Episode 53: Kidney Disease Diet: One Meal A Day (OMAD Keto) and CKD benefits and results. I announced I have changed my Intermittent fasting to OMAD (one meal a day) as part of my kidney diet to improve my renal health. The OMAD benefits include breaking insulin resistance, staying in ketosis, and activating autophagy. In this video I cover one meal a day benefits and results, including:
What is one meal a day (OMAD)
• Type of Intermittent fasting.
• IM is eating all of your food, nutrients, and calories in a reduced amount of time.
• Average person eats 6 times a day (3 meals, 3 snacks), which keeps your blood sugar elevated and puts continued stress on your kidneys.
• Intermittent Fasting is NOT starving - your just eating less often.
• Drink water all day - even when not eating
• With OMAD you usually eat everything in 1 hour.
o BIG meals
o Can be hard to eat it all, have to focus on QUALITY food
o You are FULL - STUFFED after your meal and have no interest in eating the rest of the day. Helps stop snacking.
Am I hungry only eating one meal a day?
• No - I will start to get hungry as my meal time approaches, but it is a normal hunger.
• Remember, these are BIG meals, you are stuffed once you eat it and have no interest or hunger the rest of the day.
• Wake up in the morning and busy getting ready for work, easy to push off eating until close to noon.
Why do I do OMAD Keto?
• Break Insulin Resistance. Managing your blood sugar is important in improving your GFR when you have CKD and Insulin Resistance is harmful to improving your health.
• Save time - making only 1 meal a day, no always planning my next thing to eat
• Stay in Ketosis - don’t have a gallbladder
• Helps with my weight loss goal. You always need a reason for doing something!
• Autophagy - the body’s way of cleaning out damaged cells in order to regenerate newer, healthier cells. So many benefits for staying younger, healthier, and living longer.
Will I do OMAD KETO forever?
• No - this is part of my strategy to lose weight.
• Will go back to Intermittent Fasting with 2 meals a day once I hit goals.
• Will do it a few times a year for autophagy benefits
Should you do OMAD?
• You must talk to your doctor to make sure you are healthy enough to do this and to understand how it will impact your blood sugar.
• I’m doing Keto, Renal, Intermittent Fasting, Low Inflammation, Vegetarian - it’s a lot to think about, but only having 1 meal a day makes it easier.
• If you do want to try OMAD, it is best to work up to it. Get used to eating for just 8 hours, then 6, then 4, then 1.
• Must focus on QUALITY food - this is your one meal to get everything you need, including calories.
• Keep yourself busy - having lots of free time leads to wanting to snack
• Keep temptations away.
• Get your family to support you
o No easy
o They have to keep snacks and food away
o They have to understand this is important to you
• Drink plenty of water ALL DAY LONG.

Oh gosh I don’t know if I could ever do that. I really would like to repair my body, but I have nobody that would support me, as I have to cook for my husband and he eats only meat 🥩 and bread or with porridge, very hard for me to diet. But I try my best. I defiantly want to look for a iron pot, I saw one but can’t remember in witch shop. I have a skillet pan but it’s with the grains like for grilling. Can’t do veggies or Frye’s in it except meats. One thing you have reminded me is to correct what is in your blood works. So I looked and I was not anemia, but I see it has dropped, got to get that up. And like you I also had my gallbladder taken out a year before my kidney had a cancer tumor in it and it was removed. So only have the one kidney and no gull bladder. But nobody told me a Bout fat like you mentioned to take in....so I will have to find out about that. My Cholesterol is a bit over the normal , and are working on that. You are my encouragement to keep going. Such good videos you make . You are helping so many people . 🌹❤️🌹❤️🌹❤️

I did was worried about OMD. I am just 6 months on it. I thought it was a mistake? But my water tea was added salt and apple cider vinegar. Was it what caused it?

Not only do I not think you're crazy, you inspired me to do IF (18/6), and my Creatinine is now 1.1. My GFR isn't budging at 51, but I feel great, even though I'm severely anemic (I'm also following your anti-anemia tips to good effect). Thanks, James. You're looking healthier every video. What an inspiration you are.

ive been taking creatinine monohydrate do u think that is bad i put it in my morning protein drink. do u know if suplement creatinine s bad if kidneys are not great froshy urine also. anyone list the ingredients please. is bfast 10 am then dinner at 5-6pm i do weights 3 times a week i walk evey day can u hep

I’ve Ben on omad for the last two months. I was stage five kidney failure, and my blood 🩸 pressure was through the roof. My weight was 221lbs starting out. I’m currently down to 195lbs and counting. I’ve Ben spinning on my bike for one hour four days a week. All this has led to my being in tip top shape. Pants 👖 size waist was almost 40’ now I’m currently down to size 36’. Now my shirt size has dropped from a Extra large to a large. For any kidney patients out their I recommend you try this method.

