I feel terrible for the last controller. He tried to help, he had to be devastated. Saying "Please respond." I hope he doesn't feel like he could have done anything better. He did his best.

David Parry Commercial flight sims are great and all, but they dont prepare you too well for real IFR flying. The issue is when your body is lying to you about the aircraft's attitude, you start ignoring the instruments and trusting your gut. You need to practice IFR flying in the air with foggles or a hood, with a CFI next to you of course. And dont cheat, if you can still see outside then your body is not lying to you so much anymore rendering the lesson useless.

@@rykehuss3435 Yea, my dad was a CFI (but not a CFII) and he was instrument rated. I have spent many many hours flying with him inside ping pong balls (if you know what I mean). That feeling of not moving, just sitting there, looking out the window, waiting to see the runway.... He would have me watch for the runway so he could concentrate on the needles. I always felt better when I saw the approach lights, then the runway come out of the grey. I'm not sure anything can prepare you for that, other than actually doing it. Years ago, we were flying into Concord (Buchanan) and conditions were right at minimum and getting worse. The tower closed right after we landed Later that night a plane tried to fly the approach but missed then stalled claiming out and crashed into the mall. He hit right above where the Santa Village was and it killed a bunch of children waiting in line. It was a bad one.

High hours can make you overestimate your capabilities. Secondly, having agreed to bring your friends or family to a destination may place external pressure on some people, making you feel like you really really need to bring them to the destination. Also there can be a desire to show off your skills to your friends, which is risky when combined with the thought that your 2200 hours will somehow magically make your flight succeed no matter what.

Just the fact that he tried to go cross country as a VFR-only pilot without even getting weather reports or filing a flight plan proves that she is mistaken about her father. Even if the weather report looked promising it would still be a stupid risk without IFR rating because weather has a sneaky habit of changing.

Knew a guy who flew out of our airfield who refused to preflight his aircraft, refused to properly maintain his aircraft. Died in a crash a year later. Cause of the crash? Engine failure due to water in the fuel on takeoff. No preflight. Some people are just stupid and shouldn't be anywhere near airplanes. As a A&P/pilot I've seen plenty of them.

@@GermanAviation I assume this was the beginning of the flight. Dude's out of gas, there's no way he's overweight at the point of landing. I wouldn't even call that a factor. The biggest contributor to this crash was fuel exhaustion, very poor flight planning, ignoring fuel signs until it was at critical, and accepting an IFR approach when not IFR rated. Quite a bit of spacial disorientation it looks like. This guy could have been at it all day and never made that runway. He was right to declare an emergency but he only made his situation worse by saying he was IFR rated.

This is a very good guide on everything NOT to do. This pilot: *scruffed through preflight checks *ignored well-meant and accurate advice *Flew overweight *Flew with very minimal fuel reserves, even if his destination was ok. *flew VFR into instrument conditions. * +1 for the pilot, he did actually ask for help. eventually. *but then LIED to the controller about his IFR abilities. *Panicked. This is very clear in his deteriorating radio protocol. *Died. taking 5 innocents with him

I find it weird how many apologist there are here for this pilot, he lied and lied and lied until he died which would be good riddance but he took five souls and a perfectly good plane with him not to mention mental anguish for a controller who went way about and beyond to the degree he could have all but landed the plane for him but the guy clearly lied so much he didn’t believe the controller apparently.

I think he lied about IFR since he wasn't going to get there any other way. Still, it was a mayday, so the controller would have done his best talking him down even if the pilot had been truthful.

When he was asked "are you capable of IFR flight" that might have been interpreted as - the plane is ifr certified. If the pilot was certified, ATC would suggest a precision approach

That controller had to have known this wasn't going to end well...the pilot couldn't follow a single instruction given.....Like a dog wandering onto a busy street. "come here boy, no no,, here, no, come to me,,,, eventually SMACK"

@@N224DW He just couldn't fly because he was really a VFR pilot in IFR conditions. He had no legs for instrument flying and frequently became disoriented. If there's anything I've taken away from all these reports, it's get IFR certified and stay current. If there's two thing's I've taken away, then it's get IFR certified and have back-ups for all critical instruments.

