Abdominal distension caused by Schistosomiasis. Basically parasitic worm causing body fluid to accumulate in the belly. For undead hollow living for hundreds of years the symptoms are much more pronounced.

@@rodgersb1248 more like he speaks as if he has a degree in parasitology, and indeed studied them irl. Schistosomiasis is an irl thing, Dark Souls 2 have nothing to do with it. And as people above said, it actually looks more like Hernia than Schistosomiasis, as parasites causing schistomiasis infect humans at their next-to-last stadium when they're being expelled by an animal host to the water with feces or urine, where then they search for human, and enter his body through skin. Since it would require all of these goblins to delve in the water and ALL catch the parasite, and also it would require various stadiums of said parasite, AND Schistosomiasis doesn't look 1:1 with this as you gust get a giant belly from it (remember the pics of starving kids in Africa with giant bellies? That's Schistosomiasis), and you can easily get Hernia if you push your body to too much extent with full intestines - like lifting too heavy weight - and these goblins just keep eating flesh and run around like wild, it looks like severe Hernia to me too. No i don't have a degree, i just like to know stuff and i spend hours googling scientific articles, wikipedia and generally do a somewhat extent research on shit i find interesting before i'm content with my knowledge/understanding of the topic and can write a random shitpost comment in a random place of the internet. They are modeled to look like 餓鬼, "hungry ghosts". The hungry ghost has a massive belly that is always starving and a small neck that cannot swallow food. For some, food all turns to ash in their mouth. They are often spirits who suffered violent or otherwise unpleasant deaths, which is fitting for the narrative of Dark Souls II.
I remember reading about a type of yokai that looked very similar to this enemy (grotesque, hunched over, emanciated silhouette with massively bloated abdomen) and was said to stalk abandoned battlefields - it was speculated to be based on local people who suffered from extreme starvation, especially during war or in the aftermath of natural disaster, and turned to scavenging battlefields in search of food or valuables to trade for food.
I suppose that they're playing into the theme of DaS2: the curse of life is the curse of Want. Desire is fairly well established as a motivator in the game, and it's represented here as a hunger that they can't ever satisfy.

Yeah, I heard of something like this - some researchers think these yokai were based on real-life cases of extreme starvation, since it can cause stomach bloating. Weren't they also associated with battlefields and other places of mass violence/death (referencing starved locals in war/disaster-afflicted areas looking for food or tradeable valuables among corpses)?

I forgot the name of Yokai this creature is based on but basically there is a demon in Japanese mythology that looks similar to the goblin DS2, whos belly is also enlarged, symbolizing that no matter how many times it eats its hunger will never be satisfied and they will starve for eternity.
To me they're very evocative of gaki or the hungry ghost genre of yokai. From what I recall they are spirits with incredibly lean and starved appearance but very distended bellies. In myths they are constantly hungry but unable to eat for various reasons such as anything they put in their mouth turning to sand but it depends on the myth/region. Though I could be totally wrong as I only know about this from light googling.
They remind me of pretas, also known as hungry ghosts. Spirits from buddhism that are individuals whose bad karma derived from greed causes them to reincarnate as humanoids with large stomachs and insatiable hunger.

Yes I agree I hear it’s due to this game using motion capture for its animations but they all just look so awkward compared to other games even the original demon’s soul.
They look very similar to depictions of Preta spirits/hungry ghosts, also called Gaki in Japanese. The emaciated body and bulging, distended belly represent how they're eternally hungry despite constantly gorging themselves on anything they can find.
Ooooooh it's a bulging stomach, not a giant nutsack. I see now that it's a similar kind of thing to the Preta (a mythical monster that also appears in Shin Megami Tensei), and is meant to be a hollow consumed by its appetite.
Undead don’t need to eat, which might also mean they actually can’t ingest certain foods they used to. It could simply be undigested food which continues to eat in their stomachs

Well, they are cannibalistic as shown ingame, so perhaps continued consumption of Undead Flesh caused their condition they remind me a bit of the strange Gremblin People from Blighttown only minus the odd face, maybe they are related in some way their Running Animation is so funny to watch

It seems to me that this is a reference to gaki from Japanese mythology (or preta from Indian, but Fromsoftware is Japanese, so I'm inclined to think that this is a reference to gaki). They are hungry spirits, with a disproportionate body, and most importantly - a thin neck and a huge belly. They are constantly hungry, and they eat anything. And goblins, in part, correspond to this description, they have huge bellies, and most often we find them devouring someone, even their grab-attack is a bite. And also, various sorts of weapons drops from them, which may mean that they have devoured a large number of people
I don't really like them as they demand an otherwise unnecessary sacrifice of either 1. the threat of the enemy, or 2. the consistency of the hitbox with visuals
I think that they would fit perfectly for Dark Souls 2 blighttown, simmilar enemies were in the 1 accompanied with mosquitos, leeches and other infected beings forced to live in terrible conditions. I do not understand why fromsoft gave some boring sword hollows instead. Actually the infected large dog like creatures was cool in such a place, but still i was missing such blighttown atmosphere while exploring the location.
I think the swolen belly might be due to the fact the hollow's digestive sistem is out of usage and thoses for some reasons are still extremely hungry meaning they hate mostly all they can found and it keep pilling on in their guts.
That's no goblin. I know a Hollow when I see one. A Hollow who found alot of prey. That's the funny thing about being q living corpse: you don't get fat so much as...bloat. It doesn't go anywhere else, all the meat you waste on your stomach. All of it, to quench something that could never be quenched by eating to begin with!.... What a horrible existence.
This must be one of the only cases where the in-game model is actually better than the concept art. The art is just a hideous humanoid, but the model looks... Decrepit. Like parts of it's mouth are missing, it's skin seems to sickly, and the eyes look more than dead. The jaw even seems to be either limp, or broken.
