We are finding that the panoptix lens delivers A-quality far, mid and near vision consistently. The symfony lens was our preferred lens for presbyopia-correction from 2016 until September 2019 when we started using the panoptix.
After our first 300+ panoptix cases we are finding that the Panoptix lens can be used in exchange for a symfony lens to provide better reading vision for patients that are unable to read as well as they want. This video shares our thought process as to why we decided to replace a symfony lens with a panoptix lens in a procedure known as intraocular lens exchange or IOLX.

I have the PanOptix in both eyes (second surgery was 9 days ago) and boy is it great. No need for glasses at any time! I am so glad I waited until this year.

Hi Nathan, do you have astigmatism or did you have astigmatism before the Panoptix? And are you not experiencing halos?

@@SmilingTruly888 I had a bit of astigmatism in one eye and a couple of months afterward my doctor did a little LRI (Limbal Relaxing Incision) to fix that up. After that I was at 20-20 in both eyes and loving it. I do see halos at night/twilight but it's not bad - just certain bright lights. I've heard those may reduce over time but it seems static so far (3+ months since surgery).

@@SmilingTruly888 You have to get used to the idea, if you want trifocal lenses, you will have halos, but they are not horrible, it is worth paying that price to see well at all three distances and for not wearing glasses, if you do not want halos, think about monofocal lenses , but keep that very clear, trifocal lenses = halos. I also have panoptix lenses in both eyes and I am very happy with them, but I knew very well about halos and they don't bother me at all.

Hi John, I had a symfony lens in my right eye last September and have no regrets. The halos and 'star burst' effects are a surprise to begin with, but you soon get used to them and the effect diminishes. It's worst under fluorescent lighting at first, but again it passes. One thing nobody told me before surgery is that afterward you will see the edge of the new lens in your vision as a black arc on the right side of your vision. It's most apparent when looking at something very bright like a blue sky or white background. Don't worry about this, it also diminishes over time. I wish you well in your surgery next week. - Matthew

@@JmieRU-vid I have the PanOptix lens and I get halos from point sources of light such as cars. I also get ghosting of text -- which makes reading nearly impossible. Distance and mid are good, however.

Also let me add...i am an artist so up close is important...however i am a golfer and distance is also important. So this patient of yours was a very hopeful explanation for me to see clearly someday. I never had lasic because my regular monofocus lenses were perfect until PRESBYOPIA became an issue.

I am 43 years old and developed cataracts in both eyes. I work in the dental field and have to work with loupes everyday. My ophthalmologist suggested to do mini monovision instead of going with multifocal lense for that reason. He was worried about less light going through with the multifocal and glare at night. So, I had my surgery a month ago and he placed Tecnis monofocal lense in my right eye. Following surgery, same day I started to have severe headache in the afternoon on the right side, went back to him and he reduced pressure in the eye manually and gave me meds to take as well. My far vision is 20/20 and my near vision is very poor. I have to use 3.0 glasses to read and 1.5 for computers. My biggest issue however is severe glare at night, starburst under LED lights and flickering on the side(feels like the lens is moving around) The doctor suggests that we use Crystalens on the left eye to have midrange and near range for that eye. I would rather re-do my right eye with a multifocal PanOptix since I already have glare with the Tecnis lens but at least I would have wide spectrum of vision in both eyes. Please let me know what you would recommend in my situation? Thank you

Thanks to the patient! Do recipients experience circles around lights or objects? Are these fresnel type design? Why don't they use these type of lenses in cameras so you don't have to focus?

I had panoptic lens put into both my eyes late October. Since then I have 20/20 both eyes in normal light conditions, however I have lost about 50% of my low light vision and have halo's around car lights at night and some blinking affect from LED lights. I am very uncomfortable driving at night and always enjoyed driving and flying at night. The vision improvement in normal light is amazing, however it doesn't come without some negatives.

Very true your Ophthalmologist should have warned you about night driving symptoms which are common to all available diffractive trifocal intraoculars lenses. It is a tradeoff between being able to see 20/20 at distance and near without glasses but a nightly increase in photic phenomena! That 's why such type of IOLs are discouraged in professional truck drivers with frequent night commutes

About 20-35% of patients who have undergone lens replacement surgery may experience secondary cataracts 6-18 months after surgery. It also requires treatment and consultation with a doctor.

Great topic Dr. Wong. What complications does lens exchange have? How long after (maximum) first surgery can it be done? Retinal/corneal/macular trauma? Will the exchanged lens fibrose in the capsule again? And of course you cannot exchange if you have an open capsule? Would love to match Symfony with non-yellow Synergy. PanOptix is yellow too.

Our panoptix are clear (not yellow). IOLX is very safe in the presence of an intact capsule...risk profile similar to cataract surgery or elective lens replacement. Theoretically, if the capsule is intact, the 1st lens can be exchanged for a second lens.

