In this video, you will learn some of the basic anatomy of the lumbar spine, L4 L5 and L5 S1 disc, how to understand the low back anatomy so you can relate it to your lumbar spin MRI report results. You will also learn 4 of the best exercises to help relieve pain associated with L4 L5 disc bulge and L5 S1 disc bulge.
Of note, when you see the picture of the spine and disc cross section, not that even though you can see the spinal cord in the picture, the spinal cord in the lumbar spine actual ends at about L1 or L2 and it continues down as a bunch of nerves in the spinal canal called the cauda equina. This cauda equina bunch of nerves then branch off into each of the spinal nerves in the lower lumbar spine.
Low back exercises should always be done to tolerance and never if they cause more pain. As well, one should always consult with their primary health care provider / practitioner before starting any sort of exercises, especially low back exercises when there is an underlying condition.
What exercises should I avoid for a disc bulge is a FAQ. L4 L5 and L5 S1 discs bulge treatment without surgery can be done with corrective chiropractic care and rehabilitation exercises. What if you can't sit in a low chair or you can't get into the floor? My hamstrings are so tight and my legs are weak. I haven't been in the floor in a year.

Is it necessary that bulge touches nerve , I have l4 l5 degeneration but recently when I was mopping floor I got pain burst in lower back , too much ,I don't have pain in legs but very much concentrated in start of hip line then in plus sign pain , are these exercises for that as well

Hi I had these issues for the last 7 years, I'm a painter. I've tried lots of stretches over the years but the one that worked for me in 1 day was lay face down on a bed with 2 pillows under your pelvis and hang your arms over the edge. It was a miracle first time in 7 years I painted skirting boards with no pain.

These were some of the same exercises I did in physical therapy. They hurt at first but it did help eventually. Between chiro, pt, back brace, and meds my pain is manageable. Its not completely gone, but I am able to work and do daily activities I couldn't do before.

Did it hurt alot? My mother has the same issue. She is on bed rest right now and her leg hurts all the time even when she isn't moving her leg. She did try physiotherapy for a week but her leg hurts even more

I've have had lower back herniated disc for years now and started this exercise last year. It's a life saver! So now when I feel my hernia acting up, I proactively use this exercise to prevent any future pain. I also was advised to use ice pack on my back... and both methods works great

I have an L4 and L5 S1 disc bulge and central annular tears. They're also degenerative. The McKenzie method has been amazing. The extension exercises really do work! I'm going on 7 months of pain after my injury and it's now tolerable!

1 - Cat/cow on hands & knees, flexion/extension ROM 2 - Cat/cow seated 3 - pulling knees to chest, laying down, then alternating single knee to chest...then rotating single knee twisting across the midline 4 - laying face down, up dog with resting on elbows or with straight arms.

I've been a roofer for over 20 years and my lower back went out on me a few times. The pain I can say was so extreme I had to crawl out of my bed to my kitchen and slowly try to get up. It's almost as if my legs didn't want to work correctly. Like I have sacks of concrete in my back pockets and can't stand up straight. When it happens it takes me almost a whole month to recover fully. I watched this video and learned a few things. I'll definitely give these stretches a try thanks.

More than likely you had temporary compaction. Compressed nerve in fact. Rest and pain med should do the trick. And yes I know many will say stay away from the narcotics. But using a low dose narcotic such as 5 mg oxycodone and half a Tylenol tablet for 2 or three days once every 6 hours as needed isn't gonna turn you into a opiod use disorder patient! You're a roofer! Your temporary pain (pain lasting from 1-3 days should be addressed with decent medication and not just slap you with only acetaminophen or ibuprofen that's only going to relief a small amount of the pain. When you're reduced to barely being able to crawl because of pain, you deserve only what you should require to properly address your need for pain relief! I know because I built log cabins and roofed homes also! Often doctors have no idea how much some of use in construction work do suffer, and at the high level of pain we do incur.

You may need a script for 21 days if you're suffering for as long as a month. But try to only take as needed, and space doses out farther apart if possible. As your body repairs itself, even try taking half-doses! You may find you'll save a few which could be saved for future pain from roofing related injury! Just do not use opiods unless your pain is off the charts! You don't want to create problems! You only need to address moderate to severe pain my Brother!

@@randythomas3478 terrible advice all levels. Discs don’t have a blood supply to heal on their own. The only way they heal is by moving your body to get blood flowing. You will never heal them laying down. Painkillers also just mask the pain. You still have the disc irritating the nerve. What you want to use is anti inflammatories, which definitely do a better job at pain relief than Tylenol or narcotics. The only narcotic I took was when it was unbearable pain, and the only one that worked was tramadol.

