During one of my statements I said, "I came to get help and documentation in case they try and take my child, but in my report they said in case I have to sue. Hahahaha. So many lies are in my records. Goes to show that nurses can document they did something, or a patient said something, and the task they documented, or the words spoken could be false on so many levels. Yet as long as it is documented it must be the truth. Goes to show paperwork has ruined the art of caring.
