Great video Tom! My T1D diagnosis was in 1963 and I have always tried to maintain tight control. Severe hypoglycemia unawareness since the late 1960s has resulted in loss of consciousness many times and caused several injuries, includung a fractured ankle. The t:slim X2 and Dexcom G6 with Control IQ have made a big difference, and I highly recommend both. During the last two years, A1Cs have ranged from 5.5% to 6.3%. Last A1C was 5.7% with 94% TIR, so my endo team, and I are pleased. Diabetes is still stressful, even with the best technology. Hang in there everyone, and remember that incremental improvements count.

This is really useful! After 20+ years type 1, I’ve gotten into some really bad mental habits such as feeling more comfortable running slightly higher as lows give me anxiety. I’m trying to retrain my brain to help me get back to better numbers! Your videos are the most relatable and informative I’ve seen on RU-vid!!

Hi, My mother has had Type 1 Diabetes for 65 years (from age 16 to 81) she is also considered brittle and has had her thyroid medically destroyed because of Grave's Disease and suffers from Other Tremors Undiagnosed. She's in good health now and looking forward to 82! She is 100% dedicated to a zero sugar and low carb diet and has checked her blood sugar 6 to 10 times a day for over 30 year! She is living proof that if you do the right things you can live a long healthy life with Type 1 Diabetes! Keep up the good work, 81 will be here before you know it!

Tom I love all the videos I have encountered so far. You definitely have a can do spirit and very good way to explain your journey. I thank you for that. I was diagnosed with d2 back in April 2021 this after 5 months being sick with things no one could put a label to prior to that. Needless to say I lost about 15 pounds a month and it fell off fast. I thought I was doing everything right I mean tir was nearly 98% A1c from 13.6 to 5.3 after more testing it turns out it’s T1D. And I am well over the hill and quarter down the dale. It can be done even with oldies like me.

Tom I totally agree with your comments regarding A1c being a poor indicator of overall quality of bG control. A1c tests are just another example of quick and easy Silver Bullet solutions to costly, complex, time consuming, thought processes the medical establishment would rather avoid. Now all the good doc has to do is glance at a single A1c number in order to instantly judge how well your diabetes is controlled! I have been a Type 1 insulin dependent diabetic for nearly 50 years and have been monitoring my bGs throughout the day since 1980. Over the last several decades my lowest HbA1c was 5.4 and my highest was 6.8. While the obvious winner here is the 5.4 it's useful to consider that the cost of maintaining that level was an average of about 360 bGs below 60 mg/dl a year! On a typical day during this period I would take between 15 and 20 fingerstick tests per day and my bG would swing wildly between 55 and 240 mg/dl. As you rightly point out in your video while this might well average out to a great A1c the wild swings were very damaging to my body. Initially I was very hesitant to utilize CGM technology due to negative comments I'd encountered about accuracy. However after watching all your Libre 2 videos I was very impressed with what you had to say and decided to give the Libre 2 a try. For the past 12 weeks I have done so. Overall I have been massively impressed with the Libre 2 system! Time after time after time my fingerstick readings have matched Libre 2 readings within 5 points with only an occasional divergence of more than 10 pts. Using the wealth of information provided by CGM I have gradually fine tuned my protocol in an effort to increase my time in range and decrease wild bG swings. As a result my A1c has crept up a bit from 6.2 to 6.4 but in the past 12 weeks I've achieved 91% of readings in the target range of 70-180 mg/dl, 8% in the 181-250 mg/dl range, and...drum roll please 1% in the 54-69 mg/dl range. According to my AGP Report my average bG is 130, my A1c is 6.4%, and my Glucose variability is 26.1%. I looked for the SD figure you mentioned but can't find one in any of the reports. Is the Glucose Variability (percent coefficient of variation) the same thing as SD and if so how do you convert one to the other? FWIW bolusing for extra carbs might look good in theory but in practice I found it to be little more than wishful thinking at best and an invitation to disaster at worst...at least for me.

Thanks for the video! It's easier said than done, totally understand that (esp. with icecream 😋). My a1c was lower some years ago, with lots of peaks and valleys. After I tried Libre I tried to reduce the fluctuations but then the a1c increased. Now I'm trying to stay longer in-range , as well as lower the a1c.

I find your videos very helpful for somebody just beginning with cgm measuring. My type 2 diabetes got out of control with an hba1c of 10 and my doctor subscribed me with the dexcom Sensor. Luckily my health insurance covers the costs for a year, so i can try to Come back to a better range.

