Dr. Neal Guymon, O.D. goes into detail about lipiflow and how it treats mgd. Meibomian Gland Dysfunction (MGD) is a chronic condition of the glands in the eyelids. Poor oil consistency, blocked oil glands, and poor functioning oil glands result in mgd. This can lead to dry eyes and dry eye symptoms. Lipiflow can heat up the oil glands from the inside of the eyelids and will use a peristaltic massage to help clean out the oil glands.

This is interesting. So , i had dry eye for a long time, including inflammation, red eyes, the whole thing. I used lipiflow for like 5 times already and it does help but it effects last for about a month or 2 max. (It just helps, doesn’t cure anything), then i mixed LIPIFLOW and IPL , and it does help even a bit more, the effects feels better, but again DOESNT CURE ANYTHING. Oh one thing about the video, my insurance did over all these procedures , every time. So I didn’t need to pay anything. BUT, here is the big thing, i have the same question as all of you guys, “why do i have dry eye so severe that seems nothing helps”, for years ophthalmologists just told me that’s life, we can treat the symptoms blah blah. And i understand that may be the case for a few, but why doesn’t anybody ever try to think WHY am i getting this now. So after long time finally came across a few seasoned few specialists that they themselves wanted to find out why. Turns out i got some immune issues, very rare ones, hard to find and pinpoint cuz apparently this body inflammation come and goes, till today im still trying to find out why i have this immune problem, but they did a test, trying to control some sitokines in my body, and my eyes went white and no inflammation within one day of this test. Without any eye drops or machines or anything. So proving the DRY EYE is a symptom of it. NOW, im not saying this will be for everyone , but this was my issue, and i got a good insurance so it allowed me to do as many tests and consultations as i wanted, so that’s why i took this path. My point with this is, your dry eye may only be the symptom of another issue, could be issues with muscle inflammation, could be infection of nerves, could be you need to see endocrinologists, could be arthritis , could be so many things behind of it, and the only way you going to confirm this, is if you find a doctor who is willing to ask “why” and want to find out the answer same as you want to. Very informative video with good demonstration . I got this treatment done and follow the eyelid scrub and warm compress pretty religiously but still… within 2 months my glands are over secreting and I don’t know why. Could this be due to pre menopausal hormonal imbalance? what else should I try?

Thank you. I learnt a lot from this video. I got my lipi-view taken of my oil glands. Unfortunately, my ophtalmique made little effort to explain. She told me I have thick oil and clogged glands and gave me eye drops. Doctor, could you please tell me What does it mean to have those little dark spots at the top of glands. Right around the eye lips?

Not necessarily, unless you have a combined form of dry eye issues. If you have MGD with your sjogrens, then it could help with the MGD portion possibly improve dry eye symptoms that way.

Thanks for the video. My question is why does one have to keep continuing with the routine of cleaning, warm compress etc? What is the use of the treatment in that case? What I’m trying to figure is does the treatment not take the redness and inflammation caused due to dry eyes? Do we still have to depend on eye drops after this? Thanks.

My doctor explain that this is just something that now I have to keep an eye on, kinda like someone having more dental issues than normal, especially with an autoimmune disorder, So continued maintenance with cleaning the eye lid, and the warm compress is a must. depending on my progress, my home maintenance an annual lipiflow treatment might be needed. It could be longer the in between lipiflow visits.

I don't think there is a "perfect" treatment for severe dry eye disease. Some treatments will work better then others depending on the person. Both IPL and Lipiflow are great and effective.

Yes I have! Also a great meibomian gland treatment. I like lipiflow and ILux better, but miboflow is also good.

I have severe anxiety about things being close to my eye and I was able to tolerate the lipiflow procedure. What I was not able to deal with, which me and my doctor had to take breaks was the eye lid cleaning. I almost passed out twice, but that’s my anxiety with stuff being so close to my eye. The lipiflow was a breeze compared. It did feel weird but it wasn’t bad. This is my first year dealing with MGD, my Hashimoto’s might have something to do with too. I have also been prescribed steroid to help with the inflammation. I have a follow up next week, which I’m hoping will be a positive one.

I started to develop dry eyes after a lash lift/tint a month ago. The optometrist only imaged my lower lid meibomian glands only (not upper) and I had severe drop out already. Once you get to that stage does lipiflow even work? Would IPL be a better option?

It really depends... there is a certain point of dropout that it becomes difficult to recommend lipiflow because it might not be worth it at that point. I would lean towards IPL at that point. It's for sure a case by case basis.

I can help to hopefully prevent further styes, but not necessarily get rid of current styes. I tend to lean towards IPL as a helpful treatment for styes.