@@mjudec No, they couldn't. They would have lost control just like the pilot in this case did. That's exactly the point: he lacked the training to fly by the instruments on the aircraft and so would a non-pilot.

The flight was destined to fail. Added to the entire absence of any pre-flight planning, this pilot couldn't stop thinking about "that asshole" who questioned his abilities, I'm wagering.

​@Terry Melvin He couldn't even keep the wings kinda level and the plane nose roughly at the horizon. That makes it kind of impossible to follow compass or altimeter. And likely the pilot was panicked that he'd kill them all ... or worse, someone would find out he wasn't that good a pilot after all.

Also begs the question why the other pilot felt the need to go talk this guy before he took off. Certainly what the other pilot was seeing in the moment raised flags but my guess would be the Seneca pilot was probably known at the airport for being sloppy with procedures.

@@mattpope1746 I totally agree. I have only 200 hours (and IFR) and been around my local airport, aka the second busiest airport in Indiana, only for about 4 years. I know most people here and only something REALLY major would trigger me to say anything. Which means, either I see a pilot loading 3 overweight passengers and 20 dumbbells in a Cessna 172, or he/she is one of the (very few) pilots I know who I think at times are quite sloppy or complacent.

A flight in Austria had a very similar event. Had 6 or so passengers with a pilot that was under licensed. He flew the plane despite clearly being overweight and flew straight into the mountains, my father in law was one of 2 on board to survive. He lost all his best friends that day.

I believe sometimes it’s not an oversight a pilot makes to not contact flight services about conditions. They’ve made up their mind they’re going and they don’t want to hear info that tells them they should cancel. It’s really sad to think that his friends would’ve placed total trust in him that he wouldn’t do anything that would endanger their lives.

The guy couldn't fly a heading. He was clearly in panic, trying to sound cool on the radio, but he was terrified. Was low on fuel after 3 hours flight time. Even in perfect weather conditions he wouldn't have made it. What was he thinking?

@@angeloquattrociocchi5487 I must disagree, any imbalance requires trim action and more drag from control surfaces. Perfect state is when static and dynamic CoG are placed together. Of course aft heavy aircraft is more safe than nose heavy.

@@adamw.8579 When the CofG is forward, the elevator is trimmed such that the tail produces extra downward force. This adds to the weight of the airplane so the total amount of lift that must be generated by the wings to maintain level flight must be increased. That results in higher angle of attack and therefore higher drag in cruise, leading to the reduced fuel efficiency (increased fuel consumption). Also, aft-heavy definitely doesn't make an aircraft more safe than nose-heavy. It's the opposite.

The Air Traffic Controller was a personal friend of mine when this happened. I spent time in that tower with him (I’m with the Government too) and this was devastating. He is retired now but was a tragic experience

So sad that the controller has to bear the guilt of this incident because of a person that made a series of egregiously bad decisions. I hesitate to even consider the person in the left seat of this plane a pilot. I prefer to think that pilots make good decisions and keep themselves and their passengers safe.

I was hired for ATC but I was disqualified due to my vision. Still, I have remained interested in ATC and aviation. I’ve run scenarios in my head, regarding emergencies. I feel for your friend. 😰

Please tell your friend that he went well beyond his duty trying to save that Seneca.Even a nogyro approach is an almost impossible task for a non Ifr certified pilot.The ceiling was also almost below an Ils cat 1 approach.Trying to go in visual was just a lottery….Very sad,but the controller did a hell of a job.It‘s just that the pilot got himself into an impossible situation.Best regards from Spain.

When ATC said, "Please respond" I almost lost it. Try imagining what was going through his mind in that moment of utter helplessness. IFR is no joke for a VFR pilot. I can say that based on first hand experience.

On Average, there is at least one aviation death every day in the United States(From NTSB 2018 data). Someone 'like this' is almost always flying somewhere. Sad but true.

