Guys!!! I had the absolute craziest/busiest shift of my very short career last weekend and i had to share it with you all! This took place last weekend but I didn't have time to edit until today because of my physiology final! Although it was SUPER crazy, I learned a lot it was amazing! I had a patient pull out his foley cath while trying to get to the bathroom, finally got to the toilet, crapped like a good boy, then got up, turned out, and proceeded to 'wash' his hands IN THE POOPY TOILET! That was yesterday. I'm an aide, in hospice, and average patient load is like 7-10. I had *12* (One of those a NEW admission). 2 people were jumpers, everyone was pooping, 1 lady had literally 2 COMPLETE bed changes and 2 new gowns by the end of the 12 hour shift. OMG. I was RUNNING from 7pm till about 3am, it went Q-word until maybe 5am when I needed to get caths & start checks and changes again too, and then on out it was craaaaay again. WORST NIGHT in a long time. OMG lol

Hey justine! I am only starting 4th semester, so i dont have any really crazy stories because my nursing scope isn't as big! 2nd semester, i had a patient have a vasovegal syncope while she was doing a BM on the toilet. She fell right on top of me, and I thought she had died! It was scary, but thank God she was ok! You are going to be a great RN Justine! Keep up the good work, and push through those hard days just by staying focused, and dont let anything distract u :) Keep up the good work!

OMG the same thing happened to coworker on night shift! And it was a big man so she was stuck under him for an hour before anyone found her! Thanks for sharing!!! xo

Working in a pediatric ICU, I have lots of stories. Like the DKA that went from being super agitated one minute to crashing into ECLS the next minute, or the girl with a splenic contusion who was just in icu for monitoring when she suddenly started bleeding internally from it and we had 2 nurses and 2 doctors each pushing as much packed cells into her as we could and as fast as we could until the OR team were able to take her into the OR., or the patient that unexpectedly crashed on me while I was about to give shift handover and we weren’t able to save her, or the kid with necrotizing fasciitis that went to the OR 4 times in one day- they would come back from the OR completely hemodynamically unstable and we had just enough time to resuscitate them before they had to rush right back to the OR again because the bacteria was spreading so fast before our eyes. I have soooo many stories! I just found your channel and it is refreshing to see fresh nurses who are eager, determined and enjoying it, such as yourself :)

I happened upon your channel the other day... I've been anxiously waiting to hear if got into nursing school for this fall for over a month now, so I've just been watching tons of nursing related videos in my spare time. This is a good shift report. I am currently doing my hospital rotation in my last year of nursing school, and I can totally relate. It becomes very complex when they keep changing orders, this forces you to change the care plan or make certain adjustments. Flexibility is key in nursing!

Craziest story? Well the other day, I walked in and had to orient a newer nurse- the charge nurse tried to give us 3 patient- 1 on high-flow oxygen, that I just knew would end up on the vent, a guy having an MI and a little old man with CHF. I refused because it was too much. Another nurse took over the high-flow guy. All was well with the MI guy on a maxed out Nitro drip until his BP dropped like 80 points and he had crushing chest pain. EKG showed a STEMI and a troponin was through the roof. The ambulance came (we are a small hospital) and I had to go on transport an hour away to get this guy to the cath lab. I got back 2 hours later (I have not had lunch yet) to find my orientee getting an active GI bleeder. We stabilized her and I went in the breakroom t o eat lunch when I hear a Code Blue called- it was the guy they tried to give me as a third patient!!....seriously. It's not my craziest story, but it was pretty rough- there are many days like this in the ICU.

Wasn’t sure for the last couple of years whether I should pursue nursing as a career, but bit the bullet this year and applied ☺️ so happy I did 😍 listening to your videos makes me really hope I get in now, I just wanna jump in and start learning and helping ☺️

No no no honey, you don't need new lines for everything. Just change the med, lower the IVPB bag below the main NS (or maintenance bag) to flush with fluid and hang....unless of course they are not compatible. You can do a quick micromedex search to check compatibility or call pharmacy. It will save you a ton of time.

