Yes please do, my skin problem is I have combo skin, mild eczema and hyperpigmentation, I change my skin routine got better, lost most of the old one but new one came, its too weird....
Thank you for posting this. I now understand that the goal is not just getting rid of the spots, but maintaining a skincare routine that will reduce the current and prevent new ones. I'll be much gentler with myself from now on and think long term about it. EDIT: Mine got kicked off by a bad allergic reaction in my 20's, and it has been back and forth ever since.

In darker skin tones be careful with hydroquinone which can cause rebound hyperpigmentation and glycolic acid which can cause burns at higher percentages
Yes, skin of color shouldn't be using hydroquinone at all. It's irritating and can lead to worse hyperpigmentation after a few months, and this can happen to caucasian skin as well. There are also studies that show toxicity. There are many great alternatives to hydroquinone (and which you can use indefinitely, unlike hydroquinone), including the tyrosinase inhibitors alpha arbutin, azelaic acid, tranexamic acid, dioic acid, kojic acid dipalmitate, licorice root, octadecanoic acid, even vitamin C and green tea extract. Niacinamide is a good supportive ingredient. And the most effective and quickest treatment for hyperpigmentation is one that uses multiple tyrosinase inhibitors at the same time. Glycolic acid is a risk for skin of color because it's a tiny molecule that when penetrating the skin can lead to burns and inflammation, which can lead to hyperpigmentation. There are also may alternative exfoliators to glycolic acid - in the AHA group, mandelic acid and lactic acid are safe and effective alternatives; the BHA salicylic acid is great; and PHAs gluconolactone and lactobionic acid are also effective, safe alternatives. Glycolic acid is not necessary to use. If you want to use it, or a product you love has it, then use a maximum 5% of glycolic acid.
@@jaylove1931 Alpha arbutin is not hydroquinone. Alpha arbutin doesn’t kill the cells that produce pigmentation, it inhibits tyrosinane (enzyme) which stimulates the pigment-producing cells, therefore preventing hyperpigmentation. The effect in the skin is similar, but alpha arbutin is a safe, non-irritating ingredient, unlike hydroquinone. It does work at a slower pace, however. Like I said above, using a product with multiple tyrosinase inhibitors is a more effective and efficient way to clear and prevent hyperpigmentation than just using one.

Hyperpigmentation can also be caused by medications and a liver that needs to be cleansed. I used to have horrible hyperpigmentation when I was taking a pain medication for a couple years. When I stopped the meds the worst of the hyperpigmentation faded
Can you do a video about life after hyperpigmentation treatment. No one ever discusses how to maintain the results of treatment. I’ve had my trouble areas of my face successfully treated multiple times and each time the hyperpigmentation returned. I’m aware that a patient can not use hydroquinone forever. A maintenance video will be deeply appreciated ❤️❤️❤️
Have you ever tried African black soap? The real one its brown with mineral specs. It’s drying but it helps me. It fades dark marks and helps me maintain a bright look after hydroquinone. I just use a good moisturizer w/ sunscreen after

I’m not even sure if this comment will be seen by anybody, but I would be remiss if I didn’t express how life changing this video was for me and my sensitive skin challenges! I have black/brown skin. So after watching this, I bought only 3 products: Alpha Arbutin with The InkeyList, Azelaic Acid Suspension 10% with The Ordinary, Faded with Tropicals. My skin routine is minimalistic. It’s been about 3 months now and my acne pigmentation has dramatically reduced! I can’t say thank you enough to Doctorly! God Bless you!
I’ve been suffering with dark spots since my skin went crazy in my 20’s. I tried everything and one thing I did try was Hydroquinone . Back in the day I wasn’t educated that well in skincare but hydroquinone I found out wasn’t the best for people of color. I remember when I used it I found it actually made my skin darker but it should be noted that I bought this at a Sephora and as a person who worked for Sephora we were not educated well enough about this product. I’m still on my journey but I’ve been using the FADED serum and it’s actually pretty great. Patience, consistency and sunscreen 👏🏽 that’s the key
You are correct hydroquinone not good for people of colour can end up with rebound hyperpigmentation which means more problems.I use the ordinary products and recently purchased paulas choice 0.3% retinol which is good for people of colour.🙂

You talked about sunscreen, but I would love to hear about what the effectiveness of things like clothing with SPF versus regular clothing is. Also, I have made it a point to wear wide-brimmed hats when I know I will be outside for long periods, and don’t know how effective it is. I have very fair skin, and the sun damage from my youth really shows.

