Do you have trouble getting into ketosis? Dr. Keith feels your pain. It can be frustrating to eat a keto diet and intermittent fast regularly only to be met with a blood ketone reading below nutritional ketosis. In this video, we share some reasons why it may be harder for some people than others to get into ketosis.

First off, I love you guys ,, I envy your relationship with one another ,, You can tell the respect and love for one another and the fact that you are Christians together is so awesome.. I was stalled for two months ( September and October ) stuck at a 50lb loss since Feburary this year..(2019) However, watching you guys encouraged me and listening to you both seperately and together puts my mind frame at ease and takes the stress out of me.. I recently went to just Bulletproof coffee for my morning meal and then started 1-2-3 plan ( It's on my fridge ) I eat 1 or 2 meals a day now depending if I am hungry or not.. My Carbs stay low like 5-10 a day and I eat 1500-1800 Calories a day..IMy eating window is between 1 and 7 pm. Since I started this I Lost an additional 3 lbs. and another inch from my waist. Stall is over Thanks to you both,, Thanks for the support and encouraging videos . I am 65 yrs. of age and looking forward to living a little longer and healthier .. I wish I had known sooner but better late than never! I am working on just getting rid of the last 7 lbs. and I know I will make it ,, Thanks again and God Bless you Both!

I am a 59 (60 in Feb.) year old female that has been a food addict all of my life. At 5’ 4” my heaviest was around 210 lbs. Up until three years ago I was prediabetic, really, diabetic in denial. I have had a life time of diet then weight gain repeats. Diagnosed with fatty liver several years ago, but my doctor didn’t make a big deal over it so neither did I. Three years ago I had a major health issue that was, I feel, in a major part due to my cumulative, horrible eating habits. I now have those same ketogenic issues that Dr Keith has. Out of ketosis every morning with high glucose. I have been on a ketogenic way of eating since January 2019 without one single pound of weight loss. I lost weight before keto. I now weigh 160 lbs., which is still too heavy. I want to get more weight off, but my first priority is regaining my health. Some days I am completely discouraged by my lack of weight loss, but I want to keep on keeping on in order to heal my body. Watching you two and seeing Dr. Keith’s struggles help me to know that I am not alone in this. I’m so thankful that I found you all! Sorry this is such a long post. Thank you for all you do for me and all like me. ❤️

Larkin Whidby I feel your pain - it’s taken me 3 months to loose 10lbs and I am not optimistic about loosing 50 lbs by this time time next year when I have a trip planned with friends. There is something about being older and being fat all our lives that keeps our bodies from loosing the weight the youngsters achieve on keto. But I figure I feel better and this way of eating is doable (though holidays may be tough) Good luck !

Larkin Whidby I’m 60 and have dealt with the same issue...I dropped 9 lbs over the past year but cannot break out of the plateau I’ve hit now even tho I have cut carbs down to 15 g per day! But I won’t get discouraged and neither should you because 1. It’s harder to lose weight as we age. 2. It’s far more healthy even if we can’t seem to get to our weight goal! our lifestyle promotes much more benefits so keep on keto and stay strong. I am going to try an egg fast for 4 days to see if I can kick start some weight loss, but beyond that...it is what it is....cheers! You are not alone!

I've heard of cases where people were following a strict keto diet for months or even years and not losing weight, and then into say years 2 or so, suddenly began dropping the excess weight. One theory of why is that for the first couple of years, keto, the body was healing its fatty liver. That is, healing was happening on the inside, and only after that did the body start shedding weight. So stick with it--I bet you feel better than you did when you weighed 50 lbs more--right? If you're feeling better, sleeping better, have better energy, etc., then you're on the right track. Good luck.

