Dr. Sten Ekberg thanks for your videos. I have one question, I stumbled across the “ketoacidosis confusion” while shopping on amazon for a Ketone breath anylyzer. It had a scale of when your body is in ketosis and when it’s too high it would turn red and instructions say that this means you are in ketoacidosis. What do you think about companies selling products that actually include this in their instructions? I’m sure this is very confusing for so many people! Thanks so much!

I have the highest ketone values on the pee strips for like a week is this dangerous i am still.type.2.diabetoc but almoat off.medicine is this dangerous or normal i was hoping/thinking they would go down can you please answer to comfor me ❤❤❤

I saw a video here on RU-vid with a clip from the US tv medical talkshow "The Doctors" in which they were discussing the keto diet, when one of the doctors mentioned she had done the keto diet for a month in medical school as a class assignment from one of her professors, and another doctor blurted out in amazement "You voluntarily went into ketoacidosis?!" to which the first doctor replied "Yes." Obviously neither one of them had any clue of the difference between ketosis and ketoacidosis. That's pretty sad.

Hi Dr Sten 😊 love your videos. I’m doing hard core keto, and I F right now, and my ketone levels have averaged 4.7 for this month so far; I got as high as 7.1, but mostly down around four. I am feeling great and I’m down about 20 pounds. In the last three months. Can’t tell you how much I appreciate your videos. 😊👍

My blood work which I got today showed that my ketones was +++. This is after doing OMAD for 29 days with keto diet (did the best I could). My GP told me to drink more water and be careful. My glucose was 4.5; So that got me worried. Now it's clear.

Excellent explanation. The most easily found info correctly states that, in ketoacidosis, your body makes too many ketones, which leads people to wonder if ketosis can cause it - which it can't!

I was a little worried about ketoacidosis so I came looking for this video. Hopefully Dr Ekberg can see this. I'm a diabetic but under control and on zero medication now. I've been doing IF for a year now and dropped all 6 tablets I was taking in the first 6 weeks of IF. Before IF my A1c was 138mmol or 14.5 in your currency I believe and after 3 months I was 47mmol or 6.3... not sure about that conversion. A week ago, inspired by another video, I started a fast to see if I could get into the ultimate autophagy zone... after 2 days I had 1.2 ketones at that point. On the 3rd day I played tennis for 2hrs and then my ketones had gone up to 3 so my GKI was 1.5mmol. After the tennis, I developed a migraine which is very normal for me so I broke the fast with about 1000cal, about 20g of carbs 20g of protein and the rest was fat. That night my ketones had gone up to 3.3 and the glucose down to 4.2 mmol so GKI was 1.27mmol. Next morning ketones had gone up to 3.9 but glucose had gone up to 5.8mmol. GKI was 1.48. Another day eating about 1000cal worth of food ( trying to mimic ometd to shifts stubborn weight) that night my glucose was 3.9 mmol but I forgot to write down my ketones level. After a bad night sleep my ketones were down to 3.4 and glucose up to 5.2 I divided the 1000cal in two meals The following morning ketones were up to 3.7 and glucose still 5.2. The meal was about 1000cal 23g of protein which made no difference. 3.5hrs after my meal glucose was 3.8 and Ketones 4.1. The following day my glucose around midday was 4.6. I did not measure ketones. The meal was 1000cal again with similar proportions as the day before. 4h30 after the meal glucose was 4.1 and ketones was 4.3. Yesterday morning glucose was 5.3 by midday 4.4 and after 90m of cycling was 4.7. I felt good so decided to fast for longer....so no meal, just water and coffee. At 20:00 last night my glucose was 4.2 and Ketones 4.7 - after 30h of fasting. This morning, after 41h of fasting I feel very well and my glucose was 4.1 and Ketones have gone up to 5.4mmol. I'm not sure if I should continue... I feel totally normal. Over the last year I've done 3 days fast a few times already and 2 x 4 days - 96hrs so I'm experienced. I'm planning to increase the protein to 30g today and see what happens....ideally, I'd like to keep ketones between 3 and 4.... is that wise? And if so, for how long? UPDATE: I did carry on for another 2 days but my ketones level went up to 7.4 mmol so I stopped. Moving on from that, I couldn't lose any more weight with that keto / IF plan so I'm currently starting my 7th day of an extended fast. I'm feeling fine but my ketones level by day 5 were at 6.2 mmol so I decided not to do my usual 10000 steps a day and by the end of day 6 ketones were still at 6.2. I'll brake the fast tonight, by the end of the 7th day. So, I'm a controlled T2D, IF for over a year with the extended fast thrown in here and there and I can easily go above 5 mmol in ketones. Perhaps exercise has a bigger influence than I anticipated. The good news and based on another video of Dr. Ekberg, with this extended fast I lost more pounds than I thought I would, not counting the 1st 4 pounds that are probably water. Ever so grateful to Dr. Ekberg ( my favourite cause I like to understand why things happen the way they do), Dr. Berg and Dr. Fung. I changed my life thanks to their videos.