Another great video taking a complex question and making it easy to understand. I started keto a few weeks ago after a ketoacidosis event (Type 1 Diabetic) and have moved to OMAD. After a few days, it now seems normal - no hunger during the fasting periods and lots of energy! Thanks so much for making keto easy to understand!

I have back pain at the kidney area a week ago i got itchy skin and a headache plus i not feeling well..then i start fasting and try change my diet imediatly...start praying to Jesus and read the Bible more...its been more than a week...before that i never seen your channel yet,and now i know im in right path...pray for me to cure my kidney desease...now im feeling just tired only..the itchy slowly not itching anymore just a little bit....but the back pain sometime its come...

Thanks for sharing - but you should get a blood test done by your doctor to see how your kidney is functioning. The itching could be high phosphorus and by changing your diet you reduced the phosphorus. The tiredness is usually anemia - a low red blood cell count and/or low hemoglobin. Your doctor can help you address these in your diet once he/she has your blood work to see what you are low in, and what you are high in. Getting things into the right balance will really help the way you feel. Good luck!

Hi James, still loving your videos...always learn something new. I am learning about the ketogenic diet after getting “permission” from my dietician...also working on intermittent fasting...only for 14 hours atm...taking it slow....autophagy sounds really interesting so that’s something I will look into as well. Thanks for sharing your journey...very happy to know your steadily improving :-).

Getting there bit by bit. It's tough for various reasons esp adrenal fatigue. Slowly getting no cravings and controlling my carb intake and successfully come off insulin! Now OMAD next goal.

I do my quarterly 3-day fasts at the start of each season (summer, fall, winter then spring). End of September for the next one!!!

Great video as always James. Thank you for going what you do to inspire others. We all need to question our doctors. If you don't ask... you will never know as they will not tell you the whole picture. Constantly question your DRs. When family all come to visit... I find it a challenge at present. I am thinking IF may be one way to see if there is a change. Anaemia is still a battle (looking like a vampire) and had to have a blood transfusion. Still a challenge with the water intake! I keep telling mum she needs to drink. She feels its too much. We are dealing with stoke recovery too... and you also need to be hydrated to avoid other things like seizure. I had heard about OMAD via folk doing Keto on many videos and autophogy I want to try a basic Keto (if there is such a thing?) or maybe a simple GAP diet. I think with any type of diet is to try short term. We are all built different and so it may work well or may work so so. Always try and document. Mum does not need to lose any more weight though. Maybe you could show snapshots of your bloods from a few months? Mums GFR after over a month is up a few points, so that is good but I think its from drinking more water only. Given in to the odd cup of tea and chamomile infusion. Renal appt on the 20th...

Been doing keto for one year today. Lost only 30 lbs. can’t lose the last 15. Must re evaluate. Started OMAD months ago. 6 hour eating window. My creatinine, and bun are high last 6 months. Just underwent sonogram/ultrasound today and I’m scared I have damage. We’ll see!! You’re doing a great job here. So glad I found your channel in case I will need info, it seems it’s all right here. It’s also time for me to get a new GP.

Drs Eckberg, Fung, and Mindy would be very pleased with u Mr Dadvice. Good content in a down to earth speaking style. I've moved from 97-80 Kg over 5 mths doing OMAD and faltering attempts at keto. Settled off btwn 83-86 kg over the last 3 months since I've relaxed on keto and extend my window from 1 hr to 6 hrs. I'm T1D and was having too many hypo episodes. 48 yr old male African-American @ 173 cm. eGFR 33-37 and creatinine 180. I've switched from weightlifting to just 10 km walks.

You are doing fantastic! I found 2 meals a day to be the best balance. One meal a day was too difficult to maintain while traveling - and that is a lot of food to eat in 1 sitting.

