This is the most knocking question when someone wants noninvasive or less invasive treatment for their eye bags, whether it is a good idea to have some injections like kibella. Dr. Amya you always get us informed about any sensitive issue and area with an unbiased opinion. In fact I personally appreciating you because you always discussed the topics and issued in great concern of public knowledge and their benefits as well as their interests. People have variety of options through internet knowledge but the most important thing is: insight of the outcome and complications after getting any treatment. Dr. Prasad I always wonder how can the fat be be trimmed to retain smooth surface so there are no peaks and valleys under the skin? I have 2 ugly but perfectly smooth fat pockets under each eye. My concern is that if I had blepharoplasty the area may no longer be smooth. Is there such risk with this procedure? In fact what are the risks for lower eye lid blepharoplasty? Thank you for your very informative videos.

Hi Taksobie, The risks of lower eyelid surgery are minimized by the expertise and experience of the doctor performing the surgery. A specialist like myself can sculpt only the excess fat to preserve contour, and do it without an external incision of the lower eyelid skin, so it looks completely natural, with no visible sign of surgery. Results are also determined by the expertise and experience of the doctor, as some doctors may remove too much or all the fat, resulting in a hollow and changed appearance of your eyes. I totally agree on this video. Why no one wants to pay for surgery is beyond reasoning. I called a surgeons office and the receptionist quoted $ 6000.00 Canadian for under eye surgery. Is it really that more expensive than all these other treatments average costs Canadian ( Fratora $2000.00 , Facetite $5000.00 , and Ultratheapy $? which don't address the problem in the first place for eyes which they promote. Fillers are not cheap either and don't last long ....4 months to a year depending on the type of filler. Maybe needed until bags are uncontrollable but to use them non-stop to avoid eye surgery i shake my head. My question...how long does under eye surgery normally last and the average cost from your office ? Thanks

Hi Blendor Art, Thanks for your insight! Lower eyelid surgery can last a very long time since once the fat that causes eye bags are removed, it doesn't return, but it does not prevent new fat from prolapsing, but this doesn't happen too often. I recently got a review from a patient who I did lower eyelid surgery on more than 20 years ago, while I was still in residency, and until now her eye bags have not returned. Hi @MrGB1999, I generally don't advise dissolving of prolapsed orbital fat under the eyes because it is inaccurate, and you don't want to dissolve all the prolapsed orbital fat because it will leave a hollow appearance, so you will still look tired. Sculpting of prolapsed orbital fat in lower eyelid surgery is precise, and a little fat can be left behind to prevent a hollow appearance, and to create a beautiful contour to blend the lower eyelids with the upper cheek. Hi evan wallace, I suggest you ask for transconjunctival blepharoplasty by name which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality. We don’t have set costs for surgery as each patient has different needs, and some surgeries are more extensive than others. Surgeons all have different aesthetic styles and practice focuses, which makes it hard to recommend a specific one, so I suggest finding one whose aesthetic style seen in their before and after pictures resonates with you. I want to try mesotherapy for grafted fat under my eyes. Do you ever use it? So many surgeons told me it is practically impossible to remove grafted fat surgically. I am so devastated. Famous Beverly Hills surgeon put 2.5 cc under each eye. It was way too much. Not sure what to do😢

Hi Kristina Hill, Mesotherapy, which is vitamin injections, is not practiced in the US. I don't recommend or perform fat grafting under thin lower eyelid skin. However, as an eyelid revision specialist, I do perform surgical removal of grafted fat under the eyes. I hope this clarifies things for you.

Thankyou doctor. What about for upper eyelid lipid deposition. Which is asymmetrical. I have saggy upper eyelid only on one side which I developed after chronic conjunctivitis (during my teenage years) am in my early 20s now. Is tretenoin effective in treating saggy upper eyelid.?

Hi @rockysmart1234, I don't think this is a safe approach as sculpting and removal of fat should be precisely done by the surgeon to leave a little fat behind for a smooth contour, and to prevent a hollow appearance. If you are relying on a dissolving agent, then it becomes imprecise, and dissolve too much fat, leaving a hollow appearance; or it doesn't dissolve enough fat, leaving a puffy appearance. What about peptide based injections that are supposed to dissolve fat such as Prostrolane inner-B SE? I know it is not approved in the US but it is approved in the EU. It seems like a good way to start solving my eye bag problem, but are there major risks for complications? I cannot find many reviews based on first-hand experience. My problem is not huge and I don't have sagging skin (yet - this is also why I want to tackle this problem before gravity takes over).

Hi Padam91, Even if it was safe, I still wouldn't recommend it because it would be highly imprecise. When I perform lower eyelid surgery, I leave a little fat behind to prevent a hollow, and for nice, continuous contour under the eyes. If you melt fat, you don't know how much will be melted, and how far it will reach. Also, the eyeball itself is highly delicate, so you don't want to put anything near it that can potentially affect it. I hope this clarifies things for you.

do you ever use ultherapy to dissolve eye bag? I had a surgery where the surgeon put in too much grafted fat. I heard ultherapy can disolve your on adipose tissue so i thought maybe it could dissolve my extra grafted fat that was put in. please give your opinion.

