When to STOP a PPI???

Do you have acid reflux and take a PPI, but think it might be time to stop? Have you been told to stop a PPI, but can't figure out how to do it? If so, watch this video for more information!
First, lets discuss why someone would be on a Proton Pump Inhibitor (PPI) and what other changes you can make to better your chances of reducing acid reflux. Additionally, are there medications you could stop that would reduce acid reflux? How long does it typically take a PPI to work? Can you increase the dosing or take the medication for longer if you haven't gotten relief?
Is it ok to take a PPI once in a while? Should your actions change if you take a PPI for an ulcer? What if you have Barrett's Esophagus?
Additionally, lets talk about what PPI dose is beneficial and when you could stop taking the PPI. Are there consequences for stopping a PPI? How do you avoid those consequences?

Hello doctor cooper . What will happen if I cut the Prilosec in half . I heard not to do this , but I’ve read where others have done this without any side affects , because they are only cutting off one end and the remaining shield is still there ???

I only have reflux issues when I sleep on my right side. Is this warranting the PPI medication? Can I choose to take as needed instead of daily?

With GERD is it generally acceptable to take PPI for a period of time when there’s a prolonged period of bothersome reflux sx (not just a few days or after a certain meal but ongoing day to day sx) then cease taking it once sx controlled say after a couple of months and to intermittently do that when sx warrant versus never stopping taking it?

Hello doc many thanks for info. What about with someone gastritis, lose sphincter, 1.8cm hiatal hernia? I’m very careful of what i’m eating so basically no much symptoms but i take them twice a week 40mg nexium and twice a week pariet 20mg.. Two questions: 1) Is it enough to fight the inflammation? 2) Is it safe for long term use?

Red apples such as red apple delicious and Fuji apple fruits are proven natural remedies which can neutralize hyper-acidity in the stomach and also can help in getting rid of acid reflux. Eat those red apples for a few days to ensure success of treatment.

I’d taper as needed. If you need it twice a day then try that for awhile before then decreasing to once a day.

Hi, I've just come across your videos - brilliant! I have a hiatus hernia and have stopped taking my omeprazole which I'd been taking fir some years! I have had a chronic cough for some months now - I've had a chest x-ray and all is ok there. What would you suggest? I don't want to go back on omeprazole.

What if you have chronic gastritis caused by h-pilori. Took the triple treatment for 14 days. Was negative after two stool tests and endoscopy. There has been no stomach pain and no heartburn but have been on 40mg omeprazole for four weeks now after endoscopy. Trying to stop the ppi.

I started having muscle pain pains, and now my neck hurts. I stopped taking the medication due to side effects. three days ago and now taking a multivitamin hoping to feel better no relief yet. Anyone else experience muscle pain joint stiffness on these medications

It’s certainly a side effect but uncommon. Given its persistence I would be inclined to think it’s coincidental just as soon as causative. I’d expect massage, stretching and exercise as tolerated to be more helpful than vitamins.

I’ve been on ppi for a year and I was supposed to start tapering off and I was fine until like 2 weeks now I’m too nauseous to eat but before I started the medicine I only got nauseous after I ate

Hi doc, I'm hoping you can answer cos my doc isn't giving me any advice on this. I've tried running with Gerd & after every time i feel extreme fatigue, chest pain & shortness of breath for days/ weeks. Walking is fine. Im 37 taking ppis 4 years. When 1 ppi stopped working they switched me to another. Omeprazole/lanzoprozol & another. Have had scopes & acid probe test done which came back clear. Diet is good. I dont understand whats going on here. Whatever this is, i do not feel like I'm recovering. Legs are constantly heavy, chest/hip pains that always come back. Constant sore throat that they've told me is from the reflux. Can do situps/ weights with no side affects. Need running for my occupation but feel it's definitely affecting my gerd. The shortness of breath is a real concern.

Sounds like you may have esophageal hypersensitivity. Try drinking alkaline water that’s readily available at a grocery store, after you run. It will neutralize acid and pepsin and rehydrate. Avoid sport drinks as some of the dyes or flavorings seem to have affect on the lower esophageal sphincter. Did you have motility test?

