Episode 57: Diuretics And Kidney Function - How Diuretics Impact Your GFR.
What is a diuretic
Commonly known as a “water pill”, a diuretic stimulates the amount of water and salt expelled from the body. These come in a number of forms, including prescription, over the counter, and natural.
Why use a diuretic
• Control blood pressure
• Control Potassium levels in people with elevated potassium
• Fluid Retention (sodium)
POLL - Do you currently take a diuretic?
Are diuretics safe for CKD patients?
Always check with your doctor first - it is important to never get dehydrated and proper hydration is important when taking a diuretic. For those with CKD, dehydration is even more harmful. That said, there are cases where doctors will recommend a diuretic for a patient with CKD. These including treating high blood pressure, Edema (swelling), release sodium, and to help lessen fluid restrictions.
How does a diuretic work?
Do diuretics help flush toxins from my system?
Technically yes - to a small amount. But do not see diuretics as a boost to your kidney function. They do increase the workload on your kidneys and by doing so, they can help a slight amount at expelling more toxins, but for most people they are not…they are just diluting the concentration within your liquid waste. Diuretics do help flush potassium, sodium, and chloride.
Can a diuretic increase my GFR?
In certain cases there can be a bump in a persons GFR, but for the majority of people, extended use of a diuretic can actually cause your GFR to decline and your BUN to increase.
Should you avoid diuretics?
No - but don’t prescribe one to yourself without consulting with your doctor. When properly used at a doctor’s direction, they can be beneficial in helping control blood pressure, reducing swelling, and allow a person to lessen their fluid restriction.
When it comes to living with Chronic Kidney Disease, balance is the underlying key to a successful treatment strategy. From eating a balanced healthy diet, to finding the right range for your blood pressure. The more things we can get into the right ranges, the right balance, the less our symptoms will be and the better we can enjoy our lives with CKD.

I'm glad my story helps you when you feel down. Living with CKD can be overwhelming at times - the uncertainty, the restrictions, and seeing others who are not doing very well in their fight against it. Even I face these same demons - none of us are completely immune to it. What keeps me excited is my family and friends. I keep myself busy, love sharing what I learn on my RU-vid channel, and absolutely love meeting new people who have CKD all around the world. We are part of a very unique and special community - I find nearly everyone is very supportive, helpful, and we all have this instant friendship. I have an ask for you - I really enjoy your videos on RU-vid - how about making a short little video saying hi to everyone and telling us where you are located and what stage you are. I really want to feature short video clips like this in my future videos to encourage people to meet more people in our CKD community. You can even mention your channel and encourage people to check it out. You can email the video to me at james at dadvicetv.com

Hey James. I said I’d report back when I got my next bloodwork. Last bloods my creatinine was 190 and my Egfr was down to 22. I switched three months ago to a whole foods plant based low oil, sugar and salt diet. This week my creatinine is 142 and my egfr is 34. Not great for many but that’s back up to where it was before my heart issues two years ago. I’m really happy.

Any improvement is reason to celebrate! Congrats on getting back to stage 3! There are new renal diet recommendations getting ready to be released and they recommend eating a plant based diet far stronger than in the past. With your improvement (which was about a 50% increase) you would probably benefit from a renal probiotic like Renadyl. You take it for 3 months straight before it starts showing results, but it helps to metabolize (break down) toxins in your gut so they pass in your bowl movements instead of getting into hour blood and needing your kidneys to filter them out. I have been using it off and on during my improvement and currently am doing a case study with it. I got new labs and then started taking it daily and will get another set of labs after the 3 month period (late January). I will then share my results with everyone to encourage them to add it to their treatment strategy. Here is a link for info on it and how to get a discount from the manufacturer www.dadvicetv.com/renadyl

@@rhondasternberg2881 Not exactly but very close to one. I will eat some egg from time to time and occasionally a small amount of cheese. I got new labs at the end of October and am on a very specific diet right now with a probiotic and will get labs at the end of January and post the before and after results (including exactly what I have done). I'm expecting my BUN and Creatinine to drop and GFR to bump up. I feel AMAZING and it is taking everything I have to keep from talking about it before I get the labs to show the results of 3 months following a plan. You can see what I'm taking to help at www.dadvicetv.com/renadyl - also recommended in the December issues of the AAKP Renal Life magazine.