My hospital has PCT’s which is patient care technicians (basically advanced CNAS) . I’m a night PCT and I’m sitting here watching your videos ( i just discovered you 😂) as i get ready to start my 7pm-7:30am shift and it sounds like your hospital doesn’t have PCT’s? Because were the ones that take all the vitals , monitor intake and output ( the only time i don’t empty the Foley is when it’s a fresh transplant that’s still on hourly dumps) , check blood sugars , do bladder scans , ECG’s and ambulated the patients so that patient you had that needed to use the bathroom after the enema wouldn’t have been your problem it would have been my job to tackle all the tubes and bring him to the bathroom ...

Oh god girl.. i feel you. I think regardless of whether we're staffed.. there's never enough. We always need more nurses/cnas to help bec man.. its so damn busy.

I'm a 4th year nursing student doing my final placement right now on the General Medical Surgical Intermediate care unit. This unit takes patients that aren't sick enough to be in ICU, but not well enough to be on the floor, and usually has a 2:1 patient nurse ratio. I can relate to all the line changes as a newer nurse, and not knowing what is compatible with what, and not having a user id to log in with to check compatibilities (I often had to ask other nurses who are also busy but extremely helpful). My craziest day was probably when we had a diabetic ketoacidosis had 3 Iv in her arm, was semi-comatose, she was very busy (Q1h chemstrips, and adjustment in insulin drips, as well as blood work q4h and calling the doctor for new iv fluid orders to correct underlying electrolyte imbalances. We also had to do venous blood gases to make sure the CO2 was rising, and to see if there was any correction in the bicarb. So that patient alone was super busy). Then my second patient was a necrotizing fascitis, who had an arterial line, catheter, IVJ line, peripheral IV's edematous, comorbidities it was just a mess. they ended up picking at their arterial line, and in the midst of trying to do all the stuff for the diabetic ketoacidosis, we had to go and try to fix up the art line dressing (Which the doctor said we could just pull since they didn't need blood gas monitoring or continuous blood pressure monitoring anymore). On the IMCU, vitals are also Q4H, and can change depending on how stable your patient is. And I just found that between going through all these orders for the DKA, and calling doctors, calling pharmacy, getting respiratory, and then trying to make sure our other patient was safe, omg just such a busy day. Neither of them were stable, and both were pretty heavy! Wish you all the best in nursing and thanks for sharing your story :)

Oh yeah! I definitely remember those days when you're so busy with one or two patients that you barely get to spend time with your other patients. Nursing is never boring lol.

Hi Justine! I'm Lucy I just wanna share my crazy working experience it happened one day i'm on morning shift ( btw I'm not a nurse I'm a nursing assistant ,, but I also plan to take Nursing if I have a budget for tuition) so back in my story i was morning shift that day ( it was december 26th that day) I was the only nursing assistant on duty on our ward we had a few patients that has to be discharged that time and it's just so crazy everytime I try to take a bite of my packed lunch exactly on time when I was just about to take the spoonful of food in my mouth they will call my name because a patient has to be discharged and I have to accompany them to the Hospital Entrance where they had to give their gate pass because patients are not allowed to go out if they don't have a Nursing Assistant with them ( btw before I forgot I work at an Orthopedic Hospital a.k.a Bone Hospital ) that happened for like 4 times that day so yeah I hope you appreciate my story ... sorry for my wrong grammar I'm not that good in english so yeah thank you.. :)

Ok here's one of my craziest days on the ward. Where I work it's the norm to end up having 8 or more patients daily. I've even ended up with the entire ward (28 patients)and by the way that's the smallest ward at the hospital where I work. I had the 24 hour report to do, I had to medicate all 28 patients the majority of which had IV meds to get as well as po and IM. Prepared some for OT, received two from OT, had pethidine and morphine to serve. Had two category three patients on the ward, had admissions coming in and where I work we have to hand write nurses notes for the patients so I had 28 to write and vital signs to chart. This is the norm where I am from and believe it or not despite the crazy work load we are expected to do a perfect job -_- . That was a night shift, morning shifts are ten times worse cause there's more work to be done (dressings, ten o clock med 2 o clock meds etc)

