Would love to hear more about modular medical! A low cost pump is a great idea, I’m curious what is on their roadmap in terms of automation, closed loop systems, etc.

Most interested in the omnipod - but I am waiting for integration with both G7 and iPhone (both of which I already use). Definitely want to try a waterproof patch pump but without needing a pdm 🤞🏻

When I got my first pump 20 years ago OmniPod wasn't yet available. Three of the 4 tubed pump companies that were available no longer exist, or at least not in the United States (not sure if Roche/Accu-Chek still makes pumps in Europe). Tandem & OmniPod both came out not long after I got my first Medtronic pump. It's exciting to see all of the options that we'll hopefully have soon. I'm not interested in the Mobi since it's still tubed and the size of my t:slim isn't an issue for me. In another 3 years when my warranty is up I hope to have a lot more to choose from, especially with the more customizable algorithms.

Everyone seems to be excited about the ability to use the Libre 2 Plus sensor with the Omnipod 5 and other insulin pumps but there is a big downside to this solution. Libre 2 Plus with the Omnipod 5 is now availble in the UK but it has a significant disadvantage over the Dexcom G6. If you are use Libre 2 Plus with the Omnipod 5 then you start the sensor using the Omnipod 5 PDM. That means that the only device that you can get rtCGM readings on is the Omnipod 5 PDM itself - there is no possibility of rtCGM readings to your phone, smartwatch or other apps using this combination. That is a huge black mark against using Libre 2 Plus with the Omnipod 5. If you're the type of patient that is happy with BG readings to just the one device then that is fine - but if you want to use your smartwatch, phone or other apps/devices as well then to me the Libre 2 Plus is a no-go.

This is SO TRUE and something that Abbott needs to figure out - the same can be said about the new and upcoming Libre 3 Plus. We need share/follow for all, otherwise it's hard for me to see anyone under 18 using it as an AID system when other options exist.

If a pump doesn’t have integration with a cgm, then it’s basically not worth it at any price. That’s like going back in time 20 years. I really want a pump that I can control from my phone. And esp clear alarms. I can bolus from my x2 on my phone but can’t clear alarms. It’s irritating. I have a job where I’m often in a clean room and I can’t take my pump out, but I can have a phone in a plastic envelope. The pods never worked for me because the tubing would come out and I couldn’t see the insertion site. Hoping the Mobi works out. My warrantee is up in July. Currently on my 6th pump. Type 1 for 37 years. Crazy how tech changes so much so fast

Wow. I honestly had no idea there were this many players in the insulin pump market. Great overview. I hope the insurance companies don't get to pick the ultimate winners of which pumps are available to us. Hard to imagine some of the smaller pump companies will be able to cut good enough deals with the insurance companies to get their pumps accessible to a large enough group of us. I'm just going to hold on to my Insulet stock for now, but don't think I'll buy more of it just yet.

Big companies do like to make deals with other big companies. The biggest companies have a lot of power in the big grocery stores too and it's hard for a newer/smaller player to compete. I know...I'm in the suburbs of Cincinnati, Ohio where Procter&Gamble has it's headquarters.

Good stuff! Started iAPS (soon to be Trio) a few weeks ago because of your discussions on it. It has absolutely changed my life. So, I'm interested in pump manufacturers who are going to open an API surface for DIY use. Thanks again for your work!

The clip from the MODD1 intrigues me, not because of the pump, but the infusion set. The set in the video clip looks a lot like the Ypsomed Orbit set, which has not been available in the US since Ypsomed bought the small manufacturer that created them here. I used to use their steel sets (Orbit Micro- 32 gauge is amazingly comfortable and the spinning connector made moving the pump easy) and was sad when they were bought by a company without any licensing in the US; regardless, nearly all pumps went to proprietary connectors, so it was only a matter of time anyway.

I'm on the Tandem x2 insulin pump with the G7 sensor. With my insurance I am limited on which pump I can go with. I really like the Tandem insulin pump. I am looking forward to the Tandem x3 insulin pump. From what I've seen it will be so much better than the x2.