Thank you for all your personal information you share with us. I am prediabetic and have been so for 20 years. I find my glucose testing in the mornings after 12 to 16 hours of fast to be 125 and up to the 150. Has this been your experience and if so, what have you personally done to lower the readings. My doctor told me it is directly due to my fasting. Thank you.

I’m type 1, are you ? We all have good and bad days, I’ve stopped worrying about it, it’s just how it is, even though you eat at the same time, same carb and insulin, same exercise etc etc but 1 day will be completely different on your BS readings. As I’ve learnt to tell myself “it’s just the change in the weather” But it could literally be that. I’ve been type1 10 years (in June 2023)

I’ve been type1 for 48 years. I do have a few too many peaks and troughs due mainly to disordered carb eating. I never ever get ketones. I’m going to take a closer look at my Libre data now. Glad I just discovered your channel xx

IMHO a CGM is the only practical way to get anywhere near decent control. Even if you do finger sticks every 30 minutes 24 hours a day those numbers won't necessarily be accurate due to the stress you'd be under.

Tom I am enjoying all your videos on type1 diabetes. I want to ask you. I have a Dexcom G6 and I have to put on the new one every 10 days at 1am in the morning which to say the least is so annoying as I have to stay awake until the time and I would rather be in bed and put on the new unit in the morning. How can I do this as the clock on the timer keeps buzzing to tell me that I need to replace the unit on my arm too early in the morning. Is there any way to replace the unit later in the morning say 9 or 10am or am I forever stuck doing this at 1am? I appreciate any help you can give me. Thank you so much Karen

Tom, Congratulations on your 30,000 viewer mark---that's great news! I have a question: how soon in advance do you take your insulin before eating? I have those spikes you talk about and I usually take my insulin just before I eat. Do you take your insulin an hour before or several minutes before you chow down? Hi Leonard. Thank you so much. It depends on what kind of food I eat. I don''t prebolus for all kinds. On average I would say 15 mins is my rule of thumb. But I have a separate video on pre-bolusing I made some time ago. You might want to check that one! Good luck!

I'm a T2D, and I set my range to 70-120 mg/dl. I stay with that range 99% of the time. I think that A1c is an important metric, but it one of many that need to be looked at. My A1c is 5.0 - 5.2% now. I'm trying to get the blood sugars to maintain a range of 60-100 and an A1c below 5%. This is proving to be more challenging than I thought.

I have been testing on non-diabetic people, and during the day, if they do eat, it can actually get as high as 9 for them also. But then again, it does not go over 10 as far as I have seen. Good video :) I don't count carbs, but I have a "feeling" when I think of how much I normally take and use that as a base to balance it out. Still works for me :)

Great video. This put into words some of the thoughts I have been having recently. I just got my D2 diagnosis a few months ago and it has been hard getting up to speed with so much to learn. Some questions I haven't really found a good answer for; 1) Is a spike that is higher in sugar but shorter duration better or worse then a spike that is lower in sugar but longer in duration? 2) Do spikes matter much if it is in range? I found your vids a few weeks ago when I got the Freestyle 2 and wanted a vid to apply it. As an aside, I am in the US I kind of regret getting the Freestyle 2. It doesn't have a phone app in the US for some reason. Anyway thanks for all the great content.

Thank you my friend! I really hope they will make the app available soon. It has been frustrating so many US users for so long now.... I am not a medical professional but in my opinion the worst scenario is big spikes with long duration. Second worst bog spikes with short duration. Third worst small spikes with long duration. And then small spikes (within range) with short duration and/or no spikes. These last two I consider ideal when I look at my graphs:) Just to clarify, I am type 1 insulin dependant so I can control the spikes by taking insulin which I am not sure if that is your case as well. @@TypeOneTalks In Abbott's defense, I don't think they can do anything more than they already have about the app for Libre 2 in the US. Our counterpart to EMA, the US Food and Drug Administration, is taking forever to review/approve Abbott's request.

I intermittent fast, so I sometimes get large increases in glucose right after I break my fast. I worry more about how long it takes the peaks to get back down to a more normal level. I also found, with the CGM Libre 2 system, I have "Dawn Phenomenon," where my glucose is high right before/after I wake up. BTW: Thanks to your Libre2 video, I asked my doctor to prescribe it for me. I have been using it for 2 weeks and have found many foods I thought were "safe," but which were increasing my glucose by quite a bit.