@@gypsyswingsw111 have you consider that your scale is not accurate. I actually lost several pounds but my scale did not show that it had occurred because it was a faulty scale. I am 76 years old, active every day and only lost 15 pounds. I’m half way to my goal. My blood pressure is in teens. There is growing evidence that you can actually use fat directly for fuel / energy without the production of ketones. Your body is all about efficiency. So by not waisting energy for conversion you are more efficient. So, you may not need as many Ketones to be full of energy. I think people need to consider changing their eating like the seasons. - While rabbit starvation from lean meat is bad, it is a change to your body. Try it for a week. - Extended fasting periods should be considered each month. - Eating "Zero Carb" ( technically not possible ) is a great way to change your bodies use of fuel. - Eat a low carb vegetable & olive oil diet for a week. - Eat only fish / seafood for a week. Your body always wants to conserve energy. So by changing things it cannot become efficient and store fat. Gaining weight is a sign of caloric efficiency. Good luck.

You my friend are not alone! I've been keto almost 3 years, lost 155+ lbs with 30ish more to go. In those 2 years I have tested sugars and ketones daily multiple times. They were slightly higher in the beginning, but I am lucky if I get up with a .3 and in 2 hours they are .7, I might hit 1.0 before I eat my first meal. I am convinced that being fat my whole life, (I too did gene testing and man did I get a bad mix of gene zips) I will have to fight this fight the rest of my life. But I do believe my body is lazy and is not going to produce anything it does not need. I have to be tight as I have no room for error! lol So I keep carbs at total 20-23, hit my protein and fat.

I just absolutely love you guys! So real and willing to share your knowledge and struggles with all of us. I've mentioned this before, but I'd love to see Dr Keith go on a 90 challenge meat only no plant diet and see what happens. Maybe it's elements in plant material (lectins, oxalates, other phytotoxins) that are interfering with his body becoming as healthy as it can be. I know Dr Becky has a great following helping people with a low carb diet and this is an awesome help for them. But, for Dr Keith this type of elimination diet may be a great 90-120 day N=1 bio hack. I'd be interested to see his ketone and blood work numbers and the effects of a meat only, zero carb diet. Just a suggestion. Thanks for letting me pitch a suggestion. God bless!!!!!

I’ve just completed a 22 day water only fast where I got and stayed in Ketosis for the entire period, I then started re feeding as my son was turning 30 so I could enjoy a keto meal with him, I have completely cut out carbs and sugar and am only eating one meal a day between 4 and 6pm and have days when I’m hardly in ketosis. I am 58 6’2 started the fast weighing 264lbs, I haven’t weighed myself but have lost a visible amount of weight and will re weigh when I fit comfortably into size large shirts and 36” trousers! It is frustrating not staying in ketosis and I’m considering going back on a water only fast for a month or when I fit into those clothes and then continue back on the Omad keto diet going forward as I definitely feel a lot more healthy, no headaches or heartburn in over a month, oh and I can comfortably tie my shoelaces 😂 stick it out and thanks Doctors for this vid as it definitely helps not being the only one worried about being in Ketosis Cheers G

What ketones are you testing for? I test for blood, urine, and breath, and only produce a measurable amount at zero carb or extended fasts (usually 50/50 water/chicken broth) and or water only. On my 10 day fasts, I’m producing measurable ketones by 3-5 days. In ten day fasts I lose 4-6 lbs, and put 2-4 lbs on 3 days post fast quickly. I retain water easily but also use a lot of sodium to keep my bp and pulse up. The positive side is, Dr Keith and I will survive any famine in our future. I was 10lbs at birth and only lost weight fasting or going long term under 500 calories (while jogging 3-6 miles daily). Oh and my heart rate is 36-42, and only reaches 60 when excited or adrenaline is pumping. Body temp is 95.5-97.4 with a fever I never reach 98. My bp is 90/50 max. At 9 months pregnant my bp was never reached 100/80. I’ve had my fasting insulin tested and it was 2.0, my fasting bs is in the 70’s and can go much lower. My point in all this is there are averages, and some of us are exceptional and not necessarily unhealthy because we’re not in range!