This video is fantastic! A common issue with pump manufacturers is their approach to software development. They often prioritize Apple software for smartphones and watches, which seems unfair. I have a preference for Android platforms, yet they are typically the last to be integrated into their devices.

It's great to see that Medtronic is coming up with new pumps, but until they address the high frequency of inaccurate readings with their CGM's, Medtronic is just spinning their wheels and doing a "don't look behind the curtain routine" Their CGM's look just like they did in 2010, the year I started with Medtronic, yes, I know they have a new CGM, but it's not in the US

G6 with the iPhone app. I get good control with the G6 and Omnipod 5 but it’s a hassle to carry the PDF in my pocket. I already have the iPhone in my other pocket. I have read mixed reviews on the G7.

All are pretty cool, but... I'm clumsy and run into walls alot. I have to have a super thin set. Even the CGM on my arm is a hazard sometimes. It get rips off about 7 or 8 days into my session.

In my country unfortunately we only have Medtronic currently. It is so frustrating that they're not advancing with the other companies. One thing is the size of the pump!! I don't have issues withe the guardian accuracy for me it has been relatively close to my blood readings. Another thing is not able to use my smartphone to control my pump, if there was another option available in my country I would definitely change to one that is smaller and able to be controlled with a smartphone!!! keep up with the times Medtronic!! I feel sometimes they don't think of the user and what would be easier and useful for them

Being a medtronic user: IF the problems with the smartguard 4 will be fixed in the Simplera, then the 8 series pump will keep me with Medtronic for sure 😍👍 Especially because I’m very happy with the 780G system already 😉

The pump itself is great but certainly not the guardian sensors, those were the reason i switched over to dexcom G7. Hope Simplera Sync will do better

There is no ability to use galaxy watches with the app and I really think the no screen on new pump would put me off getting it because medtronic are so slow with there app updates and improvements for Android and starting to lose my respect

I've been on the Mobi a couple of months and I love it with just regular infusion sets, and have no desire to patch it on. I never liked how the Omnipod felt on me, and I patched the Mobi once and was like oh yeah, I don't like this, so I'm good with tubes. I would LOVE to be able to bolus from watch, though!

@@KarenRoseTank I sometimes miss having the screen, but the overall pros of it work for me. And even if you don't have the phone, the pump will still run your active profile and let you quick bolus, and will still run control IQ if it's connected to CGM. It's not like it stops working if the phone is off or missing or out of range. It's just so small and light and unobtrusive that you don't even feel like you're wearing a pump most of the time.

I do not understand the Niia making essentially making three tiers of pumps. To me insurance will have a field day with that and push everyone into the cheapest version of it. If you want any features it will have to come out of pocket.

Here's a note on Tandem for the community. If any of you were unfortunate enough to replace their Tandem pump at the end of 2023 with the promise of the Mobi upgrade in February at a small cost, they will be disappointed. Tandem is not honoring that upgrade path right now while they are shipping to new customers. I cannot get a commitment from them on when they will honor the program but their bait and switch tactics are frustrating at best.

I don’t know of any updates coming for it! Beyond the bi-hormonal pump, but that isn’t coming for years. I’ve got an interview on the podcast with Beta Bionics telling me everything about it. Coming out in 2 weeks.

Within the last week ILet entered into partnership with the company that will supply the glucagon for that future pump so seems like it is just a matter of when not if. I just started the beta bionic pump a week ago and so far I am pretty impressed with it. T1D for 36 years now and beta bionics is perfect pump for those that want something easy. Got tired of Omnipod repeated promises of an iPhone app. Who wants to carry a receiver. This is the technology age. Should have never released the pump before that was ready to go.

I love the idea of a tubeless pump but I am so insulin resistant that 200 or 300 units is like nothing for me. Probably 2 days max if I don't go to Starbucks or anything. I don't understand why they don't make a pod that you can pop off and refill for ~14 days. I would jump at something like that. As for Medtronic, I will never ever ever go back to them even if they gave me the refillable tubeless pump!