Type 2 Diabetic. I'm just getting out of the hospital from Ketoacidosis. Before this I've been doing keto for over 6 months successfully. My doctor prescribed Jardiance to lower my blood glucose. 3 days later I'm in the ICU. Nobody said but I'm pretty sure that Jardiance triggered it. It's even listed as a side effect on the Jardiance site. I think more knowledge on the GP's part would have prevented this.

Same thing happened to my husband. He had lost a lot of weight in the year before he was diagnosed. Was started on Metformin and started feeling nauseous and sick. Was started on Jardiance 4 days later. 3 days later was in the ER for 36 hours with Ketoacidosis. He was put on an insulin drip and they measured the PH of his blood every hour. I’m beginning to wonder if he doesn’t have LADA. He has the symptoms of a type 1 even though he’s 70 years old. So weird.

Thank you so much for your knowledge!!! I´d like to ask you if water kefir, as it´s fed with brown sugar, after the 2-3 days it is processed in a way that it´s not sugar anymore and we can drink that sometimes a week without breaking the ketosis??? that, as well as colostrum Dr Ekberg???

I am winning the battle though... went from an A1C of 8.5 to 5.3 eating only 30 G of carbs a day... it’s very doable... but not easy.. I pack all my food every where I go... thanks again!

I have been doing low carb OMAD for about 10 days now and I just got my Keto mojo. I haven't had any food in 18 hours, I have had water and I had 2 cups of black coffee this morning. I just took my first readings and they are: glucose 83 and ketones 3.4. Is this ok?

I'm about to go on an extended fast, again; however, last year, when i did a strict week fast, my Ketones went well into the red (i think they were 7!) on the 7th day, & my glucose was in the low 60s. (I used a Keto Mojo for blood monitoring.) When i stopped, i had a fried egg, some bacon, toast & jam, & a cuppa joe w/cream. That ended that. I'm overweight (5'10"/210#), mostly sedentary, & in my 70s. I've always had good healthcare, & now am on VA care. I've never had diabetes, not even close. What in the world happened? BTW, i also used the urine strips, & they too indicated i was in the danger zone. Weird.

Is it safe for Diabetic Type 1 to be on a Keto diet? I understand that DKA can be deadly. Will being on the Keto diet make them be more likely go into DKA for those who are Type 1? Should T1 Not fast or eat differently from those who are not diabetics? Thanks!

My friend is type 2 and over the last year lost all her excess weight, plus it’s still eating away her muscles. I’ve been telling her to cut down her excessive sugars as the advice she’s received about how to make nutritional changes is nil - they just increase her dose of insulin instead of telling her to stop drinking sugary drinks/ carb-filled snacks and meals. (I’m sharing your videos with her too.) She takes a high dose of insulin so when she does cut down on sugars etc she goes hypo. All the time she is frequently drinking and urinating so I guess my point is, is she in Ketoacidosis? Can she slowly adapt to the keto diet or does she have to stick to using glucose as her main fuel source?

I have some worries while eating low carb diet, not more than 50 grams/day carbohydrates. My ketones measured with Abbot blood sticks achieved within a week a value of 130 mg/dl , means 7.2 mMoles. Glucose values between 90-120 mg/dL. Is this situation dangerous?

