I had no idea that the details we find obscure like 'daughter's wedding is soon' could become important like that lol. If the previous doctor hadn't wrote that down you wouldn't have found out that the patient was probably lying. Thank you for the video and your insight!

In regards to the last story: I know that as a chronic pain patient myself, I tend to be incredibly honest with doctors. One of the worst things for us is to get labeled with “drug seeking behavior” (especially if you’re in a city that only has one hospital system) and thus lose access to meds that you not only need to function, but also to stronger meds you might need in the future if you get a bad case of kidney stones or a bad broken bone. I’ve found the best way to do this is just to be exceedingly honest about everything. Sometimes you’ll still run in to docs (especially in the ER) that seem to want to label *everyone* that’s in pain as “drug-seeking”, but it’s less often if you share everything, including the embarrassing stuff or stuff you might not otherwise want to admit to (example: admitting to marijuana use or taking “too many” Tylenol or ibuprofen out of desperation). The other way I avoid it is to not complain about pain at all, especially not go to an ER, unless it’s BAD. Like, life-threatening bad. Like, contemplating knocking myself out with a hammer, bad. If you don’t go to the doctor/ER often, they’re more likely to take you seriously when you *do* go. It’s a sucky situation because those of us with chronic pain often wait until we’re in dire straits get help for new pain.

@@Doc_Schmidt PLEASE do imperial vs SI units. Can the switch be done within hospitals - I think it has been done in the UK. Does the switch cause deaths? Should we skip it? Could an American doctor safely practise in an SI-unit country?

I found your channel while I was hospitalized for the last 2 months trying to get a stent for a biliary stricture. It's such a funny coincidence to be admitted in the Hepatology ward and then RU-vid recommends a GI doctor's RU-vid page. 😂 P.S. if you could do something on PSC, it'd be great. I had no clue about the disease untill I got diagnosed with it. Thanks for brightening my days during the time I got the diagnosis and was spiralling. You touch more lives than you know. Not OP but GERD is extremely common, it has nothing to do with H pylori, as H pylori has to do with peptic ulcers (ulcers in the stomach or the duodenum)

Most people confuse GERD with heartburn in general. Heartburn is a symptom of GERD, but not all patients with heartburn have GERD. You need to be tested to see if you actually have severe acid reflux.

I had a patient many years ago that called 911 because he “sat” on a gun at home. He said he didn’t know there was a loaded gun between the cushions and his male friend took off before we got there. No way I could figure out how he didn’t shoot his butt cheeks if his story was true. The gun shot completely missed his cheeks and started right at his anus. Needless to say, we drove really fast to the nearest trauma center with him. He was looking super shocky when we got there, but he was still conscious and talking.

Hey Dr Schmidt! Few questions for next videos - 1. Do you plan to further specialize within a specific population in GI (Liver, IBD, IBS and motility clinics, clost.diff and fecal transplantation etc) or do you plan to practice as a General GI? 2. How common do you have a shared clinics with Clinical Immunologists/Rheumatologists for patients who’s IBD is clearly on the background of a more systemic immune tolerance failure (like primary immunodeficiencies)? What are other specialties you work closely with (Infectious diseases? General Surgery?) 3. Do you plan on having an Academic career in Medicine? How much interest do you find in the preclinical sciences? 4. What disease is the most interesting in your eyes (GI or generally) 5. GI is a pretty competitive IM fellowship like cards. Was there ever a worry that you would fall back on general IM, especially since you wanted to specialize? Or did you feel like a pretty competitive applicant by the time you applied?

Any info you want laypatients to know? General info? Procedures- besides a colonoscopy once 50+, are there any routine tests we should have done? Are there any things we should watch out for? Like how a woman should come in to an OBGYN or cancer doc if there's hard lumps inside her boobs, is there an equivalent for a GI doc? Also how do you deal with patients on their period? How do you know that the blood on their poo isn't from their period? Do periods complicate your job?

Here are my questions (reposting my two comments from your community post cos u probably didn't see them idk): Two random questions: Why is PicoPrep actually Satan incarnate? What are your thoughts on "leaky gut"? (because it sounds like bollocks to me but I'm not a gastroenterologist 😉 ) How do you approach that age-old "non-specific abdo pain" conundrum, especially when patients aren't satisfied with "we don't know exactly what it is"? I'm theatre RN but happen to be currently lying in bed healing from my own gastro surgery (an open laparotomy due to a bowel psuedo-obstruction caused endo adhesions on the left colic flexure, I was throwing up poop but I kept all my bowels and my colleagues know what my intestines look like now, yay!!!) but I know it's a cause of a lot of distress when people don't get clear answers, especially with acute pain that is sufficient to take someone to the ED.

Have you done a video on C. Diff? I was looking for a good one to send a family member who recently had an infection after a diabetic foot infection in hospital. He also has an autoimmune disease. I worry about reinfection. The videos I did see are great for insomnia. Emphasize handwashing and sanitation, please!

