It seems that Holmes' disguises in this aren't that good - anyone who has met him before in one identity seems to recognize him in another. Maybe it has something to do with how he rarely changes his voice

Why does the doctor end with saying besides & doesn't seem to continue his sentence? Is that a form of speech common among British? Since American's seem to add to the sentence when it's used.