Hello @Dr. Sten Ekberg Sorry for being off topic but in another video you mentioned taking fish oil to help with the dry skin. When you say fish oil are you referring to oils such as cod liver oil? And what would be considered essential fatty acids? Also what are your thoughts on sprouts and microgreens and which one do you think is more nutritious? Thank you.

Cod liver oil is great as it has vit a, vit d and omega3s, which are essential. Look up reversetrol and its anti-inflammatory properties.

Cod liver oil is one good source of essential fatty acids and it also contains natural fat soluble vitamin A and D. There are other products that contain higher concentrations of EPA and DHA, which are the primary omega 3's the body needs. Sprouts and micro greens are great sources of concentrated nutrients along with other natural whole foods. Just think of them as a good add on.

Thank you so much for this awesome explanation. I actually faced this scare the other day. I was 60 hours into a planned 72 hour fast. On a lark, I tested my ketone with a urine stick and was shocked to see it between 8 - 16 mM. I do a routine 18-6 intermittent fast normally and usually see a 1is mM. I ended up warming up some bone broth and dropping out a little early. Thanks again, cheers.

I finished one for 90 hours and checked the urine test, it says 16 mM. Should I call an emergency? This guy says you die... Are you still alive lol?

@@cookedbacon4245 I'm fine. I freaked a little and ended my fast a little early. My ketones dropped expectedly. I might pick up an over the counter glucose monitor for my next fast just to give me an extra reading.

@@blacfiberx7x I just dropped out of a 5 day fast. I don't think these urine test kits are acurate. I'll buy the keto and glucose one for next time. I wasn't feeling bad, I was acutally great on my forth day, but when I saw that number I freaked out.

My husband, a diabetic, had Ketoacidosis. Believe me, if you have it you will know. He says it’s the crappiest he ever felt in his life. Nausea, throwing up, headache, aching joints, confusion, weakness. He spent 36 hours in ER on an insulin drip until they got his Ph level normal.

Dr. Elberg I have a question i am 65 been type two diabetic for 5 years i have ketoacidosis with tingling in my feet. Doing IF since Jan 1 would like to know if the tingling in my feet will go away with time on the keto diet already lost 12lbs so something is working numbers are down below 150 were in the 200 range

What about Type 2 diabetes? I started doing one meal a day with relatively low cabs and measured ketones at 7 to 8, although blood sugar measured 3.9 (mmol , I think, how measured in Canada) where I normally measure average of 6.8. The reason I started doing this was after watching your videos and they talk abut controlling blood glucose and eventually improving insulin sensitivity. But was concerned the ketones were to high. Your comparison between non-diabetics and Type 1 diabetics is great, but not sure how to apply to Type 1

Dear Doctor Ekberg, I just read about research done by the University of British Columbia, because I've just started doing the ketogene diet, and suddenly was craving for icecream. So I looked it up, whether icecream is allowed. I came on an article, that was done by the University I just mentioned. Before and after 7 days of keto they have 9 young men a 75gr glucosedrink of 70% fat, 20% protein and 10% carbs. The researchers, lead by doctor Little, had expected to find markers of IR or inflammation, but to their surprise they saw that the blood vessels had been damaged. That was kind of shocking to read! Is it so, that one weak moment of binching can damage your body while in keto? If so, how to return to a eating pattern with let's say half the carbs? It sounds frightening. I'd like to know what your vision is. Thank you.

I’ve been keto for 2 weeks. My ketone levels go up to 6 on the urine strips. I’ve been increasing my veggie carbs but it’s not a quick fix. I had to eat a banana this morning to get my level to 1.5 mmol.

I've been doing low carb/carnivore for 3 months and have lost 25 lbs and yesterday I had labs and my A1C was 4.9 but for some reason my urine showed I had a level of 80 mg/dL for ketones. Is this bad? Why is it so high? I'm not a diabetic.