Hi Pat. It is not in the LibreLink app but I believe it might be available in Libre View app as I mentioned in the video. Have you checked this one? I am using Dexcom these days so I did not check lately for Libre. Hope this helps- Have a great day!

Thank you for a such an informative video. I just want to share something I discovered a few days ago. I was looking at my graphs at tidepool and tried figuring out what I could get out of them. I can see my 1, 2 and 4 weeks average there AND the weekdays! My weird pattern is that my sugars is best on Mondays and then gradually gets higher until it peaks on Saturdays. It might not be weird since we eat Taco on Fridays or Saturdays and on Mondays all the "carby" food is gone. It would be nice to find out what more there is to discover on other sites or apps.

Another great video and great advice for those that haven't studied statistics :) Always look at the trend over time - NOT one day. I love your videos - just moved up to Libre 2 and the alarms in the night when they happen make a big difference to time in target. Libre 2 and my new TSLim are such a different world to a metal and glass syringe and only knowing what your blood sugar sort of was yesterday with a clinitest test tablet in a test tube of urine I used in 1978! With the Libre 1 I managed to get to 80% long run time in target - with Libre 2 I'm aiming for 90%. 85% so far. What always messes up the stats is those infusion sets that take FOREVER to start working and then you go from a high to a mega low. Still, after 43 years I guess parts of my stomach find it hard to be an efficient infusion system but then they are never obvious.

I remember Clinitest and Tes-Tape days from 1963-1980, and recall that the test tube sometimes got so hot I had to hold it with a cloth to prevent finger burns. Did you use the 5-drop or 10-drop method lol?

Great video Tom. Absolutely agree with your take on HbA1c results.. I relied on it to gauge how well I was managing my blood sugars, but recently I've focused on time in range instead. I've been 98% TIR lately with less than 2% SD.. Gave myself a pat on the back for a job well done! 😁😅

Hey, thanx for The video about Libre sites! Myself and my bf that is type 2 found it really useful. The leg site is the best 👏🏼 As a diabetic myself for 24 years (proud and counting) 😂💪🏼 I would like to know how do you keep on going through the bad days (or maybe weeks) and deal with the burnouts 🙏🏼 Sometimes I just go on, relying on corrections, but sometimes it’s just..too overwhelming to have four-five lows in a row ... esp when I study or exercise

To explain my posting in the chat: I aim [but often fail] to keep bg between 4.2 and 6. I have Temporal arteritis that threatens eyesight & hearing. Keeping bg under control helps keep it under control. Having a 'gun' held to my head is a great incentive! How do I get this graph which tells me at what time it goes high, I do hba1c it only shows me a reading between 1-10 in my state. Is it the free style libre or something else to get such graph?

I am going to my daughter's wedding next week and there will be carrot cake. I LOVE carrot cake but never eat it. I will be eating...a little. How do yall feel about going off the reservation or falling off the low carb wagon for special events. I am type 1.5 so I only take long acting insulin. Maybe I should get some fast-acting for those events. Otherwise, I can always abstain like I normally do. I am in range 80-90 percent of the time thanks to this youtube channel. Love the videos!

Thank you for sharing. I go off low carb from time to time (not only special occasions but when my body asks for it). It's more like I tend to have a couple weeks here and there where I eat more carbs and then I go back. Sometimes it has to do with climate. In hot weather my body always wants carbs... Not sure why:)

@@TypeOneTalks Your body wants more carbs in hot weather, because it spends/losts a lot more carbs during the day. From my point of view, it is best to adapt the diabete to your life instead of adapting your life to the diabete. More or less... because of course you can't spend your day eating carbs without taking insulin :p But "low carb" or "no carb" diets are easier to regulate blood sugar carb but nor healthier nor fun to follow.

Hello Tom, Thanks again for another interesting and very useful video!! Just one little observation on the discount products you mention. They seem all to be shipped from the US or China. With the upcoming new rules starting July 2021 items shipped to the EU will be taxed from the 1st euro and thus delayed : I believe you live in Switzerland , I have friends there who already experience problems of delays.Do you have have distributors in Europe ? May I mention the equivalence of the range you talked about : maximum preferred 10 mmol= 180 mg/l and minimum 3.9 mmol= 70 mg/l . In fact there is an android app is use its called Blood Sugar converter.

