In this video, I’m going to tell you about the best test for insulin resistance called the Homeostatic Model Assessment of Insulin Resistance, or HOMA-IR.
The body works best in a state of homeostasis. This means that all of the body’s internal functions like blood pressure, blood sugars, temperature, and pulse rate all remain constant.
Insulin resistance is a condition in which the body stops the absorption of insulin as a protective mechanism against high levels of insulin in the blood. HOMA-IR measures how hard the body is working to achieve normal glucose levels and maintain homeostasis.
A high HOMA-IR signifies high insulin resistance and a low HOMA-IR signifies low insulin resistance.
Sometimes when you start a keto diet, you may notice that your blood sugars are high. This is actually because you are correcting insulin resistance. Your body no longer has a high level of insulin pushing the blood sugars down.
The HOMA-IR is a great baseline test to determine your insulin levels, especially when you’re starting a keto diet. It can also offer clarity and peace of mind if your blood sugars rise when you start keto.

@@karenorr3050try asking for HomA IR. That’s the correct name. If they don’t have it then google independent blood testing labs near me. That will lead you to online companies that will set everything up for you for a fee and tell you where, locally, you ca have blood drawn. The online company will send you a form to give the lab when it’s drawn and they will bill the online company who will then send you the results with an explanation of the results.

Dr. Berg, I wish you wouldve explained that you can only get the insulin test done at the doctors office or even the HOMA-IR calculation and results once calculated. I was looking for a HOMA IR testing kit for home, not realizing I had to get it from the doctor. Explanation is great; just need to elaborate on when you say " just do this test..", it is at the doctors office. Thanks for all you do!

Congratulations on such wonderful success. I am 54 and battling Hashimoto's and now my A1c is going up and I can't for the life of me figure it out. The doctors are clueless. They just want to try metformin & weight loss drugs. I'm 193 lbs at 5'6". May I ask for more detail on how you corrected your issues please? What supplementations are you using? I've tried the healthy keto / intermittent fasting from 8pm-Noon and it worked great years ago but now not so much. Thank you in advance.

Hello from Italy, just did my blood works after 5 moths of keto diet and intermittent fastings and my HOMA IR turned out to be 1.2 (92 fasting glucose and 5.3 insulin). What I am still not happy with is my HbA1c which turned out to be 5.2 : it's a good value but still quite far from my goal which is not more than 4.8 or lower. I have lost 33 pounds and my blood pressure has dropped by 30 from 140/75 to 110/68 and overall I'm feeling way better. Tryglicerys scored a 48 after eating no less than 60 eggs a month which is pretty counter-intuitive for mainstream guidelines. Thank you very much Dr. Berg for your precious advices ;)

Oh my god. I’m doing keto now 3 month and I’m not taking medication. My blood sugar is 100-115. And I was so depressed and disappointed. I was saying keto is not working for me. Thanks for this video, will keep on this diet hopefully will have good numbers in the future.

u need to do IF to lower ur blood sugar. Keto isnt enough. I had insulin resistance now my blood sugar is 90 max after eating

Sv Vr 90. Whoa I’m doing two meals a day. But I cannot drop it my blood sugar lower that that. You know I’m doing everything on my own. Not with the help of doctor or nutritionist. My dr told me last time. You have to eat carbohydrates. So you see it’s hard for me I’m not a dr etc. But will Do my best. Thank you.

Its really simple and u dont have to be intimidated. I'd recommend doing 16:8(u eat 2 or 3 meals in 8 hours and then fast for 16). Eat enough fat and u wont be hungry. Remember it takes time to heal insulin resistance. Its fat in ur cells that ur body wont allow itself to release if ur insulin levels keep being high. So when you have periods of not eating ur insulin lowers itself and ur body starts functioning properly. Dr Berg has videos on intermittent fasting. U can try 20:4 if u have a bad case of insulin resistance

One thing that was slowing my weight loss was to much protein. More than 5-8 oz. of protein will spike insulin. It helps to eat your protein with fat. Hope this helps and keep watching Dr Bergs videos and you will gain understanding over time.

Great advice, except there is controversy over the benefit of eating any carbs, at all, for some people with chronic conditions manifested, or for persons not displaying any abnormal symptoms(those seemingly having “normal” good health.

This is so precious information! Feel better now with my glucose numbers of 97 - 105 in fasted state on keto, especiallly in the morning. They rarely reach 80 - 90. Thanks Dr. Berg!!

same for me..... morning fasting glucose in range 100-110 (i did the test every morning for 1 week) then i got a HOMA-IR test to confirm i am ok....my result was : 0.55

Nice to hear about Dr B. IT happened to us too, maybe a month, then the glucose started going down on it's own. We see FBS's at about 90 and during the day after meals(keto) it has a random of 80-95. All good too! Took awhile like you said and not knowing about this fact we were taking 1/2 metformin daily to get the levels down. Funny, didn't work! So now it's settled off and we're happier. Good info you give us. I listen daily! Glad we found you! ♥

That's great! Hope you've stayed on the healthy bandwagon. Make reasonable food goals, and certainly find 5-10 min a day to just build muscle STRENGTH via training... Muscle is the main organ of longevity & exercise is effective at treating 26 chronic diseases! What a superpill

Thank you, Dr. Berg. Homa IR is just the test I've been looking for. I've discussed measuring insulin resistance with my doctor at Kaiser, but he told me there wasn't any way to see how that's what's going on. Unbelievable.

