A man underwent a 4000-graft hair transplant 15 days ago. He was advised by his doctor to massage his scalp while shampooing to remove the remaining scabs. He noticed some grafts seemed to have come out as well, though there was no bleeding or pain. He wants to know if hair grafts can be dislodged with vigorous shampooing.
New York hair restoration specialist Amiya Prasad, M.D. reviews his concerns in this video, and explains the timeline when grafts are firmly in place, how proper scalp washing after a transplant is essential to help with the healing process, and that losing some hair grafts after a transplant is to be expected:
1:50 - Dr. Prasad details his background training in cosmetic oculofacial plastic and reconstructive surgery, as well as his experience as a hair loss specialist and founder of TrichoStem™ Hair Regeneration Centers
2:51 - How hair grafts are usually quite secure after 48-72 hours
3:26 - How donor area healing is one of the challenges when it comes to hair transplants
3:55 - How a 4000-graft hair transplant can be fairly traumatic on the scalp
4:08 - How the combination of extracellular matrix and PRP was found to be able to thicken existing hair
4:25 - The development of the Hair Regeneration treatment and how the treatment is performed
4:51 - How a classification system to optimize the dosage and delivery of the treatment was developed
5:18 - Why proper and vigorous shampooing is essential to proper healing of the grafts and scalp
6:18 - The added benefit of undergoing Hair Regeneration treatment after a hair transplant
6:52 - How wound healing follows a very stable schedule and consists of several phases
7:26 - Dr. Prasad’s recommendation
7:33 - How losing some hair grafts is possible after a transplant due to bleeding during surgery

Thanks Dr.Prasad I feel a bit more comfortable washing my hair a bit more aggressively. My doctor did clearly state in his Post Op Instructions that After Day 6 of the surgery there is almost no chance for the grafts cells to fall out since they are already healed in place. It seems that you also have the same consensus as him - but you also need to understand the patients worry since most of us do not have medical school training and do not know all the intricacies of how the scalp and follicles function. And also can you give me some of your hair!? Ha! You are lucky. My dad also has great hair and he is almost 60 and I have been thinning and losing hair...it's tough - but I just completed a transplant to add density, going on 1mg finasteride and using laser therapy. Praying and hoping for best and judging by your videos you also have a very warm personality and seem very humble. Take care doctor.

Hi@@RaghuveerSingh284, Some doctors are more conservative with healing. Keep in mind that there is no standardized certification for hair transplant surgery, so different doctors and technicians perform transplant surgery in different ways, so it is up to the surgeon performing it to prescribe their healing protocol. Wow! This is very interesting ! On day 5 post op I took a shower and forgot to not put my head under the shower head; the shower head in my house has pretty strong water pressure and It seemed to have knocked off the majority of my scabs... I thought maybe I might have damaged some grafts ... afterwards I picked off one of the scabs that were hanging off and I saw what appeared to be dried up blood in that spot. I didn’t see any leaking blood so I’m guessing that I didn’t lose any graft but I’m hoping that I didn’t hurt the chances of graft survival . Is it possible to hurt the chances of graft survival in this way ? Also .. if grafts are secure are 2-3 days why do so many doctors recommend such cautious measures for so many more days afterwards ? Many even gave different ideas about graft security being something that doesn’t occur until 10 days or even 2 weeks post op. Seems to not be a general consensus between doctors on that one . Is there a reason for that ?

@AmiyaPrasadMD
@AmiyaPrasadMD 4 года назад
Hi Benjamin Kew, It is highly unlikely you damaged any grafted hair follicles this long after your transplant surgery as they are very firmly in place at this time. Any lost follicles by this time would have been lost eventually as they didn't take during surgery. If you have any concerns, you should refer to your transplant surgeon who knows details of your scalp and your surgery that I'm not aware of. I hope this helps you.

Sir thank you for your great info ! I have recently 10 days ago gotten a hair transplant approx 2700 grafts to my front and crown area. I did not touch the recipient area for the first 5 days except for cleaning after the 3rd day post op. After the fifth day I began to ivoluntarily pick at the scabs. I tried to stop but somehow I kept finding myself picking at it. There was no bleeding or pain when I did it. The scabs had already become fully dried up and crusty. Now I am worried I have permanently damaged my hair. Do you think I have damaged it?
@AmiyaPrasadMD

Hi Akshay Narasimaiah, You likely didn't damage your grafts as the follicle should be firmly in place underneath the skin, but I advise that you not pick at the scabs. Your transplant surgeon can examine your scalp for any possible issues. I hope this clarifies things for you.