This is a great question, and it really depends on the person. Most of my patients it has taken a few weeks. We usually do a 2 month follow up after lipiflow and a lot of my patients usually notice a lot of improvement by then.

@@DrEyeGuy Thank you Doctor for answering my question, just a few days ago I did the Lipiflow treatment I hoped to have good results in this treatment because I already had the IPL treatment but I did not have any improvement. My eyes are very dry because my glands are blocked and others are already dead I have tried various kinds of drops and nothing helps me my eyes burn and hurt a lot all day it feels like I have sand in my eyes they scratch me it is difficult to be like that, I hope to God that the lipiflow treatment gives me good results, thank you doctor for your videos.

I have a question. I have mgd and I’m considering lipiflow because the warm compresses haven’t helped my eye dry symptoms. Can warm compresses put too much pressure in your eye and mess with your corneas? If so, should we avoid them? Thanks!

Excellent Question! In fact you are going to like one of my upcoming videos. There is a worry with the heat and the pressure from the warm compresses can mess with the corneal shape. People with keratoconus (or family history of keratoconus) and glaucoma should be careful. Same thing with doing lid massages. The heat and the pressure can effect the eyes if done repeatedly. I do think there is a way to do it with still getting the benefit and not the risks though.

Thanks for this video! I was curious to know what you think about TearCare in comparison to LipiFlow.... It seems like TearCare is better from some articles I've been reading but my doctor and the doctor my friend works under both directed me to lipiflow. Although the research I've done says TearCare yields better results and is a lot less expensive, I haven't found anything specfically mentioning more sever cases like mine... In the last 6-7 months, I've had two chalazion removals in the same eye lid (about 5 months apart) and I was on 2 month's worth of Doxcy. It was only after the Doxy treatment that my doctor recomended LipiFlow... I also formed my second stye/Chalazion on my third week of the two month doxy treamtment, which hopefully expresses how bad my case is. Do you think TearCare is a route I should explore or is Lipiflow a more preferable route when it comes to more severe cases (like mine)?

I personally prefer lipiflow over tearcare, mainly because lipiflow can apply heat to the inner side of the meibomian glands. You can always look into IPL. That has been shown to be great with MGD and a recurring chalazion.

I’ll see what my doctor says and hopefully their office or a near by office provides it. It seems like ipl might be cheaper which is another bonus for me

Yes, anybody can get it, but it treats your oil glands, not anything else that might be causing dry eye.

Hi all, I have tried it and it didn't work for me. In fact sorry to say it made my eyes dry even further and had to go through a lot more treatments and it didn't improve after that. I followed up with my dr. and found out others didn't have a positive effect too. It was an expensive treatment that could have paid for more eye drops for a normal treatment. They should reduce the price for this treatment as its to early to predict a positive outcome from trials. I am a very positive person and like to try everything that's new but I also learned to not believe everything I hear or see. So good luck everyone and see both sides of the coin.

For me yes. For 3 years good, then needed it done again. It’s not a permanent fix. Also not better immediately after the Lipiflow procedure. The eyes get better over the next 2-3 months after the procedure. So be patient.

I had the Lipiflow treatment 3 months ago with no results, I feel like I threw $1100 down the drain. Very disappointing. I do appreciate all your videos, you can't get this comprehensive dry eye treatment information from other eye doctors.

I too payed a lot and got basically no benefit. My home treatments help much more. I felt the treatment was too time limited to really express much oil and there was no objective evidence that it expressed any clogged glands whatsoever.

This is such a great question Max! The biggest pros with Lipiflow is that it heats the oil glands from the inside of the eyelids, it has repeated expressions during the procedure with different pressure settings, and if set up correctly, can express from deep down near the begging of the oil gland. That being said, manual expression pros: you can actually observe the quality of the oil coming out of the glands, you can control your own pressure depending on what is needed. The tricky part with manual expression is how to effectively heat the oil glands for best results, expressing from the base of the oil glands, and there is potential for hurting or damaging the oil glands, especially if doing it frequently. Manual expression can also be painful as well, when lipiflow is not painful. I actually think a combination shows some promise, it's called immediate manual expression. Right after Lipiflow, you have an immediate manual expression to take advantage of the heated oil. You just have to be more careful with manual expression.

When you say torn what do you mean? Are you meaning the actual oil glands being torn? The lipid layer is an oil layer, and if it's disrupted, the goal is to try to increase the oil to help repair the oil layer.

Yes. Xiidra is an alternative. Cequa is also an alternative, same medication but different dosage as Restasis. There are also some others like cequa as well.

