New subscriber here in November I was told that I am 3A CKD I am 71 years old 6’2” 245 lbs but active I still lift heavy weights throw shot put,discus,hammer and javelin in the Senior games. I don’t intend to give it up and I am glad you say lifting weights is a good thing even with CKD.

I just finished watching this video and it was so helpful for me and I enjoyed listening to both of you. I was just told that I have stage 3 kidney disease so I'm trying to educate myself in this so I know what's going on with my body and hopefully try to stop any progression possible. I am really interested in the complex cyst that I have in my left kidney which the doctor is a little worried about.. what exactly is complex cyst?

I'm a plant-based bodybuilder on dialysis. For reference, my kidneys failed over 25 years ago. I worked out hard even when they told me not to. In my experience, the frail die early. BTW, plant-based eating has wonderful effects on diabetics. I've never been diabetic though. I never have done less than 4 hr. treatments. I'm very muscular so I need longer treatments.

Good morning, I am a 24 year old female with kidney disease and a few autoimmune diseases. I have exceeded the limit of Vitamin D I'm supposed to be receiving (56.4 ng/ml) therefore my doctor asked me to refrain from taking it. Can I still supply my body with sunlight? Is there such thing as taking too much vitamin D (Wouldn't it cause thyroid problems?)? Thank you for providing your audience with such good information. God bless!

Dr Hashmi, Michelle, thanks for more great info! I suppose this is related to this video's topic of muscle building.. My question has a twist on your topic here. I had been reading specifically, isometric type exercise is both good for building muscle, but ALSO, if one uses a hand dynamometer at about 30% of your maximum strength, it can help you get stronger, but ALSO in time, reduce blood pressure as much as 10 points Systolic with doing just about 8 minutes a day, every other day. As high BP is obviously bad for CKD have you heard of this isometric type exercise actually really lowering BP? as it helps to build a bit of muscle too? Another words a win win...Thx.

There is some research on hand grip strength, isometric type exercise and blood pressure. Definitely something you could add to your routine to help :)

Thank you both so much for giving me direction on the plant based diet, your sound CKD advice and flavor-filled recipes. You are my go to resource as I begin this journey at stage 3. Any recommendations on working out with small weights or RU-vid videos on beginner weight workouts? Right now I’m just using the bike and elliptical and infrared light.

Oh man I’m stage 3B and I been consuming more protein than ever trying to gain strength and muscle to help with my spinal degeneration issues… protein has been mostly chicken and eggs 😞 I really didn’t realize I am shooting myself in the foot so to speak. I just got some labs done and am hoping for the best but I think I know already subconsciously that what I been doing is not good 🫤 but at this point in my life working out and building muscle is all I have to look forward to. Time to re asses

Similar position as you. My gGFR is at 58, while I pumped and ate up to 4-6 eggs a day to build. I have reduced eggs to 1 every day, but eating more soy beans, shiitake mushrooms and oats, nuts. But we need to be careful not to panic. These doctors, no matter how much we trust and respect them, they live off the disease they treat! Everything in moderation will be sound advice. It would be really beneficial for you to work 1:1 with a renal dietitian if you can so they can look at your labs, your exercise routine, and make individualized recommendations on protein and other foods! That way you can still meet your fitness goals and protect kidney health. www.kidney.org/ckdrd has a database of renal dietitians.

@@ディアラン I agree with you! I had been eating upwards of 200 G protein a day from eggs, chicken and diary mostly to keep up my macros to build muscle. I forgot what my last reading’s were it’s been over 4 months. We shall see what the labs say this time. I very much dislike the idea of eating soy and tofu not just cuz I don’t like it but because this style of eating is what I know works. Quite disheartening to hear all this I need to change my diet completely

People that are muscular and/or strength train should not put a lot of confidence on creatinine levels. It is much better to base your eGFR on Cystatin C. Many doctors do not know this.@@ディアラン

Hi Dr. Hashmi and Michelle. I am 55 years old bodybuilder with stage 3 ckd. I eat 200 grams of protein a day. Will it be easier on kidneys if let’s say splitting those 200 gm of protein from 4 portions to 6 ? Thank you. I love your videos.

I’ve increased since this post, only because I’m trying to add some muscle while losing fat. I’m at 65-100 grams per day now. 200 is still way too much. Also, I’m 37 and i just got a kidney panel done in June. My eGFR is 111. High protein diets are rough on the kidneys.

I would have liked to see the creatinine issue being addressed. You lift weights and creatinine goes up, so you're left with poorer GFR readings. How do we resolve that?