Hi @profsavino, Grafts should be firmly in the place in the scalp by now, and should not be lost after shampooing. However, I suggest you follow up with your transplant surgeon who can examine your scalp for issues, and knows details of your transplant surgery. Hi Doctor, thanks for your videos. I have some confusions: 1- Can we go outside on a sunny day after 48-72 hours of hair transplant? Does it cause sunburn or damage to the newly planted grafts? 2-There is a myth that finestride may cause unfertility. Is it true? For how many months we have to take it after a transplant? 3- If we stop using minoxdil, will it affect our natural existing hair?
@AmiyaPrasadMD
@AmiyaPrasadMD 3 года назад
Hi muhammad akbar, Yes, you can go outside 48-72 hours after a transplant, but you should watch your sun exposure so it doesn't cause sunburn. While finasteride is unlikely to cause infertility, as a precaution I do have my male patients stop using it temporarily if they are trying to conceive a child Yes. Since minoxidil delays natural hair shedding of native hair, if you stop using it, you will likely have a sudden hair shed of native and transplanted hair. I hope this clarifies things for you.

Hello sir! My name is faisal khan Sir I had my hair transplant done on 7th of July. today One month and 8 days have passed And in 10 days I was called for headwash. But due to the distance of 250 km What did I do at home? And in 21 days I was called for PRP. He said that your head is very dirty, clean it with water. Where did the staff wash my head I went to the mirror and looked at it. After cleaning the TB, 60% of the transplanted hair has disappeared from my face. And even after washing the hair, the hair is still there which is slightly bent from one side. What hair or grafts will affect the roots? Please answer sir?
@AmiyaPrasadMD
@AmiyaPrasadMD 10 дней назад
Hi Faisal Khan, Scrubbing your scalp this long after a hair transplant will not dislodge grafts. Even if grafted hair sheds, as long as the grafted follicle is in place, hair will regrow. As for the bending of your hair shafts, only your transplant surgeon can answer this is as they know the details of how your transplant was performed. I hope this helps you.

@AmiyaPrasadMD
@AmiyaPrasadMD Год назад
Hi Lastofus, Grafts are secure enough after 48-72 hours after surgery that a tool is needed to dislodge them. Grafts that fall out if you scrub 72 hours after surgery didn't take to the scalp, and would not have grown terminal hair. You can wait 7 days, or whichever time your surgeon recommends as they know the details of your scalp condition, and the way your transplant surgery was performed. I hope this clarifies things for you.

Hello doctor I hope you’re doing fine, just some quick questions, is prp worth it after hair transplant? And is it okay to apply essential oil such as Rosemary oil on the scalp a month after hair transplant? And last question can a hair transplant be done more than once in a life time Thank you so much for your information
@AmiyaPrasadMD
@AmiyaPrasadMD 2 года назад
Hi Sofiane Belbrik, Treatment is still needed after a hair transplant because transplants alone don't treat progressive hair thinning. I don't see the benefit of using rosemary oil or other essential oils after a hair transplant, so I don't recommend it. You can have more than one hair transplant, but it doesn't mean you will get more scalp coverage. Hello, doc. I have always loved your videos. I had a specific question. What should be the minimum gap after a FUE hair transplant for PRK refractive surgery? I underwent my 2nd FUE hair transplant on the 18th April. I got the first one done 3 years back. I also plan to get my PRK refractive surgery done after this. I will be wearing a surgical cap during eye surgery. The eye surgeon will put his hands on my scalp while performing the surgery plus I will not be able to take a head bath for 5 days post-PRK. Considering all this, how many days should be the minimum gap between the 2 surgeries so that there is no risk to my transplanted grafts?

Hi jougetsu, It's highly unlikely that you lost a graft 3 weeks after surgery since grafts are firmly in place by this time, and any grafts lost by this time didn't take to the scalp with surgery, and would have not grown hair anyway. @AmiyaPrasadMD
@AmiyaPrasadMD 4 года назад
Hi Pavan Kalyan, It's normal for hair shafts to shed, but as long as the transplanted hair follicle remains after shedding, then the hair will grow back. This long after your transplant, your grafts are firmly in place and will not be dislodged by scrubbing. If you happen to lose a grafted hair follicle now, it wouldn't have survived anyway as it didn't take to the scalp and a blood supply. You can ask your surgeon's advice about when you can go to a more aggressive shampoo for dandruff treatment, as they know the details of your surgery and your scalp that I'm not aware of. I hope this helps you.

Should they be left to grow normal, they need to be removed or they will shed naturally? As you have mentioned that the follicle embeds inside so it's not easy for them to be plucked out, what should be done ? Should the transplanted hair shed quickly so follicle beneath is allow to grow the hair ? Does the hair at the top hamper hair growth from the follicle ? Your assistance will be highly appreciated.