@diddle the poodle It's not "stupid". Overloading the plane was stupid. But, that didn't cause the accident. His over-confidence and lack of experience did. By the time he was on the radio being talked in to land, he was scared, confused and blind. Unable to think straight and follow pretty clear and simple instructions. That's a sign of stress. It can paralyze you. Suddenly, you're dealing with life and death IN your hands, and you freeze up. The mind races. You can't focus. He was doing the exact opposite of what he was being told to do. He was gone already. Just lost.

I think as a part of VFR training, perhaps the testing syllabus should have students know how to shoot an RNP approach. Just one approach.

​@@mtadc1545That's a good point... but there is already a lot to learn in the basic private pilot's course. Learning how to read approach plates, set up the avionics and learning to fly on instruments at the skill level needed to fly such approaches takes an entire instrument course to learn, let alone master. Maybe a quick overview of a basic approach would help in private pilot training but I think it should be separated like it is now.

This pilot was a fool, the ATC asked if he can fly IFR which he could not. Yet, he replied “Yes”. There is no excuse for that.

We should call people names but as commented he was out of his depth, his biggest problem was NO planning, how do you just take off without looking at the weather and flying that distance without a flight plan? Then his casual radio edicate was odd as well numerous times he would relay back with call sign just odd. Just looking at that flight track around the airport go out take a long straight final but he could fly a heading.

During pre-flight, run-up, and weather checks, I'm always looking for a reason NOT to fly. I've already got a lot of reasons to go for it; but you need that solid reason not to go and you need to try to find it--that mind frame change means a lot. Just recently, I cancelled a flight b/c the elevator cable (Cessna 172/180hp) in a rental looked severely corroded. Lose that elevator cable and there's no return. I cancelled, called the owner and told him about the cable. He said they'd replace it. About 2 hours later, I'm pre-flighting a different plane when I see some guy taxi out and take off in the 172/180. I wondered, did he pre-flight? did he think the corrosion was ok? did a mechanic look at it yet? Probably not.

It's okay to practice landing with trim tabs only, but you certainly want to have the elevator working while you're doing it! Just in case you can't!

On my ATP check ride, just about the only time both fans were turning was when I was asked to land without touching the control wheel. You're right, it can be done but it wasn't exactly my smoothest landing.....lol.

While performing my preflight one hot day I moved the elevator and was immediately covered up with wasps. Needless to say I didn't fly that day. I agree it is best to balance reason for and against flying. Some things don't even need questioning.

I was on the ground just a couple miles from KHTS that day. I've only ever seen one other snow squall like that. When it blew in, it was about 500 foot visibility with horizontal driving snow. Winds were variable between west and north and gusting at 20kts. In the air, it must have been like flying in a washing machine with that erratic wind blowing up and over the hills. He must have seen an opening and tried to hug the ground when he clipped a power transmission line. Must have been in a pretty good bank at the time because that contact separated one of the props and I don't think it was found. I remember the NTSB asked for help in finding it. Those transmission line towers are placed on the hilltops so those cables can be at up to 400ft AGL with respect to the valley between the towers. Bad weather to have the fuel mismanagement catch up. Severe low level turbulence in complete imc with 1/4mi vis and a ceiling right on the deck with no fuel is a bad way to refresh surveillance approaches. Tri-State did a stellar job trying to save them.

This pilot was an accident waiting to happen. Good golly. You could probably name two dozen things he did wrong; even before he left the ground. ATC was doing a great job of trying get him down, but you could tell that he was stressed and unable to process everything that was happening. How can you fly 2200 hours and not somehow, even by osmosis, learn to deal with IMC? Yeah, complacency is a killer, but stupidity is the real harbinger of death, It wasn't a question of IF this guy would get people killed, it was a question of when, and how many.

In this case, the pilot did not mean to kill himself and his friends. However, he did do this and it happened solely as a result of his actions. This contradiction is what makes this an accident. As opposed to a multiple-murder-suicide.

While I'm not a real-world pilot, just an experienced flight-simmer with maybe 3 hours of real world logged flight time, I am an experienced motorcycle rider. I can't tell you how often I see other riders who are nowhere near ready for the bike they're on, or to even be riding on certain roads...But they go on day trips or ride in heavy traffic, or otherwise constantly make bad decisions and it's never a question of "if" but "when". Sooner or later if you're doing dumb things, you're going to get a bad result....that's just how it works and unfortunately it only takes one. Those bad riders don't think like that though, they think because nothing ever happened before that nothing will happen in the future, then you read about them in the paper.

