Treatment of alopecia areata.
I highlight the most important elements of the recent expert consensus on how to treat alopecia areata and focus on adult patients. Chances for hair regrowth and relapse are discussed.
I also refer to my own preferences in the treatment of alopecia areata and discuss some details about cyclosporine treatment during the COVID-19 pandemic.

Dear Professor, thank you very much for another great lecture! I’m on my second year of residency and this channel really help me to understand trichoscopy and trichology at all And I have one question: often when we talk about dermatology diseases on the face and head, guidelines advise us to use triamcinolone acetonide. Is this because of the short half-life and low risk of developing atrophy?

I've been binging on them for the last couple days trying to look for answers for a friend's 17 year old daughter who was recently diagnosed with a scarring type of alopecia. Any tips for finding the right type of specialist? Rheumatologist vs Dermatologist?

For hair loss, no doubt, a dermatologist. Some dermatologists have a special focus of interest in hair, so it may be worth searching for such a doctor

Yes, all treatments in alopecia areata have some effect on the immune system. I wish you good luck with regrowing your hair!

Thank you for the video! What are the best treatment options in pregnancy? There are a lot of limitations, what should we do with this group of patients?

Unfortunately there is no good solution because the ginecologist will usually not allow to use our drugs. The good thing is that in most patients alopecia areata improves during pregnancy

@@chiarapensa7492 There is no such dose. Many of my patients receive 10 mg prednisone or less (in addition to other treatments)

With doses used in dermatology (usually around 3 mg/kg/d) and regular monitoring I had no such case, but it does not mean that it can not happen and I always remain very cautious

Madam, 1) Opinion on switching methotrexate and cyclosporine alternatively at 6-9 months cycles 2) Systemic agents for long term management of AA in children. (Have used cyclosporine as first line therapy with relevant monitoring and paediatric opinion)

1) I do not have good personal experience with MTX. Some experts say that it requires a high dose, others that it should not be in monotherapy . For me it is far-line treatment. But the idea is interesting. Follow few patients and publish ! 2) MTX is safe in children . Cyclosporine in my opinion with higher caution.

Does covid cause Alopecia Areata? If so, what are the chances of it reoccuring or advancing to AT and/or AU? Thank you!

COVID-19 does not cause AA. I have mamy patients with AA who had COVID-19. Generally it had no significant effect on AA, but you can not exclude a probability of worsening in some patients.

I have a close family member that recently got AA about 3/4 months after C-19 and the dermatologist suggested that it may have been from c-19 and stress. I'm not a doctor, but there have been several reports online (on youtube as well) that are suggesting that AA is a manifestation of c-19. I'm certainly no expert but just wanted to see if you've heard of this or not. They say c-19 can manifest TE & AA?

I do believe Covid 19 triggered my AA ,fit and healthy none stressed male , no history of it in family,had a bad fever with Covid so immune system fighting it off ,got better but not 100% after 18 months immune system still on red alert still not much hair left🙁

Yes I have hair on my head but it is very fine and it's come back white ,still trying to grow,I have no hair legs arms etc but I live in hope😄

Hi Prof.. I wanted to ask you if you familiar with isotretinoin for frontal fibrosing alopecia? My doctor put me on isotretinoin for FFA but I saw many doctors are saying it doesn't work. They all say dutasteride or finasteride are very effective and hydroxychloroquin? I really hope you'll see my message.

It works (probably the best or one of the best drugs for FFA). Some data are here: pubmed.ncbi.nlm.nih.gov/29036252/ . I use dutasteride or finasteride as additional treatment

Prof. Am I wrong or you are actually the lead researcher in the retrospective study investigating isotretinoin as a treatment for FFA?Wow of course you're familiar with this treatment. Unfortunately I got diagnosed lately with scarring alopecia LPP. It seems there is very little research on this aweful devastating 😔condition and there is no cure. treatments are vary in their effectiveness. can you please consider address this condition and hair loss?🙏🏻 and what are the therapies that are most effective based on science🙏🏻🙏🏻