@@ronnie_g5051 I was on furosemide (Lasix) and before that triemterine for years. Diuretics put stress on our kidneys especially if not hydrating with water, not just fluids. I rarely have any leg or foot swelling and add no salt to my diet. I’ve maintained my stage 4 status for 6 years. So many rx’s are very harmful to ckd. I was very fortunate to have had that adjusted in ICU 6 yrs ago. I’m also diabetic but due to adhering to the ckd diet I don’t take any RX for it. Glucose under control. Diabetic meds are stressful to kidneys too.

That is great that you took it upon yourself to eat a better diet. While there is no 1 single diet that is the best, eating plant based is the closest thing. I recommend using a cast iron pot/pan/wok to help infuse your food with some additional Iron - this will help prevent Anemia. Do you currently take a renal multivitamin like ProRenal+D? Good luck on your next labs and let us know how they turn out!

@@timmorris3883 I would be firm and direct with your doctor - Refusing to give you a referral to a dietitian is a violation of the oath they took to do no harm. SOME doctors are too stubborn and need that extra reminder of why they went into medicine. A good doctor will respect your urgency to make changes and improve your health and it will make your relationship even better. All of my doctors know me well enough that we make "deals" or pretty much everything. One wants me to come back in 3 months, I ask for 1 and we end up on 2. :-) Make sure they understand you WANT to improve. @@DADVICETV UPDATE! I was able to locate a dietitian in my area with experience with ckd and diabetic patients. We have met and discussed my max and min numbers and a diet plan. As I stated in my previous message I had already reduced the amount of sodium, phosphorus, potassium and animal protien from my diet . I went to my GP doctor this week and asked for labs so I could see where I was at and if the diet was helping. Well the good news is..... my EGFR went from 27 stage 4 in September to EGFR of 41 stage 3 this third week of Dec.You can imagine the psychological and mental boost that is to my desire to continue down this road of healthier eating and living! Now over the past month I have not been as dedicated to eating right with all the holiday parties and food of this time of year. While I have pretty much maintained my will power I have cheated some. Hopefully with this good news from the labs it will be motivation to be more aware of what I put in my Pie-hole. LOL. I have also started taking the Pro-renal multivitamins with omega 3 along with increased fiber supplements daily. I'm very interested and eager to see your results from taking the Renadyl in the coming months. My next labs and visit with nephrologist is in Feb and I cant wait to see his face when he sees my GFR has increased so much, especially since he kinda indicated that it wasn't possible. Hopefully it will even be higher then. I am so glad I found your you-tube channel and web site. Thank you for the advice and encouragement you give to all of us fellow ckd patients.
@DADVICETV
@DADVICETV 4 года назад
@@timmorris3883 OUTSTANDING TIM!!!!! I love hearing stories like yours, and that is a FANTASTIC jump in GFR. For most people, the GFR will continue to increase as they tweak their diet and they will eventually make it to a stable number that is close to their max for diet. That is where something like Renadyl comes in - it helps reduce the amount of toxins going into your bloodstream so your kidneys can focus on the other waste and toxins in your blood...potentially boosting GFR even more. I do have a favor to ask of you - the thing which really sets my info apart from all the others is that I am a REAL kidney patient and sharing what I did to get better. Too many Nephrologists are robbing people of the opportunity to try diet changes, just letting them slide down to dialysis-land. While diet and lifestyle changes will not work for everyone, I have seen positive results in well over 9 out of 10 cases. People need to discover there is hope, they need to discover the power of eating healthier, they need to know that dialysis is not the guaranteed results after getting diagnosed. Everyone who can buy more time (and energy) is getting their life back while also getting closer to future non-dialysis treatment options being developed. My ask of you - can you record a testimonial video of yourself - something to encourage others to look at their diet, to make these changes, and to see what happens. The more people we can get talking about this, the more hope we bring to others. And it is also such an amazing feeling to know you are sharing something that is actually making other people's lives better. I know it isn't easy to expose yourself on video (believe me, I am not a video person except for the guy playing "before") but this would be so helpful for so many. :-) You can send it to me via email, or upload it to RU-vid (I can download from there), or message it to me in Facebook. I want to amplify the voice saying this can work so that together we can reach more people and give them this same excitement and relief you are experiencing. So happy for you!!!!!

