TVD Fan 17 Kai is their uncle. Caroline may have given birth to them, but genetically, they are Jo's daughters. Jo Parker was a witch from their family coven bloodline. Kai is her twin brother. It wouldn't work if Alaric and Caroline actually had sex and she got pregnant, she's a vampire, vampires can't procreate. The coven transferred Jo's kids into Caroline, thus making her give birth to them. So by genetics, Jo and Alaric are Josie and Lizzy's parents, and Kai is their uncle.

TVD Fan 17 Genetically, they are Jo's daughters. Thus being part of her bloodline, that also makes Kai their uncle. Caroline is taking care of them not just cause she gave birth to them, but cause their real mother, Jo, was killed and couldn't let Alaric take care of them by himself. And over time, they grown to acknowledge, call and recognize Caroline as their mother, while not knowing the real story about the death of their actually real genetic mother.

That was definitely done for fun in my opinion.