This is great info but I have some questions. My stomach has been on fire with cramps/stomach aches no matter what I eat. So one of the nurse practitioners told me to take Florajen pre-biotic daily. So I took it for about 6 weeks and it resolved my issues. However, I stopped taking it several days ago as I noticed I was packing on the pounds. Sure enough the weight dropped rather quickly when I stopped taking it. But my stomach started raging again to the point of being doubled over in pain. So I was given Sucralfate 1 GM. I took one pill and was so nauseated and sick that I stopped. Does this mean I have an ulcer? By the way the gastroenterologist here won’t see you unless it’s a high dollar procedure. They tell you to see your primary doctor. Thank you for the video.

A natural remedy to fully get rid of acid reflux is by eating fruits such as red apple delicious or Fuji apples. Eat any of those red apple fruits for a few days to ensure success of treatment. It worked for us and the problem never came back.

2 days into being off a PPI after a long taper. I would say moderate heartburn… we will see how it goes. For some reason Apple Cider Vinegar actually seems to help

@Mark Cooper, MD My father is in bad shape right now, hard to get a doctor rendez-vous in 3 weeks, his doctor prescribed him this 13 years ago when he was 65 years old, now he's 78 and recently his energy level has been dropping massively. I've done some research on my side and he did on his side too, we believe he's at a stage where is stomach acid isin't strong enough to process nutriment or is intestinal baterial flora has been affected because of those IPP he's been taking daily since 13 years, I do not know what that doctor had in mind to never tell him to get off those after 4 weeks. Do you think taking magnesium, Vitamin D, B12, Calsium and Iron would help keep him afloat for the doctor to do a bone density test and bloodwork to take a look at his vitamins and minerals level? Also is there a good way to heal back from this with minimal side effects? He's slowly starting to panic his energy level been declining fast past 6 months and he's a bit desperate. I wanna thank you for this video and ahead of time for your response if there is one :)

I’m skeptical that this alone caused such profound symptoms. Sure he can take some vitamins. Recent evidence suggests these are useful in older populations. A general multivitamin is reasonable and likely sufficient.

I was given a PPI after experiencing acid reflux due to a medication side effect. I’m no longer on the medication that caused the reflux. However, I cannot get off of the PPI. I get rebound reflux that is miserable. I’ve been taking the PPI for over 3 years now. I CANNOT get off of it without miserable rebound reflux. I want off of it so badly.

Gradually may work. While using tums. For example using two Tums in the morning, and before eating lunch etc. And then maybe using PPI on the weekends only. As a start

@@lt4376 it’s been 2 weeks and 5 days and it’s gotten a bit better. Still uncomfortable daily. I’ve tried tums and alka seltzer and they don’t help at all. I’m also doing famotidine at night and I’m pretty sure it’s not doing anything. I do see it slowly getting better but GOD it’s miserable I will never take a PPI again

@@nandinisingh8112 I weaned off of it and replaced it with Pepcid. I gave it 3 weeks with no PPI and finally caved in and started taking it again. After 3 weeks, the rebound reflux got worse that it’s ever been. Horrible burning in my throat almost like lava!! I’m taking the PPI again. I don’t think I can ever get off of it.

I take half teaspoon of bread soda after a big meal. It really helps. Also u need digestive enzimes to help digest tour food plus don't eat big meals. Hope this helps...

Hello Doctor Cooper - what do you consider "low dose" and "high dose" PPI? Do you have link to any studies that back up what you said about a low dose having "proven" effective, etc? Thank you

The dose varies between specific Meds but most have a low vs high dose Ie 20 vs 40 or 15 vs 30. As for citation it’s best to just go to pubmed and search relevant key terms if you wish to delve into data yourself as it’s indication specific. Regarding utility of PPI for Barrett’s here’s one. It’s important to look at most recent and largest as other studies are less promising on benefit. Chen Y, Sun C, Wu Y, Chen X, Kailas S, Karadsheh Z, Li G, Guo Z, Yang H, Hu L, Zhou Q. Do proton pump inhibitors prevent Barrett's esophagus progression to high-grade dysplasia and esophageal adenocarcinoma? An updated meta-analysis. @@MarkCooperMDhey doctor, thank you for all the knowledge it’s been very helpful. Quick question, I am down to 10 MG omeprazole and I lost 20 lbs and eating more probiotic foods. Is 10 MG a high dose or not too bad? I love it because I can eat anything and feel fine all day long. I just worry about being on a PPI long term. I can skip a day and I’m fine it’s just when I skip 2 or more days I start to feel a bit asthmatic. I would love your feedback.