I just found you, so I missed the opportunity to ask on the appropriate video... If a person is (long) diagnosed with a serious GI issue that’s also rare and without a lot of research, studies, treatments, etc., where should that patient go for help determining the best treatment (asking about specific hospitals or specialists)? After bouncing from GI to GI, the current said that it’s too advanced for a regional hospital GI and that I need to see someone at a large city hospital.

When do you need to go to the doctor for bleeding & blood in the stool? I have been bleeding on & off for months now & I have severe abdominal pain when this occurs. Part of me is really scared to go to the doctors office because I definitely don't want bad news. Also, my father & brother both did the same thing for years. Please help, I am really nervous about the whole situation... Thank you so very much 😉

Get to a doctor ! If something is causing you to bleed internally, you definitely want to get it checked out. What if it’s something that could be easily addressed but becomes life threatening bc you left it untreated? Wishing you all the best

I have Crohn’s and get treatment via a six weekly IV drip (inflectra) as previous aziothioprine was effecting my white blood count and it’s like I don’t have anything wrong with me. Im 25 and I know this medicine is “high risk”, is it worth it? (Also have you ever seen crohns just… go away?).

Please find and answer this! I have an MS in Sci Ed but I still cannot understand this. I had an emergency hepatojujenostomy due to an iatrogenic biliary injury. I don't know what is connected to what anymore and what to expect as far as lifetime repair. Happened at Duke. Major FU to say the least. Thank you.

I don't know if you're even allowed to answer this, but I figure it's worth a shot. What course of action would you recommend for a 24yr old female that constantly has stomach issues, including frequent blood in her stool, but has already had crohn's and colitis ruled out by a colonoscopy? This is something that my daughter has been struggling with for about 3yrs. They did the colonoscopy just before covid blew up, then they basically just said "no crohn's, no colitis, must be your diet" and left her to deal with it on her own. For the record, it does not seem to be her diet, as she eats fairly healthily, and has done several elimination tests. We're not sure where to proceed from here, and could really use some guidance.

There are several medications that need to be titrated based on a patient’s weight/size, or other factors. So why in God’s name do you GI docs think that EVERYONE can consume the gallon of liquid required for the colonoscopy prep? I have to get yearly or every other year colonoscopies and I end up vomiting every time during the later stages of the prep. Not because the prep tastes bad but simply because my stomach is so full I cannot get another drop in. Why would you think that someone under 5 feet tall and weighing less than 110 lbs can consume as much as someone over 6 feet tall and 200+ lbs?

As a veteran of a dozen colonoscopies plus 4 surgery-related bowel preps, let me tell you what your GI won't: *you don't have to drink the entire gallon if your poop is coming out clear.* :D The best way to minimize the amount of prep you'll need is to fast; two days prior switch to very light meals of low residue food, one day prior switch to clear liquids. The day of, chug about a quarter of the prep, then sip on the rest with plenty of water and gatorade until you're squirting clear. Everyone's body is different, of course, but that's the method that works best for giving me an excellent preparation while minimizing my nausea and discomfort.

I’d also imagine that medications are much stronger and affect the body more than something like the prep solution. But like he replied... I don’t recall ever seeing a medication by weight.

I only have one Story involving an GI Doc. That would be my very first visit there. Told him that. Do i want anesthesia ... nope, need to drive home. Got told it wont hurt, it didnt. He found something .. got told it wont hurt to get rid of it. It didnt. But there was a thing he didnt tell me. A very simple thing. Go for a walk for 10 15 minutes afterwards. Dont ge tin the car immediatly. On the drive home i got massive pain. Thought that something was wrong. Thought he damaged something. Crawled up the stairs to my living room (yes crawled, on all 4s). Called my Partner back then (Nurse). Well .. during the call i passed gas. She heard it. She laughed. She asked if i went for a walk. Since then ... i always walk a bit and get some gas out. Never had that pain again. I do have a new GI doc. The old one retired. He told me immediatly that i should take a walk afterwards. That was the day i learned that there are 2 Types of GI docs. The ones that do tell you the secret of an walk, and the ones that dont. But to be honest. Still a better experience then the first Visit at my "second" Dentist. And for those wondering how often i have to go since im already on my "second" GI Doc. Thre is a Family history of Cancer so im told its best to go every 3-5 Years.

I would imagine (and hope) that ER docs and pediatricians are quicker to suspect lies when a parent’s explanation of cause of injury to a child doesn’t match the injury.

Because Drs just don't have the time to dig, think or care, I stopped going to the Dr to find out what my long term chronic issues are, yes I'm undiagnosed, have been for 20-25 maybe more years, they don't care to hear me out and I don't care to pay them for guessing, condescension and patronization. No need to pay for health insurance when they are gonna let you die anyway