Hi Kristina Hill, I don't graft fat into the lower eyelid because of the high risk of complications of grafted fat in thin lower eyelid skin. However, as an eyelid specialist, I do perform surgical removal of grafted fat in the lower eyelids placed by other doctors. I would perform surgery to remove grafted fat in the lower eyelids, and don't recommend it done by a thermal device alone. I hope this clarifies things for you.

Dr Prasad, when people such as traditional Chinese medicine doctors say that eye bags are due to poor kidney or liver health and they recommend ear massage as they say the ears are connected to kidney or liver...is this just nonsense? Since from you, it seems like eye bags are due to the fats under the skin prolapsing and bulging out....a physical issue rather than body's internal issue. Do you have any insight or comments to this? Thank you. :)

Hi Grateful Princess of God, People with no kidney or liver issues can still have eye bags. The fat that causes eye bags is naturally present around the eyeball for cushioning, but it prolapses forward in some people, and is most often genetic where several generations of the same family have them. The ears have no connection to prolapsed fat. I hope this clarifies things for you.

Hi clint8u, I don't use that device in my practice, but it’s important to remember that the particular device does not determine results, or causes complications, but rather the experience and judgment of the doctor using the device. Hi Dontrel Dontrel, I do perform eyelash transplants, but it is quite a the commitment for the patient. Since donor hair is taken from the back of the head, once transplanted, these hairs do grow long and need to be trimmed. Their may be some styling needed as well for the lash to curl, depending on the natural curl of the graft. Also, there is a high risk that these transplanted lashes may also be pulled out again due to trichotillomania. I hope this clarifies things for you.

Do you advise to inject fillers with a condition of Ocular Rosacea? I have this problem and skin. On antibiotics for life for my eyes. Antibiotic Pills daily and antibiotic gel in eyes daily.

@@AmiyaPrasadMD hi , thanks . I was curious. No family Dr., eye specialist have a clue what to do with my eye lids. They lifted the top eyelids up and all yellow and the tissue thickened to the back of my eyes. This causes no stop migrains. Neither of these Drs agree my migraines are from my condition but admit never had a patient with this condition. I cannot keep taking ritriptan they also precribe nonstop . They are drooping as to heavy. I'm not ugly lol . I can hide this condition well with makeup. But i was curious on surgery, filler or botox. I was diagnosed by a dermatologist after 2 years at emergency for infected eyes and vomiting. Have you ever came across a patient with this? Do you have any idea what is my best method to take care of thickened droppy eyes which effect the eye muscles . I'm getting no where with anyone of them as i know no cure ....but in this situation have you had any referred for eye lid surgery? Thanks you don't have to answer

Hi TheReal Chrisch, Heavy, hooded eyelids are caused by excess fat, and stretched skin. Fat dissolving injections are far too unpredictable to reduce the precise amount of fat, so they could leave your upper eyelids looking hollow if they dissolve too much, and injections won't treat the hooding at all. Hi LS F, The scary stories you hear about are from doctors and practitioners who are not experienced in injecting or operating in the eye area. A doctor experienced in the delicate eye area drastically reduces the risks of this procedure. Many doctors don't do injections in the eye area as it is highly delicate. Deoxycholic Acid is used in very little quantity in under eyes treatment, doses are different for tummy fat, chin fat and under eyes fat, need your opinion again @Dr.Parsad

Hi @rockysmart1234, It takes precision to sculpt fat behind the lower eyelids because not all the fat is removed to prevent a hollow appearance, and for a continuous contour into the upper cheek. You can't get this precision from using deoxycholic acid, as even in reducing chin fat, multiple treatment sessions may be needed, with each session requiring multiple injections. Hi Alaa Ttrabulsi, From a medical standpoint, blocking DHT-formation with finasteride should affect beard growth as DHT does stimulate growth of body hair. However, from a clinical standpoint, I don't see adverse effects on beard growth from my male patients taking finasteride as they still appear to have good growth of facial hair. Hi @marisoldunk6426, Endolift is a laser-based treatment, and results of any laser treatment are determined by the doctor performing it. Trying to achieve a surgical "lift" with a laser alone often leads to overexposure and overheating which can damage the skin, and the essential fat layer behind the skin. Hi @Mr_bigboy, 4:00 of this video is a comprehensive deep placement of filler at the foundational bone level. These are thicker fillers that are not used for areas with a lot of facial movement like the under eye area, but these thicker fillers used in areas like the cheeks and chin can last longer at up two years. Hi @rockysmart1234, I don't use high frequency ultrasound devices in my practice because I find them to heat too quickly. I do prefer devices capable of more gradual heating, and with more temperature control. I hope this clarifies things for you.

Hi salman khan khan, The face is naturally asymmetric, especially at the cheeks, and everyone has facial asymmetry to a degree, even models and actors. In most cases, this asymmetry is not noticeable by others. If it really bothers you, you can see an experienced doctor for an evaluation. I hope this helps you.

Doc my eye fillers gone wrong... What should i do?

Hi Morris Vincent, Sorry to hear about your problem. The first thing you should do is see your doctor who placed them. If they were hyaluronic acid fillers, they can be dissolved with the injectable enzyme hyaluronidase. If they are a different material, then it can be more complicated to remove the filler. I suggest you see your original doctor about this concern as soon as you can. I hope this helps you.