I obviously feel sorry for everyone in the crash, but I can't help but imagine how this impacted the controller. It would be next to impossible to not second guess yourself after this and go over the "What else could I have done to save them" chain of self-doubt. I really hope this guy got a little time off after this. I realize that the pilot was foolish for doing what he did, but I still can't help but feel sorry for the guy as you hear his voice go from concerned, to scared, to panicked to hopeless. I can't imagine what it would have been like as a passenger to witness the man behind the wheel spiraling into despair like that.

One thing I’ve learned from all Of these videos in my early training is they all gambled. If you gamble you usually lose. It’s amazing the amount of pilot error that occurs. They all just don’t get it till it’s too late. Never deviate from safety.

@@lazurm Even a 20 hour student pilot knows when ATC is giving you headings to fly you are getting vectors, clearly, this guy couldn't follow directions and his constant request for what he was already getting showed he was not only incompetent but was unable to clearly communicate. I suspect his 2200 was spent flying in the pattern.

Many years ago i got into similar problem in florida flying a c-150. Into Orlando. ATC gave me a special VFR clearance and proceeded to give me a ground control approach. Told me when to turn, stop and when to descend. He guided me on a 10 mile final all the way to 1/4 mile from airport when contact was made at 750 agl and visibility was about 1/2 mile. Thank ful for that controller and to my instructor who gave me and demanded instrument training. Having a flight plan and flight following certainly was a plus. I'm writing a book about the whole trip. KFRG to KVRB. These case studies really make you think before heading out.

It often seems like it is never one thing, but rather part of a pattern of missteps. I went to a commercial flight school, and it is really unfathomable to me not to do weight & balance, plan your flight, and file a flight plan before you step outside for preflight, etc. It takes 10 or 15 minutes, and it could save your life. Do it.

You can if you stick strictly to clear weather and short flights. He probably never experienced adverse weather and got complacent about checking weather. I've seen pilots get an attitude about people questioning them about prep work before.

Another possible explanation for poor decision-making and behavior. Somewhere I read that half or more of all private plane crashes and deaths involved "flying while intoxicated". Take a great pilot, get them drunk, and see how great is his or her decision-making and behavior (piloting skills). Not saying this was the case here, but given the statistics... maybe could be. Also note that less-than-legally drunk (for DUI purposes) still has consequences, and that blood levels fall over time. Which means, it would be higher when fundamentally stupid decisions were made back at the start of the whole process, including the decision to fly at all. Oh, and don't discount the effects of prescription drugs too.

I was busy with something else when the video started the analysis so I went back to listen again. Three hundred pounds overweight! Three hundred! Simply amazing. I get the idea sometimes that pilots forget that they are not in a car, forget that the smallest defect or poor judgement or forgotten detail can and oftentimes will result in their fiery death. We cannot just pull over gents.

Sobering. Now this is a video a new VFR pilot should watch and learn from. The pilot made several mistakes before leaving the ground. Things we are all taught to do for every flight but this pilot chose not to do them or ignore the data. what do we take from this? Prepare, prepare prepare.

GA Flying I am a new VFR. 10hrs. I am passing judgment as myself coming off as high and mighty at all. Judging of the communication alone (or lack there of). Or coming into a situation with the “eh, it will be fine” is scary. Yes I’ve ran out gas before as a new kid driver, but I am not going to plummet thousand of feet and endanger lives.

ATC deserves an award. He tried so hard, remained patient & stayed calm. This should be a training video for ATC. He was handed a no win scenario but kept doing all he could to talk the guy down. I have a question, with all the advances in technology is anyone working on a cost effective retrofit autopilot for GA? No idea how that would work or if it is even possible.