Hi Doc! Thank you indeed for your very detailed informative videos. I need to ask you what are the chances of these lenses not suiting any ones eyes? Example: skin sensitivity. Also how chronic is the surgery if cataract is done on a patient with previous retina detachment problem. And which type of lens is more suitable for such eyes? Waiting your kind response.

I just saw another new entrant in Europe. PhysIOL FineVision Triumf Trifocal EDOF IOL. So now three IOL to consider, Panoptix, Synergy, and now the Triumf. May be have to wait a while for comparisons to be made. I;m also interested in laser vitreolysis using Yag Laser to try and reduce floaters - Very few people offering this. The idea being the floaters are vaporised. I think that if I'm going to be paying for this I ought to get the best result possible.

Is there anything to reduce the halo effect? I had a Symfony lens (for cataract) over a year ago, and the halos are bad enough that I'm considering having the lens replaced with a mono-focal lens

a year ago i got the Restor lenses and can't see shit due to Ghosting in both eyes it's so frustrating a PRK touchup did not help, made Ghosting worse so far. Touchup done oct. 2019 now i see about 20/80 far and 20/100 reading with like 4 ghost images mangled up into one spot. If i could afford it i have it replaced in a heartbeat !!

I see many Restor patients with this problem. Panoptix is based on the Restor and will have the same issues and I've already seen a few Panoptix patients with this problem.. Was the ghosting you had there initially or did it get worse with time? How long ago was the lens placed?

Steven G. Safran Lens replaced October 2018 , but ghosting started after Prk touch up , lens wasn’t correcting right and some astigmatism left

@@achimenssle4766 Most likely some of the problem is related to the PRK touch up then/residual astigmatism. Multifocal IOLs are not ideal in some patients with certain issues and it may benefit you to have the lens exchanged for a monofocal lens if you are willing to give up some uncorrected near vision. A toric lens to correct the residual astigmatism may also be an option. You should see an excellent surgeon like Dr. Shannon Wong to see if you might be a candidate for this.

Safran yes, what I thought, before Prk just saw a double image, which the doc said that’s from astigmatism and the left eye (dominant) was not doing great at intermediate distance , close up great, far distance great , two to ten feet blurred! After touch up , reading terrible, at about 5 feet 20/40 at 10 ft about 20/50 ! Right eye, reading good , intermediate 20/30 , at 10 ft about 20/32 DOC is a very reputable man I think Dr Steven Vold

Thank you for yet another detailed video. Dr. Wong, how safe is explantation after poor results from multifocal lens? How many years out from initial surgery is explantation safe? I would love to hear your input on this as I can't find much information on this online.

Explanting a lens can be done safely for many years after initial implantation in most instances. The main issue is that after a yag capsulotomy - which is done in about 1 in every 3 eyes with a lens implant - replacing the explanted lens with the full array of lens options today can be very complex.

I will soon need cataract surgery and really appreciate the educational experience i am having through your you tube channel. One question...are progressive contact lenses like the progressive implants. I really dislike the progressive contacts because distance mid range and reading are blurry, double vision, and ghosting. My doctor said the implants are different than contacts and better . She mentioned the "new" implants seeming very good and now i sm guessing she was speaking of the panoptics lenses. That conversation was November 2019.

Each eye is unique. We usually use panoptix to both eyes. However, for all patients we offer all options, determine what the patient's expectations are, then make the recommendation on lens type based on the patient's preferences and expectations.

Hi Dr Wong - First, thank you for all your very informative videos. I have had Lasik for monovision, near vision on my left, distance on my right eye, and extremely happy with the results and my vision (as I'm approaching age 60). As I see these IOL implants(Symfony, Vivity, Panoptix), I wonder if you have performed or have considered different implants on each eye (ie Vivity on the right, PanOptix of the left) to address patients like me that are used to and happy with monovision? What are your thoughts on such an approach? TIA

Thank you Dr Wong for your informative videos.I am booked in to have the Vivity lens implanted in a months time (end of Feb) at the recommendation of my specialist.Having read a lot about the Panoptix lens and not knowing a lot about the Vivity,i am not sure if i should go ahead at this stage.I need glasses for both reading and far sight and my desire is for a glasses free outcome.Is there a comparison with these 2 lenses?

Good evening Dr. Wong. Thank you for your videos. I happened to see this video only after my right eye cataract surgery implanted with AcrySof IQ Toric IOL. My left eye does not have astigmatism and should I go for monofocal or multifocal to have a better nearsightedness and the surgery is in two weeks time. Thank you again in advance.