I am 81 years old Male. On 2nd May 2020 while I was taking steps upward to reach second floor, suddenly I developed right side back pain radiating to calf pain and 3 fingers of foot. I walk daily for one hour or more. Since then I am bed ridden. Now as suggested by you 4 exercises I will try. Thanks Doctor.

Literally just did the chair thrusts as I'm working from home and my numbness is subsiding in my leg and foot...I want to cry... just went to the doctor and I was given no recommendations for relief. I've been dealing with this numbness for over a week... My injury began about a year ago, but after therapy I was food for almost a year...now the numbness is back...

Doctor thank you for the great info. A month ago I developed a pain that started at my buttocks, and went down my thigh. It sat at my calve. Then went to my ankle. I had an MRI and I have a disc bulge. The one you mentioned. I'm meeting with a phyiscal and rehabilitaton doctor next week. My pain keeps me from walking fast its horrible.

I’ve suffered with similar conditions, but never in a way that would cause my upper body to be bent right forward! I have no pains in the leg or foot, just the lower back. I’ve suffered great pain when trying to move around, but didn’t know what to do other than rest. Thank you so very much indeed for all your help.

I am also suffering from same pain in the lower back for past 5 , 6 years. I can’t move around; bending forward is great issue ; whenever I do some work bending immediately pain comes. I am just cooking & then taking rest; consulted few ortho doctors not much relief. MRI results shows problems with L4 L5S1 problems. Ayurveda massaging also tried. But no permanent remedy. In Chennai one dr Mr. Krishna Raman’s exercises helps a bit. Even now I suffer from same problems. I avoid going out ; jerks in the road affects more.. Movement of the body is very much restricted; This issue is big torture. The exercise only will give little relief

I have had blown discs bulge in l4 l5 numb foot and toes for over 10 years now. When it's bad I'm bedridden. Now it's radiate to my pelvic pubic bone area pain. It's a pain in the ass deal with pain for life. I'm used to pain but it sucks not being able to function a few days a year when it does act up bad. Scan shows pinch and infringing on nerves. I knew after heavy powerlifting in 2006 I woke up with numb foot and toes and calf numbness. It has never went away it's getting worse actually. Now my calf is smaller than the other one and weak. Great vid. My family doctor is no good. I need specialist again.

Totally agree, sometimes it’s agony and sometimes not. No idea why. Drinking more water helps and definitely not lifting heavy things. I’m 76 and have had lumbar pain since having my children.

I have been doing this exercises for 4 weeks and got incredible results. I had pain going from the upper butt down to the right leg calf, 2 weeks past by and the pain disappeared form the leg completely and was felt only in the upper butt, and even the diameter of it went down, I was the happiest man.....not so fast You would not believe this, woke up in the morning after all this amazing 4 weeks recovery that I had, and found myself back in square one again, Only now the pain I had on the back of my leg was replaced by 15%- 20% numbness all the way to the toes, a slight bend for brushing my teeth and boom all the pain came back (and it was less than a minor bent). Please any suggestions and words of encouragement?

I wouldn’t do any cardio, other than walking until the bulge is released. Believe me, if you’re in pain, the last thing you want is exercise to aggravate the issue. I even aggravated my L4 herniated disc with too many stretches 😢

good explanations and demos thanks - they were part of my morning yoga routine until I got lazy for a few months and when I bent over to pick something up, my lower back got nailed ...yet again. I'm 70 and have been diagnosed with bulge(s) on my lower disc, so I know what's going on. When I recover, I will for sure be back on my yoga routine, but was wondering if you could recommend a few exercises/stretches that would help strengthen the muscles that support the lower back? Great video, my spine is a mess and will be getting surgery. I will give these exercises a try until I get surgery. I have ice on it as much as I can to try and numb the pain. Thank you for your video and knowledge .

Thanks for the video .Had operation on L4/L5 ,Left discectomy about 4 months ,still in pain down left leg to my ankle ,also the top of my right top thigh .Dont want to have another operation but I can’t be in pain and on strong pain killers for ever .Thanks for explaing in a way most people can understand 👍

Respected sir I have lower back pain which is transferred to buttocks and left side leg .some times I feel tingling effect. Butterfly like feel in back . If I take rest pain is reduced . I experienced lower back pain in 2016. My L4 L5 had bulged and the pain was almost unbearable. I wish you would share with your subscribers the first thing to do in the few minutes upon experiencing the pain. Walking, sitting down and standing up, ascending and descending the stair is pain in every move. The 4th exercise which you recommend is hard to do when L4 L5 is bulging. I couldn't even stand straight at the time. As though there was a wedge stuck on the lowest part of my spine. But I'm grateful that I had stumbled upon your video. I just wish....

