Fast forward to 1:42 seconds to see the first instance of the glitch.
Current setup: Dual Fury-X with 16.1.1 driver.
This issue has been happening since December of which I reported it on community.amd.com.
I've tried things ranging from disabling ULPS, to using ClockBlocker, to turning off Anti-Aliasing. Nothing works.
When it first happened I had Dual 290x MSI Lightning cards and it I thought maybe it was the Radeon Settings driver update. But since then I've reverted back to 15.7 and even tried 15.11 beta, but the issue still exists when in Crossfire.
This issue DID NOT EXIST BEFORE DECEMBER. (at least not noticeably).
Hopefully CDPR and AMD can take a look at this before the final expansion is released.

Just a note: This glitching problem happens in other games. Not isolated to W3. Happens in crossfire.

Similar problem with Tri-X 290 CrossfireX as well, Sapphire cards. Using 16.5.3 drivers

I have a dual 290 setup with Sapphire Tri-X cards and just now am experiencing the same thing. They operate without flaw on all other games. I hadn't played the game since last October and back then the game was flawless, so maybe the Crimson driver suite overhaul released before the holidays did something? I don't know what to think yet, but upon research (which brought me here), all I can say is I'm glad it doesn't mean one of the cards is failing. I'll keep an eye out for this problem on other games, but like I said, so far so good everywhere but W3.

+J. Maddox I'd be happy to discuss more if you need help. I spent a LOT of time troubleshooting this issue. In some ways awesomely and others sadly, I did a full upgrade on my system including using Win10 and I haven't seen this issue since. I now have dual Fury X cards. IMO, if you love W3 as much as me, and have 3440x1440 or greater, you're only gonna get stable ultra frames with dual Fury X. No regrets tho. Love it. But if I was you, I'd consider selling your 290s and go with a single Fury X (or just go nVidia). A single Fury X is super stable, ultra quiet, and low temps.

i just got another 390x graphic card and i enabled crossfire. i currently using crimson 16.5.2 drivers and i get no where near 60 fps on crossfire, i only get 30-40 fps. and i get better performance turning off crossfire just using one card and get around 35-45 fps. am i doing something wrong? i disabled ulps i turn of power saving mode. on crimson radeon settings in system tab. is says my second graphic card is linked. im so frustrated trying to figure this out.

Any luck with this bro? I have a quadfire machine and the default AMD profile for witcher3.exe only permits 2 GPU's to be in use at any one time. If I force AFR compatible or AFR friendly I get severe engine flickering.