Oh this really lifted me up so much..I thank you very much ..I shared your video because sometimes I don't think family would understand and even co workers.. because they see me alright and yet I seem to play a Nick nack on choosing what to eat..drives me crazy if someone might say if that would what will really cause to die when eating that then it should have already did it since then...I'm stage 5 iga nephropathy.. it's saddening to hear my nephrologist say that my kidneys will only have a year or two and I'm on to dialysis..I don't want to believe her..I'm fine, just my creatinine is 4.0 and a little bit high cholesterol and high blood pressure..I wanted to watch all your videos sir and get help

Thanks for your kind comments and sharing! There is a lot of information to be found in the videos I created - though the ones from the first year (going from Stage 5 UP to stage 3) can get long (I love talking). For 2020 I started to change my videos to be more education focused - less about me and what I was doing (I still share that, but its not the focus). My latest videos have LOTS of great info, especially the LIVE videos I do on Tuesdays at 6pm EDT with a Renal Dietitian. Try to join one of those if you can - you're able to use the chat to ask questions. Tonight at 6pm I'm doing a video on blood pressure that will also include a live chat.

Newly diagnosed with Type 2 CKD. I’ve had microscopic blood in my urine for decades. Could never get an answer why. Now, I am worried what this diagnosis means. My blood pressure and sugar are normal. I subscribed to your channel and hope your positive outlook rubs off on me. You are an inspiration.

Glad to hear that. The one thing that I feel sets my videos apart is that I'm an actual kidney patient and speaking from experience. I may be the only channel like that - all of the others I've ran across are people who dont have kidney damage, some are doctors, some are people sharing info they read. Not seeing any real kidney patients is what got me thinking I should document my journey.

I struggle with dehydration. That takes my 25% kidney function down down to about 14.. Then I have to go back and have more blood work done and I have to make sure I'm very hydrated before the blood t test.

Hi James I’m stage 4 CKD and my other problem I have +2 protein. My question is can I eat protein everyday? This won’t increase my protein in my urine? Pls inform me better cause I’m confused. My nephrology says I can eat protein everyday but in small amount. Others nephrology tells you can only eat protein 3x a week so I’m really confused.

I do Omad and when I do eat I do keto and I reversed my diabetes and my blood pressure I no longer take any drugs we weren’t ment to eat 6 times a day

I haven’t been officially diagnosed with CKD but my doctor says my last urine sample showed protein in my urine. She said less than 2 grams ( is that still considered high?). I am 61 years old, negative for diabetes and high blood pressure. Not sure the cause of protein in my urine…I do have borderline high cholesterol, hypothyroidism and an autoimmune disease( Sjogrens Syndrome). I am scheduled to have another urine sample and blood work in 6 months to see where my numbers are. I have not been referred to a nephrologist… yet. My question here is, does the OMAD diet HURT the kidneys in any way that you know of. I know there’s not much literature out there about it. I want to follow the OMAD diet for weight loss and also want the autophagy benefits, as long as I’m not hurting my kidneys by doing it. Thank you.

Great advice! I've been on a 16/8 intermittent fasting diet now for 10 months, I may move to OMAD, but at the moment it seems to working. In January 2020 my eGFR was 42, now in November 2020 (10 months later) it's 54, I'm 74 so it can work no matter what your age is. One question: in your 1400 OMAD diet how much protein are you eating? I'm trying to stay below 45 gms per day, and alternate one day vegan and the next I eat chicken or fish.

I have a friend who doesn't know if he has kidney disease... but he's a severe diabetic, and an advocate of OMAD, been doing it for years. It has not made a difference that I can tell for his blood sugar issues, but he doesn't watch WHAT he eats, or the portion control. For myself, I take twice=a-day medications that have to be taken with food... Hmph. I do intermittent fasting several times a week. I find that, surprisingly, I don't get particularly hungry during the twelve hours between breakfast and dinner. In fact, the opposite. I used to have uncontrollable hunger, when I was eating six mini-meals a day for a stomach condition. My stomach healed... it is SO much easier to skip meals than to eat six a day! LOL

I found OMAD too hard to keep up with while traveling for work, and went back to eating my first meal around noon and finishing my last meal by 6pm. I agree - it is so much easier to skip meals than to squeeze in extras! I rarely am hungry and sometimes accidentally forget my first meal. Your friend really needs to watch what he eats - bad food doesn't matter when time it is, it is still bad food going in. Thanks for sharing!

