Dr. Peter Coleman, who is Board-certified in Addiction Medicine and has been in recovery from opioid use himself since 1984, shares his perspective on what makes opioid addiction unique and what means about the best way to treat it.
-Safe, Comfortable Outpatient Detox
The Coleman Institute for Addiction Medicine has been a leading outpatient addiction treatment center since 1998. Our innovative withdrawal management programs, often referred to as The Coleman Method, have helped thousands of people free themselves from addiction to Alcohol, Opioids, and Benzodiazepines. Originally founded in Richmond, Virginia, our network has expanded to cover 14 cities across the US.
-3 - 8 Day Opioid Detoxification Process
For short-acting opiates like oxycodone, most patients complete their detox and begin Naltrexone therapy within 5 days. Longer-acting opiates like Suboxone and Methadone typically take 8 days. Our standard opioid treatment package in Richmond, VA, Wellesley, MA (Boston area) and Willoughby, OH (Cleveland area) includes the detox and 6 months of case management and Naltrexone therapy. As a non-addictive opioid blocker, Naltrexone dramatically reduces cravings so that patients can focus on their recovery. We have a 98% success rate for completing detoxification and starting Naltrexone therapy. During the detox, we work with patients and their families to develop an appropriate aftercare plan. We also offer a convenient, 3-day outpatient alcohol detox and a Rapid Benzodiazepine Detox.
-Caring, Empathetic Environment
Dr. Peter Coleman understands first-hand the challenges our patients are facing due to his own personal struggle with the disease of addiction. He has been in long term recovery since 1984. Dr. Coleman has dedicated his career to treating addiction patients and creating a team and an environment conducive to long term recovery for his patients.

Alcohol wasn't easy to stop but easier compared to cigarettes or opiates it's like the difference in being shot a few times or eaten by 10,000 ants. Alcohol is by the day where opiates and cigarettes are by the minute. It's much easier to tell myself I don't want to drink than to tell myself I don't want to be on opiates because drinking is so sloppy and it's a few hours a day where opiates start in the morning and continue all day just like cigarettes. This Dr knows the reality of opiate addiction, I wish more Drs did.

I think a lot of times it is more difficult to get off pain meds because the person is really in pain even if they don't even realize how much pain they are in because they've been in pain so long, but their body knows they're in pain and it craves pain meds because you are actually in pain. Sometimes I'll be going along, busy and when I slow down I realize that I'm in a lot of pain, then suddenly I want my pain pill.

I do understand depression. As a 9 year old I couldn't save my brother and watched him get ran over crushing his face in. I took all the blame and blamed life damming it deep down. This is definitely what led me to drugs. Lately, it's called slow in my city, the fentanyl, and I've been playing tug a war with her for several years. But... I'm on day 6 cold turkey. Somehow, feeling great. Am smoking weed, but that's all. No sleep, except 3 hours for the first time yesterday. Been taking it differently this time for some reason. Honestly this video sounds like it makes the case for opioids not against them. We all know about the tolerance situation but I was on three Norco tens a day for over 20 years and never abuse them and didn’t ever try to get more or move on to street drugs I crushed my hip and pelvis in a fall through a roof. The side effects are so slight that you can’t even tell people are on this drug it helps with mental pain it helps with physical pain it makes you happier, what is the problem? Not everybody pushes it to the extreme with opioids a lot of us just want to have a peace of mind and be pain-free or at least less pain and there’s nothing wrong with helping mental pain as well. My doctor retired and I lost my pain meds all at once it’s been years and I feel terrible I hurt all over, I’ve been on disability because of it for years and now that I have a medical marijuana card a pain clinic won’t even see me in Ohio. It seems the same situation with benzodiazepines. Anything that actually works they don’t want you to have any part of. They want you on mine controlling SSRIs and antidepressants that completely take you out of the game and numb your mind. They are just as hard to get off as any other narcotic. They will shove them down your throat as many as you can eat but if you ask for a Xanax or a pain pill because you have pain or anxiety they treat you like the devil. What’s the point of making anti-anxiety medication when they won’t prescribe it to people with severe anxiety and what’s the point of pain meds that actually kill pain but again will not prescribe them. I am disabled and my whole left side was crushed why can’t I get a pain pill? Literally suffering every single day of my life now. At least they have tried me on 13 different types of antidepressants! Never felt so bad in my life after taking SSRIs. The system is screwed up.

