I'm 61 and overweight but I'm working on loosing the weight. In the past 3 months I have lost 18 pounds by doing Low Carbs and Intermittent fasting. I will prefer not to have surgery. Can this be reverse.

I have had a knee replacement on right knee..left one has pieces of cartilage and I had a hard fall on it.. I do not consider another operation at 83 years...will try all your advice. I thank you. South Africa.

Hey Tina! Sorry to hear about your knee arthritis :-( It's questionable if your x-rays will show better arthritis in the future after doing these exercises. But what you can expect is to feel better in your knees despite your x-rays looking about the same. Everyone gets at least small signs of arthritis as they age. It starts as young as in the twenties. As long as the arthritis is not irritated, you can do just about everything without much problems. There may be crunchy sounds in the knees, but it shouldn't slow you down. I was painful my left knee and its almost 4 months now !! Because im working a cleaning lady in 1 company and almost standing and walking at the end of the day its swollen😂😂😂 3 doctors check my knee and til now its painful!! Hey Hilda! Sorry to hear about your knee pain :-( Your story is sadly all too common. Many doctors don't know how to really fix the problem, they just give people medications, injections and offer surgery. Do you think about 10mm defect in articular cartilage can heal with trainning and supplements like collagen ect? I have read that surgery isnt 100%sure that will fix the problem, im only 23 amd play football and wanna continue i have also taken prf injection which have helped a lot

Can articular cartilage fissures under knee cap heal? Had a fall on concrete on both knees almost 4 months ago and still have pain on walking. I'm in a part of NZ where it's really hard to find a GP who will help with injuries so I had to self refer to a specialist who ordered MRI for one knee. The other knee is less painful but unstable and clicks.

Do the articular cartilage can be heal with pt,i got focal detect 1.7cm x8mm,i can walk but pain abit with bending on land. Doctor telling me to dont play sport include too much lunges such as badminton. It been 2 year the pain come and go consistently but somehow im pain free when walking. Dr. I am scheduled knee surgery on my left knee but the right knee is also in pain. According to the orthopedic doctor I need to have my knee operated cause it’s a bone to bone. Actually the doctor inject my 2 knees but still in pain on the first of day of the injection.right now I have my physical Terapy for 6 weeks before my surgery in February.

Hello , thanks a lot for your time ,I am 37 years old according to my mri my both knees cartilages are almost gone and I have tear in both meniscus,do I any hope to regenerate my cartilages without surgery ,and suggestions will be appreciated .I have no problem walking,it hurts when I bend ,I hurts if I do squats even body weight,..thanks a lot

Hey Navdeep! Sorry to hear about your knee cartilage in both knees being almost gone!!! And a meniscus injury too :-( Regenerating the cartilage is questionable...but laying down great, supportive scar tissue that is ~70% as strong as the original tissue is highly possible. That is usually done without surgery doing the right exercises over time. Hi, at the end of your video you make two examples to explain when cartilage can't or struggles to heal, one of them being knee surgery. Does it apply to arthroscopy too? I mean not wide open knee surgery but holes to get to the knee. In my case the surgery was stupidly recommended to get rid of a cist at the meniscus and as a result my knee got worse as a result. I also had the surgery and the front of my kneecaps hurts like crazy. How long ago you had yours ? did your surgeon told you if you are going to need another surgery or this pain is permanent ?

I’ve watched several of your videos, lots of good information. But, I am still a bit confused. However, What is the problem with the knee when you bend the knee ever so slightly it feels like there is a slight popping feeling? Is it the meniscus, the cartilage, or something else? I can walk on it but I can’t bend it without pain.

Thanks for watching Patty! It can be many things causing popping. Without checking your knee in person and testing out each tissue individually, it’s hard to say exactly what could be causing the popping and the pain when bending it. Usually it’s a meniscus or cartilage problem that stops the knee from bending. It can cause popping too.

Is it possible for the body to lay down new cartilage or scar tissue, in areas of the knee joint where the cartilage has been completely eroded or torn away due to injury? If not, is there any point in trying to rehab the knee or is knee replacement the only option?

Hey bonchancedenoel, it is possible to lay down new scar tissue where there is injured cartilage. It's questionable on whether or not your knee joint will look better on an x-ray. What you should look for is how you feel once you've corrected the muscle imbalances. You absolutely should rehab your knee even if your cartilage is too far gone to heal. The reason is because if you do have a replacement surgery, then you would have fixed the underlying root problem - the muscle imbalance - so that you don't develop another problem after you have the knee replacement.

Good information, thank you. I have been very active in playing tennis, hiking, doing squats, etc several times a week. A couple of months ago I slipped on a wet road just by walking and immediately felt knee pain in both knees as I balanced not to fall. While I can walk and even run with moderate or no pain, any climbing of stairs, hiking, squats, etc gives me pain. I suspect I torn my cartlidge/meniscus. My question is: would knee support braces help, either in healing or stopping further damage when playing tennis (which is still can)? If yes, should I get those with hinges or just simple ones that wrap around?

