In an online mentoring session, I went over a case where I had to refer out in order for his sciatic complaints to resolve. In the past, this same patient had seen me at least 5 times over 5-6 years with similar complaints. The difference was that his symptoms fully resolved on all those other occasions. Make sure you know your limit and trust your gut. If you get zero improvement in 3-6 sessions, AND they're not centrally sensitized, you should refer out.