These exercises seem to help in the short term, during the exercises and for a short time afterwards, but ive only found relief by actually strengthening the surrounding muscles as carefully as possible, and that actually got me back to 90%. Now i have a second one from lifting wrong at work. Now i have a herniated L4-L5, and a buldged L5-S1. I tried these physical therapy exercises for weeks, and I never got any help with the pain until I started doing careful resistance training and actually pushing my body to adapt. I got more results out of 3 weeks of resistance training, than i did out of 6 weeks of PT.

I have bulging disc in L4/5… it was was never a massive problem until I had to have THR at 40… I recovered from that only to be flat on my back for a few months with excruciating sciatica down the leg, and agonising back pain. I can now walk again but still have permanent nerve damage, sciatica and lower back pain… however, doing these exercises daily reduces that and I can manage walking several miles daily (no more marathon running for me) better still I don’t have to rely on pain meds and have avoided problematic back surgery. 😍😍😍😍

Hey Doc . I injured my lower back while doing a weighted workout at home with dumbbells. Pain was unbearable first two days but them subsided . Got an MRI done , which reveals that I have an annual tear with the disc bulge at the L5 -S1 level . Would the same exercises apply (as I have the annular tear too)? How much time does complete recovery take? I really appreciate all the videos and content you have put up. Thanks again in advance :)

Hi Doc, great video! Love the exercises, somethings to consider though, The 2nd exercises sitting towards front of chair, some people may not be able to do that one, especially with injury to lower back, sacrum / coccyx bone. Really enjoyed this. And yes it helps .

Hello sir, Thank you so much for making this video. My mom is suffering from this and she is having pain from through back of the legs. In MRI report it says left posterolateral disc protrusion with annular tear seen in L4- L5 with mild facet hypertrophy causing right lateral recess and spinal canal stenosis resulting into impingement over thecal sac and exiting nerve roots, more on right side. Posterior disc bulging is seen at L3-L4 level with mild facetal hypertrophy causing indentation over thecal sac and exiting nerve roots. Rest of the intervertebral discs appear unremarkable. Verterbral bodies shows mild spondylotic changes. The lower end of spinal cord, conous medullaris and rest of the nerve roots of cauda equina are normal. Cords end at L1 level. Please tell me is this serious condition and what to do for this. My mom is not in canada please help me by giving suggestion what can be do? Waiting for your response sir

Quick question... What exercises do you recommend to tone abdominal muscles with a disc bulge & 3 herniated discs located throughout the spine (neck, middle, lower)? Regular crunches & sit ups aggravate these areas & cause unnecessary discomfort/pain. Thanks in advance. 🙂

Ok...the best book on this subject is by STUART MCGILL called BACK MECHANIC.The exersizes you need are in this book,illustrated with clear and safe descriptions.Stuart McGill is the Back Master.on RU-vid look him up.You can find him demonstrating what is called the Big 3 exersizes for back/disk rehabilitation.They are very safe and strengthen your spine without risking increased disc damage.I know this will help you if you listen

I recently started having sharp electric like pains along the back of my right leg, my doctor said it could be sciatic nerve related. Thanks for the stretching exercises and explanation of the potential cause of my pain. If treated early through regular stretching could it eliminate any further damage?

Hi, I've been dealing with my back pain for 3 years now. I started having pain when I was 15 and I had a MRI done a year later. I have multiple herniated discs but my most severe is between my L4-L5 8mm but again that was 2 was ago. I have gotten worse and I am scared to do any exercises. Are you sure these exercises are safe to do?

I've been told to only do extension exercises for my l5-S1 disc bulge, causing sciatica all the way to my foot. The first three exercises you recommend have only exasperated my injury

+Patrick Mcdonald, thanks for your comment. Always follow your doctor's recommendation before doing exercises. Not all exercises and stretches are suitable for all people and all low back conditions. Never do a stretch or exercises that exacerbates your pain or condition.

Tis video should be very helpful, lower back pain can be very debilitating.... I had a micro discectomy of L3-4.... One thing I would suggest in your video presentation is breath control. I have always believed it is very important not to hold your breath and to make sure to breath through the stretch....