I would be up the creek if I had to depend on an anti-depressant, I absolutely cannot take those things. I've been on pain meds for 20+ yrs and it doesn't appear to be so difficult to get into a pain mgnt here in Ga. But my sister and her husband live in SC down along the coast Beaufort her husband has MS and has excruciating headaches at times and my sister says there are no pain meds to be had there. @@amberlilly4101 they’re taking everyone’s pain meds in Ohio now. I was talking to my neighbor up the road and he was on pain meds and his doctor has retired now as well and they gave him a referral to a pain clinic but when he called the pain clinic to set up the appointment they told him they would not be prescribing opioids they were going to be doing other types of pain management like physical therapy and injections. They are making it to where you can only get opioids for end of life pain. They blame the war on opioids and all of the deaths from them on people who have legitimate prescriptions for their pain but it’s actually all of the fentanyl pouring over our border that is killing everyone. Watch the suicide rates and the accidental overdose rates climb as they crack down more on things as simple as Vicodin that you can’t even get unless you’re dying in Ohio. When I was young I remember going to urgent care with a toothache and getting prescription for 30 Vicodin’s with a refill. Nobody thought anything about it. If you’re having no issues getting yours be glad but be aware because they are cracking down no joke. I never thought I would have to go without mine after 20 years and I’m sitting here doing it now.

I agree with the opioid part. Trying to take the pills away just gave way to fentanyl and now everyone’s dying. And people like you are in pain. However with benzos I can’t say I agree. The withdrawal from those is absolutely BRUTAL and lasts many months, even years. And they can cause CNS damage. @@shadowX9365 Yeah I’ve been on the benzo‘s for years also. I was on 6 mg a day of Xanax at my highest point and I’m down to 2 mg right now. I am hoping and praying I don’t have long crazy protracted withdrawal that last for years. Honestly I do fine on 2 mg and if I could stay there I wouldn’t worry about it but they are cutting back on opioid and benzo prescriptions. It’s just a matter of time and I would rather get off of them in my own time starting now while I still have a script for them instead of on their time and having to come off too quick. I got cut off the first time when I lost my doctor and had major issues then found a psychiatrist but they’re already sounding like they’re wanting to cut back on the 2 mg. Opiate withdrawal was definitely the worst but tbh alcohol and benzos was no joke as well because I had withdrawal seizures from alcohol. While it's not the same kind of withdrawal feeling, it pretty rough. But still opiates was the worst. Oddly enough when I was younger, I tried to kick Suboxone a few times and wasn't ever able to because obviously I would withdrawal. But a couple yrs ago I finally did it and it wasn't that bad at all. Idk why it was different that time. It wasn't like I really wanted to quit or was motivated to. I hear you. Some of these doctors talk like we are taking pain meds for the fun of it. Even if you have an understanding doctor, the pharmacists give you a hard time. Now the DEA is cracking down on the pharmacies and throttling the amount they can order. Last month on my fill date Walgreens was completely out of pain meds and didn't expect an order for TWO WEEKS. The truly infuriating part is that we legitimate pain patients who need this medicine to function are treated like addicts while the government allows illegal fentanyl to pour over the border. I am 75. My body is not going to heal. The failed implants are not going to suddenly start working. The arthritis is not going away. I will never get another surgery that only causes more scarring, less mobility, and more pain. I am a Christian. I trust God. But sometimes the medication is how He helps us to keep going. I know a lot of people who have ended it all because the pain was more than they could bear. It's a horrible thing for surviving loved ones. I'm not going to do that to my family. I am so tired of having to drag myself to the pain management doctor every 4 weeks because they can't give you a refill. The added stress of not knowing if I will be able to get the medication is raising my blood pressure so now there's one more thing causing me anxiety. I cold turkeyed off this crap 11 years ago. I tried every including prednisone which caused a 40 lb weight gain. You're right about CBD: useless. Now I'm hearing about kratom. Really? Honestly, I'm afraid of it. Welp, time to get ready for the PM appointment. The doctors are very nice, but the government is trying to take prescription treatment out of their hands. I will pray for you.