@@Raison_d-etre he sorta didn’t believe the controller, that’s what liars do, they think because they’re a habitual liar, which this “pilot” seems if he moved his lips or blinked he was lying, others are lying. I’ve flown sims with weather complications set to max and the plane trying to move forward pitched and yawed at every angle to move forward. I crashed my plane but it was such one could walk away. His plane could fly into those conditions though not ideal with a trained pilot so the only reason he didn’t follow a damn instruction was he didn’t want to... fulfilling the obvious thing being he was a total asshole. Sad he took people with him but for the pilot good riddance.

He was not IFR qualified, that's what brought him down, a plus 2000 hours total VFR pilot could not say "I can not go on with this, please help me" instead he said he could take an IFR clearance and killed everyone onboard!

While it's a small point, not mentioned in this video is the fact that the flight was planned for a total of five, when a sixth was added at the last minute. All of the passengers were members of a Polish-American flying club. I would think some of them were pilots. It's asking a lot to speak up and say "we're overweight". Especially since you weren't expecting problems because you've got a friend and 2,000 hour pilot at the wheel. It's also hard to miss out on a trip by saying "I'm out. This is dangerous". The irony here is if someone HAD done that, they wouldn't be overweight (I assume). You don't really know for sure when you are averting disaster for yourself (or others). That's something to keep in mind. Rather than feel stupid when you opt out and everything works out, remember all these case studies.

The last few minutes of flight suggest a pilot with brain freeze: completely overwhelmed by the reality of his situation, and unable to follow even simple instructions, to the point of repeatedly confusing left and right.

I was freaked out watching this video. With my 70 hrs of flying, always get a weather briefing for the whole route and alternate airports. I flew right seat with my instructor in the Seneca on a rainy night. He asked me if I think I can fly by instruments. I’ll try, i thought; I have you to save us. I flew attitude, vert heading and airspeed. He said in 15 mins we would impact the ground without wind corrections. NOT EASY, ESPECIALLY WITH NO TRAINING. that was the first time I experienced spatial disorientation. Don’t fly VFR into IMC, Never Never Never. I did my solo 3 TNL’s but never CC. Then anew GF and 911 happened. 2500 (est) hrs of Hangar flying since. My 2 pennies.

The crash killed pilot Wieslaw Dobrzanski of Niles, Ill., and passengers Ireneusz Michalowski, 44; Kazimierz Adamski, 65; Stanley Matras, 65; and father and daughter Stanley and Monika Niemiec. Four of the victims, including Dobrzanski, were members of the American Polish Aero-Club in Chicago. The six were flying from Illinois to Florida, with a planned stop in North Carolina, to look at airplanes for sale.

I don’t know why I keep watching these. I’m not a pilot. This and the “Final Approach” accident studies have me tearing up at the end. I think it’s because I can hear the fear in the pilot’s voice and the desperation of the ATC to reach them when they go silent.

I'm an IT guy, but this is no different than tragedies on Mt. Everest, or people who hike into wilderness without experience. As an adult, it helps you learn better decision making, stress control/response, and impulse control. You'll enjoy life more and not die due to incompetence or obstinacy. Wisdom is learning from the mistakes of others. Less than a year ago a woman from MA went to Arizona and dehydrated to death on a half-day hike; her "date" left her to descend alone although she felt woozy.

A VFR flight plan is optional. It assists search and rescue crews if you get lost with no comms. Not filing one was the least of this guy's crimes.

Ma Nature doesn't suffer fools. Cross-country flights MUST be preplanned days before and constantly monitored during the actual flight. Weather aside, his blatant, cavalier, attitude towards W&B, fuel requirements, burn rate and the 5mins it takes to write up/file a flight plan, says a lot about his mindset with regards to being a pilot, being PIC. How did this 'accident waiting to happen' attitude slip by all those flight reviews? This guy was destined to bend metal some day, whether in his car or in something a lot more demanding. Some people are just like that: rules don't apply to me, I've gotten away before so I'll slide by again, preflight planning is for chumps, etc. Sadly he killed four innocents, including a 'student' who probably looked up to a 'real' pilot who can fly a twin. GA gets such a long-lasting blackeye when accidents, like this, happen. Fellow aviators, please don't ever be like this guy.

