To give you a cheeky answer - with difficulty. But seriously some of the really large decanters are very hard to use when serving at a table when the glass is placed on the table in the usual place in front of the setting. I would try to avoid using those sort of decanters for a formal dinner and stick to more traditional designs
Once I was at a rather nice meal and the the lady to the left of me put her hand over her wine but wasn't quite quick enough and unfortunately the server didn't notice her hand on her glass, let's just say, it was rather messy
