I’m amazed at many problems are caused by muscle imbalance. It seems like current treatment protocols are (possibly) aimed at immediate pain relief, without focus on any actual healing in mind.

It's deterioration of the kneecap cartilage.

Thanks Doctor for all your amazing videos I have been following all your tips and exercises since my meniscus tear injury in Nov and also my osteoarthritis and chondromalacia patella as seen on my MRIS ..I think my condition must be surely a combination of being overactive , genetics and. Age .and also muscle imbalance I am trying very hard to strengthen my glutes but I still can’t get the trick of how to walk by tightening by glutes alternatively !! However all your other tips and exercises have helped me a lot and my pain is reduced .. Doctor suggested I start using ankle weights now since it is 6 months since my injury What is your thoughts on it

I had the same doubt about using glute for walking. And I think I m getting a hang of it. Before trying to walk with glutes, step 1 would be to do the glute strengthening for which his clamshell exercise was very helpful. Doing it a few days i could finally isolate my glute muscle and clench it just like you would isolate your upper arm muscles to show arm strength.Very important is doing the glute clench should have absolutely no impact or pull on the quad or the other back thigh muscles that's what it means to isolate the glutes. Once you are able to do that you try to feel that glute muscle as you walk. Lets say you step forward your right leg. By the time you step forward your left leg next and the right one stays fag behind, you can feel the tight glute. Don't exert or exaggerate but just feel the tightness in the glute. And now when you swing forward your right leg again, i assume the whole leg is held together right from the glute leaving less pressure on the knee. Now again make sure you fully press the toes and use them strongly as you swing forward the left leg. When you do this, slowly you can use your own ways to tighten and slightly clench the glute muscles with every step. So my approach was to start without trying to exaggerate too much. Do as much as you can and you'll get a hang of it.

Thanks Rini will try to follow what u said .. my problem is when I contract one side say my right side and then move my other leg ( left) forward I don’t get enough time to release the contraction of the earlier right glute and contract the left glute together with the forward movement of left leg So I end up contracting just one side and walking !!😀. Anyway I guess it takes a lot of practice The clamshell exercise I do regularly

@@sujamathew1946 I get you😊 The doc may be the best person to explain but let me attempt. I would try to simply lie down or sit down and do alternating glute clench and release on both sides. Clench left while releasing right. And vice versa. If you can do this, same thing needs to be done while walking. Clenching comes easily in the right glute at the time (when you stepped it forward and now) it is behind and you are stepping your left leg forward . That's why I said notice at what stage your glute feels the tightest during walking. Don't clench in the beginning of that's difficult. If you can't naturally clench in all probability the glutes are not yet isolated. Pl treat this as a layman comment just trying to help!!

If your chondromalacia is a result of overuse (e.g., long distance running), there is a good chance you will not be able to return to the activity that caused the injury. Rest and correcting any muscle imbalances are the only way to heal the kneecap cartilage. Once your knees have healed, more or less, you may need to find a substitute activity that does not cause your kneecaps to either mistrack or an activity that does not place too much compression force on them.
