I am in the middle of treatment for PC now. The plan is for 6 mos of hormone therapy and 9 weeks of daily external beam radiation. My gleason is 4+3=7. My Decipher report puts my in very high risk for spread. I was offered an opportunity to participate in a trial involving a combination hormone therapy with Lupron and Darolutamide.. I'm three months in. So far so good as far as side effects. The trial is paying for the Darolutamide, and the rest is covered by medicare. Nice to see this study result.

Thank you! A member of our family in Europe with very high risk prostate cancer with bone mets has been given triple therapy. His doctors started it 11 months ago with injections of Firmagon every 4 weeks, then, two months later they added to it Nubeqa and 6 rounds of chemotherapy. Since the end of the chemo he is on double ADT (Firmagon + Nubeqa). He seems to be in top form; exercises regularly, and never had fatigue, hot flashes, or weight gain to complain about. I am going to share this video with him.

Great video. I am a high risk PC patient, Stage IIIC (cT2a, cN0, cM0, grade 5 [Gleason 3+4, 4+3, 4+5], decipher 0.98) . Already had 26 EBRT treatments. PSA reachaed to 0.1 after 7 month of Lupron. Started Xtandi after 8 month of Lupron. Still taking combination of Lupron and Xtandi until the completion of 24 month treatments. The side effect is manageable. Thank you, Dr. Scholz👍

I'm 66, metastatic PC. Started with Lupron shot with Bicalutamide and that treatment knocked down the PSA significantly maybe 3 months, than he changed me to Enzalutamide ($9k -30 day, I get mine for free ) and the PSA is now at .002. But the quality of life has dwindled after changing to Enzalutamide, I'm tired, I'm forgetful, Hot flashes are terrible and sometimes my head feels like it'll explode and I wonder how long I will tolerate life like this. My last appointment my Dr said I'll have to stay on this treatment the rest of my life. I was hoping I could take less of a dose but that's not the case.

I appreciate your insight. I had an uptick in my PSA to 1.5 and they found a small spot in the backside of my pelvis. I have a Gleason 9. I was on Bicalutamide for over 2 years, but recently stated taking a testosterone booster since I weight lift 5xs per toboff set side effects. Definitely wasn't the wisest of choices. Now my oncologist is wanting to put me om Erleada with Lupron injection every 3 mos. I'm very concerned about all the side effects. I had zero side effects with Bicalutamide. My doctor says I will respond well to the treatment because I'm very active, however, only 58yo. Not sure what to do?

It would be interesting to know if the study also assessed the time to resistance for each therapy, especially the combination therapy. Does using the drugs in combination accelerate time to when the cancer becomes resistant to them, or does it delay time to resistance because the cancer is fighting on two fronts?

How about a triplet therapy by adding a taxane medicine?

This info is eye opening. I had no idea some patients with mets might not be on combination medications, but I do understand the issue with insurance coverage. My dad has always been on combination, but was dx when mets were already present and has good insurance. Are there any updates on oral medications other then zytiga or xtandi. We know about xofigo and pluvicto updates, but are there any new promising oral medications for those with mets? Also is there any time that the cancer reacts to original oral meds taken before that originally stopped working? Example: returning back to casodex?

Would the Helpline know any way to get emergency Medi-care at 56 with very aggressive cancer here in L. A.? I can hardly get a second opinion with Medi-cal. I'm on Lupron and Casodex and Megestrole Acetate. I guess this 1st generation combo therapy is better than mono-therapy.

I have two questions. 1. New PSA test called episwich which has a 94 per cent detection of cancer. 2. A German stud shows that if ou have an MRI and a PSMA petscan and no cancer is detected you do not need any treatment.

Are we talking about finite term, maybe 6 or 18 months, of combination therapy, then follow up for 5 years to check for recurrence progression, or are we talking about 5 years of combination therapy? I did 20 months of Lupron+abi-pred, and am now doing TRT to restore my testosterone per Dr Thomas Ahlering's study, and feeling great at 6 months with PSA