Hey Dénes Zoltán! Sorry to hear about your fall and knee pain :-( Knee braces may or may not help. It depends on if you have a ligament problem in your knees because that's the main problem a knee brace with hinges is for. I was diagnosed with meniscus tear a bout a year ago. My calf muscle has enlarged and my leg is heavy and I can’t squat. What hope do I have? Is my calf muscle enlargement related to the tear? Thank you for your informative videos!

Sorry to hear about your meniscus tear and calf problem :-( Does the calf hurt? If you don't have pain, then it might be a muscle adaptation. If there there is pain, then it might be swelling that drained into your leg from the meniscus problem in the knee joint. The fact that you can't squat and the leg feels heavy is likely a sign that it is not a muscle adaptation and probably swelling from the knee joint. There's hope for you to heal if you can get the swelling under control. You likely cannot squat because the swelling doesn't let your knee bend comfortably. @@epmanualphysicaltherapy Thank you so very much for your reply, I appreciate it! My MRI showed degenerative tears in both menisci. My calf doesn’t hurt only my knee hurts on squatting. I think it’s adaptive swelling as you suggested. Is this enlargement permanent? I have already started the no resistance exercise and walking with the technique you taught in the video. I know prognosis is poor for degenerative meniscus tear( I am 50 years old). Thank you again for your help and the great videos. God bless you

64 yrs old, 6'2, 220lbs, very healthy. Recently had Hyaluronic acid injections for both knees. Working on gluts, losing weight. Also working on building calf muscles. Laying off hammy and quad per your suggestion. I plan of having gel injections again in 5 months. Will the gel injections assist with cartilage repair. btw - 1st gel injections gave some relief of pain but not what I had hoped. Suggestions? TIA

Hi Mark, it is common for the 1st shot to not feel the relief you were expecting. You really feel the benefit of the shot after you've had your second or 3rd shot. The shots can be very helpful if you are having mobility issues but it's more important to strengthen the glute area as it helps alleviate the pressure on the knee joint. It sounds like you are consistently working on it. Hi there I started watching your videos and find it very helpful . I had arthroscopic surgery done on my knee four years ago due to torn meniscus since than I have pain on and off and sometimes it’s very hard to be active as my knee locks ,pops and does not bend much. I love riding bike, swimming and walk but at the moment I can’t do anything just walking for 20 to 30 minutes slow walk which frustrates me as I use to be very active and I work in childcare. I Seen many physiotherapist, osteopath, chiropractor, physiologist, doctors and done every exercise given . I had cortisone done three weeks ago not much relief. I have been told by surgeon that I need knee replacement but he want to do another surgery to clean out as I have developed osteoarthritis to see if that improves before knee replacement. I would rather not have knee replacement as I am only 48 years old and single mum but looks like I have no choices please help me with some advice.

Isn't scar tissue inflammatory? And doesn't it constrict movement and even cause pain?

Hey Alan! Great questions :-) Scar tissue is a normal part of healing. It's what the body puts in place of dead or injured tissue. Generally, it's a good thing since it helps to repair damaged areas of the body. Scars on skin are a type of scar tissue that most people don't have constricted movement or pain with. Scar tissue on the inside of the body works the same normally. If the injury is really big and there needs to be a lot of scar tissue put in, then there's a chance of having problems with movement or pain afterward. We see this with people that suffer with massive skin burns. Their skin constricts as it heals. Inside the body, especially in knee cartilage, that amount of scar tissue is rarely developed. It does happen however when there is a chronic re-injury to the same spot because then you get scar tissue on scar tissue. That thicker tissue can compress surrounding tissue and cause pain and limited motion. For most knee cartilage injuries, it doesn't get that bad. And laying down scar tissue is VERY important to set you up to get back to normal again with activity.

Caught my foot under a low ottoman when I went to sit down a few years ago and tore my meniscus. Surgeon removed half of it. I would have to have a knee replacement He said. Now Bone on bone and Surgeon says Replacement. I have lymphedema In my legs So my concern is that with surgery that's going to make it worse. I am 78 years old and Making drinks of collagen, garlic, and ginger to "collagen load", losing weight, and Sitting on My massage table to swing my legs for the knee movement and recumbent bike. I know it is not an overnight process but am I kidding myself That I can make a difference and avoid knee replacement? Sometimes my left knee Seems to get out of place and then I can't put weight on it at all. If I sit down or Lift my knee up and swing my foot left and right I can seem to get it in a place where I'm okay to walk again though I am becoming more and more stiff legged. I do sleep at night on my back with my feet higher than my head supported by pillows. Any advice or encouragement would be appreciated. Love your videos.

I had a right knee arthroscopic procedure 6/15/2020 And my muscle in my knee hasn't builded back up. My kneecap hurts when I walk on cement. And my orthopedic surgeon clean out my knee l had little to no cartridge left. I'm left with a limp. I can't run are jump. I csnt Lea e the house without a know brace

I had the surgery 10 days ago on my left knee and I also have pain on my kneecap. Sometimes I hear cracking and pain radiates on both sides on the knee. Did your surgeon told you if you are going to need another surgery ? or is this permanent ?