I like how you put that. To transfer your pain to the Dr , so he can understand how you feel. Every day i wake up and some days i cry. I think How am i going to get through another day hurting. I take my pain med and wait for it to work so i can get up and get a shower. Id love for anyone that says that ppl shouldn't take pain meds, to do cbd listen to music meditation and so on, to feel like i do for one week without pain meds. Maybe they can take a small sharp pebble and put one in each shoe so every time they step down it hurts like hell. Go to work like that and be on your feet all day. Then when you take those sharp pebbles out of your shoe , it dont hurt to walk anymore. WHAT A RELIEF?? Well thats what pain meds are for chronic pain patients . We need relief we deserve to the help. Im tired of the stigma . Yeah if i stop my meds ill go through withdrawals. Ill get help . But untill this pain is down 50% ill take them. One of the problems with drs cutting off pain meds is the ppl go to the streets and get them. They can get ones laced with fyntinol and die. Or they can take more than they should because they dont have to drop for the pain dr every month , so why not? DRS cutting back too much or cutting pain patients off because they are scared of the cdc is one of the problems pain patients that are monitored by pain drs every month usually stick to the rules so not to be cut off. But tey still get punished and cut off of cut back too much or too fast Just because the government and cdc put all the statistics in one category. Opoid deaths. Over dose. From drinking with them orbuying off streets laced = all ppl and chronic pain patients cut off cut back. FK them all. They will find them on the streets, so why not take care of your pain patients for what they deserve. Pain relief so hey can have a decent life.

I relate, I’ve been on Dr prescribed opiates for over 20 yrs for chronic pain. I don’t know life without them, I was on a time released one for the last 10 yrs. Only time I had problems is when my pharmacy wouldn’t fill it, or my insurance refused to pay for it. So my dr just switched me to pills not time released, changing my dosage every month. I’ve been through wd many times, it’s hell in hell. I’m slowly lowering the dosage they tell me to take, I’d rather find other ways to deal with my pain then go through this forever. I do smoke pot, it helps a little. I don’t crave it, I don’t get a rush etc at all. I wish I was never put on them, they did give me fentanyl patches years ago. I passed out from it, that scared the hell out of me. I was done with that game, so now I have 3 different milligrams of the MS Contin / morphine . So that’s going to help me get off them, I’m just doing it a couple weeks at a time. I’ll get off them I have no doubt on that, it’s just hard learning the dosage to lower and avoid wd. I too miss my energy and my memory etc, I just forget what life’s like without them. I wish you luck 🙏

I was 7 and by the age of 14 I was using street opiates and was in and out of psychiatric units and detoxes. It's weird because in the UK they're usually very careful about what they prescribe to you....this was in 2001 though so I know it's become more regulated and understood. I've been addicted to opioids ever since I was introduced to them, first by a doctor which was Tramadol in 2012 and then by a man I met in rehab in 2014, with him I went into Oxy. Where I live, which is not the U.S, we don't really have heroin so there is no "cheaper alternative" to Oxy so getting high has always been extremely expensive. I'm on Suboxone now but I want off that shit as well! I hate that drug and I'll always regret the day I said yes to being put on it in rehab in 2017. I've been an addict since I was a teen and my whole family deals with this disease so I've been using since 14 but never ever has my life gone to so much shit as when I was introduced to Oxy and Benzo's! My life literally got ruined! There is nothing more horrific than the withdrawals from these drugs, but I will say I completely understand why that guy said "it's like getting a hug from God". I haven't gotten that high in years because of my tolerance but I'll never forget the first time I got that high, there is nothing that has ever in my life felt as good. But eventually it destroyed my life. Not worth it no matter how good it felt. I've actually been thinking about the Coleman method to get me off Suboxone but I am so scared of these withdrawals! NOTHING is as horrible!

Am a medical professional was on Tramadol low dose but for about 20yrs...was also on pregabalin for the past 5yrs, I suffer with chronic upper back pain, am now on methadone for 1yr now on 20mg/2mls per day, it takes away the back pain, I don't crave tramadol I haven't taken it for all the time I ve been on methodone which is 1yr now, and even if I miss taking methadone I don't think of tramadol, but I do take pregabalin when I don't get methadone so as to manage the pain and withdrawal, I d also take something for sleep, for nausea that's if I don't take methadone even for a day .. because the fatigue, pain, nausea, etc gets really bad that I cant even function. My question is if I start exercising will the withdrawals go away? Need to stop methadone because it's really difficult to travel everyday to the centre it's expensive too on transport that is...I need to start reducing methadone until I stop completely, but what else can I do so I don't withdrawal on methadone as well?..am willing to do anything so I can start seeing patients again...I have no energy, it was tramadol that helped me for 20yrs to attend patients as it took away pain and fatigue... please help