In this case you would also want to get a Cystatin C and use it to calculate eGFR. The Cystatin C is not affected by heavy lifting nor dietary protein intake. If you start working out and your creatinine goes up, it's likely a reflection of lifting weights and not necessarily indicative of further kidney decline.

Dear Dr. Hashmi and Michelle, thank you so much as usual for your very useful explanations. I would have a question for you. I am a 17 year old male and I fell in love with lifting weights, I love doing sports in general and over the years of training a built an athletic body, 1.90 m x 88kg, with a bodyfat probably around 10-15%, did sports beside weights all year round (track, wrestling). But unfortunately, from an early age, I was diagnosed with polycystic kidney disease. At the same time I would love to continue lifting weights, and I also would like to introduce protein powder in to my diet. Do u believe that a daily intake of protein of about 1.6-2 grams per kg would be appropriate and healthy for my case? I am very active, lift weights 5 times a week and do lots of cardio as well. Thank you very much for your attention and dedication.

I had the same problem as you, and the answer is, yea... 1,6-2 is to much, and we have to reduce it to up 1g/per kg. So, yea there is no chance for gain more musule than before.

On lifting weights, will it affect the fistula for patients on dialysis? Is there other way to build muscle that are considered safer for those with fistula? I've been told by doctors that I'm not supposed to carry heavy stuff with my fistula hand.

Pull bands - even from the floor if necessary. Fatigue the muscle so it can stay strong.

A question about fitness and common gym supplements like creatine (ATP system, cognitive functions) and l-citrulline (pre-workout for nitrous oxide cycle). The more plant based a diet become the less creatine is available in the diet. In some places it is recommended that vegetarians supplement with creatatine. The NO cycle in athletes also drops in efficiency as they age so l-citrulline helps with that. Any thoughts?

Hello doctor How much protein should I be taking daily? I am a healthy individual aged 42, 172cm, weigh 70kgs, male I do weight training (moderate to intense) 4 times a week, walk 10000 steps daily and on a vegetarian diet (consume dairy) My trainer says I should be taking around 160-180g of protein per day. Youre saying to utilize plant based protein like soy protein, but there is much research showing high estrogen associated with soy and plant protein. This would not be ideal for young men and men in general...

Hi, ive been diagnosed with acute kidney injury since i was 17yrs old and my creatinine level (normal 0.7-1.3) has been fluctuating (my levels: anywhere from 1.5-1.9) for 4 years now. I've tried everything; medication, change of diet and more water intake but still no difference. I am desperate for change and I want to gain muscle. Would plant based protein work for me?

I'm new, having proteinurea and other indicative symptoms, I stop muscle building Bcos I was worried that the broken down muscles could worsen my kidney condition. One time I did and the fatigue and illness I felt afterwards was just to much. It was then I learnt the muscles break down and kind of over work the kidney. I Luke what hear now but I am not still sure where the balance is.

What about essential aminoacids? For those with high protein requirements if you want to reduce the strain on the kidneys, you could have half of the protein dose from chicken, soybean whatever and the other half with essential aminoacids?

What’s your opinion on plant-based protein isolates (plant protein powders), with regards to kidney health? I consume a moderate amount of protein - 65-100 grams per day - much comes from these types of powders.

We have an episode on protein supplements so make sure to check that out! It really comes down to whether it is needed to meet protein needs or not. And what else is added to the protein supplement. Using a minimally processed protein powder (where it's just hemp protein powder or just pea protein powder for example) is ideal if using protein powder. We don't want people missing out on the fiber, antioxidants and naturally occurring vitamins/minerals in whole plant foods because they're drinking protein shakes/powders.

Minimally processed soy like tofu, tempeh, edamame. Legumes are another great plant protein source. Black beans, lentils, chickpeas, green peas, pinto beans, etc. They all have varying potassium levels.

I'm 43, got 2 kidney stones, just found out today. The problem is that if you are a skinny man and want to put on mass, you are gonna need a lot of proteins. ....And if you got candida and limit you calories from Carbs, you gonna need even more proteins.

You'll want to ask your nephrologist that so they can provide an individualized answer! There are kidney specific amino acid supplements that are lower in nitrogen. They should be used under guidance from physician/renal dietitian.

As an innocent bystander a contradiction in my mind is, with any kidney damage wouldn't you need a so-called complete protein for any possible repair to take place? I'm mostly vegetarian apart from fish a few times a week but don't have any real kidney problems. I should have watched this before saying anything.

This is a concern for a lot as a bigger muscular guy heavy into lifting I have relied a lot on meat protein in my years now trying to switch over .I'm finding very difficult trying to get the calories I need to keep up my bodyweight and also have to watch cholesterol and sugar levels. So it hard watching fat intake and can't increase carbs because insulin resistant it's hard to get calories