I'm by no means suggesting u anything.. I just shared my experience.. please do have your doctors opinion to check whether these foods are safe for you

Your kidneys manage your blood pressure and it is common to get a small diuretic to help manage it. While the diuretic does place a little extra stress on your kidneys, keeping your blood pressure in range is far more important. High blood pressure can quickly damage Nephrons and reduce your kidney function, so keep an eye on it. Most doctors aim for a high blood pressure of 130/80.

@fisterhr
@fisterhr 2 года назад
Question, what do you mean by blood pressure medicine with the water pill? Are you talking about a strong diuretic? I ask b/c my dad has lowered his protein thanks to a low carb diet but he is struggling with anemia and edema in his legs and ankles. The doctor takes him off the strong diuretic and his creatinine goes down, but he swells up. He puts him back on it, and his creatinine goes up but the swelling goes down. Here is another side note. After being a diabetic close to 30 years, in less than a year on the low carb diet, he brought down his A1c to being one point of being NORMAL, his triglycerides went to normal levels and is one point from being in the normal HDL range...in other words, his insulin resistance is going away. I have heard and read a lot of anecdotal testimonies where keto of low-carb diets are not only reversing type 2 diabetes, but also correcting blood pressure, and helping reduce protein leakage, so I'm wondering if my dad's blood pressure is better, yet he keeps taking the same blood pressure dosage that may be lowering more than it should, so this, in addition to the strong diuretic is that what may be lowering his blood pressure more, may be hurting the kidney. So I'm guessing that his blood pressure needs to be reevaluated to where he may ask for a lower dosage. I may be wrong but what James is saying here about blood pressure not being too bad in combination with a strong diuretic is what is making dad's GFR go down is what may be happening to my dad. Just a theory and I thought I'd mention it here because of what you said.

I hopeI can stop my function from declining...I've only had diabetes for two years yet I went from no protein to a albumin creatinine ratio of 600! That's a shockingly high amount of protein. My diabetes has been out of control and my diet poor. I'm hoping there's some temporary reason for this jump in protein, because that's a shockingly big jump! My GFR is still over 120, but I've read that can be a result of "hyper filtration" which is part of the disease.....I'm hoping I can use some of your strategies to not cause further damage. I hope it's not too late.

Fruit smoothies are usually packed with sugar and I have learned so much about how our body processes sugar that I try to greatly limit the amount of sugar I eat in a day. When I do very low carbs (high fiber, low sugar) I drop weight quickly and easily. So I personally avoid things like smoothies.

My dad’s blood pressure is under control using 3 medications for blood pressure, it’s being between 125/75 and 130/80. He has CKD with creatinine 2.09 and GFR 36. Recently we noticed some swallowing in his feet, the doctor recommended diuretics pill. I’m not with him because these pills will cause his blood pressure to go very low. How can he get rid of the water retention in his feet? Does natural diuretics help?

The one thing I do know about treating gout - Black Cherry Juice is your best friend! When I would get a flare up of gout - I'd by me a jar of Cherry Juice and drink it throughout the day. There is something in Black Cherry Juice (the real juice) that lowers Uric Acid and greatly reduced my gout till it went away. I keep a jar of Black Cherry Juice in the fridge and drink a little every day to help keep my Uric Acid lower.

It was a challenging book to read cover to cover - but I really liked that he included so much of the research. I was lucky to avoid the need for dialysis, so his strategy doesn’t apply to me (I was well into Stage 4 by the time the book was published and arrived in the mail). My bloodwork continues to improve (RBC, Hemoglobins, etc.) but I feel my GFR is needing a change in my treatment strategy to get it moving up faster (I have had 2 setbacks throughout the year, one from animal protein when I first switched to Keto for almost 2 months and the other from taking the wrong antacids on a trip where i picked up something and had to fight it off). I am planning to re-read the book this December (I am traveling a lot for work right now) and see if there is anything in it which I think can help at stage 3. So as of right now I have not tried anything from his book.

He was saying there is not actual study that proves too much protein hurts kidneys... so I am confused again. Stage4 but dropped again to stage 5 (think from blood count down) there is also a FB group for kidney support. Some folk have low GFR and still not on dialysis. Hope they do try and change diet. The worry is low GFR and tiredness. He has a cook book out.