I'm severely disabled and have been since i was a little kid. I just dealt with the horrible pain and stress up until i got around the age of 35. Well of course i knew about pain medication so i went to several doctors and they all got defensive about it. I was running my self ragged trying to get pain medication legitimately. What really pissed me off was i knew quite a few people getting pain medication for injuries that didnt require such pain medication. Like hydrocodone 10s for a stubbed toe. Or a bicycle wreck that sprained someone's ankle...Oxytocin 20s Then there's me... I was shot on accident by my dad when I was 4, it went in my stomach at high downward angle it took out 1 of my kidneys put a hole through my liver and the bullet got lodged in my spine. Well it came to rest 8n a bundle of nerves that controls my bladder, bowels and it crippled my right leg. Well needless to say my body functions isn't automatic like a normal person, i have to cath and ill spare you the horrible details of my bowels.. Well my right leg was 90% paralyzed and when I was 5 they fitted me with a brace. It was the exact same braces that forest gump had on. Well in a bad turn of events the shop who made my brace did something wrong in the measurements and the shoe connected to the brace caused a horrible wound to appear on my paralyzed foot, Well i couldn't feel it and this massive wound got gangrene no matter how much my mom cleaned it, well long story short they ended up amputating my leg-off up above the knee. So i walked on crutches from 7 years old till 17. Then i got a prothstisis i wore it till my early 30s but it caused me to have open sores that would never heal become of the way the leg anchored. I had a big almost inch deep gash under the right of my nutsac where the leg was up to my inner thigh, and it also kept busting my stump open. So it got so bad my doctor said Tom i hate to say this to you but if you don't quit wearing the fake leg we're eventually gonna have to amputate the rest of your remaining right stump off up to you pelvis. So i was like fuck that, So ive been walking on one crutch the most part of 30 years. My arms are fucked up from the crutches, tendons are torn ive had some horrendous falls and still got that bullet in my back that hurts like hell, i have phantom pain that are devastating. So you can imagine my frustration when i got to these damn doctors and they see me they see my chart and still tell me no!!! So many doctor's havr told me know i had no choice but to take advil which doesn’t do jackshit really. Well now im borderline kidney dialysis from taking nsaids all my life for pain because these heatless doctors wouldn't help me legitimately by prescribing some pain medication without all that extra bullshit mixed in the pain medication, like oxy, morphine, ect. So ive been fucked all the way around by self righteous doctors who dont give 2 hoots in hell about people who really need and is a prime candidate for pain management.. No only thing they care about is what insurance you got and how much money they can make.
@brockowings4138
@brockowings4138 Год назад
Wow! I can’t even imagine what all you have been through. It does seem like the people who really need it for pain can’t get any meds to help with the pain. In my experience, when I didn’t really need pain killers, I’d get them prescribed and the times I felt like I really did need them, they wouldn’t prescribe. The US created a monster in the 90’s when oxy came out and they used it for any little thing that a couple pills of Tylenol could have been therapeutic but instead they started pushing Drs to write these scripts for oxy saying, “it was a miracle drug with NO side effects.” After that, they started opening pain clinics and urging Doctors to write even more scripts because peoples tolerance went up and their lower current dose needed to be increased to be effective with, lstill no side effects.” Fast forward 10-15 years later the truth comes out of how dangerous and additive these drugs really were and cracked down on Dr’s for writting opioid medications. Because of that demand for them went up as well as the prices, so people switch to heroin because it was much cheaper, stronger and flooded everywhere. Now that the monster was created, it got out of hand so fast and snow balled that there’s very little chance of, putting the monster back in the cage.” Now that my books finished and you have read my comment thus far, I thank you and those that wish to comment on this with their own thoughts is encouraged. :)))

Riddle me this: I've been taking a relatively high dose oxycodone for chronic pain (so I'm told - 120mg/day) for approximately 30 years and I've always feared withdrawals - mostly because of all that I've heard and in films like Trainspotting. However ... I DO run short on occasion and from time to time I'll have to go 2-3 full days with nothing. Here's what I notice: I don't get cravings - like where it's all you can think of. I don't buy anything elsewhere. I just make way to tuck in for a couple of days and so far, honestly I haven't encountered ANY really bad symptoms like nausea, vomiting, shitting etc. I DO become extremely weak and my motivation to do ANYTHING goes right out the window. Other than that it's almost as if I don't withdraw at all and I've never been given a decent reason why - my doctor and pharmacist simply shrug and say "fortunate." Can you give me any possible reasons for this lucky break? I should add that I take them specifically for pain and not to get high. In fact I can't even see how anyone DOES get buzzed on the stuff - because it does nothing mind-altering for me. Could it be because of that? No cravings because there's no feeling to chase? Could it be the I don't metabolize OC very well and am therefore on a much lower dose than I'm getting? To that end, it doesn't really kill the pain as much as I'd like and that is why I run short occasionally ?? I'd like to hear the ideas of an addiction Dr. I'm not denying that I DO have an addictive personality. I had to quit alcohol 25 years ago because I liked it TOO much, but that went successfully, as did the cigarette habit that went away later that same year.