Congrats on your success! Personally, I use 2MAD. I eat my big meal around noon. Then I have a second smaller meal, around 5-6PM. My concern with OMAD is that it can be difficult for your body to fully utilize a full day's worth of protein from a single meal. The challenge when losing weight, IMO, is to try to maintain or increase your lean muscle mass. The trick is to get just the right amount of protein most every day. Spreading the protein over 2 meals avoids overloading your ability to fully assimilate the protein. Excess protein is to be avoided, but under consuming protein can lead to muscle loss. Also, I believe it is important to consume protein and fat together, in proper proportion. If you don't have a gallbladder, you may want to consider an ox bile supplement to help you digest fat. Also, medium-chain triglycerides (MCT) are quickly and easily assimilated and can help jump start your production of ketones. MCT oil can be helpful for people who have a hard time getting into or maintaining ketosis.

Ev S: the idea behind OMAD is to be in the fasting mode as long as possible. Having bfast after dinner last evening will give u about 12-14 hrs of fasting. Then btwn bfast and dinner is 10-12 hrs. However if u have dinner latest 6 pm, then ur next meal is 12 the next day, then that would be 18 hrs of fasting. This should give better results. Bless.

Hi I m fr Singapore. How to reduce my husband's Creatinine to drop fr 330 to below 260. He has blockage to both kidneys. If this Thursday the Creatinine still at same level or spike he needs surgery to open the blockage. I gave him 200 gms salmon or seabass per meal. Is this too much that cause the Creatinine to spike. Plz help to advise me. His is ckd 4th stage and hypertension. Tks.

I am worried a big meal can stress my kidney so I decided 2 meals a day 1 pm and 5 pm i have read autophagy seems to be very important for renal functions, but i am not sure if omad o two meals a day are good since are bigger meals in a short period of time. :S

This sounds like overkill. I have one slice of wholemeal toast after I get up - which is essential for me to have a daily motion. I then usually don't have anything [perhaps one more slice of wholemeal] until my main meal. I may have small food at other times. Only two coffees and then mostly water. I think that's probably not too much burden on my kidneys.

I've been sticking with around 50 grams and I get most of mine from a combo of nuts, eggs, chicken, and tuna. As of now I completely avoid red meat. What is your favorite source of protein?

@@DADVICETV I've been doing low fat Greek yogurt and eggs. It's easier with those to measure quickly and not worry about going over. I was in acute renal failure back in February. GFR so low they couldn't calculate it. Dialysis for 3 weeks. Doing better now though, off dialysis. GFR around 56.

I lost a lot of weight . now no high blood pressure no cholesterol pills harming my liver , no sugar problems only problem i have is malnutrition hehe. watch out for that !! now i am eating a little more fasting a little less. i think it is best to be on the top of your weight biomass at 25 than down below 22 just to have that little weight protection if something happens. I m just trying to maintain bmi at 24 to 25 . i never knew it was so easy to control. watch out for malnutrition and have fun losing weight brother.

also i heard ur kidney can only handle 20 grams of protein every 2 hours or so and so i like to spread that out if Im doing more that 20 because from what i learned anything over 20 grams may be overload. i don't have the paper that i read that said that so if i wrong let me know.

I haven't heard that, but I do know animal protein will put your kidneys into hyper filtration mode for several hours, increases stress on the kidneys which could lead to inflammation (which also lowers kidney function). I haven't had any issues eating all my protein within an hour, but very little is animal protein, nearly all of it is plant based.

I drink water ALL day long. I limit my eating window to 1 hour in the afternoon. As a matter of fact I'm just getting ready to go get my one meal for the day in a few minutes and It's almost 4pm. But I make sure and drink water all day long - we can't get dehydrated.

I ended up going back to 2 meals a day for the convenience. It was getting hard with travel to plan 1 large meal that had all of the calories and nutrients I needed. I now eat my first meal after noon and finish my last meal by 6pm, giving my 18 hours a day of fasting.

+strictly low down Lemon juice is very good for you and helps prevent kidney stones. A quick blood test can check your kidney function and let you know if you have kidney issues - I recommend getting that done to see where you stand. Then your doctor can help you with a treatment strategy and diet if needed.

OMAD is great.. seems to me it's the ideal way everyone is meant to eat. Quick tip, on those hungry days try taking a teaspoon or two of MCT oil. It immediately stops hunger.

I'm going to recommend NOT doing 1 meal a day - instead ask your doctor about intermittent fasting. 1 Meal a day is a LOT of food to eat each day, and it can get "blah" very quickly. I kept it up for a month and then went back to eating all meals between Noon and 6pm. The benefits were nearly the same and it was MUCH easier to get all the nutrients I needed.

I love doing OMAD and have done it for a long time now. On days where I do feel hungry, I will do 18:6 or 20:4 . I never have breakfast no matter which fasting regime I am doing.