Hi James, what do you think/know about pomegranate juice? Is it good or bad for people with kidney disease? I'm concerned about the potassium it has (is it something we should avoid?)
@DADVICETV
@DADVICETV 4 года назад
I have no say one way or the other on Pomegranate juice, except that it doesn't fit my taste. Our goal with our diet is to avoid EXCESS. Your body needs things like potassium, and with a low GFR you need to watch how much you consume per day. Using an App like Chronometer will help you to track your nutrients and makes it easier to fit something like Pomegranate juice in. Even though Pomegranate juice is on the higher side of Potassium (around 67mg per ounce), most CKD patients can easily fit in an 8-ounce cup (about 536mg) in their daily diet. Your doctor will give you your daily targets, commonly 2700mg a day for potassium. Remember, watering down strong tasting juices like this allows you to drink more in total without adding additional potassium - I do this often with Cherry juice.

They put me on furosemide but my caregiver said that her sister's doctor told her that they don't want to put her on furosemide because it can give you kidney issues. I am now quite worried and concerned and I do not want to take the furosemide furosemide. This is a loop diuretic. I haven't yet but my card gets filled on the 1st and I am going to be cutting out all processed foods, I'm ditching the salt shaker and going to be getting the weight off because I am also quite a bit overweight. Even though I have congestive heart failure, I don't have swollen feet and I don't have swollen ankles. My hands are not swollen either. I wish I had an answer to this, but it's beyond my experience as a kidney patient. Your dads doctor will know best - and my advice would be to use a quality source for his salt, like Pink Sea Salt and not the overly processed common iodized table salt. Stage 5 is very challenging to find the right balance and it's important to work very closely with your dads doctor during this stage. Wish I could offer more. Best of luck!

It can be challenging getting a diet to address multiple issues, but a good dietitian may be able to help. I went through a number of dietitians - the 5th one was the one I liked and have stuck with. I was pre-diabetic (never got officially classified as diabetic type 2) and a reduced carb renal diet had a huge impact and got my blood sugar back to normal. I still continue to eat low carb, though it is getting challenging with all of the holiday events going on now. See if you can get a referral to a dietitian - a good one can really change your life. Good luck!

So sorry to hear that Alicia. I remember when I couldn't fit into my shoes - and I have massive feat to start with (11.5 6E - that is mega super ultra wide). My doc had me GREATLY reduce sodium for a while and they kept an eye on how much I drank and urinated. They had me switch my diet to low inflammation foods which can help reduce leaking protein. My problem continued and they had me remove all SOY from my diet and WOW - things got much better. Turns out I have an allergy to soy, though I never knew it and used to eat it all the time. Soy is normally an anti-inflammatory but for some people, it does the opposite. I hope this helps. See what your doctor or dietitian says when you share my experience with them. Good luck!

Violet - thanks for sharing your story. Kidney disease is a unique in that there are so many things which can impact it. Even people following the exact same diet can end up with different results. Here is what I've learned in a nutshell - first you need to find the underlying cause and manage it to stop further damage. Next you need to manage your blood pressure and blood sugar - these can both cause more damage to your kidneys, including inflammation which puts you on a downward spiral. After that it's lifestyle changes to remove anything which is bad for your kidneys - no smoking, no NSAIDs pain relievers like Ibuprofen, limit or avoid alcoholic beverages, etc. Last is diet - this has to be modified based on your last labs (and re-adjusted each time you get labs). The diet has to be balanced and give your body what it needs. Many people take this to mean avoiding lots of things - you can't starve yourself healthy. For example, getting too little sodium is just as bad as too much. To help I have a food list I use and a worksheet for your doctor/dietitian to help you - you can download it from www.dadvicetv.com/food - others may charge for a similar list, don't pay for it, download it free. Part of your diet is proper hydration - not staying hydrated can cause a big swing in your labs. Some people try to drink a bunch the hour before getting blood drawn - that's too late. You need to stay hydrated by drinking your daily target throughout the day. This includes while fasting - you continue to stay hydrated during your fasting window. If you are diabetic, fasting can also have a negative impact so check with your doctor if fasting is good for you. Hope this helps - and Happy upcoming birthday!
