A the “I am wearing a hat” line is from the song earlier in the show by the same name, it’s sang by Mrs Goldberg about doing what’s socially acceptable and settle for what you don’t want in life, Marvin hearing this during the song is a metaphor for him being told to do what’s socially acceptable by society and now he’s finally breaking this rule

Some people say it was for Marvin, some say he quit and the line “ I don’t smoke, don’t do drugs and there comes the bad news” is in the present which can mean he used to smoke but doesn’t anymore and some people say the cigarette is just a sex symbol and not actually a literal cigarette

A couple things I'd like to make clear: Marvin says two V BIG THINGS! I think the cigarette line was to symbolize that Whizzer is the more feminin of the two, as cigarettes were associated with lesbians prominantly at the time, but I'm probably wrong.
So I'm presuming that entire weird whispering sequence is what's going on in Marvin's mind, and I'm pretty sure "I am wearing a hat" is one of the songs Trina sings, so he's thinking of Trina and probably feeling kind of guilty.
shea please listen to the rest of the soundtrack. The song I’m wearing a hat is about doing what’s socially acceptable and easy (a metaphor for Marvin’s marriage to Trina) and having sex with whizzer is him completely going against this
