Here I show you a biceps rupture evaluation and how I have modified the way I evaluate it to isolate a rupture especially in those who have ruptures but are not as easy to identify. I also throw in a few biceps evaluation pearls to help identify the rupture and talk about how to treat a biceps rupture.

I don't find it to be very reliable but I do teach it in my CME courses. Also, putting the arm up actively right after injury can be pretty painful. thanks for commenting☺

My right bicep has gotten pointy from an injury. It’s been a week now and now it’s starting to harden up on the “inside” 😊

I have a proximal tear of the short head with a 12cm gap between coracoid and current position but only found out 2 months after injury. Would appreciate your input. Do you have an email address I can contact?

helo sir is this possible bicep tendon tear at the labrum connection cause also a SLAP tear ? therefore we have bicep rupture or loose bicep tendon with tear of labrum?

4weeks post tenotomy - my lhbicep tendon has come out of its groove. no bulge or bruising - pain comes and goes - learned i have a subscap tear. hope conservative treatment work.

I tore my biceps in both arms like this and my doctor didn't perform surgery. He actually advised against it saying it didst offer much outside of making the biceps look normal. Should i seek a second opinion?

I think I got a partial distal tear about a year ago. The injury was very painless but I knew something was wrong. Doctor did an ultrasound and found nothing and I have done physio and it makes it feel better but it gets reinjured easily. It seems like the muscle has slid up slightly towards the shoulder and I have lost much supination strength. I am a power lifter but can’t lift anymore I feel like I need surgery.

3 years late, but Luddington’s is indeed a good way to suspect the long biceps head rupture. I use sonographies to search for the tendon, and the luddington test has been positive each time the tendon is missing in the bicipital groove.

Sir after my bicep workout yesterday I'm feeling bicep stiffness around tendon area I didn't listen to any pop or even visible bruise or Popeye syndrome I dont have any pain around that area but it's very tight it's like pulling I don't know what have I just done to myself 😭 can you please say what might it be ? Both biceps looks normal while flexing as well

Does a tear or partial tear always bruise? What would the pain level be from 1-10?

They don't always bruise but they do most of the time. Usually the pain isn't too dramatic. People panic about the noise it makes when it pops. Good questions!

I hear mine pop as I was benching I have bruised lump on my bicep like on the side I can use force pretty good it’s more on my inner left side of the bicep that is balled up could this be an indicator it’s rupture ? And would I need surgery ?

You could also have snapping elbow syndrome. This is not usually affected with benching and you should definitely see an orthopedic specialist

Does all bunching of the long head bicep mean a rupture or could it be a tear that can heal overtime ?

If it's bunched up and the other one isn't, then probably. Some partial tears will heal themselves but can be susceptible to complete rupture.

​@@OrthoEvalPal I had mine checked today at the doctors because of the Popeye muscle, my long head tendon is still in tact as the doctor was able to locate it and feel it not being ruptured. I can supinate my hand through resistance which contracts my long head but it is definitely a little bit lower so I'm going to the A&E shortly so they can do a scan, my doctor believes it could be a partial tear which may require surgery. Will provide an update later for anyone interested.

Sir my father's bicep muscle got down from his shoulder and is now near to his elbow. Looks like the muscle has broken. But he don't face any kind of pain in movement or in anything . Sir please tell me what kind of problem he is facing. Please sir reply

Hi Karmanpreet, That is the more common tear of the biceps and many people live the rest of their lives with that one torn and they do very well because there is the other biceps tendon that usually stays attached and therer is a muscle beneath it that bends the elbow too. Should feel better after 6 weeks but should see a doctor just to make sure there isn't something else going on.

My Doctor said once the long head tears there is no way of fixing it, is that true?

I tore my long-head proximal. I had it repaired there's still a little bit of a gap but not even as bad as before started working out 6 weeks after

They can be fixed by some shoulder specialist but there really is no need to. You can function well without the long head. They are often such a big pain generator that surgeons with just cut them off and let it drop if it is partially torn.

@@OrthoEvalPal your wrong. If not fixed looks deformed and you will for sure loose 20% strenghth. Also deal with lots of cramping. I know 6 people who tore proximal bicep. 3 had surgery way better result. 3 didnt big regret

Studies show that the difference between doing a tenodesis vs tenotomy is that there is more cramping after the tenotomy. Very few people have the proximal long head reattached to the original foot print. Yes you will lose some strength, especially with supination. It's really about patient preference. Now, distal biceps tears should almost always be repaired if they are of appropriate age. Much more functional loss there. Thanks for the input.

Yes. It's never to late to get it repaired.

My doctor said I didn't need surgery I tore my long head and I had the surgery surgery done best decision I ever made didn't come out a hundred percent but it's better than what it look like

@@joecaballero1749 100% new. Well worth it. I started exercising 2 weeks after. Light of course. It's very strange how most doctors don't want to do the surgery but my arm looks so deformed with that popeye look thank got he did surgery