Thank you for sharing Paul. I was not aware of the new rule. Yes, these products ship from US and China. I do not work with any distributors. From my experience local distributors of MiaoMiao for example charge significnat mark-up which might be higher than the tax/custom duty so not sure if this would help in terms of price (maybe as you mentioned it could help with speed of delivery but I am not sure). As long as we stay grey we’re good. I personally try to stay low carb for the most part as it’s less complicated but as soon as I eat carbs and inject Fiasp things can go a little wild, just a little wild. Not terribly high or low just raised/dippy so I find not complicating things in the first place serves me well, 96% (DG6) over the last 90 days isn’t too shabby!

Love your videos. My main issue I‘d love to discuss is managing my diabetes „in front of people“. I think it‘s difficult to bring this topic up. I am not ashamed of my diabetes, however I prefer to go to the bathroom for taking insulin in most cases.

I feel weird about it too. People looking at me lifting clothing etc. I have an ex-friend who told me I wasn't aloud to take insulin around her because she didn't like needles or the smell. She wanted me to take it later when she wasn't around. Sometimes it's just easier/more comfortable to take insulin in the bathroom. However I try not to do that too often bc diabetics shouldn't have to hide to make others comfortable.

It's not always easy indeed. I have no problems discussing diabetes and insulin at home, taking it in front of my kids etc, but even at my in-laws I feel weird about it, even though my mother in law is diabetic and takes insulin too. It does feel too intimate. Outside of home I just take it in the bathroom unless I'm completely alone or the bathroom look like it might be a bad idea, hygiene-wise)

In 1992, I visited my parents in Florida and had to board Peg, my elderly cat, at a veterinary clinic. Peg developed diabetes symptoms about 3 weeks before the vacation. The vet called on a Monday morning and said that Peg was in a diabetic coma with a BG of 538 mg/dL. Peg also had a chronic gastrointestinal inflammation that required taking Prednisolone, a steroid, so I made the sad decision to have her euthanized. On the flight home to Chicago, I took a pre-meal bolus in my seat, rather than getting up to use the restroom, and noticed the elderly woman sitting next to me staring. I explained that I have Type 1 diabetes and take insulin before meals, and added that my cat had recently developed diabetes and that earlier in the week I had to have her euthanized. The woman asked if Peg caught diabetes from me. I explained that diabetes is not contagious and that cats cannot catch diabetes from their owners. Can you imagine?

I spend my phone teen years going to the toilet… I’m on a pump now so it’s easier but when I became an adult I thought why the hell should I go to the bathroom, and I started to dose discreetly under the table into my leg (I learnt that you could do it through trousers!!!) ignore comments from others, you’re a hero, you’re saving your own life!!

IF ANY ONE KNOWS an alternate or a cheaper CGM i would be interested-- i was hoping the Apple Watch would be my SOLUTION- but it seems not this year!!! MediCare does not cover a CGM AND i can not stand PRICKING my fingers MORE THAN 1x or 2x per day!! i know the CGM info would help a lot towards my eating issue but $500++ is just too expensive!!! as a pre-type2 i am testing in the mornings around 115 to 125 mg/dl-- so like you i want to steer away from type2 BEFORE i am too deep!! Junk food/SUGAR is my biggest problem!! GREAT Video/Info as usual- i really appreciate the bookmarks- although- i usually watch the entire video the 1st time i view it- BUT when i go back to review certain topics i use the bookmarks! -

Up until I went on pump, I was always able to hold my HbA1c at a reasonable 6.8 over last 37yrs, with the peaks and valley method. The only reason I ever really cared about it was to avoid a lecture from a Dr.(s) that lacked an understanding of the difference between reality and what the HbA1c was indicating. The unfortunate side of the peak/valley approach is I became tolerant of extreme lows. I don’t notice until dips into 40s or just “LOW” on CGM. Probably missing a large number of 🧠 cells because of it. The pump/CGM/IQcontrol has now made the HbA1c much more representative and relevant of my good BG control and is great for plotting beautiful small deviation graphs in range.

Same for me. When I finally got my hba1c down to 7 my endocrinologist was so happy with me, while I was crying because all I had was extreme high and lows all the time. The medtronic cgm had alarms all the time and it stressed me out so much, I over compensated just to make it shut up! The change was dramatic after the change to dexcom.

