This new CGM is not only capable of monitoring glucose levels, but also a second vital parameter, that could potentially save your life. And while theres many other CGM's on the market like the Dexcom G6, G7, Medtronic Guardian, and Eversense E3, no other CGM on the market has a feature quite like this.

As a very brittle Type 1 who test ketones 10 times daily and a person living with mitochondrial disease who has suffered from lactic acidosis I am beyond happy to hear that my dreams of what I’ve been saying to my Endo for years are coming true A CKM continuous ketone monitor and Lactic acid monitor are coming true! This could not only help me beat off ketones but also prevent mito crisis induced lactic acidosis.

Thanks for providing the news on Abbott's upcoming CGMs. As a longtime Freestyle Libre user along with a Medtronic insulin pump, I recently switched to the Dexcom 6 and Omnipod 5 to achieve the imperative blood sugar/insulin compatibility. The great news about Abbott's prospective Libre 4 is not just its ability to measure ketones but also its connectivity with Omnipod and T-Slim.

Great news and tech advances. It is a shame that many of these devices are only available for the US and European countries and for patients living in LATAM are not available. Best case scenario is taking 3-4 additional years to be launched in this region. Right now, we are still on Freestyle ONE in LATAM and with very low probability to even get the TWO.

Continuous insulin monitor and continuous glucagone monitor would be even more important to me.... The only problem I have with Abbot and it's "Freestyle Libre" is that from the version 2 upwards it is impossible to get the data on linux. It is only possible with an official Abbot's application which according to what is known puts the data into their "cloud". I don't want any big pharma company to own my medical data. So I now use occasionaly old FS libre 1 reader with the newest firmware coupled with FS Libre 2 sensor to be able to get all my data without sharing it with Abboth or anyone else...

BTW, I expect the FreeStyle Libre 4 to be have some stupid alarm with even slightly elevated ketone levels which would make it completely unusable for people on ketogenic diets / people doing prolonged fasting or even intermittend fasting combined with vigorous excercise - in all those cases ketonaemia is desired and not something to be "alarmed" about... Even FS Libre 1 complains about ketone levels if these are only very slightly elevated ("> 0.4 mmol/l", I think). It is just that Abbot makes money out of diabetics and they definitely don't want to see people on the ketogenic diet...

Hi, I am a 61 year old non diabetic who has been in Natural Ketosis for 3.5 years. In the first 4 months I easily reduced my bmi from 33 to 22. I used a freestyle libre 1 cgm for 2 two week periods to understand how my glucose levels were affected by food and exercise and took daily finger prick and regular keto breath levels. A freestyle 4 would give a much better understanding of peoples metabolism. The treatment potential for the obesity diseases, types 2 and 3 diabetes(dementia), for people to stay in ketosis is a huge market. a combined alcohol level reading would also help those who want to try it and continue to drink in moderation. Doctors would also end up understanding how each patient was doing without relying on their patients verbal answers. Diabetes type 1 individuals using less than 20 carbs per day and geting their carbs through the gluconeogenesis of protein, thus substantially reducing their injected insuline to maybe a 10th of their current dosages, would be able to gain control with much less stress. A type 1 diabetic who is fat adapted can have their brain fuelled by ketones when their blood sugar goes low. preventing them becoming unconcious and allowing them to fix their own glucose lows. The highest Natural ketone blood level that I measured was 7.1 mmol/L, well below the 15 to 24 mmol/L that only type 1 diabetics can reach. my concern with the Libre 4 woul be in the software being set to not allow this kind of value to be reached without alarms being forced to be reached. A step in the right direction by abbott - next step is to educate doctors and other health professionals in nutrition and human metabolics. Hopefully big pharna, the medical establishment, and food industry will not sabotage this step in the right direction. Thanks Rob in Sweden

On listening to this clip, those were my thoughts also. It's useful to know one's ketone levels but this device would be disastrous if was going to frighten you off getting into ketosis by raising alarms when ketone levels were well below those reached in ketoacidosis. As long as the device is sensible about where the alarms are set, then its market appeal will go way beyond just diabetics. It will be attractive to many millions attempting to improve their health through a low carb diet.

This is amazing. I want the dual glucose/ketone monitor right now. I have been using Freestyle Libre2 for two years, and the addition of ketone monitoring would help me when I fast. I have tried urine test strips for ketones, but they didn't work for me. I am not excreting ketones in the urine when I fast. I did not bother buying ketone test strips for blood drops, because of the expense. I wonder if the Abbot Labs dual monitor will be more expensive than the current glucose only monitors.

Hopefully things will change by the time it comes out. Many insurance companies are starting to cover these continuous glucose monitors as they have realized the benefit they have in improving glucose levels.

Yes acidosis should be added, an A1C should be added, If the freestyle libre 3 hasn't had the shortcomings fixed, what makes us think they're going to do the right thing on the freestyle libre 4, everybody knows about the shortcomings of the freestyle libre 3, I know we need as many features as we can get, and when the four comes out we don't want to see no shirt Cummings that we got to wait for, we'll have to see.

That's important news: ketones, and +++ the ability for the new Freestyle to give glucose levels informations to your insulin pomp, if it matchs. I like my Medtronic 780, but I think I will have to change for Omnipod or Tandem. We will see, maybe by the end of this year, depending on FDA and insurance aproval.

If its possible to detect ketone bodies via libre, why isn't it yet possible to measure insulin levels? It would be great to be able to monitor insulin level to detect early insulin resistance. BTW is it a viable option to perform a OGTT via libre? Is it accurate/sensitive enough?