This accident mostly was on my mind today, as I looked at the weather for a flight from Montreal to Halifax. Weather is decent here. OK at destination. But extreme convective activity over Maine. Without a weather brief, I would be toast. Anyway, remembering this video made the decision to go from a 4 hour flight to a 12 hour drive an easy one.

It really had to be hard as hell on those ATC people who did their damndest to get that pilot on the ground. The pilot didn't seem to talk much, why wasn't he more communicative with the ATC? Such as; I'm low on fuel and I need to land. Can you bring me in on a straight approach? Can you give me a rate of descent? Am I lined up right? Communicate, communicate, communicate. There was a student pilot on board, why not have HIM look for visual clues while the pilot flies the instruments? A little cockpit resource management might go a long way... I'm aware that there is a huge difference between VFR and IFR, but if one is flying VFR why NOT practice flying instruments only? At least enough rudimentary skill to do a 180 and get OUT of low vis conditions?

Obviously he made a lot of mistakes, but you are jumping to conclusions, an emergency is ,,an emergency..it does not matter how he got into it. It is the unknown that kills.Very easy for the rest of us to judge. He declared an emergency - therefore he knew his limits. The video suggests him as ignorant - maybe to make a point, but I am not that sure. he managed to fly VFR into IMC and the accident happened because he stroke power lines. Loss of life leaves deep marks every time.We should be more respectful.

My primary instructor insisted that I learn how to make ILS approaches as a student pilot. He was an x Navy F-14 pilot and a hard ass however lol. I could hold commercial standards before testing for my private. Your instructor matters! He'd bitch me out for being 50ft off my altitude or 3 degrees off heading. In hindsight I'm glad he was a hard ass because if my weather went to shit I would have landed that plane like a boss, even at 60 hours TT.

It's not that the pilot thought he might not be able to do it --- the pilot was unable to consider non-good outcomes (bad weather, not enough visibility, fuel reserves, ...) because he was way overconfident and _knew better_ than the smart-arses who determined maximum load and allowable centre of mass positions. After all, it worked before, so they were clearly wrong. Nothing could happen to this God's gift to planes ... Unskilled and unaware of it.

When i was a student pilot i did my long cross country i checked weather as instructed and VFR was forecast along entire route. I kept timing ETA. Weather moved in sooner than forecast. I got in icing conditions and light snow i knew my 3rd airport was within 5 miles. I immediately set up to enter the pattern and talked to them on the frequency and made a landing immediately. It was a huge inconvenience but no one got hurt and no airplane lost. This was the action i did. Other students missed reservations the next day with the airplane. A cessna152 doesnt do good in icing at 5000ft and snow falling.

Great videos and analysis, thanks. And suggest that instead of having the "this will never happen to me because I am a lot smarter" type of mentality , think about "what elements of this accident and behaviors can I recognize and learn from them"

Wow, what a graveyard Pilot. Well, ignorance and negligence sometimes never give you a second chance and I think that played a significant role to this tragic event.

I have held my license for years until my health went south. When this started, I knew the outcome. I live near Huntington and remember this crash. One word sums it up; arrogance. When your mind believes you can travel over 500nm without a flight plan, without recurring weather updates, overloaded, and braindead, you should not be in the pilot's seat. This was avoidable. Plain and simple. As I am listening to this, I remembered my flight school days and about establishing personal minimums. This pilot had no business in the air. This was almost painful to listen to. Sad. Very, very, sad.

+2008Flyboy I agree. I dont understand people who fly lots of xc without an instrument rating, especially if you are flying long xc's in a twin. It's only a matter of time before weather closes in on you.

+Herve B It's not that difficult, but does require more commitment, especially to stay proficient. Cost varies greatly dependent on the individual and if you choose 141 or 61. Lots of variables in the cost factor.

You should consider that 1 hour reserve as "unavailable fuel" that MIGHT be sucked up, if your lucky. That way, that 1 hour reserve should only be used in an emergency.