I went to a chiropractor for the first time in my life last week. A friend suggested I should go. I had been feeling pain in my right hand, wrist and arm. Two fingers near my pinky were tingly and sometimes a little numb. I thought maybe it was carpultunnel because of the shooting pain in my forearm. Sometimes a burning sensation to my elbow. These symptoms happened on nights I worked a lot with the mouse on the computer. Icing after my work shift helped. Chiropractor took X-rays and said I had a bulging disc in lower back, another in lower neck and I have a loss of curve in my neck. I started my first treatments this week and I feel lost, I have learned nothing so far during my visits. I thought I would get suggestions on things to do at home to help improve the situation, but nope, no info, nothing. I just go in to the clinic, he puts the vibrating gun on my back a few times and on my neck, then sends me away to another room to lay down on a massage bed machine for 10 minutes and then I hold a band behind my neck a few times to help curve my neck and that's it, I go home.. I have two questions... Should I look for another chiropractor? And is it necessary to crack/pop the spine in order to fix a bulging disc. I'm really afraid to get that done... The chiropractor knows, that's why he has been using the gun, I'm not looking forward to when he will need to start popping/cracking...

Hi GPH22. Sorry to hear about what you are going through. For starters, the fact that the chiropractor took xrays is a good thing. Bulging discs can only be seen on an MRI or CT, but not on xrays. Xrays may show disc height decreases indicating degenerative disc disease. In my office, I spend time with patients explaining the corrective process, how the techniques we use will help the patient. I do a form of corrective chiropractic care called Chiropractic BioPhysics. The fact the this doc is giving you the elastic band exercise to help improve the neck lordosis (curve) is a good thing. I give the same exercise to my patients. I also focus on posture correction with exercises and adjustments, as well on spinal alignment corrections with traction (home and in office). I can't comment on if adjusting the spine corrects a bulging disc. However, adjusting the spine may help correct the alignment problem that is triggering disc inflammation and disc damage. When spinal alignment and spinal function is improved, then inflammation reduces and pain from the disc bulge may improve. The disc bulge may still be present but it may not be symptomatic. This is still a good thing and many people have been able to function in their day to day life with no pain and still the present of disc bulges. If you are afraid of adjustments, ask the chiropractor to modify the technique. I also use an adjusting instrument called Impulse that give light taps on the spine. It's backed by research, is gentle, and still effective. It is possible to still benefit from chiropractic care with full spine Impulse instrument adjustments. If you are not happy with the level of communication the chiropractor has provided, you should ask the doc that you have some questions, would like some clarification, and also have the doc review with you the care plan, why it's necessary, how long you can expect to see changes/improvements, what kind of changes/improvements are they measuring (spinal alignment, posture, range of motion, functional ability, spinal function, balance, health status). Also, ask if you can do home exercises that would complement the chiropractic care (traction, mobility exercises, stretches, posture correction exercises, balance work, core training). This is what I do with my patients. Dear Nisha, I also have been through the same issue for about 7 years now. The answer is, it depends on your lifestyle if you can get back to normal or not. First of all, I would eat healthy diet with more greens, and some meat as well. Secondly, Keep yourself hydrated, lack of water in the body makes your back stiff and eventually less mobile and more injury 🩸 Take those supplements that are good for bone and nerve health - such as, Vitamin C, Magnesium, and B Complex - we can get C and Magnesium from food, which is the best way to go. Drink Turmeric tea or capsules on regular bases, possibly 2-3 times a week. and sleep on a good mattress - not on the Sofa - all these will reduce inflammation and heal the back naturally! Stretch before you go to bed - specially the McKenzie stretches. Do NOT stretch right after you wake up, trust me on this one - I have had a herniation due to this and many professionals do NOT recommend stretching back right after waking up. Thank you I had an accident a year and a half ago. I broke my right femur bone and damaged my back. These ones you are talking about. It has also caused my 3 nerves in that area to not work, I am having major problems. Some of those exercises I can't do because of the scars on my right thigh. I was told it was a miracle I didn't lose my leg or that I am not in a wheel chair.

Excellent video Dr. Salubro - I just found your videos, not by coincidence but by looking on RU-vid to try to understand the vertebrae as I have no idea of how this works. 6 weeks ago my back started getting tender and it is now at the point where I cannot walk without a walker - I also have a bit of a problem with controlling my "bodily functions". I just had and emergency MRI and told that I have a herniated L5 and L4. Your exercises seem to be the right direction for me along with your great videos that I have had a chance to watch now. However, I cannot perform the exercises as I cannot get onto my stomach or any of the positions you showed without excruciating pain. Do you have a "lighter" or starter exercises for less advanced situation. I wish I could give you the details of the MRI but I do not have that yet and refuse to consider surgery. Thanks again for the very informative/helpful videos and any potential advice you can offer.

Hi Charif. Thanks for your comment. I can’t give a specific answer for your condition. Generally speaking, swimming may be helpful and tolerable. Riding the bike with an acute disc may not be tolerable. Every patient condition and situation is different so it’s best you consult with your doctor.