That’s a lot of insulin.Large doses of insulin is not good for your arteries and veins. Have you considered adding metformin or some other type 2 drugs to lower your insulin requirements. I’m 6’4 220 lbs and I use 40 units a day that’s on a low fat high carb mastering diabetes diet so it’s not low carb.

Do you think there will ever be a closed loop pump that will use a sensor like the Eversinse E3 that is 1 sensor that will last an entire year and is super accurate? I'd love to see a long use, super accurate sensor with a closed loop pump. PROBLEMS WITH MEDTRONIC 780G PUMPs and Guardian 4 CGMs As a medtronic pump user for about 20 years I have been EXTREMELY disappointed in the durability of medtronic pumps. I've had an average longevity of 1 pump per year in the last 4 years with the 780G for just normal active use as someone who enjoys hikes, mountains and beaches. Medtronic's old pumps were indestructible. The new pumps are absolute crap in durability and cannot be trusted or depended to function in a normal active life. They are made for people sitting in an office all day. I have almost been killed 3 times because of their inability to function in sustaining life by just the fact they unexpectedly cease to function because of critical error 35 that also make alarms go off for hours. It's complete trash in quality to deal with. The software has been crazy problematic with critical errors that make the pumps unusable and annoying alerts of high glucose that take you out of bolus that you are entering in when the alert appears forcing you to start all over. Great idea: maybe let the user give a bolus when glucose levels are rising instead of interrupting the bolus to tell them that glucose levels are rising and making them start the process all over again. The software has so many issues and is just not well designed for usability. Medtronic CGM's becoming less accurate with each new version. It is also a problem that medtronic sensors are deep and painful with only a 7 day longevity and with a 2 hour warm up that is way longer than the competition. CONCLUSION Medtronic's algorithm is still one of the best for closed loop use but all other aspects of medtronic pumps and CGMs seem to not be able to keep up with the competition. I have been greatly disappointed in them for the last 5 to 6 years. I'm not sure that they can be trusted in providing life sustaining equipment with such bad hardware and software issues. I'm currently using the 780G system with the Guardian 4 and it often does not always suspend insulin delivery when going hypoglycemic and the alarm drives me and my family crazy even after acknowledging every alert on the pump. Medtronic has a huge amount of forward movement and advancement in quality control to make to be trusted. Currently the minute I find a pump and cgm with a comparable algorithm and closed loop system I am likely to switch away from medtronic as they have been absolutely undependable in the one thing that matters most which is their continued function to deliver insulin when issues arise (critical error 35). I feel like with medtronic I'm paying for a Bugatti, Lamborghini, Pagani or Ferrari and receiving the quality of a Pinto or Yugo. It really is that bad. They need to make some major changes and improvements. I have not seen them make much of an effort to address user concerns. The new simplera addresses some issues but did not really advance much and is still behind much of its competition. Hopefully the new pump will move forward. Medtronic just needs to remember their purpose and to focus on producing a pump and CGM for their users. Medtronic has lost its focus. I hope they step up soon.

The new tech is nice, but the quality of the dibetechnicians is more important than the capability of the dibetechnology. WpEver clinical trial show the same result- every new tech tested since 1990 produces a 0.5% decrease in average A1C for the group - but the effects are not additive. 75% using the new tech do not achieve the AACE or ADA targets. 5% do "too well" with A1c below the diagnostic level for diabetes. The doctors of both those groups don't feel they are safe. Imo, if we spent 10% of what's spent on more expensive new tech into educating those of us who have diabetes instead, we'd get a better return on investment.

Twist pump sounds really good I also like the graphs and symbols on the screen where you can bolus 🍕 as such and reminds me of Tamagotchi or an apple mouse 🖱️ 🍏… That would be my choice if I wasn’t transferring to Omnipod 5 🤖📟