One Pantaprazole does me for 3 days. I've tried the ACV, Aloe, Slippery elm and Betaine HCL without any luck. I was scoped about 5 years ago and the diagnosis was mild. Geez I'd hate to expeience severe if only mild hurts this much. It's not so much reflux as it is just the burning feeling.

Yup. I can’t tolerate ppi’s so I use h2 (Pepcid) and I used it before on an as needed basis but this last flare up of gastritis had me using it every day for 6 weeks. Now I have to wean off of it. Very painful. Wish I would have just used it on and off like I did before.

I have a question. I started having symptoms of gastritis and was prescribed esomeprazole 40mg and famotidine 20mg. I have taken these for almost 6 weeks and recently started having issues with possible esophagitis which I think is from the PPI weakening my les. I was finally able to see a GI doctor yesterday and have an endoscopy scheduled for December 11th. the doctor told me to keep taking the PPI and Pepcid and also prescribed Carafate. He kind of dismissed any concerns I had about the PPI and the new symptom that were not there before. I want to ween off the PPI to see if I have indeed improved and if the esophageal symptoms go away but am scared to and also do not know what is the best way to proceed.

If you are suffering from hyper-acidity or acid reflux, try eating red apple delicious of Fuji apples for a few days to neutralize and get rid of the problem. It worked for me, my wife and some of our friends and the problem never came back.

We had the same problem. I got long fast then Get gastritis, then using ppi quite long that start getting gerd that initially not exist. Im going taper the ppi half the dose take when symptomp appear

Dr . I’ve been battling Gerd for 0:30 6-8 years I’m 68 years old now I’ve always been on 40 mg of either a generic version of Nexium or pantoprazole . Always took them daily ( big mistake ) Well now neither seem to work at 40 mg . I tried 20 mgs of rabeoprazole good success but after 3 weeks severe stomach cramps, bloating constipation I had to stop. Im in fear of what to do next . I am having a endoscopy 1-15-24 i do not have Barrett yet . Right now just to ease pain im taking 40 mgs of pantoprazole 2x daily.

Hi how did the endoscopy go? Wondering if it was helpful? Been on PPi for quite a while, 10 MG and it helps tremendously. I also am on a diet and eat more probiotic foods and fast but definitely understand. I can skip a day but more than a day it gets difficult.

The drug made me very dizzy and nauseous. Also, I most fainted several times. I stopped using it and will consult my doctor.

Hello Dr Cooper, I hope you do reply as I been trying to learn more about BE. I am 29 years old male and in shape doing CrossFit daily. I recently did an upper Endoscopy for GERD and found out its complicated by BE which is benign but precancerous mucosa directly related to chronic acid reflux. I do eat healthy, but I do drink a lot and do nicotine pouches. I am now aware of my condition and will limit my drinking and try to stop the acid reflux. My question is I am scared as I am young, and do you think long term It will get worse to develop to cancer or any health issues? I really appreciate your videos and hope you will reply. Any recommendations I will take fully as I would like to live a long health life. I did start taking PPi recommend from the Dr, but only read bad things about it and how it stops your body from breaking down food and nutrition' and people suffer from b12 deficient. Thank you!

The overall risk for cancer is low. Your digestive system has many more tricks to successfully absorb nutrition and is far from being solely dependent on acid production to succeed in its efforts. The people who hype up myths that you won’t absorb nutrition are fear mongering. If the medicine benefits you then enjoy its benefit. If you can modify other things to get off the Med, that’d be great, but not essential. If it does not help then avoid it.