Nah, the pay sucked from day one but inadequate staffing is the number one problem. Then again, come to think of it, It's inadequately staffed because the majority of nurses can't take it anymore and are leaving. The government claim they don't have enough money to pay nurses so that's another factor. I see online how nice the hospitals are abroad looks amazing, where I work we have nothing like that. So we here working in conditions you can't imagine, trying to do our best with the little we have and trust me the nurses here are exceptional. We get the work done and done on time, everything for each patient assigned to us is completed by the end of the shift.

you all sound absolutely amazing, i wish i could learn from you!! where are you from? we have that problem in some hospitals too, my friend was telling me she is receiving ICU patients on her unit because of overflow, but she already has 6 patients it's impossible to take care of an intensely acute patient on top of that! the government is cutting in health care here too which sucks, you'd think it would be a priority but apparently not!

I'm from Jamaica. I work at a public hospital in a volatile area. I can relate to your friend, the ward I'm currently on has quite a few patients that should be in ICU however no bed space :(. I don't mind doing my job but the conditions under which I have to work is discouraging. Mind you, not all the hospitals in Jamaica are like that, just the ones that offer free healthcare. The system is unfair; unfair because nurses in that setting have so much to do and even more is expected from us. Nurses are viewed as 'Educated maids' the majority, they have no idea what a nurse does! They think it's blood pressure checks and temperature -_- My ward holds 45 patients but still we have nearing 60 on the ward at times with very few nurses to care for them. Doctors want everything done, Relatives of patients arguing with us, patients cuss us out but still we try. Trust me, I've had to hold back tears or just find a room and cry, take a few deep breaths and continue.

That is ridiculous. How does this even happen? A patient like that should have been transferred to an acute unit. The ICU Dr refused to accept the patient because "he wasn't unstable enough". This is a complete disregard of patient safety. You don't want your patient to get to that point of being unstable and deteriorating...

Justine G.Feather It's unfortunate it happens often. I've also experienced many hectic days like how you described but what I have learnt is that at the end of the day the priority needs to be the safety of your patient/s. Even if that means advocating for yourself to get more assistance from colleagues or escalation of acuity. It's your registration and livelihood on the line that you've work so hard to attain.

my craziest shift so far was when my patient who was totally fine while I took the patients vitals and helped with a toilet visit and out of the blue the patient went into respiratory acidosis and our mobile acute team came - it was chaos :O and weirdly exciting

overall yes the job is stressful, but it varies from day to day, depending on your patient load :) some days are absolutely hectic, but you always make it out alive haha! other days you're bored because all your patients are stable and there's nothing to do for them, it really depends. The hospital I work at is a smaller hospital, but we still get acute patients, especially on my floor, so it's usually pretty busy :) The one thing I can recommend is you have to be able to leave work and not bring your day home with you, otherwise you will burn out very quickly, especially if you've had a tough one! :)

Melyssa yeah it’s a big time saver! but i don’t think it’s wrong, i literally just check off my meds and keep them in their packaging until i’m ready to hand them out, and make ajustments depending on my assessment! instead of doing it after my vs/assessment i do it before before i get busy! :)

Whoa, all of ya'll with 2:1, 3:1 patient ratios, on my floor we have 5:1 and it's awful D: Techs are 12:1 usually.

Question, I may be missing something but wouldn't the doctor switch the patient from lasix to spironolactone (k+ sparing) considering the patient's potassium level? Either way! I just graduated, and I start at the end of May! I like your videos!

that probably would have been smart haha but I don't think we have any IV K+ sparing diuretics on my unit, the only one we have is Lasix and the patient reallyyy needed to be diuresed asap... I figure that's the reasoning behind it but you definitely have a good point!!

can't u back flow your piggybacks and then give your antibiotics? If I'm not sure if the primary line is compatible I just call pharmacy and check with them first. I feel you though cause when I vent to my husband he can't really understand what the heck I'm talking about at work. Lol! What system do u use? We use Cerner at where I work and it does suck when u find out that the doc prescribed something new that you didn't know about and end up giving it late. Or they'll prescribe something not so significant like at the end of shift (that can actually wait the following day like Synthroid or vitamins) just when you're trying to catch up on your charting. Craziest days are always when we are short handed! I've had to work as a PM shift lead and CNA (taking care of 16 patients) at the same time before. Honestly, I cried at the end of the day but I feel that days like that are the ones that make or break you and ones that really build character.

