This was one of the best podcasts! Dr. Gersh hit the nail on the head with the Biest.. I tried it 2 different times, and my estradiol went back to nothing. I'm post menopauseal .. I guess Dr's don't know how to use it properly to compensate. Thanks to all!

Prefer Dr Gersh's approach to the H deficiency- to mimic what we see when we were younger in cyclic- that periods are a vital sign for women- do not try to micromanage the system-- appreciate all the health professionals here trying to help women who have been abandoned by the health care system- like myself

This was a much needed discussion. It’s astounding to me how differently doctors practice hormone therapy. So many different approaches! It was people like these docs who were practicing hormone replacement when no one else would after the WHI. I appreciate how everyone was so respectful to each other. Thank you Karen for this. And thank you to all the physicians. I learned a lot.

Wonderful discussion, I learned a lot. Thank you all so very much 💛🤗xx The points raised on the difference between alleviating vasomotor symptoms, and therapeutic protective doses, oh my goodness. It's a struggle to get on any hormones in the UK NHS, doctors want you on the smallest dose, and then try any excuse to stop prescribing HRT. There's a hysterical panic from any related bleeding, clogging up the hospitals with urgent hysteroscopies. Is it because doctors don't know what hormones do in women's bodies, nor do they seem to want to know? Underdosing women makes doctors feel better, but it isn't stopping the big diseases is it? And if these low doses are pro rather than anti inflammatory, that's worrying me as someone with autoimmune conditions already. We have Dr Louise Newson here, she's pretty much the only one putting herself out there for women's hormone health, and was berated a few days ago in the major Press as a hormone evangelist, some kind of pied piper leading women to terrible outcomes. This is British establishment, including the medical establishment, for you. The great news is we can at least get hormones if we beg for them, but symptom control is the limit of prescribing, health outcomes aren't on the doctors radar here. Yet. Thank you again 😊xx

In my 10 years since starting my own stage changes of Life and my hormone education, the two top doctors I absolutely prefer include my own hormone doctor and Dr. Gersh. They are on the same page for so many reasons. My science mind, inquisitiveness, and love of learning, has grown my database of knowledge. Understanding the body and how everything works together and has effects everywhere, makes it so much easier to look at BHRT. It’s all fascinating and my different stages of life Perimenopause , Menopause, and Post Menopause, have been wonderful. Life is great! ❤

Dr Taylor (Menopause Taylor on here) says 0.5 is the right amount. She has a charts on her vids and site for each form of delivery as well.

Dr gersh says physiological dose in post menopausal women should be at or above 100 picogram per ml. This is GENERALLY achieved by 0.1 mg transdermal estradiol patch biweekly. Dr Gersh also has a chart of how to administer both estradiol and micronized progesterone by a presentation, for providers and cost $200. To access it. Hope this helps

A different doctor who did not participate in this discussion. She also goes by the name menopause Barbie. She presents as a kind of goofy person. But her information is fabulous.

Very informative and so refreshing to see the different views but the unified front on enough estradiol. I've never had success trying for years with Bi-est as it always seemed to tank my estradiol both in testing and symptomatically but maybe 80/20 wasn't the right dose. I've had a uterine ablation so I don't know how I'd ever do a cyclic HRT and no one has ever been able to tell me that. But thank you all!!!!

I need HRT for dummies. I didn't understand a lot of this. I'm post menopausal and body is falling apart, A1C is creeping up and I've gained 40 pounds in 3.5 years. I've been low carb, high protein with IF for 7 years. I lost 60 pounds doing that and now I can't stop the weight gain

The first time I listed to a presentation that is dense with complex medical principles, I listen like it’s a song in the background. Then I will listen to it again a little more closely. I listen to this one, three times to make sure I understood the different types of estrogen, the receptor sites affected, the various ways to monitor effectiveness of the therapy… all of the details. I feel good about it now. I have absorbed the information.

I was on 80% Estriol and my body started to decline I was feeling horrible I asking to my provider to change to Estradiol game change I’m feeling much better also my face skin is much more firm!

Cream or gel.. you don't take Estradiol orally. And if you know how to use the patch properly then use it. You need to do way more research. I'd advise any woman to do a lot of research.