Thank you for the info..being 39 and gfr being 36 is not helpful snd on top my kidneys keep on taking out albumin whixh is at 22 right now..36 and up being normal...frustrated by food etc...maybe I should just try IF diet...and have one meal a day...need to be alive for my son..I know for a fact that if I go on dialysis as a diabetic I won't survive more then 6 months on it..

I only take medications for my blood pressure. I have lost a great deal of weight and will be adjusting those at my next doctor appointment in about a week - they have become too powerful now that I am less over weight and make me very sleepy.

James are you following Keto(high fat) or DaVita Kidney diet for your OMAD meal? My husband, Erik, does not need to lose any weight but Fasting 16 hours would give the kidneys a good rest. Still waiting for his labs this coming week to see how less beef for a month did.

I'm doing Keto with extreme high fat, mostly plant-based protein, and very low carbs (20 grams net carbs a day). I'll always do low carb going forward, but most likely going to switch to a more Mediterranean style diet in the near future.

I have tried IF two meals a day but not every day, usually during the week at work, on weekend I relax a bit more, sometimes I will add breakfast depends on how I feel. I think I am too obsessed with food to only eat once a day. if I did do that I could only do it occasionally, I think.

The OMAD didn't last long for me - my job has me traveling nearly 100% the last 5 months of the year and it was just too much food to find and eat during an hour of lunch. I went back to eating 2 meals a day - one around noon and the second by 6pm. That made it much easier to consume all of the nutrients I needed while still being social with coworkers and friends. I try to eat low carb, though right now I'm back to having a lot (and I mean A LOT) of apples in my diet. Most of my protein comes from nuts, beans, hummus (homemade), and some protein drink mix. I am unfortunately one of the people who has a negative reaction to soy, so I have to greatly limit any soy I consume. When I make my meals I reach for a lot of spices - curry, cumin, ginger, turmeric, cinnamon, and chili peppers. I also use coconut oil, Ghee, and nut to change up the textures and flavors. My go-to meal is a stir fry - chop of veggies, add some nuts, black olives, water chestnuts and cook it up. Then flavor with some type of sauce I mix up. A lot of kidney patients don't consider Ghee since it is animal protein - but it is amazing since you only need the smallest amount of it to give something a buttery flavor. A small jar goes so far and you are consuming very little animal protein while still getting "butter" in your food.

Soy is much higher fat compared to other legumes and perhaps that is why it is not as well tolerated. I used to do keto, but I am finding going low-fat vegan feels better for vascular health. Nonetheless, I sometimes miss the easy energy of coconut oil based stir frys.

I don’t eat breakfast, I eat one 4 oz cup of Greek yogurt, then I eat a small salad 🥗 with avocado 🥑 and pouch of tuna or salmon then I’m good until supper around 6 o’clock. I don’t have Kidney Disease I have always eaten like a bird 🦢 lol 😂 I have a high metabolism and burn off calories fast . On September 3 I will be 60 years young !!!! My Husband is really learning to fast after supper and not eat until his breakfast 🥞 the next day. Drink up H2O oh yes !!! We both have totally stopped drinking Sweet Tea ☕️ holy cow 🐄 I never thought I would be able to give it up! Thank you Sir for education you share 🙏🏻

So I did not know that last part and yesterday I went to quest to get labs just to see where I was at. Rejected - and my family doc would have approved if I called him, but he is on vacation. My Nephrologist works at a hospital and I get labs there too, but it is a shame I need a doctors order to pay to get my own labs. :-(

My first meal of the day is a glass of lemon water. Some hours later is my main meal-o-the-day = OMAD. Two hours later is my second meal. A couple ounces mixed nuts with some sourdough pretzels, grapes. My info's come from countless hours doing my own research. My Dr's are good for dispensing some med's. Outside that they're not worth much

I had someone ask the same question on facebook. Here is an example of a meal:For the most part, my main meal is a lot of mixed veggies (carrots, broccoli, cauliflower, snow peas, zucchini, cucumber, onion) stir fried in a cast iron wok using Ghee (clarified butter) with few eggs (scrambled in at the last moment, or boiled), a side salad (romaine lettuce, cucumber, a little cheese, sunflower seeds, blueberries), some macadamia & almond nuts, and usually a little keto desert I make with Ghee/Egg/Almond powder/Blueberries. I stick with vinaigrette type dressings, but will chance up the difference dressings or sauce I make for the veggies.
