Can we please stop minimizing the pain of IUD inserts and removal . Why are women subjected to this while men get anesthesia for vasectomies

Men get local anaesthesia for vasectomies (local freezing injected into the surrounding area). The studies on local freezing to the cervix for IUD insertion are limited in their numbers and have shown mixed results- some studies show some improvement, while others show it might actually make the pain worse (because of the pain from a needle going into the cervix). My practice is evidence based, and so I will not put needles into my patient’s cervix unless there is strong supporting evidence that it will definitely improve pain (and not cause more pain). IUD removals are typically not as painful as insertions, so much so that I am unaware of any study in the literature that has been done specifically regarding pain at removal. My video descriptions of the IUD removal and insertion process is not meant to “minimize” pain, but to describe what the majority of patients can expect. I 100% acknowledge that a small percentage of patients may experience significant pain, and this description is not meant to minimize that experience. In my own office, if a patient has had a very painful IUD insertion (or, much less commonly, removal)I provide oral analgesics, I have a separate area to allow them to recover where there is a heating pad for them, etc. I acknowledge their pain and treat them accordingly. -Dr. Dervaitis

I’ve had an iud 3 times and not once has it ever really hurt during removal. IMO anesthesia is not necessary.

“Brief” yeah no this hurt EXTREMELY BAD. No I didn’t have an infection, but they did say I was too small and shouldn’t have gotten it I’m the first place …

I believe you because i had to go to a specialist before. I went to our tribal clinic and they said they had to right tools but it was extremely painful going in now im afraid for them to remove

@@tamikagreen593it was extremely painful for them to put it in probably bc they measured your uterus before putting it in. That was the absolute worst part for me, was having a ruler shoved in. It was insane. The removal process shouldn’t be quite as bad just cuz you shouldn’t have to be measured again. At least that’s what I’m hoping for as well! Lol they say you just grab the strings in the end of the IUD and pull. Like a tampon. Some drs are even leaving the string about a cm longer if their patients say they would like to self remove it. You can remove it on the toilet, in the shower, etc at home. That’s how easy it is! So I’m hoping it won’t be painful at all. lol Best of luck to you, on this journey!

​​@@AM_Entertained naw I tried and it was soooo painful, I have a high pain tolerance, I've given birth twice, once with just gas & air and the other time with no pain management at all put the pain pulling on that thing was nauseating & I was cramping bad for days after... I want it out so bad, I just need to work up the courage and bite the bullet.

My removal was excruciating and traumatic. The strings were short and my IUD seemed to get lost. I'm sure it was more difficult to find because i have a bicornate uterus. The Dr had to fish around because I was from out of state and couldn't really afford to go back. She finally found it and removed. I cried the entire time and cried out when it was pulled. ☹️

I just had mine removed today with similar issues. I have a very high pain tolerance and I was crying. This is my third IUD and I have never felt that level of pain during the removal. I've been bed ridden all day and it sucks!! I'm sorry you went through that but I'm happy I'm not alone and not being dramatic with how painful it was.

Hi Dr, I got an iud put in a month ago and it was the most painful thing I have ever experienced in my life, and then I suffered severe cramps every day. I booked a checkup appt with my doc 1 month later like you suggested and got an ultrasound which showed the iud as sitting in the wrong position. The doctor had to remove the iud and I spent all that money and went through all that pain for nothing. I wish I had gone to a gyno to get it done rather than my gp. I am so upset and traumatised from the whole experience that I dont want to get another iud put in and have gone back to the yasmin pill. Is this something that happens often?

So sorry to hear this. Every patient’s anatomy and situation is unique, it’s impossible for me to comment on this since I was not personally involved in your case and your insertion. my iud insertion was the most painful experience i’ve ever had. and i stepped into a fire when i was a kid. my personal advice as someone who’s been encouraged to try nearly every form of birth control by my OBGYN instead of finding the root cause of my hormonal balance. I am never using birth control again. it’s terrible for our bodies and we are shown no genuine care or mercy when it comes to prescription. ( my personal experience)

same thing happened to me with a gyno! it’s def best to get a checkup and see how comfortable u r with ur gyno before getting one inserted or removed, my new gyno makes me so comfortable and she’s extremely informative where as my last one just wanted it done as quick as possible and always seemed annoyed by me, i’m assuming it’s because i was young and she was busy but still mine was inserted wrong and when she tried to “fix it” she moved it further out of place and i’ve waited a year now to get it removed because i’ve been scared!