Hey Melody! Great question!...scar tissue only develops where there's been an injury. It's not really possible to guide scar tissue into a specific place without there being an injury there. Surgeons have experimented with this using a "microfracture" procedure in surgery. They drill small holes and make a surgical "injury" so that the body can fill it in with scar tissue. Besides that, there's no supplements or medicines that generate scar tissue in a specific place within a joint. If you're looking to heal knee cartilage, then you've got to trust that your body will make the scar tissue right where it needs it. You've got to do the right things to give it a healing environment. Does that make sense?

Absolutely Karim Roig. Most ACL tears can heal without surgery. It depends mainly on the individual preference and if the ACL is at all attached. Sadly, most people come to us after they've had ACL surgery and are still in pain. 7:50 weigh the pros and cons - Wondering if when surgeons say you need a knee replacement ; Is heroin or some other pain drug an option ? I tore my meniscus in 2018 after walking on a bad ankle for years . Doctors recommended arthroscopic surgery to ( Remove the torn cartilage !?!?! ) . That made zero sense to me , because they even say that I’d have to get a knee replacement soon after !?!? . Anyway I stopped walking too much ; and Am doing ok . No Surgery , NO PILLS , no doctors - Also i had to get a new mattress - Don’t forget to always have the best mattress possible

Side story - when the pain started 2018 i went to 3 doctors and they all wanted me to take ridiculously large pills like 800mg Motrin , took x-rays , which I knew would be worthless from my experience with the ankle injury . REFUSED to let me get an MRI , and REFUSED to give me time off work !!! Absolutely Blanking Amazing .

Hi! I had had a surgery 7 years ago when I was 16 and I'm quite active. I workout regularly. But, I've been experiencing pain in my knee from time to time. Also, It aches when I'm putting all the body weight on it & bending it (while the other knee has no such problem & moves smoothly). In the surgery, the surgeon removed a small broken piece of cartilage between the joint, cut an another piece from side (where weight is not loaded) to graft there. Please suggest something. I'm young & I can do better to heal it in the long run. 🙏

Hey I had the same surgery 2 weeks back..if u don’t mind, just wanted to ask you on your rest period and how long has it took for full weight bearing on your knee and if u are able to get back to sports.. along with the cartilage damage i also had a partial acl tear for which drafting was done and then meniscus repair as well.. and also which area in the knee does it pain..

@@UdayBhargav1 hey. The doctor suggested 2 months of bed rest ( but it could be different for you). After that I was allowed to walk. I prefered playing sports after 1 year (just as a precaution). I think the front outer part of the left knee miniscus hurts. I had been running very fast on a tar road which was kinda slanted, so, it must have made a tear. But, I keep getting pain from time to time. Then, I stop running for a few days & the swelling goes away.

@@yashrathi5219 Thanks for the reply! The doctor advised me not to play any contact sports and advised a lot of cycling, swimming types of exercises for 2 years once I recover. But I am glad that you are able to run and return to sports. I guess the pain in the outer knee like you said is because of a meniscus tear because it hurts for me too even while I'm at rest as I had the meniscus repair. But could not really understand why your meniscus hurts if it's cartilage damage for you.

Hey Yash Rathi! Sorry to hear about your knee surgery and continued knee pain :-( The 2 biggest problems after a cartilage surgery like this are: #1 - Going too fast through recovery. Whatever your surgeon says, you've got to 2x or 3x as far as recovery time. Go slow and make sure to listen to your body. If it hurts, you're probably doing too much for what your knee is ready to do at this point in recovery. #2 - Knee alignment isn't addressed - Surgeons and doctors aren't trained to look at this or treat it. Great question Charlotte! Scar tissue develops on it's own as long as your body has a good healing environment. The main concern you should have is that you don't irritate it frequently. In other words, if you feel your knee swells and flares up several times per week when you do certain activities, then you're not giving it a good healing environment because it keeps getting irritated. It's the same as if you kept picking at a scab and making it bleed every other day - that wound would take a very long time to heal because it doesn't have a good healing environment. That's the number 1 problem that people face with healing cartilage, they flare it up too often and don't let it heal as it will on it's own.

Great question Florence Su! Scar tissue is what the body uses to replace injured original tissue. It's the same thing that makes up the scars on your skin. It's usually about 70-80% as strong as the original tissue.

Hey Muhammad Shahab, it sounds like there is likely LOTS of pressure inside your knee joints which usually happens from muscle imbalances. Based on what you're saying it also sounds like your knee joints are very irritated. You might need to rest a bit while the knee joint calms down so you can begin to do exercises that can help reverse the muscle imbalance.

I received a steroid injection now getting 3 viscosupplementation Injections. I’ve had 3 no difference yet. However doing your glute walking exercise and no weight dangle knee movement, I feel better already in 3 days. Butt muscles are😂 sore. Thank you, I continue to watch. So helpful.