This makes sense. Is it possible to get into a ketoacidosis though your blood sugar is below normal. Because I’m pretty sure I had that situation. I’m treating my type-1 diabetes with a pump. At that moment I had been on a carnivore diet for a while. Reduced my base insulin from 2.7 to 1.8 per hour. And I’ve been in or at least beginning of ketoacidosis before, always been able to fix it my self. It can happen if the infusion set has failed or similar. But in this case my blood sugar was not sky high. I also since a few months back monitor my blood sugar continuously, and thought the system had failed. So I did a normal blood sugar test and that one also showed low. Just below 3 mmol/l. Can ketoacidosis also occur e.g. if the blood sugar is ok but something is wrong with the insulin? And also how exactly does the insulin regulate ketones? This was mentioned in the video but not explained.

I can't think of a scenario. You can have low blood sugar and make ketones, but they should not get into a dangerous range in the presence of insulin (meaning you blood glucose does not go high). For a type 1 you have to monitor glucose carefully when you go low carb, because even a small amount of insulin makes a big difference, but I think you'll be better off for it. If your blood sugar goes too low, you took too much insulin. 3.0(54) is too low even in ketosis. 4.0 (72) is probably the low end. Good luck

Thank you for your answer. I’m still perplexed over this situation I had. Regarding sugar levels I’ve been at as low as 1.9 (34) and above what the equipment can read >35(>650) and never had this as bad situation nor at a low blood sugar level. Those situations was during a time I used a lot more insulin, around 200-300 units of insulin per day. With the pump I got down to ~160 units without any other change. Now I’m down to an average of ~55 (42-78)units per day. My blood sugar can still go low sometimes but it seems like my body is regulating it. Now it flattens out when it’s getting low. When on the carnivore diet this didn’t work as well. I was trying to deplete my liver storage then. Thanks again for your answer.

I know this post is old, but I just wanted to tell you that I've experienced the same thing (also type 1). It's called euglycemic diabetic ketoacidosis. It's also caused by a lack of insulin, and almost exclusively happens to insulin pump users. That's because we use fast acting insulin in pumps, and it's "burned" off after only a few hours. If there is any disruption of insulin flow for some period of time, we risk devoloping ketoacidosis, no matter the blood sugar level. It will usually rise rapidly after stop of insulin flow, but not always. If you've been sick and/or vomiting or fasting for a period of time or having done exercise while also not receiving insulin, and you've depleted your glycogen reserves, you won't get a rapid rise in blood glucose when you start lacking insulin to cover you basal needs. You won't feel it in the beginning, until you get sick from ketoacidosis. It also makes it pretty difficult to treat, because you need to eat a bit of sugar, take insulin, eat sugar, take insulin, eat sugar take insulin over and over until you're clear of ketones and have replenished your glycogen reserves. But it's safer to go to the hospital if you start experiencing ketoacidosis even if it's mild.

Dr Ekberg would you kindly explain why do ketones go over 10mmol/L in type I diabetics causing ketoacidosis. I understand that they do not secrete insulin, so zero insulin. Therefore, their blood glucose is sky high, especially if they continue to eat carbs. Their body, not being able to use the blood glucose, erroneously thinks it's starving, so it starts burning fat as fuel and ketones increase in the bloodstream. OK. But.... what isn't clear to me is why doesn't the body metabolize the ketones released in the bloodstream ? Why do ketones increase so much going over 10 mmol/L ? Why aren't they entering the cells and being used as fuel ? Why aren't they being consumed, instead of being kept in the blood and continuing to increase, causing a drop of the pH of the blood ? If the body can't use glucose, because of zero insulin and therefore shifts to fat burning, producing ketones, then why aren't these ketones being used up by the cells. Ketones do not need insulin to enter the cells through the cell membrane. They are liposoluble, so just go through the lipidic double-layer cell membrane. This is the part I don't understand of diabetic ketoacidosis. Would very much appreciate your explanation. Thank you for the great content and greetings from Italy 🍀☀️💖

Today I measured my blood ketone levels after 9 days of LC eating at about 30g carbs per day. I was shocked to see 4.7, then later 5.1 after a bit of sour cream. Am I too high? I'll keep measuring and hope my ketones will lower a bit. I am thinking my ketones are too high because my body is not fat adapted and doesn't know what to do with them. I am not diabetic btw, blood sugar is at 4.7

You're absolutely fine. Just did a 3 days water fast and my keytones were crazy high. But my glucose was also within standard range. My gp also didn't know the difference between ketosis and ketoacidosis.