We call what you in the US call a surveillance approach, a Ground Controlled Approach. I have done just one of these through several thousands of feet of cloud. I had an instructor with me as I was pilot u/t and only VFR. All that needs to be done is to follow exactly what the controller tells you to do. He won't tell you what speed you are at but keep airspeed constant throughout the descent with normal use of pitch and throttle. Sorry but this pilot was completely useless at IFR flying. I find it hard to believe that this bloke had over 2000 hours I only had around 30 hrs of flight instruction when I did this GCA and though it was draining it was very doable, no input from the instructor was required. Very satisfying to come out of the cloud with the runway there in front of you.

Done that myself, it was a PAR. Heading and altitude call outs from the controller. I said nothing during the whole thing. I'm not sure that would have helped the Seneca pilot as he was outside the Center of Gravity limits. Very difficult to control the aircraft even in nice weather.

Raoul Cruz .... Yes, with you .... you say nothing on a GCA, just do what is given to you and I don't know about you but I found it very draining .... but very satisfying and I reckon it should be part of the PPL syllabus, i.e. compulsory before getting a licence. Yes he was overweight and c of g was a few inches out .... surely, a higher minimum and approach speed would deal with this. Well, I believe it would but he needed to be aware that higher speeds were required. We're only talking between (a guess) 5 to 10 knots difference in these minimum / approach speeds. He should have replied to ATC that he wasn't IFR rated but could likely handle a surveillance approach. He should have been able to do that with so many hours! If he'd climbed to 2500 feet and maintained a heading of 170 degrees as instructed he'd have been fine if he'd followed all instructions. He was multi-engine rated .... more than I am and I, probably like you, would have landed safely in my single engine PA28. A very sad affair .... he was completely out of his depth but should not have been given his experience. To be honest I find it difficult to believe that he had 2000 hours.

Precision Approach Radar, it's a GCA type approach,they give heading and distance callouts and your position in relation to an imaginary glidepath (same glidepath as the ILS). In the video, the Huntington controller is trying to give the pilot a SAR approach but, it's obvious the pilot was not capable of following simple heading instructions. Sad indeed.

Not a pilot, but these uploads are a terrific resource for wisdom that can be applied to just about any decision making event anyone might encounter. The pride and arrogance of most of these widowmakers is pretty common, taking unnecessary risks, showboating to make a good war story for thier friends. I'm reminded of that pompous bastard that crashed that Italian cruise liner into an island, hotdogging to thrill someone he knew on the shore.

Regardless of how he got there, that feeling of how bad you've screwed up and not knowing if there is any solution must be terrible. I've had that feeling to a lesser extent. I remember trying to rebuild an automatic transmission in one of my old cars. It wasn't until I was surrounded by a pile of clutches and bolts and valve bodies did I realize I made a horrible mistake in attempting it. The difference being is my failure only cost me a few thousand dollars buying a new transmission - the idea that I could die very soon was never there.

Yes, on the ground all you have to do is stop. In the air you have to get to the ground, the plane will always find the ground, the question is will you be in control when it happens. I hope you old car gave you many more years, I love the old cars. I think i typed this to feal less sad and angry.

300 lbs over gross is 50 gallons of fuel. How do you get to 2000 hours, multi-engine, complex and not get an instrument ticket? He doesn't know what MDA is....doesn't have deicing...has no skill to turn on vectors in a cloud.

If you are going to survive as a pilot you should RESPECT and avoid IMC even with an instrument rating. Fully competent pilots die in icing condition all the time.

IMC is not fine if you have no training or experience by flying without reference to the ground and cannot scan the instruments properly and interpret them readily for your orientation and direction of flight.

with an instrument rating, his added confidence he'd probable have only lasted a few hundred hours..... read the Nall report. Most weather accidents involve pilots with an instrument rating.

There should be a way to report dangerous pilots. Someone on the ground felt strongly enough about his situation to talk to him, it's like the seaplane crash at Oshkosh where they were pleading with the pilot not to fly and he did....killed himself and a passenger before getting in the air.

To decide to fly a small plane that was not only 300 POUNDS overweight but not even balanced correctly then compound this with absolute disregard for weather is truly the height of arrogance and absolute sheer stupidity. May all pilots never lose the respect for the skies so many have paid dearly to instill in the industry.
