In this video, Dr. Matt Cecchini and I discuss how we use immunostains in small lung biopsies in a variety of scenarios. We cover adenocarcinoma (including mucinous cancers), squamous cell carcinoma, poorly differentiated carcinomas, spindle cell neoplasms, and much more! Wonderful and wholly practical summary! Thank you. This topic always reminds me of Bayes theorem. "Pre-test probability" is so essential to correctly interpret IHC, and/or any other test.

100% agree to do high-risk HPV ISH instead of p16 to assess for metastatic HPV-associated SqCC in the lung. Due to limitations in specificity, the only time I use p16 at this point is when I have an oropharyngeal biopsy (I don’t do gyn) with tumor in a middle-aged to older male because the correlation with hrHPV in that situation is extremely high (and the ISH takes slightly longer). Great discussion!

What do you think about using CK7 and CK20 (or anything aside from p40, p63, GATA3) for differentiating squamous cell cc from urothelial cc?

I'm not a big fan of CK7/CK20 for any differential in the lung

In a known case of endometroid adenocarcinoma with now lung mass....do u recommend put ihc for ruling out fetal type lung adenocarcinoma in case of absence of radiological details ....

I would usually use TTF-1 and PAX-8 in the setting you describe
