I love this, insight into why as a patient there is so much waiting that happens. It is so easy when you are seriously sick to forget what an enormous heroic responsibility doctors have taken on and how much of themselves they are giving to help us get well.

One could argue that this is due to being understaffed on doctors. We shouldnt be working them so hard. They are only human and need just as much rest, food, and other basic human needs as the rest of us

I appreciate your comment. We do work very hard, and we don’t like being late all the time either. We are very often understaffed; many of us would gladly accept lower pay for job-sharing positions, but those are rarely available.

I work as a chaplain at three different hospitals, including many on-call shifts. I hear these challenges of on-call work. Most of my overnight calls are for end-of-life needs or emergent crisis support. Thanks for sharing this, it is nice to see what it is like on the rest of the team.

I used to volunteer as a patient greeter for an outpatient endoscopy unit. I basically brought patients to change into gowns and send them to their beds. I'm a nursing student now but I appreciate the videos! I love seeing a different perspective and learning from a doctor

You explain things really well in layman terms - which I realised it's really important not only in patient education but also case presentations and actually - being able to do so well shows that your understanding is actually very strong. I'm learning so much as a PT 😊 Please consider doing educational videos on days where you're not feeling like doing meme-types (tho I enjoy those alot too as they're very accurate!!)

Finishing ophthalmology residency in a few weeks! Weekend call is definitely sphincter puckering! Why do people like to punch the face!! I’ve always been grateful for my medicine colleagues! You don’t get any appreciation; you’re work is simply expected.

Hi Dr. Schmidt, I've seen your tictoks before but I did not know you were a GI Fellow. I'm really glad I found your channel because your GI content is fascinating and gives great insight into the GI specialty which I think is spare on RU-vid. I am currently a Nutritional Sciences PhD student and RDN with plans on attending Medical school in 2023. I hope to become a gastroenterologist. Love the content, looking forward to seeing more!

Thanks for this great peek into your weekend on call. I've been enjoying all your short videos, love the different facial expressions & the various wigs! I especially appreciate the humour mixed in with all the medical terminology as I am a retired RN & worked for a couple of years in an endoscopy unit. Only one of the GI doctors I worked with seemed to have any sense of humour - good to see you do. Best wishes from Sydney, Australia

I have a lot of respect for doctors being on call. I personally am interested in medicine but I could never do on call. Keep up the great work!

I love videos like these since I think they do a great job of reiterating to the public how difficult and involved being a doctor and the training involved is. I think we do a bad job of making that clear and most people don’t understand what the problem with doctor shortages is and feel we get paid too much. You’ve inspired me to make some ophthalmology content in the same vein as yours! (Although I may not hit the mark as well with the comedy bits)

What if they hired twice as many doctors for half the pay? Would that help with the doctor shortage?

@@3_up_moon If the cost of hiring a physician were the problem it would. But it isn’t. The doctor shortage exists due to a lack of Medicare expansion and “budget neutrality” which has led to a fixed number of residency positions leading to an insufficient amount of training. Beyond not fixing the problem, it’s insulting. Asking people to take care of patients and halving their pay essentially implies that patient care is a service that can be cut

@@EthanMitch half the pay for half the work. I dont see the problem here. If you free up twice as many positions for care, you would have half the work load. Are you saying you feel entitled to the same pay for half as much work?

@@3_up_moon because that's not even how the work works. I don't see half as many patients or train half as long or care half as much for people. I have no idea what you do for a living but understanding healthcsre is not one of them

“Fellow” is an official title/job. The fellow is a doctor who has finished residency and is now doing advanced training in a subspecialty. Fellows are usually recent residency graduates, but I did my fellowship mid-career.

This was fascinating to watch! I am enrolling in a PA program this year and am highly interested in working in this field. I grew up with 2 family members with IBD and this field has evolved so much to provide more effective solutions, and hopefully one day that will include a cure. I have so much respect for gastroenterologists and their dedication to this work!

How often do you think a patient knows another doctor consulted on them? Only time I have ever heard about it previously was when my primary doc said “oh that’s weird, neuro asked for a consult on your MRI”.

This is really interesting to see! I’ve actually been in the ER twice before and was told point blank “we won’t hear from the on call specialist until tomorrow”. They never reached out. Finally we got ahold of the department and all they did was refer me to a private specialist group (urology). It took about 3 years for me to get diagnosed. Who finally cracked the case? Not any general medicine, emergency medicine, or urologist. It was the pelvic floor physical therapists that they sent me to as a last resort :/

I just found this channel! Absolutely love it! I actually have a methadone question. I'm a bit worried about my liver, I don't have any issues now. But I'm currently on MAT. Will prolonged use of methadone destroy my liver?

How senior are you in the US when you’re a fellow? I’m an Orthopaedic Surgeon (equiv. of an attending) in Australia but when I was a fellow doing on call, I had done my specialist exams, and could have worked as a specialist at that point already if I didn’t want to do more specialist training. I had a team of registrars or what you’d call residents below me who would see the patients as they came in through the ER and I would only have to go in for urgent operations that were needed to be done.

SHIT! As an intern in 1982 (yes, 40+ years ago), my weekend call started Friday 5 pm and went until Monday 7 am. UGH!

Just wondering...some people only require 4 hours of sleep in general to function. Are you one of those people? Also, some people are great nappers by nature. In your field it might be an acquired skill but I was just wondering because if it is acquired in your case that would be great. You seem to be brimming with energy all the time which is also wonderful !!! 👍👍👍👍

Do you do octreotide routinely for variceal bleeding, esp. in a post code that isn't stable enough for a hair cut let alone a scope? Kind of seems variable per facility...

That really shouldn't be variable per facility. Using octreotide is a cornerstone therapy for variceal bleeding as its in one of the very few classes of therapies shown to reduce mortality in variceal bleeding.

No registrars are equivalent to residents . Doctor shmidt is already done with specialization plus an additional year or two(i guess )to become a fellow instead of just an attending specialist) . He is very young for his accomplishments , its actually impressive .

In theory the weekends are less busy. I’m only covering the 4-5 doctors for a finite amount of time and then in exchange they cover me on other weekends when I have time off 😁

Just wait until you are the attending and you can hire a PA or NP to take first call. After years of fellowship torture, it is soooooo nice!

Your slurring a bit at 3AM on Saturday morning. I do believe that our higher senses are significantly compromised during those hours. We are depending exclusively on highly tuned reflexes to respond to critical situations.