Hi Dr. I’m going to start weaning off of 40 mg of Protonix a day. My next follow up is not until June and I’ve been on them since November I believe. so I think I’m ready to get off of them, but should I wean at the same time as taking a Pepcid or take the Pepcid and stop the Protonix completely?

I started to using Nexium 20mg daily and it solved my reflux immediately. After several months my finger tips started peeling, cracking and splitting to the point that I couldn’t open a screw top bottle without them bleeding. It was so painful, and it’s lasted over a year. I’ve had blood tests and am awaiting a dermatology appointment. Just over a week ago I’d ran out of Nexium and a few days went by and I started to notice that my finger tips started healing. I’ve now stopped taking them for about 10 days and my fingers seem much better. They are not healed but at least I can use them again. Is this connected. I’ve been at my wits end?

What specific pill did you use? What color and capsule type? I’ve seen people have odd dermatologic reactions like this.

@@MarkCooperMD thanks for the reply. I started with the purple/pink solid which was in the over the counter 7 day Uk version. Then I moved to a 28 day over the counter supply which was much smaller with particles in capsule, multicolour I think. Then I moved to esomeprazole on repeat prescription which would sometimes be solid or capsule. My GP has not suggested any link with my finger condition and the medicine, it’s just that I made a link as I’ve been recording my foods and environmental factors. Previously my fingers would peel, crack and bleed for 2-3 days, then heal within 24-36 hours and then the process would start again, hence why I noticed this change so much. I’ve been tested for auto-immune, and skin tissue disorders and both show clear.

Pink/purple solid with 7 day over counter, then multicolour capsule with 28 day over counter, then mix of both on 28 day repeat esomepraole script from GP.

You can and should do it but do it very s-l-o-w-l-y. Otherwise you will feel the wrath of the rebound acid King. I’ve only been using Pepcid for 6 weeks and I’m having trouble weaning off of them. I think I tried to do it too fast so I’ve been going slower these past 2 days but the rebound is very real.

After completing a GI scope I was given a PPI by the physician. I have no signs of any GI issue, no reflux, no pain nothing. As the PPI have very nasty side effects I have decided not to complete the course of treatment.

I stopped the omeprazole after I had fundoplication surgery to correct a large hiatal hernia. After a few months (post surgery) I started choking and gagging om my food. My GP said to take 40 mg daily, and I restarted the medicine. This in spite of tests that show normal GI function and no Barret's. When I had DXA scan showing osteoporosis, I lowered the omeprazole to 20 mg. Still having a little difficulty swallowing, but I am still hoping to ditch the omeprazole entirely. Maybe try an H2 blocker. My GP and GI doctor seem agnostic, leaving it to me to decide.

Some trouble swallowing after a fundoplication is typical. A patient having it at this stage I might dilate and would not so much place them on Omeprazole as it’s less likely a reflux problem.

Most patients should have minimal acid reflux after fundoplication surgery so many won’t need any acid suppression.

​@@thomasschellberg8213 I won't as they will be likely wrong, IMO. Instead go to an alternative medical provider and discuss the use of betaine HCl capsules and night time melatonin. Hint: PPI meds are the devil's version of melatonin. The former is an antioxidant and the latter has an azo functional group and hence is toxic per in part the free radical at the functional group.

GERD is extremely common and esophageal cancer is relatively uncommon. Esophageal cancer is considerably more frequent in men (though still less than 1:100 lifetime incidence). Smoking and overuse of alcohol and obesity are other risk factors. The benefit of PPI is demonstrated for patients who have Barrett’s. The majority of people with GERD shouldn’t be worried about esophageal cancer.

@@MarkCooperMD thx for the reply, I’ve been on ppi for over 10 years and really want to come off, I was put on them with never having an endoscopy, had one 3 years ago and showed no Barrets, I think docs should slow down on prescribing these meds before an endoscopy, but I know it’s really not the docs that come up with these medical protocols, they are sadly just following an algorithm. But it is what is I guess, I’m hoping these ppis haven’t caused any damage. Your info is good to know, as I try and stop the med.