I don't know why I didn't think of that haha I think I was so stressed i just thought "screw it i'm starting over to not make a mistake!" Omg we have doctors that do that, they'll prescribe a bunch of things either hen your shift starts or when it's about to end, like REALLY?! I agree that days like that definitely make you stronger and build your character! :)

We learn the most when we make mistakes. Being a nurse is so unpredictable and some days are just harder than others. Just always think of your patients first and you'll be just fine. Good luck! :)

Yeah, we have a great tool at work that we plug in the meds with the maintenance IVF and it tells us everything that is compatible and not. I'm an ICU nurse, so giving a billion meds at once calls for reusing the line for the 3 days the tubing is good for is a must! :)

Hi! I am a junior in high school, and I'm very interested in becoming a nurse, but I want to be a psych nurse. Do you still go to school for nursing or phycology?

you go to regular nursing school and then find a job in psych :) or, I'm not sure if this is everywhere but I know they have some nursing programs that specialize in psych, but you'd have to look it up for some more info :)

Sammie Schmall no i mean work work haha, in quebec we can get a nursing license from a "college" type education (3 years) and then go on to do our bachelors in university if we want, which is what i'm currently doing🙂 we're the only place that works this way lol

Omg. Working in MRI I have million stories. I have the nut jobs who give me attitude when I tell them to change into a gown , if I have to do a needle stick more than one time , those who tell me they have had an implant for 20 years and will be fine in the MRI , or the crazy patients who just pee all over the MRI table. Ugh. So done with Radiology

They should have intubated your guy desaturating the moment he became obtunded.Like, how can one not be "unstable" enough?!

ZiMakeUp it’s very rare they intubate patients, which they only do in icu, unless they’re in major rest distress where they’re desatting on a 100% non rebreather.. they always try everything (lasix, full 02) before they take him down..

Justine G.Feather I’m curious how did he end up doing after? And that’s very true. We try to do the least to most invasive. But your patient wasn’t not saturating well on a venti and was obtunded. Was his ABG good too as well?

ZiMakeUp tbh this was so long ago i dont remember😩 i think the lasix did help and the new antibx must have helped.. but i was working every other weekend at that time so i didnt get to follow up

Justine G.Feather OH. My craziest story. I got my patient from ER who was on a NRB. The first thought I saw when I saw her...”uh, why does she look dead?” Granted she was 100% on NRB, she was not arousable to sternal rub and pain. I asked the Nurse Practioner if she wanted an ABG because she didn’t get another one for 7 hours after she was place on the NRB. Guess what? PH of 6.9 and so we call ER to intubate. Not only that, nurse from ER calls and says, oh. I’m sorry. I forgot to mention she has c.diff. OMGGG. I died on the inside. Not only that, her temp is like 34C. I basically had a dying patient. So I’m getting her settled after intubation and I’m assessing her and I watch her heart rate drop like 10 points. I told myself that’s interesting. I call my other coworker and was like, uh. Her heart rate just dropped. So we just keep watching and all of sudden she drops down to the 30s and I’m immediately at the HOB and I say SHIT and he asks if I can feel a pulse. NOPE. She’s in PEA so I immediately pull the code button and start performing CPR. As I’m performing CPR, poop and bile comes out of her mouth and I’m not gowned up. Again, I died more on the inside. I tell someone to cover for me so I can get a gown. We get her back but decided to code on me again on shift change. Wooo! Unfortunately she didn’t make it. May she RIP but that whole night I didn’t sit down and couldn’t wait to go home and bathe.