Dr Wong. Have you ever placed a synergy lens in one eye, and a symfony lens in the other eye. I had a symfony in one eye and yet to have any lens placed in the other. Im somewhat disappointed with the near vision with the one symfony lens and, before I have one placed in the other eye, I'm wondering what my options are. Could I have a Synergy lens implanted in the left eye to improve my near vision.

A synergy and symfony combination would be a good option for some patients. There are many options. I personally have not yet mixed these two lenses yet.

What are the risks of doing another surgery. My overall vision with Symfony lenses is worse than it was pre surgery. My optometrist says the risk of blindness is huge. The dr who performed the surgery just shrugs his shoulders and says sometimes it doesn’t work.

Dr. Wong I had two Panoptix lens replaced in March 2020 in Trinidad. I have noticed the halos and the shadows around my eyes like if there is something in my eyes. The glare bothers me a lot. I see the letters with shadows. Its now six months and nothing has changed. I am seeing much better but those are the issues I am living with. In addition I have dry eyes to deal with. I am 64 years old and still work on computers. It is very uncomfortable but there's nothing I can do here at this time.

I had a PanOptix Multifocal IOL placed in one eye last month. Poor night vision, halo, glare, sparklers, 20/25 distance, intermediate blurry, closeup blurry. Disaster. I'm getting this out as fast as possible. considering Vivity.

Good afternoon, Dr. Wang. Thank you for your videos. Tell me, is the yellow lens filter useful ? Or it is better to choose an IOL without a filter, because it reduces light transmission. Now most IOLs are produced with a yellow filter. There are Symphony Tecnis and Acrysoft Panoptic transparent ? (Russia). Thanks.

I have not found the yellow lenses to be beneficial in any meaningful way. We use only clear lenses.

We stopped using the crystalens in 2017. Newer lens options emerged that in my opinion delivered better overall results than the crystalens.

I will shortly be having lens replacement. I have a moderate cataract in one eye and minimal in the other. But I am horribly nearsighted, worsening rapidly when I had a heart attack and was started on meds. I have worn contact lenses for about 50 years with near vision in one and distance in the other. Do you have a recommendation of the type of lens in my case?

I hadthe multifocal last week its my seventh day. I cannot go outside, cannot drive, I have to wear shades inside the house and keep the house in darkness this is causing depression. Went to the optamologist yesterday told me to come back in 3 weeks. Can you give me some advice

I'm at the stage where I need cataract surgery. My left eye had a macular pucker and was corrected and that lens was replaced with one that only corrected far vision. I do need to use readers to work on computers and reading. Can that lens be replaced with the panopticx lens? My right eye also needs cataract surgery. I was a pilot and my far vision has always been excellent, but cataracts have reduced that excellent vision. What do you think my options are?

With the Panoptix lens, does distance vision degrade in darker (evening) conditions since the pupil will dilate and include more of the near-vision magnification? I currently wear multi focal toric contacts and notice my distance vision degrades as light conditions darken

I had panoptix put in left eye. I'm 35. I get a fluttering effect in certain light likening is trying to focus or under water. Also contrast sensitivity. So some words I can read and others I can't. Is this Nirmal? Will this go away in time?

I’ve heard that the panoptix lens gives a changed colour perception and that the night light halo effect is worse than that of the symfony. Is this true ?

In my experience, the halo with panoptix is less than symfony (at least that is the feedback I am gathering from my panoptix patients). The only color perception change with the panoptix or any implant is a more normal color through the implant that through the natural lens that discolors as it ages.

I was surprised at removing a lense and replacing with a newer better lense was possible. Is it common? Wondering as I was putting off doing mine as i have early stage cataracts. But I would like to see better and without glasses. Worn glasses from age 16. Then laser 25 years ago. Then needed glasses again 13 years ago.

Any info on the Tecnis Synergy (new Sept 2019) vs Panoptix lens. Supposed to go near sight to 33cm vs Panoptix 40cm, and allow more light in.

We have implanted about a dozen, very similar performance! So far no noticeable difference in distance intermediate or near vision

We have implanted about a dozen Synergy IOLs, so far nonoticeable difference comparing with Panoptix Rayone or Finevision! Confortable Jaeger 1 reading and distance 20/20 are achieved! So far early experience to comment on nightly photic phenomena

@@gondwana6303 Do you know the IOL implanted in your eyes? almost all diffractive multifocal IOLs have low light photic phenomena... but usually well tolerable! If You have significant distrubance You should consult you Ophthalmologist as there may be capsular opacification, or the central ring of the IOL may be decentered from the pupil leading to "comet trail" or "ghosting" aberrations

Hi doc. I would like to ask approximately how much would it cost for an iol exchange from monofocal to multifocal iol. I just had recently a cataract surgery and had received yellow monofocal iol. But i am near sighted before and i am used to not wearing glasses when reading, now it bothers me that i cant read near after surgery.