120mg a day isn't all that high, a lot of heroin users would be doing what is equivalent to at least 4x times that dose in a day. and like you say you're not abusing them, and clearly aren't even succeptible to addiction since you dont even get buzzed off them. you have a physical dependence which is significant but no mental addiction which is what drives a user nuts in withdrawal... it's like heart ache, you are in love with the feeling and can't stop chasing. the mental aspect of withdrawal an addict experiences make the physical aspects beyond unbearable.. it's like completely falling apart in every way

Alcohol withdrawals are deadlier. Not nearly as painful though. I mean, if you want to go down that road, then a strong benzo or barb addiction would be "the worst addiction" but that's not my experience. Like, seizures can kill you and you simply cannot cold turkey tranquilizers or anticonvulsants including alcohol, but I'd rather go through alcohol detox than methadone detox if I had to pick. Also, worth pointing out, it's not about what "the worst" addiction is. Like, it isn't a contest. End of the day, every withdrawal syndrome sucks and no one is handing out medals for enduring "the worst one."

This is a very good video. I had a shocking unbearable unexpected withdrawal after just 3.5 days on oxycodone for shoulder surgery. I had had a minor episode years before coming off a short term perocet use post surgery - remember thinking wow if this got worse it would be devastating. It was - within a half hour there was no way I was waiting to hear back from my doctors office and ended up taking one pill - whatever it took to make it go away. I knew the risk of taking more when not in pain - it worked. We think my episode was due to reduced kidney functioning which created a higher level than normal in my body. It left me interested in the topic of - are doing enough to put the fear of God into potential users - scare the crap out of them - I think too many think oh you might get addicted then you get help no big deal. I think more prevention could happen if people know there is no way in hell you ever want anything to do with withdrawal. Addiction is not the worst bad, you need to be very afraid of withdrawal, and the opiates are the worst.

I have ckd stage 3 . Been on opioids for decade for pain .hey NSAIDS make our kidneys worse. So what the hell do we take for pain. I agree withdrawal is the worst. Why can't we just stay on instead of living in chronic pain?

@@JonKonik11 Thank you for speaking up. I should be more careful. In case it might help... This is what I asked ChatGPT to do and her response: I want to taper off Kratom. I am taking 4grams 5 times a day. I would like to taper off to zero within 4 weeks. Please send me a table format ChatGPT Here's an example of a tapering schedule for kratom, based on the information you provided: Week 1: Day 1-2: 3.5g, 5x/day Day 3-4: 3g, 5x/day Day 5-7: 2.5g, 5x/day Week 2: Day 8-9: 2g, 5x/day Day 10-11: 1.5g, 5x/day Day 12-14: 1g, 5x/day Week 3: Day 15-16: 0.75g, 5x/day Day 17-18: 0.5g, 5x/day Day 19-21: 0.25g, 5x/day Week 4: Day 22-23: 0.125g, 5x/day Day 24-25: 0.125g, 4x/day Day 26-27: 0.125g, 3x/day Day 28: 0.125g, 2x/day Note: This is just an example and may not be suitable for everyone. It's important to talk to a healthcare professional before starting a tapering schedule, as they can provide personalized guidance and support. Additionally, listen to your body and adjust the tapering schedule as necessary. Nostromo: Ian...I have empathy for you. I know what wds feel like. If I can help in any way let me know. For what it is worth I would never go Cold Turkey again. Find the minimium dose that works and go from there. I got much more help from ChatGPT than just the schedule. OpenAI.com is the webpage

FYI first switch to kratom Then switch to only stem vein Taper as much as comfortable Then Mega dose vit c Over 3days increase tolerance to high doses of it while still on kratom 4th day stop kratom On that day take as much as 5grams every two hours in juice You can do this up to 80grams per day Do that for 3 to 4 days while getting plenty of protein Then when ready taper off the vit c I also used niacin and NAC for anxiety and cravings. I got off it. U can too. The rest is-all in your head.