Thank you all for the invaluable information! You mentioned doses of E2 that are therapeutic and not only symptom-alleviated focused, what E2 levels do you like to see? I realize this differs from woman to woman, but is there a minimum level that is necessary for bone,brain and overall protection? My level, after 2 yrs on a .1 mg patch, is 40. This seems low.

This conversation is so important and has been needed for a long time. what an interesting group of presenters. great idea. I will need to listen to this information a few times. I am an RN, still working out of necessity @ 71y/o. I feel like crap. After 6 months/2 cycles of estrogen/testosterone pellets /c oral microlized progesterone, I stopped due to an Atrial arrhythmia that looks like a heart block. Now I feel like I am in menopause. GRRRrrrrrrrr. The practitioners in my area don’t believe that women my age need hormone replacement therapy.

@@123STEP23 absolutely true I am never using a product that I cannot remove or stop using immediately. I may have to wait 72 hours for the product to eliminate. I have friends who are using estradiol patches and oral micronized progesterone without complaint. However, nobody I know is being monitored for their hormone levels. I can’t even find a practitioner in my area who will prescribe HRT. Supposedly I don’t need it at my age. I don’t need my brain? Remember that I’m a nurse and this really winds me up

I would suggest that you see if you can find a compounding pharmacist in your area that knows a lot about hormones. I found her through the North American Menopausal Society, which has a list of doctors that you can find in your area. She isn't a doctor but she is very knowledgeable, Iike Dr. Gersh and is helping me with my hormones.

Thank you. This is a great time in history is right!!! Brilliant minds caring for the forgotten gender. Science is progressing. We need help. Finally at 57 on HRT. I learned a lot and will have dose pushed past symptom relief. I have BRCA2 mutation had breast cancer 15 years ago and have been living a deplorable existence since. I want to feel balanced again I have no idea who I am right now. Hope I get there quickly. Interested in the immune cell response- interestingly I’ve been on HRT 2 weeks and I have a bad cold maybe covid idk. Haven’t been sick in a while wondering if there’s a correlation.

I'd rather be dead than live like that. There is no excuse to not be on hormones. Especially the body identicals. And your nutrition is key and proper excercise. The amount of malpractice and down right abuse dolled out on women is mind blowing. Under the auspices of cancer treatment. Remember that saying "the cure is worse than the disease ".

The only Estrogen you take is Estradiol. 1.mg.. everything else is useless or toxic if highly synthetic.

Is it possible that the difference in estriol levels in the urine of healthy young women vs. women w breast cancer could reflect less excretion in the women w breast CA and the inability or difficulty excreting estriol (therefore lower levels in the urine) somehow contributed to the disease state?

I like your thinking. That is a good question. I found it odd that they would extrapolate that lower levels of estriol in the urine of women with breast cancer, resulted clinically, to giving estriol in HRT formulas. I don't think they proved what mechanism was at play there. Why was estriol lower in women with breast cancer? Was it being used by the body to fight the cancer? Lots of questions....so much research needs to be done. But to immediately translate into the clinical application does not seem evidence based to me. I think the new research on estriol is very interesting as a therapeutic agent. Again so much more research needs to be done. Do we need to get our PhDs and do the research @lauraadamowski9186? 😃

I’m a menopausal woman 62 yrs old. Did a saliva test my estriol E3 was a lot higher then my Estridiol E2. I had a surgical hysterectomy in my early 40’s. Why do you think this is? 34:25

Your Estriol is higher.. that makes no sense. I don't rate those saliva tests. Gersh only does bloods. Estriol is produced in high quantities during pregnancy. You should have higher Estrone which is E1.

Interesting facts. You mention thyroid, I take a T4 only medication as a replacement and never feel well. Estrogen impacts thyroid how do women such as myself initiate use of a patch and keep thyroid levels under control)?

T4 only medication will keep us hypothyroid because majority of us thyroid patients cannot convert T4 to T3. Are you getting your Free T3 checked regularly? Adding in T3 (Cytomel) will help improve the thyroid for patients who don't convert well. Using estrogen can help thyroid function but it isn't used as a primary method to regulate the thyroid but you can work on both at the same time.

Have you ever tried thyroid extract? A thyroid contains around 30 different hormones and not just T4! Also you might have problems with the conversion of T4 into T3, check also your f-T3 levels.

Why is it prescribed? I know with the bio identicals creams they do Estriol with a smidge of Estradiol. It seems ludicrous to give it out at all.