Sunscreen tip for super oily skin…after an skincare, I apply Jane iredale powder me dry spf then apply tinted mineral sunscreen. It creates a really nice matte look that lasts for hours
So true that growing up in a household where we tan quite easily SPF wasn’t a priority. Lots of years running around the pool without SPF & around 35 severe discoloration started to appear. Now at almost 50, after about 8 years of lots of work, patience & lots of SPF I’m at a manageable level. Anyone who asks for what I’ve used. I always advise seeing their Dr. first to identify the cause so the treatment is appropriate for the cause 🙌🏼 Also - I invest in really high quality UPF 50+ hats in addition to SPF 🌟 Thanks for the info about the actually formation of the discoloration 🌟 very informative video 👏🏼👏🏼👏🏼 I’m excited to pick up & test Faded!
I laugh with my kids now about how my son is dark middle eastern skin tone, I'm Portuguese/Lebanese so I ran beautifully and he takes after me. My daughter on the other hand, has porcelain white skin that burns quick ( takes after her dad) When they were young we lived at the beach... I'd slather my daughter in sunscreen and tell me son " you're allset you don't burn" I cringe when I think back to those days... We live and learn
You two are the reason that I started my permanent skincare journey. I have so many different creams and serums and have spent so much money, that my skin hasn’t had a chance to get used to anything. So when I found your channel, I went back to basics. It’s so hard when another product comes out, promising the world in a bottle. Then I remember to look for the main and supporting players in the ingredient section, like you said, instead of looking at the pretty package and promises. I now have a simple routine, which I wish I had started years ago. Thank you. And you’re correct, I wasn’t raised to bother wearing sunscreen. Just to moisturize. In fact, I completely avoided it because I had cystic acne. The last thing I wanted was a greasy film suffocating my pores. Now with hyperpigmentation spreading like butterfly wings across my face, I came to see what you both had to say about it. As I was negatizing (it should be a word) my treatment’s efficacy, by using a strong acid peel, I was getting a rubber band effect and making it worse, I needed a new plan and to actually understand why this was happening. So thank you for that also. Retinol (gently gently), and Kojic for the win, I hope. But I’m like a 6 year old, dragging my feet, hanging my arms like limp noodles, and looking up at the sky, (with sunscreen on) while whining: “But why does it have to take sooooo long! Ugggh!” 🤗

I use the Heliocare as well! I have post inflammatory hyperpigmentation after fraxel laser and I hated it! I also use the tretinoin as well! Good stuff

I’m confused if there’s actually a difference in active percentages and/or ingredients allowance IF ANY. Not medical grade as in prescription drugs but skincare marketed that way that you can only get as an esthetician or from an esthetician or that are incredibly expensive usually, cosmeceuticals, etc. hopefully that makes sense. Are these skincare brands any better active-wise/formulation/research in anyway and if so how. Thanks you I’m a faithful follower!! Also, I’m getting mixed answers about if esthetician’s are actually allowed to use higher percentage actives than over the counter concentrations in a non-medical setting.