I think you mean sympathetic dominance. I don't see why though. Once you are fat adapted and your blood sugar is more stable, the stress on the sympathetics and adrenals are less.

Loved this video.. my sister is just 45 days into keto and does 18 hrs IF.. she has hypothyroidism.. she has blood ketones of 3.5 and has no urge to urinate even after 8 hrs.. is it something to worry doctor? Please help.. she drinks plenty of water though

What ever water you put into the body has to come out, so as long as she drinks enough, I wouldn't worry too much about the timing. 3.5 is a typical level. Ketoacidosis would be increased urination and thirst because the blood glucose is spilling into the urine

I’m type one diabetic . I have been doing the keto diet since the 1st January. I was admitted to hospital with Keto acidosis. Blood Keto reading of 12 . But my blood sugars were normal. Through cutting out cards I had to reduce my insulin injections to stop my blood sugars going low this in turn caused the acidosis. So would you say it’s not safe for a type 1 diabetic to do Keto?

Many type 1 diabetics can do keto, but I would monitor very closely and change very slowly to give the body time to adapt.

Aha I’ve had something similar with low blood sugar levels. I did then 0 carbs. Now I try to have 20g a day but I’m not pedantic I’m happy also with the double or sometimes tripple of that. The avocado seems to be a miracle fruit. 18-20 g carbs but hardly raises any blood sugar. This strategy to always have some carbs but still in a keto range doesn’t seem to cause any problems. So yes it’s not without reasons many doctors don’t recommend it. But neither have any of those doctors been able to explain how it works and what the difference is between ketosis and ketoacidosis. Apparently a keto diet should keep your keto reading between 0.5-3. I need to get those keto sticks so I can measure. And I always keep some candy around just in case. I started with a carnivore diet, and now I can say that is a no no for a type 1 diabetic. Started on the evening of 23 February so on a fast learning track.

Be very careful reducing your insulin injections! Type 2 is expected to reduce them with Keto, but with type 1 you still have to maintain a certain amount

On a 9 day fast and ketones are running between 6.5 and 7.4. Blood sugar running between 54 and 65. I’m fine.

If you over fast and do not consume enough electrolytes on a ketogenic diet. Can you risk ketoacidosis even if you are not a diabetic?

You know ketoacidosis is when both glucose AND ketones are high in the blood...more osmolarity..chaos Happens only in diabetics.. In keto diet, glucose is low and ketones are present.. In normal people, ketones are not significantly present when glucose is already there... That being said...Not drinking enough water is unhealthy even with a normal regular diet

I did Keto for about 6 weeks and my Ketones got into the 6's? That higher than 5, does that mean my body isn't making any insulin? With Carb IF my ketones are about 2.4.

Your ketones go up before they come down. Your body has to have time to reawaken the dormant enzymes that utilize ketones. 6wks is hardly enough time.

I’m doing it and at first I wen’t extreme. Learned about it through some carnivore youtuber and I did not go slow cut all carbs and fiber at once. 23rd February. Can’t recommend my method lots of issues occurred. But keto diet is great, know when I have more knowledge of how to rather than just experiment, it’s really good. I recommend to get a way to measure blood sugar continually this helps a lot. The type I use is not 100 accurate, but it’s a consistent error so one can just recalculate to the exact value. For me it shows too little, a colleague of mine used the same and for him it was showing to high but consistently so also.

The ketones in my urine are above 5. I feel sick, I puked once. I cannot stand, I have no power in my muscles. I don't have diabetes neither type 1 or type 2. It's my 2nd day on keto-diet. Yesterday I ate 1500kcal of fats and proteins and 25g of carbs. Why do I have so high ketones and I feel awful???

That's the keto flu. Plenty of factors can make it a bad experience (low electrolytes, high carb diet right before starting keto, dehydration to name a few). Urine ketones will be very high when you first start keto until your body stops overproducing them. Do not use urine ketone strips for anything besides telling you if you're producing ketones. It only measures what is being flushed (excess), not your blood ketone level.