@@julieprieto2209 Great Question! MGD is considered a chronic condition, but there are some things you can do to hopefully prevent atrophy and more gland dysfunction. Omega 3 supplements, warm compresses, lid massages, diet to reduce inflammation, lipiflow or other gland treatments like IPL and Ilux. Also trying to use the 20/20/20 rule if using computer/screens during the day. Blinking your eyes to try and help those glands keep functioning is a big one.

Did you eye doctor go over the results with you? It might be tricky to determine because there are many different things that can determine if you would be a good candidate for lipiflow.

Hi Dr, just had an exam with my ophthalmologist yesterday. My oil glands aren't functioning at all. Going back in 3 days for additional testing by dry eye expert to see if my glands are savable. Can I still do lipiflow?

You are not alone. Lipiflow is one of the most expensive dry eye treatments out there... unfortunately. I think it will continue to come down, or cheaper options will become available. There are already other treatment options to look into: IPL and LLLT (low level light therapy) are a couple of options that are cheaper... still expensive, but cheaper then lipiflow. Depends on who you talk to if they are just as effective.

It's not likely to get the glands back... but... some studies have been showing possibly a mild regeneration. So it's possible, but we need more research to know for sure.

@@shubhamgarg2021 This is a good question, and some times can be a tough decision. Ultimately you and your doctor will want to make this decision, just because he has observed your glands. Lipiflow is great for preserving and hopefully improving the glands that you still have, so for that reason it would be worth it. It would be a harder decision if you are hoping that lipiflow will try and bring your glands back, because I wouldn't fully count on that.

I have very dry eyes due to mgd. The tear production etc. Are all healthy. The only thing that helped me thus far is IPL. But after 1/2 months the effect is gone. Also my dry eyes are better but not gone after IPL. My question is. You think lipiflow results will last longer. And is it possible that my symptoms will be completely gone? Thanks for this video!

Excellent questions! Hard to say. I go back and forth between lipiflow and IPL being more beneficial. I think it depends on the person. I think its possible to control dry eyes, and control symptoms. I usually shy away from saying the symptoms will go away.

This is a very good question. You are doing the right thing trying to get down to the core issue causing your red eyes. IF the redness is dryness related to meibomian gland dysfunction the lipiflow and IPL are both great options. Hard to say which one I would recommend. Some people lipiflow works better, and some people IPL works better. Eye redness is usually from multiple issues, so I don't know if I could make that claim the liliflow or IPL would completely get rid of the redness.

Daily maintenance is there to improve the results, also to hopefully keep you from needing to the repeat the procedure earlier than you need to. It's kind of like getting a teeth cleaning at the dentist, but still need to brush your teeth and floss afterwards.

She has prescribed a combo steroid and antibiotic ointment to heal my ravaged lids.Having to pause warm compresses, what else can I do to stimulate my meibomian glands to produce oil? Already taking an excellent Omega 3 oil.

@aquarius11 If you go to a good practice that performs the procedure properly, you'll probably have a better outcome. If you can find an ophthalmologist to just heat and express your glands, that would work too. I've healed and use a antihistamine gel on my tear duct to help. Dry eye is a lot!

Hello, I had inflammation on my eyes a month ago due to dry eyes so I am taking Arex (Steroid drop) for almost a month. I just scheduled for lipiflow next Friday. However I read the info about the contraindications " active ocular inflammation or history of chronic, recurrent ocular inflammation within prior 3 months. Should I reschedule later ? Please advice

Excellent Question. Lipiflow can actually cause some inflammation. Sometimes we prescribe a steroid after the procedure. At the same time, inflammation is a big part in dry eye as well. I would try and calm your inflammation down as much as possible before the procedure. Then expect some inflammation after the procedure as well.

I got it done today because I’m getting LASIK here in a couple of weeks and it didn’t hurt at all! It felt really nice tbh, like a comfy eye massage haha! I was terrified as well but when it started it really wasn’t what I expected. Its also very warm and my eyes felt better afterwards. I highly recommend it!

IMPORTANT! We can recreate this at home by putting hot pressured water over our closed eyelids (from a shower head) for a few minutes. I have severe dry eyes and before doing what I mentioned, I would use eye drops every 10 minutes or an ointment every 20 minutes. I do this every 24 hours. I need to use drops once every few hours. Alhamdulillah (All praise is to God).

@@aravindram1480 "cured" is the wrong word. But about 7 out of 10 usually show significant improvement in signs and symptoms. We have gotten better and finding the best candidates for lipiflow as well. There are some patients that don't notice an improvement, and some patients that we don't recommend lipiflow.