I’ll try to address this in a future video as there’s a general misunderstanding of why PPI are prescribed up front. But to address your one point - this is not just an algo we follow. It’s based on research by medical doctors and scientists.

To neutralize hyper-acidity or get rid of acid reflux, try eating red apple delicious or Fuji apple for a few days and you'll see positive results. It worked for me, my wife and some of our friends and the problem never came back.

I'm on 40mg Protonix twice daily, Trulance daily and Levsin as needed. I'm still miserable, probably due to my obesity. I have a horrible paraesophageal hiatal hernia, Gastroparesis, Tortuous Esophagus, GERD, IBS, another hernia. My specialist says weight loss surgery won't help me much whatsoever due to my severe Lipedema and Lymphedema. My body won't change much. I live on Zofran most days. I'm just miserable all the time.

Prayers for you, Gerd is very difficult but we must do better and change diet and lose weight because it’s very difficult weaning off. These companies hook you and keep you coming back for more. I am luckily only on 10 MG omeprazole, lost 20 lbs and feel a lot better. Hopefully the goal is to lose another 20-30 pounds and try to wean off little by little. I also make sure not to stress, you need to be patient and keep getting checked by your doctor. Losing weight made a huge difference! We are all in it together. God bless you. Amen

@@alexlabra1562 ask your doctor about H2 Blockers it might be better and easier to wean off. Honestly best thing that works is dieting, no soda, cutting sugar and having less food. The Japanese say “eat until your 80 percent full” and begin feeding your gut probiotics and healthy food. It’s important to know that it takes time to heal, like a bruise or cut on your body, it won’t heal over night. Give yourself time and be patient. All will work out. I pray for your success. Amen.

@@alexlabra1562 is that 40mg once a day or 2x a day? That’s an awful lot. I personally would recommend an h2 blocker. It suppresses some of the acid but not a complete block of the acids. I am taking Pepcid 10mg 2x a day and I am having troubling weaning off of it so you can imagine how hard it will be to wean off a ppi at that high dose when the time comes for you to do so. Take the least amount of medicine that benefits you and get off of it after 4 to 8 weeks. Slowly. OR take it as needed. I did that before this last time and then I didn’t need to wean off of it. I don’t know what I was thinking this time around.

Theres a study which demonstrated drinking alkaline water can decrease heartburn by neutralizing the acid and disrupting Pepsin.

Have a question. So my mom has been prescribed to take this every day to help with her acid reflux. And I had acid reflux so she told me to try it every day and I don’t have acid reflux anymore but here recently I’ve been having malaise, my #2’s haven’t been put together right, random bouts of nausea and I’m wondering if it’s the pill making me feel this way. Most of the time though I feel fine. My blood pressure is good, I always stay hydrated, I eat moderately healthy, it’s just here and there I feel funky. And I have no doctor to ask questions. If anyone out there can grant me some insight on what you think is going on and what I should try it would be well appreciated.

The reason to go to the doctor is to have a twenty minute conversation about this and gain much more than you can from a message exchange. With that if you have no alarm symptoms, risk factors and feel somewhat improved then the worst that happens if you stop the medicine is your old symptoms return. The best that happens is that if these are side effects they resolve and you find your old symptoms have largely resolved. The other thing to consider is that if a 20mg pill can produce side effects then the side effects of 2kg of the wrong food could be even more substantial.

I’ve had doctors prescribing me ppis for years. Why do they give me these medications when they can cause so much more problems. I mean they help me so much. But I want to stop.

PPIs actually cause few problems - robust studies consistently show that their long term detrimental effects are relatively uncommon and minimal. If you don’t benefit from them then you definitely shouldn’t take them. If you can successfully do without them then that’s preferred. Avoiding them when they help is a personal choice of course but not what I would recommend to my family.

They turn off the acid cells in stomach, then stomach makes more. Then when you try to stop taking PPi`s you have double the acid production which is agony.

I was just telling someone the other day that enough patients report they find lemon water soothing that it’s certainly an easy thing to try. Yet straight orange juice is a typical trigger of heartburn. I’d sooner recommend trying alkaline water, which I discuss in one of the gerd videos.