I agree about the Sunscreen. After watching you guys, I started using the CeraVe mineral sunscreen and my hyperpigmentation has lightened so much. I’m super excited. I always had the idea that darker skin tone shouldn’t wear sunscreen and I never wore sunscreen and my hyperpigmentation was kinda bad and I felt so insecure. I started to wear sunscreen this summer and oh my goodness my hyperpigmentation is almost gone.
@@feliciaadams8545 bliss tinted sunscreen, my skin tone is a bit darker than Dr.shah and it does look a bit white on my skin. I’m currently looking for a different one. It makes my skin a bit dry. If it’s within your budget Sephora has great options. If not try target. I got mine from target

Mandelic acid gave me hyperpigmentation around my mouth area. 1 to 10, my hyperpigmentation is at 9! It’s been 6 weeks, I have been using everything mentioned! I wanna cry!
Touch Bright & Clear Cream, has niacinamde, tranexamic acid, azelaic acid, and kojic acid. Their website lists the exact percentages of each active. Regimen Lab also makes a multi-active fade cream. Btw, hydroquinone can still be found otc
I tried to order Differin hydroquinone from Walmart and instead I got the new formulation with "4% niacinamide and botanicals". What a ripoff! I'm using it since that's better than throwing it out, but I don't think anything but hydroquinone will do much for my dark sunspots (that date back to my teenage "suntan oil" years before sunscreen existed...)

I hate cystic acne, it hurts because it stay under the skin for ages. I've seen a dermatologist and got rid of mine with an antibiotics, Tetralysal, I think. It worked quite good.

I have been fighting melasma for over 3 decades now and watching this video has got me worried about the usage of hydroquinone even though I have only used it as recommended by doctors. Get yourself a plasma pen on Amazon. I bought the Ophera pen. I had dark spots, sun spots, a few skin tags and I had them completely removed. 6 days recovery time for me.
I have been an RN for 50 years, and have spent the last 22 years of my nursing career doing laser hair removal, Diolite treatments, IPL and Botox. I am taking notes and learning so much from your videos. The ones on rosacea and hyperpigmentation were particularly valuable for me. You are both a delight to watch. I have 1000+ active patients, and I am spreading the word to all of them about your Doctorly channel. Like myself, many of my patients went through their teens and 20s before sunscreen existed. (What was Sea and Ski anyway?) We slathered on the baby oil and cocoa butter, and are paying for it now with brown spots and crepey skin. Like you say, it is so much easier to prevent sun damage than repair it. I greatly appreciate your product advice, explaining the purpose of the various ingredients, and teaching us how to choose effective products that don't need to cost hundreds of dollars. You have inspired me to nightly retinol (purple jar Olay from Costco) and to greatly step up my sunscreen (EltaMD) use. I plan to try the tinted version next. Thanks so much for what you do!
I have gotten so many more spots on my arms this past year - I’m a big sunscreen user, so this has been so frustrating for me. The dermatologist I went to for my yearly skin check only addressed the spots stating they weren’t cancerous. Great, but I am wanting to have my arms cosmetically look better too. I feel like it ages me. Thank you for addressing this.
Everytime I get a pimple, it leaves behind a mark for weeks. This is a new occurrence for me. I wear my sunscreen, but I’m not great at reapplying because I work a retail job and it’s just not easy for me to reapply multiple times. But I wonder if that’s the issue…
The mark has got nothing to do with sunscreen. It MAY worsen under intense prolonged sun exposure but acne will always leave a scar. Use the products they mentioned and ensure your sunscreen is not causing new acne. Treating your acne should come first.
My skin has been clearing since I've been following your advice, thanks so much for this channel :D 14 years I've been suffering with acne and I finally feel there's hope of having clear skin without using birth control. I've tried expensive green beauty products for years and also every "natural" recipe I could find with never lasting or great results. I see now I was putting too many different ingredients on my skin and irritating it. I unfortunately do now have scarring too. I used to have a big scar on my face. When I was 18 I burned my cheek with a chemical peel I did at home (I had no business performing it on myself and regret it) and I left the acid on too long..then I put hydrogen peroxide on it afterwards...BIG mistake. I had no idea what I was doing. My entire cheek was severely burned. My texture and tone has never been the same since. It's much better now though since it's been 10 years, not even too noticeable at all. I just know it's there and hope I can one day have it not make me feel self conscious. After clearing my skin, I'll most likely opt to do some laser treatments to the area.
@@nafeesa14 Honestly, the best recommendation is to see a dermatologist if you can afford it. That was not always an option for me. It depends what your concerns are. They have multiple videos on acne and scarring that should be useful to you. What helped me personally is micro needling and more peels done by Estheticians. I still have a faint discoloration and texture change. I may get more micro needling & peels done. If that doesn’t help I’ll see my options with laser treatments.
I stared watching your videos maybe 2-3 weeks ago and finally got out of using Proactiv that I’ve used for the last 15 years. Using your tips I have customized my skin care routine and now my skin looks way better than it ever has in my life
I’ve had all kinds of skin issues, from dryness to redness, but after trying the subliminal affirmations on borlest, my skin has become so much healthier and balanced. I’m really impressed!
One thing I always have to remind myself is moisturizer. I do my retinol cream at bedtime because it can cause my skin to be super sensitive and I don’t want to go out after putting it on. I of course let it soak in then moisturize. Otherwise my skin starts to peel. I also have to remind myself to use sunscreen. I grew up the same. We rarely used sunscreen. My sister actually used suntan oil. That coconut stuff. Now I can’t smell coconut without thinking of suntan oil. I guess I was lucky because I hate the heat. Never laid out to bake. I do have dark spots on my face after taking birth control pill for years. They are stubborn. 😤