Great information! Researching deeper I’ve learned there may be differences with resulting readings being less accurate then conventional measuring methods. Likewise, it’s important to realize what overall reading accuracies are expected from the new system. They don’t seem to be absolutely correct and need to be calibrated. Once calibrated the readings may be understood as close to accurate maybe within a certain percentage of true accurate reading. This usually is acceptable if user knows what to expect. Currently Only real system use will show these issues and doesn’t appear to be published.

I guess future is already here, I use the real-time CGM on watch without the need of a phone. I have been utilizing Ambrosia's real-time CGM system, which incorporates the FreeStyle Libre sensor along with the BluCon and LinkBluCon applications. This regimen has been instrumental in my weight loss journey, aiding me in shedding 15 lb within a span of 30 days. I make use of the app's food insights feature to monitor the impact of various foods on my glucose levels. This information allows me to minimize consumption of foods that cause significant glucose spikes. Moreover, it's proving to be a valuable tool in my practice of intermittent fasting.

I get my libre 2 free from NHS. I wish it was insulin pump compatible as I would definitely use one of those, that said I wish the NHS would issue the NFC insulin cartridge that sent dose info straight to the libre app

Brother was recently diagnosed with diabetes. He’s pricking his fingers but gets 2 different reading where one finger will say 99 and the other will say 110. How do you know which one is correct???

Does he have low heamatocrite/heamoglobin? There are some glucometers which measure Hb/Hct and adjust the glucose readings. With each glucometer you will find in the manual which blood parameter ranges these work realiably for. He should look for his bloodwork if any of these are too low or too high. If unsure, just go to the lab / your doctor and take your glucometers with you and let your doctor / your lab check your blood glucose in a real lab and take your samples from your glucoemeters at the same time to see which comes more to the actual result from your lab. Also, you need to assure that you really get blood and not the intrastitial fluid - so there are ways to push fresh blood to the fingers and also you take take the blood sample from the 4th or 5th drop of blood and not the first one.

Abbott CGM progress and development is great news but their shortcomings is keeping up with Apple IOS which punishes IPhone users because they are forced to use old Apple operating systems to stay connected to existing Free style, libre CGM’s.

Too bad Abbott and other CGM developers didn't consider the thousands of blind diabetics when they designed their CGM and the Reader that comes with it. "Line up the marks", and the Reader does not have audio output, which makes it inaccessible to the blind, which I consider inexcusable. My watch speaks to me, my alarm clocks speak to me, my HAM radios speak to me, my smart TV and cell phones speak to me, etc, yet my CGM, which is critical to my health, DOES NOT speak to me?? There is no excuse with the modern speech technology that is out there, and so affordable to implement too. You can purchase a talking watch for under $10 for example. I do own a cell phone, but unfortunately it will not work with the Libre 2, and I don't plan to go out and spend hundreds of dollars for a new cell phone that will work with the Libre CGM. To reiterate, the Reader should speak and be accessible to the blind, dyslexic, etc. Sad, and again, inexcusable in my opinion.

Am I missing something? As far as I was aware, ketones would only be a problem if you had high blood glucose values for long periods. If you are using a continuous blood glucose monitor - or even finger-pricking - you would be aware of these high values and adjust accordingly. The "Ground Breaking New CGM" you describe strikes me as a solution in search of a problem. There are better things that Abbott can do than this.

My worry is non-diabetics consuming all the supply of CGMs as they start monitoring more and more functions of the body. We’ve seen this with some insulin.

Long time ago I followed a low carbohydrate diet ( lost 25> kg), If I remember correctly the objective was to get into a state of elevated ketose .. this new device will help people to measure their levels of ketones. As soon as the market finds this option people on a lowcarbohydrate diet …. the availabillity for diabetics might drop (unless the production capacity is significantly increased)

I imagine that this will be helpful for some T1D's. For me however this will do nothing at all as I have during me 20 years as T1D i have never had any problems with ketones.

Agree. I monitor my blood sugar all day and night. Recently with CGMS of course. But before that I manually tested my blood sugar 10 times a day. And night. And if my blood sugar was ever high, I corrected it immediately. Never ever let my blood sugar stay high for days or even hours. So never ketones for me. This seems to be a product for diabetics who are in poor poor control of their blood sugars. In the long run they would be much better off keeping blood sugars under control at all times and then they would not need to worry about ketones. Kind of like those surgeries where they staple a person's stomach so it is smaller and they cannot eat as much to allow them to lose weight. Why not avoid the stomach stapling by eating less and losing weight.

I don't consider it as a big plus - a CGM itselfe is already a very nice indicator for possible ketones, therefore the effort for ketone-sensing isn't that necessary in my oppinion.

I realized the libre 2 sensors got much better about a year ago. Before that, If I was going for a prolonged water-fast, after about 4~5 days of fasting it basically showed "low glucose" / 40mg/dl all the time which was not true - my levels changed between 40-70mg/dl. Since the last year there is a real improvement and FSL2 sensors actually measure something much more meaningfull, although It definitely is not THAT accurate. But it is a generall problem with FSL that it actually does too many "adjustments" that you cannot trust it really much...

I used the Freestyle Libre 14 day. I used 4 out of 6 boxes prescribed to me and paid by BlueCross. I had high hopes after listening to these promotion videos. My experience is that it was very inaccurate and worthless. They were on the average 30 to 50 points off all of the time. Now I hear others saying the same. What a rip off they are. You know that 4 grams of carbs = 1 teaspoon of sugar. Do the math and save your money. I hope in the future they get better. Good luck.

I wish they would make it little less expensive so peasants like me can afford it. I have type 2 and take Jardiance and Levemir and United Health Care won’t approve it. BTW UHC’s stock price before ACA (Obama care) was around $28 and after ACA was implemented it is over $500, go figure.