Why are you using old lenses when newer ones are already available like Trifocaal lens Carl Zeiss ? Those lenses are much better then symfony and panoptic lenses.

Short answer is its not FDA approved (obviously) If you look at the defocus curves of the panoptix and AT LISA you'll realize saying its "much" better is disingenuous on your part since they both have tradeoffs. Panoptix excels in the 1m to 40cm range while AT LISA is slightly sharper from the dropoff of infinity to 1m and offers slightly better vision closer than 40cm . The majority of people getting trifocals care about their near vision (1m to 30cm) performance else they'd just get symfony or monofocal.

I have mono lens on English health service the distance is blury now from floaters it was done May 2021 the near vision is just done a month ago ,previous to it i used my myopic right eye for near vision until the cateract got to bad was PERFECT near and distance vision for a short time then the distance was affected by floaters after 2 short weeks and is blury ever since and near sight with the new implant in right eye though good does not seem so compatible with distance but is only one month since was done , i live in hope of improvement as it is not working in conjunction with my distance in left like my near sighted own lens did for some reason ,hopefully it will futher settle down , is thought floaters are a problem but i have often got vision as if im looking through binoculars with rims round at sides that keeps coming and going yet my retinas are said to be fine. Distance is often blured over especially on my moving around which is said to upset the floaters which does seem logicsl but the perfect vision i has at 1st for 1zt time in my life at 69yrs old is now sadly much missed i had eye glasses from very early age . being disabled i can well do without glasses in greater old age !!!!!

Hello Sir, I'm 24 and have cataracts in both of my eyes. I'm trying to get as much input as possible on this. I really dont know what lense to chose, my doc recommended edof with blended vision, but I really dont give away my ability to drive at night with all these glares and halos. Also one important thing is I'm doing Martial arts, I also wanted to compete in the future. I dont know which lense would be good to see the other fighter with perfect vision. Do I have to give up my plans on competing? I'm really stressed out with this and researching all day on what is best for me. I hope you can advice me on what you would chose for a young person like me.

Hi I hope this helps, I had symphony in left eye and hated it vision was just not clear and it did not help with needing reading glasses, and night time halos are the worst. I opted for just regular lens in right eye and vision is 20/20 and clear with no halos. I still need readers I think if I had let them put symphony in right eye night driving would've been impossible.

@@surfbird448 i also have symphony in left...it's been 4 years...during day time performance of symphony is like natural lense for me...i can read from 60 cm to distance..but nightime there are huge starbust while driving...also low light contrast is 30-40% less i guess compared to a natural eye...should i go for Technis eyehance in right eye??

@@TradeNiftydaily I have never had that lens,I dislike the symphony lens so much that I am going to replace it with just the regular lens like the one in my right eye. I just cannot handle the halos and Starburst, my right eye is perfect except I do need readers. Best of luck.

3’close & midrange blind zone... cannot see anything in hands with symfony iol implant... opthamolofgists say cannot take it out... left in over a year , saying blur zone would improve. not correctable with glasses. consider ciliary sulcus additional lense!?!;)

We do intraocular lens exchanges so regularly. With generous viscoelastic use, I am not seeing signs of visually significant corneal changes.

I just got cataract surgery and the use a distant mono focar and in top of that ihad a peace of the caratat fall in sided the eye making my react go to emergency after a week because the doctol said you be fine after that ihad a second doctor whish he save my great person this doctor did a vitrtomy as a urngent surgery and know in better but pressure remains some were in the 25 so we hoping it goes back to normal. my point after all this was to have all better but i dont think was worth it in not to happy with this lens .all this sacrifice and now in 20/30 but it kind of bright and bother is it normal ?anyway i which i heard this video before i woul'd of choose this type if lenses

Thank you Dr Wong for the excellent info on intraocular lenses. How does the Finevision trifocal lens compare with either the Symfony or Panoptix lens? My doctor is recommending the Finevision lens for me ( 66 year old male that had lasik surgery 7 years ago and having difficulty with vision in all three zones without glasses)

In order to know the answer, an examination of your eyes would be necessary. We have placed the panoptix lens in many patients who had prior LASIK. We have also chosen not to place the panoptix in some patients that had prior LASIK.

Hi Dr. Wong, how about the glares? I am afraid the glares will never disappear, and after how long I can change my lenses again if I am not satisfied with Panoptix due to halos and glares.

Both the symfony and panoptix lenses will create night time artifacts for 100% of patients. In my experience only about 1% are seriously bothered by the night time halos. For these patients, if they are super bothered, the symfony or panoptix can always be removed and replaced with a monofocal lens that will give less halo at night but also cause the individual to need glasses more to achieve full range of focus. Basically, in my experience any lens can be removed and replaced with a monofocal.