This is a good conversation, but I swear I'm more confused than ever when I listen to all these different opinions. I don't know what is best! Like, I still don't understand why some people say you have to start hrt within a 10 yr window from menopause, but others disagree. Which is right? And why? And some say don't take progesterone if you don't have a uterus, period. (Dr. Bluming, etc.) Other doctors say it's fine. (I don't have a uterus and I take it, and it has been a game changer for me sleep-wise.) I'd just like some simple answers , not more confusion, esp after the ultimate decades long confusion resulting from the whi study.

You start the first or second year of symptoms. Everyone is different. Some women have anger issues and a few headaches and just harder periods. This all starts in the late 30s... it can go on for over a decade. Once physical symptoms start to present themselves you need to do your research thoroughly. I'd also recommend good supplements and even see a homepath. Change your lifestyle. Then you start the body identicals. The earlier you start the better. You were listening to Dr.Gersh she knows her stuff. When you are fluctuating you are causing damage to your body overall. That can't be reversed and you will age rapidly too. It's not chronological age that ages you fast its having no crucial hormones that will age you overnight and even have you bedridden. And I'm on about VITATILY!! Your mind is also been disrupted and starts to decline.

I'm glad your talking about the xeno estrogens and pollutants. We are not in the same era even 25 years ago. This is what drives me bonkers listening to these natural channels. The food is contaminated..

Interesting discussion, to the Wonderful doctors i have one concern. The estrogens made in a woman's body and synthetic estrogens are expressed interchangeable in the discussion as if they are exact identical because of the term bioidentical . The term biological, i believe is a mind control marketing strategy than a medical truth. The phytoestrogen is about 2000 time small than what the made in the body, but the lab estrogens are about 2000 time bigger than what made in the body. All the lab made estrogens is as counterfeit dollars, you spend until the body catches up with you. 1. The synthetic hormones trick the body and further complicates imbalance. 2 blocks the receptors, preventing the obsorbsion of plant estrogen. 3 because they are extremely big they're difficult to be broken down by the liver/ gut part ways, hence they will lodged on the cells of the organs 4 creating protein, lipids and glueclose spikes causing AGEs which is the perfect condition for all forms of immune deteriorations. Women need to know the truth , that they will be able to make better informed decisions.

EVERY DOCTOR HAS THEY OWN OPINION, I DON'T THINK I WOULD LIKE TO GO THE THE PAIN AND CRAMPIN FOR BLEEDING. IT WILL BE GOOD IF THEY UNIFY IN WHAT WILL BE THE RANGE OF SAFE DOSAGE FOR EACH HORMONE!

Cramping mostly comes from not enough progesterone and too much estrogen compared to progesterone. My cramping have stopped with taking progesterone. If all your hormones are harmonized, I do not think you'll suffer from cramping.

You can take organic Maca powder you won't get cramps. But yes cramps signify low Progesterone.

I'm a fan of hrt, but estradiol has triggered a debilitating histamine intolerance in me. As this is a known risk of hrt, I would like to see this addressed by these experts. It's ruined my health and life (I can't function as I now have breathing issues, and constant severe fatigue), and no medic is helping me. No one talks about this aspect. Let's acknowledge estradiol induced histamine intolerance. It's not that rare.

Are you taking oral estradiol or using a patch? I just wonder if the route of delivery matters were also taking progesterone? Had no idea how complicated we females really are.

What is scary experience? Did your MD determined that it was histamine response to the estradiol? What was the delivery system? Was it oral or a patch? Is there a resolution of your symptoms yet?

You weren't a candidate for it then. If your taking that orally well... if not you were not ready for the Estradiol. Women have to be healthy and fit and have already changed their lifestyles before going this route.

Suzanne Somers died of cancer and she got millions to take bioidentical hormones but I read she and her husband took the jab.

Sommers had cancer since her late 20s. And she died at 77. I don't see the the tradegy here. Also she fell and seriously hurt herself a few years before if I recall. Yes the injection will do stuff to you alright. I never touched it. I care for my reproductive system and overall health. And bio identicals are not body identical. Unless she was lying.

Yes, I was one of those women that had minimal amount of E2, which I thought was bone protective, I just found out I have osteoporosis 🥲. I’m so disappointed in these fears of Estradiol😣
