Prometric Skills Demo for taking respirations on a client

According to Prometric guidelines for the nurse aide state exam, this is a step-by-step demonstration video for measuring and recording a resident’s respirations. This skill is one of the 21 testable skills on the Texas State CNA Exam. Remember to include indirect care with this skill. You MUST not tell the client during your test that you are taking their respirations. For this skill, you can state, “I am taking your pulse or vital signs”. This is a very important point.

I failed my exam today because I said end of skill before recording my results 😭 the instructor was extremely intimidating & my nerves got the best of me. It does make me sad that something like that can cause me to fail my entire exam but at least now I won’t forget that step next time.

Dr. Lee is a fantastic instructor! I just finished my 3-wk CNA classes with her and just recently passed the state written exam. Hope I'll pass the skills test, too. Hi Dr. Lee, I have a question. On test day would we be allowed to place a paper towel on our partner’s abdomen for taking respirations, like it was demonstrated in class? I’ve been practicing, binge watching your videos, reviewing my notes and counting down to my skills exam one week from today!! Are we allowed to touch the stomach or place the paper towel on stomach to count for this skill although they aren’t allowed to know we’re doing respirations or will that cause us to fail?

Hello Morgan, Yes, you are allowed to place a paper towel or touch the abdomen if you ask the patient for permission.

Hello Barakat, The only vital signs that Prometric will ask up to take are the pulse and respirations. The blood pressure and the temperature are not tested with Prometric.

I teach my students to buy a watch with a second hand and to keep the watch on while washing their hands. By the way, it's 20 seconds for the hand and wrist wash, then go to the fingers and under the nail beds.

Hello, You have to simulate handwashing before touching a patient and at the end of the skill. So, yes for this skill, handwashing is simulated twice.

Hello, If the evaluator asks you to take a respiration on a client, then you MUST take a respiration. If you take a pulse instead of the respiration you will NOT PASS the skill. Now, with that being said, pay close attention to my next comments. Prometric placed a note in the instructions for the respiration skill that says, DO NOT TELL THE CLIENT YOU ARE TAKING THE RESPIRTION. Therefore, I teach my students to tell the client when testing that you are there to take VITAL SIGNS. After saying that, the student should proceed to take the respirations. I hope this is clear. Hello, I can understand why this skill can be confusing. First, Prometric tells us in the instructions NOT to tell the patient that you are taking their respirations. Therefore, a candidate is allowed to state, "I am taking your pulse" or " I am taking your vital signs." This is in the instructions from the state. When you tell a patient that you are taking their respiration, the patient may change their breathing pattern and therefore, the healthcare worker will not have a true result. Hello Glendalyn, The normal respiration rate is between 12-20. The student will count the inhalation and exhalation as one breath.

Hello Tochukwu, The normal respiration rate for adults is between 12-20. The evaluator will take the respiration rate of the client with you. In order to pass this skill you MUST be within 2 numbers (Plus or Minus) from what the evaluator got. Therefore it does not matter if you got a normal or abnormal number. What matters is that you are close to the evaluator. I hope this makes sense. Take care.

You didn't do the respiration Dr. Lee. You did the pulse. You weren't looking at the patient's chest for the respiration.

I can look at the chest or the abdomen when counting the respiration. Both areas of the body will move with respiration. Some people will look at the shoulders moving. There is no rule that the healthcare worker has to look at the chest ONLY.

This video is incorrect. For respirations you have to look at your patient the whole time, this was partial apical pulse. I get that you don't tell the patient what you are doing on this skill but you can't count respirations looking back at the clock, that is impossible. Just my thoughts