The insertion was so bad that I start getting a panic reaction whenever the doc mentions removal so I am sure it will hurt like a b... cause I will tense up. That thing was the one of worst decision of my life. I keep having infections and doing anything warranting that thing just hurts now.

@@tracymayer5519 I was asked to cough. It was a short pain that lasted 2-3 seconds then I had a couple of mild cramps and it was finally over. The pain wasn't too bad cause it was gone almost right away and far from the insertion pain level. No bleeding, nothing.

I have super mixed feelings about people discouraging each other from getting IUDS. It’s super effective and some people just weren’t educated properly. I: -had complications getting mine -had to be re-dilated 3 different times for my procedure. And despite the discomfort I felt I still recommend it. It’s not as invasive (in my opinion) as the bar that goes in your arm and though my procedure took almost an hour because of the complications I had, the OBGYN that handled it for me did an amazing job of making sure I was comfortable and safe. It was not a horrific experience. I will get another one. I will continue to use IUDs as my birth control.

It hurt. A lot! Starting from the insertion. There are some people that are blessed with painless experiences but for myself, I couldn't move after going home because of the cramping and pain. And I didn't stop bleeding for another 6 months.

I had a removal and refit 5 years ago. I’ve got another one today And from what I remember, the refit HURT, as the uterus has to measured and then the iud is inserted. I’m dreading it

I removed my IUD myself at home after a month of having it. It was so painful, cramps everyday and severe bloating.

Great video! Reinsertion has the same symptoms of accomodation? I mean: if it takes you 9 months to feel ok with Mirena, when you insert it the second time, will you have the same symptoms or it will be easier?

Generally speaking, the adjustment with subsequent IUD’s is often easier.

I had my mirena removed November 23rd and I've been bleeding for 10 days and passing clots, is this normal? Is this my period or withdrawal bleeding? I can't find information anywhere and my doctor told me as long as I'm not bleeding heavy its fine...

There is no evidence to link IUD use to vaginal or vulvar itching, irritation or yeast infections (a common cause if itching). See your doctor.

I had my mirena 3 minths ago. But i have alternate spoted bleeding and prolonge menstrual cycle, i have used primolute n for the backup medicine, is this the reason of prolonge periods?iam 45 yrs having 3 children.

It took me 9 months to surpass all the symptoms of Mirena insertion: pain, cramps, weight gain(15 kilos in almost 2 months), daily menstruation, heavy menstruation. After 9 months period stopped. Maybe it takes longer for you too.

I just had my Kyleena replaced may 31st before it expires in October this year 2023 ,the removal and reinsertion was so much better than the first time I got it inserted, but the only question I have is when is it safe to have unprotected sex again since I had it removed and replaced in the same sitting ? I know I had to wait 7 days the first time but no one told me anything about this time and I can’t find anything informative on this on the internet tbh , is it immediately effective and that’s why no one informed me or do I still have to wait 7 days ? I’m just trying to be safe but I’m literally finding no answers anywhere

At replacement, efficacy is immediate- HOWEVER, there is a chance of IUD expulsion or shifting with a new insert, so my own practice is to advise my patients to use back up contraception until their one month check up appointment (where I check that the IUD is still in good position). -Dr. Dervaitis

Dr. Kristina I have a concern to the 21 years old IUD inside my mother. In tge mid of this corona virus pandemic she experience a severe pain in her abdomen area that cause her to not stand or move. But we don't want her to go to the hospital because of corona virus so we just comfert her inside the house with some pain reliever. And now she had a small wound that grows bigger and did not heal I guess she had diabetes but we don't know what type because we still did not go to the hospital. So my question is will my mother will ba fine if she remove her 21 years old IUD? Is there a high risk that she will survive with out complications? Will it not scar her?

The chance of pregnancy with an IUD alone is 2-8/1000. Most patients feel comfortable with that low risk and do not concurrently use condoms (unless they are using condoms for STI protection with a new partner). Some patients prefer to also use condoms to further reduce the chance of pregnancy.

Can we do workout after having IUD. Because sometimes it's giving some pain. Many people saying that after having IUD we will gain weight.

Yes, it’s absolutely OK to exercise/ workout regularly (no activity restrictions with an IUD) There is not any evidence for weight gain with an IUD (see video #10). -Dr. Dervaitis

I know this video is old, I hope you can respond. My Paraguard has been slightly poking out of my cervix for over a month. And currently I can tell something is wrong/infected by my discharge. Is it better i try to push the iud back up or take it out myself and get a new one placed?

It is generally a similar experience (although the level of your experience of the person doing the insertion can impact discomfort… if you have the second insertion performed by a different health care provider there may be some variation). -Dr. Dervaitis