I really enjoy your videos Dr. Berg but you tend to just graze the actual information mentioned in your titles. An explanation as to how to get a Homo IR test and what the results would mean would have been a lot more helpful than yet another explanation on what insulin resistance is. Just some constructive criticism.

As Abacdef says, a doctor or practitioner has to order the test. Dr. Berg did say something about what the test result means. ... But, it might also be helpful to have some words about how to convince or ask a reluctant doctor to order the test.

Thank you bf., I watched your video I called my doctor to include Home IR in the blood test I am going to take and he didn't know what it was. I came across your video because, I need to understand why when I am fasting and doing the carnivore diet for four months now my glucose reading at 4 pm was 109 and I've only had coffee, a tea and some water. You hit every question I had and made me feel I'm on the right tract!!! Thank you so much!!!

I've been mostly keto since 2007. My average fasting blood glucose is 85 (4.74 mmol/L). I get a higher reading of over 100 very rarely and that's only after intense exercise.

Excellent information - - this is much needed support! Many people who have not kown how to take care of their health because of being raised in North America and elsewhere where corporate food and pharma advertising has led what is "healthy" and what is not - and then being told by many doctors in the medical profession that they have various conditions that can only be controlled by drugs and often a diet that then makes the person sicker - NEED to know that healing is a process --when the body which is miraculous -starts to heal because it is given the optimum conditions for health - it often fixes one thing at a time Many don't understand so they get very discouraged - I have often seen over the decades people stop doing what will ultimately help them to health So THANK YOU!!! Thank You Dr. Berg for answering my question without me having to ask lol. I've been doing Keto + IF for just under a year, and had lost 70 lbs up until May 1st. I have Type 2 Diabetes So after learning from you, & Dr. Jason Fung how insulin prevents fat burning I quit taking my insulin. Also began incorporating OMAD 3 - 4 days per week. Have never felt better. More energy, better sleep, etc. Since doing so I have lost 15 lbs & 2" off my waist. But I noticed my blood sugar levels had gone from avg. 4.2 - 5.2 mmol/L while I was on insulin To avg. 5.7 - 6.4 mmol/L since I've been off. So I was wondering why, and how long it would last. But you just explained why. - Because my body is repairing my Insulin Resistance and so there is more glucose in my blood since there is less insulin to lower the level. Hopefully it won't take much longer cuz I want to see that 4.2 - 5.2 mmol/L range on a steady basis.

Abacdef Mnoprs what’s incredible to me is that I’m a nurse and I have not heard of this test nor in my years of going to multiple doctors for my many ailments have they ever mentioned or ordered this test. It’s quite aggravating and I’ve been losing faith in the medical field that I am a part of. I’m going to ask my provider to order this and I can’t wait to see the look in her face.

he needs to give more info in these videos. but anyways i read that this test is a calculation of 2 other blood tests. i really doubt my doctor knows about this. HOMA-IR = fasting insulin X fasting glucose / 22.5 im still not sure 100%, i hope he replies

I tried keto diet seriously and only learned it's absolutely not for diabetes as it raised not only ketones but also my blood glucose. So, please make note of it when you make video to protect diabetes.

My doctor did an insulin level on me and it was high. She told me that I had insulin resistance. My HgA1c and glucose was normal. She started me on metformin.

How do I get my dr to order this test? I asked him if there was a test for insulin resistance and he said no just blood sugar. I’m not diabetic and since doing keto my fasting blood sugar went from 106 to 88. Love your videos, dr Berg!

I tested my homa-ir today it was 0,96 (out of 0-2,5 range), happy with my result though. Just that damn cortisol came out higher. Other than that, I feel good about the results. 🙂👍 thanks doctor!

Many thanks as always. I have a very off situation. First three months keto fasted and non fed baseline blood glucose quickly normalized low 80s. Month 4 it’s mid to upper 99s, same everything.

Yes. Insulin resistance starts years before you see a change in glucose readings. You can have low glucose readings but high insulin level. The effects of high insulin on the body can begin to show way before you are diagnosed as diabetic. I.e. blurred vision, retinopathy, neuropathy, kidney problems etc. Dr's talk about glucose, it It's actually insulin they should be looking at as well, perhaps more so.