@AmiyaPrasadMD 3 года назад
Hi AF B, If you are already washing your hair after your transplant, and the scabs are gone, that's all you need to do in physically handling your scalp. There are different rates of shedding and regrowth for individuals who had a hair transplant. However, treatment is still needed after a transplant for treatment of progressive hair thinning, as the transplant does not treat progression at all. You can refer to these videos about why treatment is still needed after a transplant, and how to go about it, and how scalp coverage can still be improved after a hair transplant: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-rIlI55F478w.html ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-A3huSHGYrFU.html

I’m 6 days post op and currently talking finasteride minoxidil pill I’m curious as to when is a good time to do hair regeneration. Will you have to do the hair regeneration treatment the rest of your life to keep the hair thick or is a 1 year process?
@AmiyaPrasadMD
@AmiyaPrasadMD 4 года назад
Hi Antwain 84, The ideal window for the TrichoStem Hair Regeneration treatment is within 1-2 months after hair transplant surgery to help with graft healing, to thicken native thinning hair, and to stimulate growth from hair follicles that are still viable, but currently not growing. The treatment lasts between 3-5 years, and some patients get a second treatment 15-24 months after the first at no additional charge. While a large percentage of men do continue with finasteride for maximum effectiveness of the treatment, all TrichoStem Hair Regeneration patients can stop or be weaned from minoxidil. I have treated patients again after 5 years, and their hair still looked pretty thick at the time. I hope this clarifies things for you.

Hi Sofiane Rricco, It is highly unlikely you damaged the follicle this long after the surgery as the grafted follicles are firmly in place. It is normal for hair to shed, but the hair will grow back. Tissue that comes out with the hair is likely keratin or dead skin, not the follicle. I hope this clarifies things for you.

Hello Doc, thanks so much for the info! I want to ask you, when is it safe to do SMP in our scalp for areas where there is no transplant? I had a transplant done to my crown but in my mid to front area I have hair but quite thin and having my hair short shows it as it is see-through. Can I do SMP right now in these areas? I am 10 days post-op. Thanks again!

Hi iamMusicman, I suggest you consult with your transplant surgeon who can examine your scalp during healing, and knows details of your transplant before any more aggressive treatments to your scalp. Transplanted hair needs to grow before scalp micropigmentation can be considered as it's too soon to assess your transplanted hair growth. Hi Zain Lala, While treatment is still needed after a hair transplant for progressive hair thinning, for men I usually prescribe finasteride to slow hair thinning, and our TrichoStem Hair Regeneration treatment to thicken thinning hair, and stimulate hair growth from dormant hair follicles. Unlike PRP alone, our treatment only requires 1-2 treatment sessions, with benefits lasting 3-5 years. Our TrichoStem Hair Regeneration patients don't need to use minoxidil after our treatment. Mesotherapy is not practiced in the US. You can refer to these videos on why treatment is still needed after a hair transplant, why our patients don't need to use minoxidil after our treatment, and how our treatment is different from PRP alone: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-rIlI55F478w.html ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-T5iPdCfZ2Ng.html ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-vZAGaAssewY.html

Hi Nakkeeran Srivasan, I'm not sure what you mean as minoxidil is a topical drug, which isn't applied with syringe. Treatment is still needed after a hair transplant to treat progressive hair thinning, which isn't treated by a hair transplant at all. I wouldn't recommend minoxidil as a treatment as there are better, more long-term sustainable treatments available, like finasteride to slow hair thinning, and the TrichoStem Hair Regeneration treatment to thicken thinning hair, and stimulate hair growth from dormant hair follicles. You can refer to these videos about why treatment is still needed after a hair transplant, the limitations of minoxidil, and why our TrichoStem Hair Regeneration patients don't need minoxidil after our injection treatment: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-rIlI55F478w.html ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-sv82XSpQfnI.html ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-T5iPdCfZ2Ng.html

Hi Brain Hax, Scabs are just dead and dry skin, not the follicle. A small bulb at the end of shed hair can be dead skin or keratin, and not the graft, so hair still grows after shedding. Hi doctor. I am on my day 10 after hair transplant. While washing my head today, I noticed a lot of hair shaft coming off. I don’t see the root or the white bulb. I am not sure if they r the grafts or the existing hair. Does it happen after day 10? Am I washing it in the wrong way?
@AmiyaPrasadMD
@AmiyaPrasadMD 3 года назад
Hi govardhan gopi, It's likely just normal shedding, and with the grafted follicles still in place so the hair will grow back. If you have concerns about your transplant, you can consult with your transplant surgeon as they know details about your surgery and scalp that I am not aware of. I hope this helps you.

Hi Raghu, While it's OK for my patients to do it gently with baby shampoo, I suggest you ask your transplant surgeon about it as I don't know the condition of your scalp after your transplant surgery, and I don't know the details of how your transplant was performed. I hope this helps you.

Hello doctor. I'm now 8 days post op and not sure when to start vigorously washing the scabs off. Also do you recommend going under the full pressure of shower or to pour the water over my head when removing the scabs? Thank you

@AmiyaPrasadMD 4 года назад
Hi Brian Minard, I suggest medium water pressure at the most, and gently scrubbing your scalp. You should follow your surgeon's advice about this since they know the details of your transplant surgery. I hope this helps you.