There is merit in the speculation that Keith is a more efficient ketone burner. I am 38 was obese, started keto 9 months ago and have done clean Keto. With that said, when I started and for at least 3 months my ketones ranged 1-3.5 and for the past 2-3 months they range 0.5 to 0.8. I have been very careful with carb creep and alternative sugars, I do drink alcohol 2 times per week, but from what I see I am a more efficient ketone user as my diet has not changed. Just my take on it.

Interesting theory that you're super efficient at burning ketones...Hmmmm...I'd like to believe that's what's happening for me! It's just so hard for me to get into ketosis. Thanks for the discussion. Helpful! (PS I do not have a history of obesity or even overweight, but still have a hard time.)

60 year old, female body, 5'6" - always active, always ate whole foods, never over 135lbs but with a family history of diabetes and early cataracts (there is a relatoinship b/t cataracts and blood glucose). I had been doing IF and low carb for about 18 months but without testing blood glucose or ketones - felt and looked great. Went off it for about 6 months while travelling and living with friends. Now , getting back on low carb >>> keto - while testing ketones and blood glucose - finding lots of glucose spikes, not easily in ketogenesis and also having blurry vision. Blown away by the spikes, my body's reaction to occaisional 'treats' (potato chips, bagel, etc) and indication of possible insulin resistance, especially the vision issues. I will get a fasting glucose blood test, thinking about a continuous glucose monitor, and would love to see a Functional Medicine doctor but they are really expensive!

I never for the first year checked my ketons then at about 13 monthes I started and im rearly above .6most of the time .4 .5 and its frustrating here I thought I was in ketosis carbs below 20 then I start testing and I think it was a mistake for me to start testing I was happier not knowing. Even after a 48hr, water fast I was at only .7 how is that possible? So thank you for this video it makes me feel better knowing more of the things that factor into it.

I totally relate to Keith! I just barely get into ketosis even after 36 hours of fasting (I do keto when I eat). Sometimes I wonder if it's a body fat situation? I'm lean and maybe the body doesn't use fat as readily when there's under a certain percentage? Maybe it uses a mix of fat and protein (muscle)?

Dr. KEIRH, I too almost always have low ketones. I attribute it to being overweight for much of my adult life, was insulin resistance, its slowly changing and working to unravel type 2 diabetes. I'm on my way. I'm surprised you dont keep your carbs under 20 more consistently. I need to be close to 10g of carbs due to the damages of type 2. I bet if you got your carbs consistently under 20, and even closer to 10 your ketones would be higher. Try it for 2 weeks.

A good Multivitamin/mineral supplement while doing IF and Keto worked for me. I was at 6.0 between meals ! BUT I suffered from serious metabolic acidosis and had to shut it down with carbohydrates ! I used SWISS Men's Ultivite for anyone interested, but I also took Centrum Men's Advanced 50+ from time to time for Copper, Vitamin K and Iodine. I would only take half at a time of both tablets. I made sure to eat dairy for Calcium, Beef for Iron or an Iron tablet, eggs for Choline and sometimes eat tomato and/or avocado for extra potassium. Of course salt for sodium and chloride too. If I was doing Keto for extended periods of time and ketones are not showing, I would probably request a full blood works be done by my doctor... EDIT: I recently found an article that explained what was happening to me. Apparently those Ketone test strips measure only the ketones you AREN'T USING for energy ! I did not know this. WE CAN BE BURNING FAT WHILE HAVING LOW STORED KETONES (High ketone readings) ! And it was the Vitamin B3 and B2 that apparently was causing the metabolic/Ketoacidosis during the fasting state. Be careful with those vitamins while fasting ! I had to consume lemon water with vinegar plus reintroduce glucose into my blood to stop the acidosis. I am now off Keto and looking to do low carb (under 120g per day or 40 to 60g per meal), high fiber, low to moderate fat (definitely low saturated fat and cholesterol), low GI (glycemic index), low GL (Glycemic load) and moderate to high protein diet... WITH intermittent fasting... that means no grains except for maybe high fiber bread, no legumes, and no to very low sugar (maybe just the sugar from a piece of fruit a day)... after learning what I have, I think this is a much healthier diet... and you will probably lose just as much weight as keto, if not more ! I hope this information is helpful... and I think getting vitamins and minerals from natural whole foods is a much better option to taking a multivitamin/mineral supplement although for Magnesium, Zinc, Folate, B12 and B6 I will be supplementing... to keep blood pressure and Homocysteine in check...