@@sandrabentley1420 but would insulin not reflect on the A1c in a large way? The correlation is about, say 75%, yes? So is A1c not the easy way to test for both sugar+insulin? You thoughts please thanks.

@@contrarian717 I don't think so Carel. A1C is really just a 3 month snapshot of your blood glucose level, whereas insulin is the true test of how much of this hormone is resistant or sensitive to glucose. A person could have had insulin resistance for 10 years or more but you would not necessarily know that by doing a fasting glucose or A1C. Insulin levels are a PRE cursor for having glucose intolerance or not. So apparently we should all be getting an insulin test and see where that stands in relation to glucose and A1C. Then it can be determined if the person is diabetic, pre diabetic or not. I could have good fasting blood glucose, good A1C and have high insulin levels. It is the insulin that must be controlled.

You're welcome. I'm diabetic (type 2) and always interested in what you say about the subject. I can't afford you, I only wish you could take over my care. I feel that I would lose the weight and maybe ween off of my diabetic medications.

beatriz aceiton I believe he has some videos on that topic but it's a good idea to get a thyroid panel done if you haven't already. The TSH blood test isn't enough. At least request your free T3 & free T4 be tested as well. Losing hair & even balding can be a sign of hypothyroidism so it'd be helpful to rule that out with your Dr.

what kind of carbs? If u eat carbs that arent allowed on keto ur body doesnt know what to do with them and it raises ur insulin and stays in ur blood. U need to look into what you're eating and how much. Also dont eat fruit.

I just came from my doctor's appointment . I asked for HOMA IR HE WOULD NOT REFER ME TO HAVE THE TEST. I showed him this video, he got upset . At first he said he doesn't know a test like this . I also asked for coronary w calcium score , he refused it ! Doctors work for the system not for patients. He was referring me for a mammogram which I refused .

Thanks so much for educating us. Lately I’ve heard a bit about this test so I asked my GP since she’s just acknowledged I do have pre diabetes. She’d never heard of it. Maybe it’s not available in Australia? I’ll keep looking. Thanks again for your hard work.😘

You can calculate HOMA-IR yourself if you have results for your blood glucose and insulin.

Dr. said that there was noting to be gained( not in so many words) to doing a fasting insulin test! He said that checking glucose level accomplished the same thing.

I am doing that now and my sugar went from 240 to 97 fasting and keto and light exercise in a week...and I watch what I eat KETO all the way lots of salads also

I did Keto for 9 months, lost 24 lbs in the first 6 weeks, then Nothing...nothing. My blood sugars went up from 120 to 140 and Never went back down. I quit when I started having heart pains 9 months later. I was eating No Carbs except for what was in my Greens, and NO sugars.

Dr Berg or anyone else who can answer, (Short Version) Would being on a Keto diet already (about 3 months) affect the results of a test like this? (Long Version) What I mean is, I was diagnosed with pre-type 2 4 months ago, for multiple reasons, Wife and I decided to try Keto. We have had great success and my fasting blood sugar is pretty good, look great, feel even better (thank you BTW). With respect to long term health, I would obviously like to reverse my insulin resistance, not just be able to better control my blood sugar WHILE ON Keto. So my question is while on a Keto diet, will tests like this have the same kind of result range as someone who is eating a heavy carbohydrate based diet? Yes all foods will invoke an insulin response, but lower carbs would mean less insulin "stress" (don't know the real term but I think you know what I mean). Thanks in advance to anyone who can answer this

Oh wow, that is interesting. In Keto adaptation period, glucose level seem to go up. No wonder. I was wondering what on earth is going on. I guess I willl have to just keep hanging in there. One things is definitely immediate. Weight loss. And second thing ( in my case) blood pressure went down, within couple of months at the tune of 10 points. I don't count those times when it reads exceptional low.

sadly, i've been doing keto for 5-6 years and my bg readings are often 70-90 range... and my ketones are often 1.5-3.0 (most of the time)... I also practice 1-2x/mo 60-72 hr extended water fasts. I thought all was good until I did the HOMA-IR blood test only to find that my HOMA-IR score is 2.88 which is considered high insulin resistance. I must be doing something wrong.... and the daily BG and KETONES tests are misleading me to think I must be insulin sensitive when I'm not. I've also read that it's possible for ketones (during my blood test my ketones were +3).... to skew the HOMA-IR Score. But, I'm not sure if this is likely in my case or not or how to know for sure one way or the other. I may do this test several times spread out over a period of time and see if I consistently see the same results or not but, it is a bit discouraging for now (since I just got my test scores back a couple days ago). Anybody have any thoughts?

I have a question, my wife has blood sugars of 250, her C-peptide is normal, but she can’t seem to get her sugars down, even when she’s fasting and not eating at all her blood sugar stay in the 250 range. Any idea what’s causing this? She’s been dealing with this for a while, many insulins don’t work on her and we’re really at a point that we don’t know what to do. Any input would be greatly appreciated.
