Clinical Examination of Head and Neck Lymph Nodes - Clinical Skills - Dr Gill
The clinical examination of head and neck lymph nodes is the first clinical skills session which new medical students at Warwick Medical School encounter and is a comment examination station in medical school OSCEs
Most people will be familiar with the concept of having "raised glands" when they have a sore throat. This is not actually having raised glands but is due to enlargement of the lymph nodes of the neck in order to facilitate the immune response to the infection.
Lymph nodes are exactly that, nodes on the network of the lymphatic system that allows the main immune system to react and to focus resources where they are needed within the body.
Here will look at the steps required for examination of the lymph nodes of the head and neck. Starting with the Submental submandibular parotid and tonsil lymph-nodes, we will progress upward To assess the pre-and post auricular lymph-node Before moving to the back of the head to check the occipital nodes.
Then we move down the neck, looking at the anterior and posterior cervical chains, before getting the patient to shrug their shoulders, in order to assess for supraclavicular nodes, and hopefully ruling out the worrying Virchows node.
Some people watch on this medical examination may experience an ASMR effect

I appreciate your method of mentioning what is happening and of obtaining consent. It is not only quite professional, but modern. I wager that many people have had bad experiences with doctors who were not informative and treated them as a specimen rather than a person (I know I have). It's good to know the next generations of doctors are learning to treat others with respect.

I have had problems with the lymp nodes in the left side of my neck for around a year, including a very painful biopsy recently. It is extremely helpful to see things from "the other side" and understand what is being looked for

As a person whose only in-class education relating to the human body comes from public school health class and a single introduction to biology class, the lymphatic system has always been an enigma to me and was not as thoroughly covered compared to some of the more “well-known” bodily systems (primarily reproductive, digestive, and circulatory). Therefore, I’ll be looking forward to learning more. Thanks for the great content Dr. Gill!

As someone with autism and a dislike for physical contact, I've had way too many uncomfortable experiences with doctors who don't explain their process but just get to it. Not only do I like to hear what they do when they do it as it helps me understand the procedure and purpose, but it also relaxes me for incoming touches and examinations. I'm glad you highlight this!

This is really interesting - I knew "lymph nodes" were a thing but not where they were, but curiously whenever I've ever had a cold, sore throat, flu etc I've instinctively wanted to massage these areas and felt relief when I did. Learning through ASMR for the win!

I have been on the internet for 2 years trying to figure out as to why I have these swollen lymph nodes.. with severe dry mouth after I got Covid in Jan 2021. I’ve been to ENT, Endocrinologist.. ultrasounds, MRI & still no proper solution. Just from your video I feel I learned so much & have a better understanding, now I can give the doctors more of a guide with proper terms/ knowledge! Thanks so much! Blessings to you! 🍀✨

Do you have any dental issues? I’ve had swollen lymph nodes for over a year now, following an incomplete filling of one of my teeth, and a botched filling of the tooth next to it. I had thyroid cancer and had my right thyroid removed, sept 2022. Still dealing with these issues, I believe I have a long lasting dental infection (due to the root canal I had when I was little, I have a crown and believe this to be infected). I have many teeth with issues as my gums are receding and my doctor labeled it severe periodontal disease. I have had dry mouth for years, I got Covid back in December of 2020 or 2021 I don’t even remember. But I take medications that a side effect is dry mouth so this issue has been dismissed rather quickly. Trying to see a dentist who will look at all my teeth and not just one tooth per appointment is rather challenging. I have tonsil stones as well, and an ENT I saw April 2023 told me to plan for surgery to remove my tonsils and adenoids. Then she left the practice and I won’t see another ENT until February 2024. I’m in constant pain, unable to eat a lot of food. I struggle swallowing my own spit at times. My situation feels kind of hopeless. I’ve just been dealing with this for so long now.

One time I had a swollen lymph node and didn’t know what it was so I had completely convinced myself it was a tumor, and it got worse because of my stress. So when the doctor was like “this? It’s just a swollen lymph node no biggie you probably had a cold” I burst into tears in relief so hard it scared her

It’s very easy to scare ourselves with changes to our body. We do it a LOT at medical school, and not infrequently as doctors too - sometimes you just need someone else to say “it’s ok”

I'm 20 and one on my neck on the middle left side got big and hurt a bit, usually the one in the tonsil area get swollen when i'm sick The next night i got a high fever, after 5 days or so the lymph node went down in size rather quickly, in about some hours, but it still is bigger than normal but isn't paining when i touch it. 4 days passed and it still is that size. And i'm so anxious i can't even remember what i felt like before i noticed the node.

I remember one time going in for pink eye and right after introducing himself and I explained what I was in for he just started examining me and touching all over my face and neck and I was so weirded out and confused

Thanks for this video. All my life I had wondered why I experience a little sting behind my right ear whenever I had an infection. In fact I have the biggest scar from chicken pox just in that node.

Very informative, thank you. I have an MPN (ET), and sarcoidosis, both rare disorders, and some other disorders. Recently I have noticed a lump on my neck, near my throat, which I’m unsure is lymph, salivary or thyroid issue (such as a nodule). When I was first diagnosed with sarcoidosis a consultant mentioned salivary gland, but no more mentioned, however that was before the ET dx. I plan to try to see my GP and ask to explore this, emphasising my conditions. It can be like a no man’s land when you have conditions and speak with a GP, as they can be quite dismissive to a patient’s concerns. (When you can get an appointment).

Also one question. I didn’t know there were lymph nodes at the back of the neck. Are they there for a specific purpose or do they have the same purpose like for transporting immune cells like the other lymph nodes mentioned? Thank you, sorry if my question is confusing.

ah yes. so you’ll be using the lymph nodes “backwards” ie here is the node, so the problem is there

all the nodes are draining from different locations. so far example groin lymph nodes suggest pelvic issues. axillae suggest breast or possibility of lung issues

Lovely video-very informative & relaxing! I do have a question. Do different countries teach doctors to palpate lymph nodes facing different directions? The reason I ask is I'm from the U.S, and while I've had my lymph nodes examined in this fashion many times in life, I've never had a physician do the examination whilst standing behind me-they've only ever stood in front of me. Is there a reason for this, and does it matter as far as having an accurate examination goes?

I’m a zoology major who’s failed human biology and anatomy countless times. I’m actually super interested in this though. (Also- your voice is so soothing. I’d never survive your class) 😂

What if there is a lymph node in front of the neck under the voice box, behind collar bone base of neck thats questionable?

Dr. James Gill, Thank you so much for the informative video. My have two persistently swollen lymph nodes since my pfizer vaccine doses back in May 2021. They have been persistently swollen for over a year now. One on each side of the front of my neck. They are very swollen, move easily and are very tender upon palpation. What kind of doctor should I see for this issue?

i have inflammation on the right side of neck and near the shoulder. An MRI scan on shoulder shows that it is rotator cuff injury,.Is the inflammation is associated with rotator cuff injury? Thanks .
