I’m always appreciative when my dentist does a thorough exam of my mouth and tongue at my checkup.

The best part of this channel? I'cant find a question raised in the comments that doctor Tom does not answer. Really the best OMFS channel for general dentistry.

Its in this borderline lesions where dilemma lies. Thats why dentists should be profficient enough to perform IBs as well as the comprehensive head and neck examination. You might only get a couple of chances to pickup CA at this stage.

Very true, excellent points! This was one that could have easily gone untreated. Thankfully her general dentist was vigilant and sent her over as soon as he saw it. Thanks for watching!

Very good channel, congratulations and thanks a lot! Ok, the lesion is very small and have already and ipsilateral lump. I prefer an excisional biopsy and positive just need to go to the neck.

Thanks for the comment! This was treated with a large local excision and a selective neck dissection

Hey Tom, I was wondering. When you removed the mesiodens in your video about that topic you didnt talk about potential damage to the n. nasopalatinus nerve. Is there no risk to damage it when raising flap in the front of the palate? If not, why not? Thank you

Thanks for the question! I try to avoid the bundle whenever possible, but with mesiodens removal it is sometimes necessary to free the bundle from the palatal mucosa. Thankfully I have never had a patient with persistent hyposthesia after this treatment, it heals well. I don't use electrocautery to minimize trauma to the nerve.

Interesting case. Could easily go undiagnosed.

Thanks for the comment, appreciate you watching! This is a good teaching case since it's not as obvious as most oral cancers.

please, please, please, STOP with the background music. Some of us have a hard time hearing or making out words with background noise/music. There is need for music. We would much rather hear your voice clearly.