Yes! I'm 20 years with type one. My HbA1c was awlays on the good side, the highest 9%......but i get used to very, very low sugars......and I lose not only many brain cells but many friends too. I'm a dentist now I have two kids I can't do this anymore, i'll try a sensor for the first time and i hope no more low, low, low on my glucometer!

@@Abidosss it will definitely bring better control and less lows as it did for me. Unfortunately, I still can’t sense when I’m going low unless I notice it in my vision. Hopefully you won’t get so many lows and get back that ability to sense the lows, when they do happen. Good luck.

Hi Bill. I think I confused you because my TIR and SD numbers were in mmo/l. 1 mmol/l equals 18 mg/dl so 27 mg/dl would be 1.5 mmol/l which is great SD (better than mine in the past 90 days). Hope this helps! Great to hear about your 100% days btw. Keep it up!

Hi don’t want one to one but ice cream is a big no no for me and frozen yogurt is even worse as it’s very high in sugar. I also have had a big delayed high with the yogurt when it had corn syrup instead of sugar. I wish manufacturers were forced to mark this clearly. Good luck will it all. I have HBA1C of 40 (5.8) - I am very happy with my control. I am also between 92 and 100% in range constantly. I have to use a lot of long acting and short acting too (Total of 150 units per day) but, I'm happy with everything. I also prebolus by one hour at least to take account of food and drinks.

Another great video But....Although keeping time in range and the variation both reflect a good blood glucose control..., nearly all the studies done showing they are not related to long term complications ... Only They found the A1c is the indicator that is related to complication

I try to get as close to 70 as I can at night when I'm sleeping. That way you get good control for a 1/3 of the day, and when your sleeping you don't need higher blood sugar for energy.

Try to go through the reports in Libre view on your computer. I need to double check but I believe it is there somewhere. In LibreLink app another indirect indicator of SD is how wide the blue range is that is bounding the black line in the “daily patterns” tab. Hope this helps!

don’t think SD is available as such. If you use the LibreView app on a phone, all you have is the blue shading on the Daily Pattern graph as an indication of variability. If you log in to your LibreView.com account and look at the AGP graph, it shows: Average glucose e.g. 7.6 mmol/L Glucose management indicator e.g. 6.6% (which is an estimate for your HBa1c) Glucose variability defined as percent coefficient of variation (%CV); target

I was under the impression that the HbA1c result was an ‘average’ of your blood/glucose levels over a three month period, as defined by a test using the red blood cells… If your HbA1c result was 6, this would indicate that the highest point would be around 12, and the lowest around 0, surely…showing obviously that you had peaks and troughs….the same result would also occur, theoretically, with someone who kept their levels between 3 and 9… As the average is going to be roughly around the middle of what your peaks and troughs have been… I ma6 of course be completely wrong, but it would bear put your opinion that the degree of peaks and troughs don’t really make that much difference… my own, having had illness and covid following that, when they were all over the place, was 7.1…

My A1C was 8 points a year and a 1/2 ago And my PCP never told me what normal blood sugars were supposed to be or even what keto asidosis was when I was in the hospital Practically in a coma ! Thank God I found doctor Richard berntein on RU-vid! He has been a type one diabetic for 70 years and still going with normal blood sugars of 83 mg per desoletor. My blood sugars were around 250 they were going up-and-down all the time and I looked like death warmed over. Now my A1C is 4.6 And my meter is set for 75 low and not over 120 high. Doctor Richard Bernstein is a miracle!

If you eat a burger with a bread bun(carb) your Blood sugar will rise faster than the insulin you inject can catch up so will lead to spikes, not a big deal as long as it is not sustained at the high level. Carbs are what makes it hard to keep stable.

I agree with you that insulin puts the visceral abdomen fat on but your not curing diabetes your putting in remission by not eating at all and who wants to go a week without eating by fasting? No one in their right mind. I'm on mounjaro and it's basically taken away my appetite to the point my sugars been excellent and I'm off the insulin which if your correct I'll soon lose the visceral fat, the metabolic syndrome, and high blood sugars. The fact is though I haven't cured my diabetes I've put it in remission and you as a doctor should realize that if I again begin eating like I was that initiated the diabetes I'll be back to square one where my sugar levels increase significantly and I again will have the high sugar levels that you know are damaging themselves to your body. The point is I haven't been cured and you know it. I'm in remission as if I was cured my sugar levels wouldn't again rise when I increased my food intake.