Sir I remove crusts & scabs on vigorous massaging on 4 day of hair transplant....I saw lot of hairs like white bulb on my hands...there is no bleeding ,paining& redness.. myhead is clean now....is it any problem the hair root is still there or not sir.... . . Please reply doctor
@AmiyaPrasadMD
@AmiyaPrasadMD 4 года назад
Hi Siva raman, The white bulb at the end of the hair shaft is likely keratin, or dead skin, and not the follicle. I usually recommend scrubbing with baby shampoo this soon after the transplant to clear scabs. If you have any concerns about your transplant, you should consult your transplant surgeon who knows the details of your scalp and your surgery. I hope this helps you.

How come some doctors say 10 days till the hair root is attached and some say it only takes a few days ?
@AmiyaPrasadMD
@AmiyaPrasadMD 2 года назад
Hi, Hair transplant surgeons have differing amounts of hair grafts placed in a single procedure. As I explained in your other comment, nutrition from surrounding tissue is important in graft healing, so I make sure that graft density does not exceed what the skin tissue can heal effectively, or you risk losing starving grafts and losing them. However, it after 48-72 hours it takes for hair grafts to be firmly in place, with more healing time the surrounding tissue becomes softer, and the hair grafts become even more firmly rooted in the scalp. I hope this clarifies things for you.

Hi Umer Tariq, Grafts do shed hair, but as long as the follicles are in place, they will regrow hair. I strongly recommend you don't pull out grafts. @AmiyaPrasadMD
@AmiyaPrasadMD 3 года назад
Hi Jake Smith, This long after surgery, the grafts are firmly in place. Any grafts that may have completely dislodged after the first 48-72 hours didn't take during surgery, and would not have grown hair. I hope this clarifies things for you.

hi doctor, what do you mean by a graft above the skin attached by a clot will likely not survive? Im quite worried as a few of my scabs are above my hair and when i removed the scabs the hair is still there on my hairline.
@AmiyaPrasadMD
@AmiyaPrasadMD 2 года назад
Hi Haziq Shah, A grafted follicle only attached by clot did not take to the scalp, so it won't grow hair. However, even if a hair sheds with the scab, as long as the grafted follicle is rooted to the scalp, it will survive and grow hair. I hope this clarifies things for you.

Hi karthik keyan, As I don't know where you are with your general healing after your transplant, you should be following up with your surgeon. It is generally safer to remove scabs through scrubbing than by peeling them off. I hope this helps you.

H Doctor, i haven't understood the meaning of "a graft above the skin attached by a clot will likely not survive". Do you mean in the first 48 hours or also After? What Is the difference between hair in Scrubs and hair in a clot? Thank you for your kindness.
@AmiyaPrasadMD
@AmiyaPrasadMD 7 месяцев назад
Hi @giovannifioccola, A graft above the skin attached by a clot means the graft didn't take to the scalp, and is only held by the clot. Without taking to the scalp, the graft will not survive. It is possible for high blood pressure from nervousness and anxiety to cause a graft to be pushed out of the scalp during surgery, which is commonly called "popping". This is why it is important for the patient to be sedated and relaxed during transplant surgery to prevent popping. I hope this clarifies things for you.

@AmiyaPrasadMD
@AmiyaPrasadMD 3 года назад
Hi Mike Philpot, A pimple on the scalp shouldn't be a problem for graft survival, and it's actually pretty common. It's important to keep your scalp clean for proper healing after your transplant. If you have any concerns, you can refer it to your surgeon who can examine your scalp, and knows details about your surgery. I hope this helps you.

@AmiyaPrasadMD
@AmiyaPrasadMD 4 года назад
Hi momo momo, Using a razor on the scalp this long after a hair transplant won't affect growth of transplanted hair, but it might cause ingrown hair, or an infection, so your surgeon is likely trying to avoid unnecessary risks to your scalp. I hope this clarifies things for you.

Very good evening dr prasad. Sir I have gone through 1st hair transplant 1.5 years ago. Hair transplant with 2000 graft.Result was good till 1 year 2 months .But recently I have faced lot of hair fall. My working area is so dusty..I don't bother to shampoo for some days.i am concerned that I have lost so many transplanted hair recently..Is there any chance they might grow back?
@AmiyaPrasadMD
@AmiyaPrasadMD 4 года назад
Hi Arghya Roy, It is highly unlikely you are losing transplanted hair because of dust or not shampooing. You should be aware that transplants do not treat progressive hair loss of native hair. You need treatment after transplant like finasteride to slow thinning of native hair, and a treatment like Hair Regeneration to thicken thinning native hair, and stimulate growth from dormant native hair follicles. I hope this clarifies things for you.