i found your tier chart for acne EXTREMELY helpful. it is what helped me realize i needed to prioritize azeleic acid into my routine for both acne and PIH and i have already seen amazing results so far for my acne. so i think if you could try that chart for some of your topics like hyperpigmentation that would be super helpful for people (:
New kid on the block here and I am falling asleep every night watching your videos. Good job! This video is one of my favorites because of the common issues associated with pigmented skin. Question: Sun exposure obviously increases hyperpigementation, but doesn't heat alone aggravate and increase melasma specifically? It's super frustrating that I can be wearing sunscreen and a sun hat and still have melasma spots darken in less than an hour in the heat.
Same case here...so it means you also need to use tyrosinase inhibitors like Kojic acid or alpha arbutin... Im also using Kojic acid every night 2hours before bed and it's fading a bit but needs a little time

I am a youthful looking 76 year-old multi-racial woman, and a couple of spots have begun showing up on my face. I’m having great results, but I’m also using Spiro, so they’re definitely working together. I can’t really speak on the results of using Faded solo.
You guys are the best. Thank you so much for these videos. My husband and I have been using Ambi's fade cream for my malesma and his sun induced hyperpigmentation. We are almost done using it for 3 months with retinol and nicenamide and spf50 of course. We will now move over to different serums with alpha arbutin, tranexamic acid, kojic acid, niacinamide and retinoids and tinted spf50 of course. Thank you for the detailed advise and ingredient list. We don't have all the suggested products here in South Africa but there are good alternatives like Skin Functional, a South African brand. Using faded by topicals and other topicals like niacinamide, vit c, alpha arbutin, kojic acid serum and cleanser soap, mandelic acid, TXA, licorice root and tretinoin. It is getting better and lighter. I also started going for microneedling treatments. It's going well just have to be slow and consistent.
I started getting a bit of hyperpigmentation in my early 50’s, mainly on the sides of my face near the hairline where I obviously wasn’t applying enough sunscreen 😭 I’m currently on a compounded hydroquinone cream and it’s over 75% faded now I’m really happy, my dermatologist alternates it with HQ-free treatments to avoid the risk of discoloration. None of the OTC treatments worked for me, it was really frustrating as my spots were not that dark or that bad. Those ingredients are pretty ineffective compared to hydroquinone. Even tretinoin did nothing to lighten the spots.

The peels actually made it appear worse and it’s so true the only thing that worked was the hydroquinone. I’m so happy you warned about long term risks of that product; I was not educated on that. mine came back even wearing thick sunscreen and reapply 3 times a day even in the house. It got worse after a trip to Las Vegas in the summer. Like you said came back with a vengeance. :-(