Keith - don't sweat it. Your body is far more ketone adapted than Becky's, and not only for the reason(s) you mentioned (i.e. Your body produces and uses ketones more efficiently than Becky's does). Your body only needs to produce enough ketones to fuel the brain, as your muscles no longer need ketones as they now burn fatty free acids directly. Genetics too play a role: Take the Inuit, who were rarely in ketosis despite traditionally eating a very low-carb diet. It takes several days of deep fasting for them to produce measurable ketones. Yet, they are adept at burning free fatty acids, almost as if they “skip” keto-adaptation and proceed directly to burning fat.

Hey Dr. Keith, I wonder if you've ever checked your inflammation markers, like homocystine? Hypothesis: High homocystine (or other inflammation marker)= high cortisol = high glucose = low ketones. I think you might be on the right path regarding SNPs analysis. Homocystine levels are affected by a couple of MTHFR snps. Supplementing methyl Folate and B12 would bring it down... Wonder if it would improve ketone levels. Love your channel. Thx. : )

How about an endurance test (i.e., an aerobic event greater than 2-3 hours without feeding during the event) to see if he's actually a "fat burning machine"? Maybe even go into the event in a fasted state? Bicycling could be implemented to reduce the risk of injury. If you have a positive outcome, then make these rides a habit. Start with one or two rides a week and work up to more, if desired. You've already shown in an earlier video that your ketone generation is higher with endurance workouts.

I think the fat adaptation in Keith could be right and that would be great. On the other hand he might be hyperinsulineamic from his insulin resistance which would not bode as well. You should get homa ir test and find out what your fasting insulin level is like.

Maybe due to that I won’t see full benefit of adapting? My eating pattern is conducive to low card eating as long as calories from fat equal half of my daily total calories. I do probably less than 30 carbs a day and have had no trouble dropping weight. Down at 210lbs. My goal is to reach 200lbs but I don’t pay any attention to whether or not I’m in Ketosis. I just do what I’m supposed to do. This all started when my doctor said I had to get my weight down due to HBP, high LDL and blood glue close levels. That’s all down now, way down so my goals are still to get to 200lbs but also to adapt this as a way of life. I think that I will ultimately be on a low carb diet for the rest of my days, which s fine cause I already know how to do that.

Hi 2 Fit Docs, I believe I am insulin resistant. Although Keto in the strictest form doesn’t seem to work no matter what I cut out, low calorie diets with fruits/veggies and portion control doesn’t work either. My body simply will not let go of the weight unless I starve myself or eat foods I don’t like and work out very very hard everyday. This is not sustainable. I test key tones using urinalysis sticks that show a small amount on my best day. This is my first week of no sugar, intermittent fasting, black coffee, less protein, nothing is working and it seems like such a waste of time to go each week only to find that I need to adjust and remove something else. There is nothing else to remove, and since the remaining foods are high in protein which must be kept under control, what else can be done?
@thresamatthews9096
@thresamatthews9096 2 года назад
Urine strips are fine when we first start keto, but the body quickly stops overproducing ketones. We make less. I guess we get more efficient--maybe it's demand-driven. I now have a keto mojo and it helped me manage better. I thought I would NEVER get into ketosis and have to eat 20 g or less to maintain ketosis without exogenous ketones. But the meter helped to find out my carb tolerance which is, alas, LOW after cancer treatment. Maybe you will also have to do some detective work to get it sorted out. Good luck!
