OP feels like her husband is holding it hostage, yet she's holding affection hostage. It's probably a major turn off for him to think she's only being affectionate to get what she wants out of him and not because she actually wants to be affectionate with him out of love. You say your confidence is sh*t, but I can't imagine he feels particularly great either knowing you have no desire to show him how much you care for him. Bottom line is both of you get therapy or counselling, work on yourself to start giving affection more frequently without making it transactional, and get toys. Just because he has low drive for the actual act doesn't mean toys can't be used.
