Realistically, if there’s a difference in facial characteristics here it could be from corticosteroids like prednisone as the guy does have IBD and that’s a common method of treatment which can cause water retention and the appearance of a more “filled out” face to varying degrees. Or it’s just the lighting. Or he shapeshifted. We’ll never know.
